15. Moving beyond the stage: how characteristics at diagnosis dictate treatment and treatment-related quality of life year losses for women with early stage invasive breast cancer

Abstract

Background:Although evaluations of breast cancer screening programs frequently estimate quality-adjusted life-year (QALY) losses by stage, other breast cancer characteristics influence treatment and vary by mode of detection – i.e. whether the cancer is detected through screening (screen-detected), between screening rounds (interval-detected) or outside screening (community-detected). Here, we estimate the association between early-stage invasive breast cancer (ESIBC) characteristics and treatment-related QALY losses. Methods:Using clinicopathological and treatment information from 675 women managed for ESIBC, we estimated the average five-year treatment-related QALY loss by detection group. We then used regression analysis to estimate the extent to which known cancer characteristics and the detection mode, are associated with treatment and treatment-related QALY losses. Results:Community-detected cancers had the largest QALY loss (0.76 QALYs [95% CI 0.73;0.80]), followed by interval-detected cancers (0.75 QALYs [95% CI 0.68;0.82]) and screen-detected cancers (0.69 QALYs [95%CI 0.67;0.71]). Adverse prognostic factors more common in community-detected and interval-detected breast cancers (large tumours, lymph node involvement, high grade) were largely associated with QALY losses from mastectomies and chemotherapy. Receptor-positive subtypes, more common in screen-detected cancers, were associated with QALY losses related to endocrine therapy. Conclusions:The associations between ESIBC characteristics and treatment-related QALY losses should be considered when evaluating breast cancer screening and treatment strategies.

Abstract

Although DNA immunization is a safe and efficient method for inducing cellular immune responses, it generates relatively weak and slow immune responses. Here, we investigated the effect of hepatitis C virus (HCV) antigen modifications on the induction of T-cell responses in DNA immunization. It is likely that the strength of T-cell responses has an inverse relationship with the length of the insert DNA. Interestingly, a mixture of several plasmids carrying each gene induced a higher level of T-cell responses than a single plasmid expressing a long polyprotein. Moreover, the presence of a transmembrane domain in HCV E2 resulted in stronger T-cell responses against E2 protein than its absence. Taken together, our results indicate that the tailored modifications of DNA-encoded antigens are capable of optimizing the induction of T-cell responses which is required for eliminating the cells chronically infected with highly variable viruses such as HCV and human immunodeficiency virus. Abstract

Yoido Full Gospel Church of David Yonggi Cho is the world's largest church, and naturally Cho deserves attention from scholars for his work. Some claim that his work and theology arc related to the shamanism of Korean culture because Cho emphasizes healing and blessings. However, his messages on healing and blessings originate from the American Pentecostal/Charismatic movements rather than from Korean shamanism. Therefore, one should consider that Cho's work is part of a process of spreading the American Pentecostal movements in Korea rather than one of shamanizing the Christian gospel. Cho has kept relationships with missionaries (especially Pentecostal ones) more than with other pastors in Korea. He was an associate to Pentecostal missionaries in the beginning. However, the foreign Pentecostals and Charismatics have become Cho's friends, helping his ministry to globalize. Through his fellowship with foreigners, he has been able to discover recent trends of the Pentecostal/Charismatic movements and apply them to his ministry in Korea. Among the Pentecostals and Charismatics, Oral Roberts is the most important person to have had an impact on Cho. Roberts not only stands in the tradition of Pentecostalism, but also participates in the Charismatic movement. Roberts supports Kenneth Hagin, although he does not agree with every aspects of his theology. Like Roberts, Cho also has a close relationship with American Pentecostal and Charismatic movements and has some similarities to Hagin's faith movement. However, Cho's messages are based on sound evangelical theology. In particular, Cho's teaching of the three-fold blessings originates from Roberts. [ABSTRACT FROM AUTHOR]
