Until recently, heart transplantation was the only treatment for ATTR-CM. Regulatory approval of tafamidis transformed treatment for patients. In the phase 3 Transthyretin Amyloidosis Cardiomyopathy Clinical Trial (ATTR-ACT), which established the safety and efficacy of tafamidis, medication adherence was high with 97.2% of patients taking ≥ 80% of scheduled doses. Evidence of real-world adherence to cardiology drugs demonstrates low adherence and suboptimal outcomes; however, real-world adherence to tafamidis has not been investigated. The main objective of this study was to describe adherence patterns of patients filling tafamidis in the Symphony Health database.Methods: This retrospective analysis of the Symphony Health Solutions claims database used secondary adherence measures, including modified medication possession ratio (MPRm), days between fills adherence rate, and compliance rate, to assess adherence patterns of 2020 patients filling tafamidis free acid 61-mg capsules or tafamidis meglumine 4x20-mg capsules from June 1, 2019 to August 31, 2020.Results: Patients receiving a tafamidis formulation had characteristics consistent with the expected patient population; 71.6% were aged 75– 84 years, 83.2% were male, and the highest proportion resided in the Northeast region (30.5%) of the United States. Adherence for tafamidis was high, as 75% to 100% of the patients across subgroups met or exceeded the commonly defined adherence threshold of 80%. Median number of refills ordered and received was six refills per patient. Most patients received refills with no gap (n=1633) or a gap < 30 days (n=1267/1317 patients). Adherence was high across follow-up time, sex, and age subgroups. Adherence varied by geographic region, with the Northeast being significantly higher than the Midwest (mean MPRm 94.41% vs 88.21%, p=0.0007).Conclusion: These results provide evidence that real-world adherence to tafamidis in patients with ATTR-CM is high.Keywords: claims analysis, amyloidosis, cardiomyopathy, transthyretin amyloid, adherence

Photosynthesis is a process where solar energy is utilized to convert atmospheric CO2 into carbohydrates, which forms the basis for plant productivity. The increasing demand for food has created a global urge to enhance yield. Earlier, the plant breeding program was targeting the yield and yield-associated traits to enhance the crop yield. However, the yield cannot be further improved without improving the leaf photosynthetic rate. Hence, in this review, various strategies to enhance leaf photosynthesis were presented. The most promising strategies were the optimization of Rubisco carboxylation efficiency, the introduction of a CO2 concentrating mechanism in C3 plants, and the manipulation of photorespiratory bypasses in C3 plants, which are discussed in detail. Improving Rubisco’s carboxylation efficiency is possible by engineering targets such as Rubisco subunits, chaperones, and Rubisco activase enzyme activity. Carbon-concentrating mechanisms can be introduced in C3 plants by the adoption of pyrenoid and carboxysomes, which can increase the CO2 concentration around the Rubisco enzyme. Photorespiration is the process by which the fixed carbon is lost through an oxidative process. Different approaches to reduce carbon and nitrogen loss were discussed. Overall, the potential approaches to improve the photosynthetic process and the way forward were discussed in detail.

This study was conducted to assess caregiver preferences for pediatric asthma treatment delivery systems. A total of 186 caregivers of children with asthma who ranged in age from 1 to 4 y completed a stated-preference questionnaire to assess the importance of specific treatment attributes in terms of caregivers' preferences, and to determine the percentages of caregivers who preferred specific treatments. Other outcomes assessed included caregivers' likelihood of adhering to pediatric asthma treatment. Most respondents (75%) preferred treatments that have been approved by the US Food and Drug Administration (FDA) in children as young as 12 mo, that require minimal effort in coordination and inhalation on the child's part, and that take up to 10 min to administer. A greater proportion of respondents indicated that they would be able to follow the physician's advice (increased likelihood of adherence) if given this treatment (54%) versus an alternative (35%). This difference did not achieve statistical significance, however. The conjoint analysis method used in this study enabled investigators to assess caregivers' relative preferences for specific attributes of various pediatric asthma treatments. Overall, FDA approval in children as young as 12 mo was the most preferred attribute. Caregiver preference for pediatric asthma treatment alternatives is an important consideration and should be a component of discussions between healthcare professionals and caregivers. In the treatment of asthma, the conventional measures used to monitor a patient's progress and health status do not address the impact of functional impairments associated with the disease that may affect the patient's daily life. Unlike those measures, health-related quality of life (HRQL) reflects the physical, psychological, and social difficulties a patient perceives on a day-to-day basis. This study was conducted to determine the effects of once-daily budesonide inhalation powder via the Pulmicort Turbuhaler on the HRQL in adult patients with asthma previously treated with other inhaled corticosteroids. A total of 184 patients 18 to 70 years of age who previously received inhaled corticosteroids were enrolled in this double-blind, placebo-controlled, parallel-group, multicenter study. Patients were randomly assigned to budesonide 400 microg once daily or to placebo for 12 weeks. Each patient's HRQL was assessed at randomization and at weeks 4 and 12 with the Asthma Quality of Life Questionnaire (AQLQ). More patients receiving budesonide than those receiving placebo reported statistically significant (P < or = .05) improvements in HRQL at weeks 4 and 12. With the exception of the domain pertaining to exposure to environmental stimuli, differences from placebo in overall AQLQ scores and individual domain scores were clinically important (> or = 0.5 units). In addition, 2.4 patients needed to be treated with once-daily budesonide for 1 patient to demonstrate clinically important improvement. Budesonide 400 microg administered once daily via the Pulmicort Turbuhaler provides statistically significant and clinically important HRQL benefit in adult patients with asthma previously receiving inhaled corticosteroids. Abstract Background Bone metastases are a common painful and debilitating consequence of castration-resistant prostate cancer (CPRC). Bone pain may predict patients' prognosis and there is a need to further explore CRPC patients' experiences of bone pain in the overall context of disease pathology. Due to the subjective nature of pain, assessments of pain severity, onset and progression are reliant on patient assessment. Patient reported outcome (PRO) measures, therefore, are commonly used as key endpoints for evaluating the efficacy of CRPC treatments. Evidence of the content validity of leading PRO measures of pain severity used in CRPC clinical trials is, however, limited. Methods To document patients' experience of CRPC symptoms including pain, and their impact on health-related quality of life (HRQL), semi-structured in-depth qualitative interviews were conducted with 17 patients with CRPC and bone metastases. The content validity of the Present Pain Intensity (PPI) scale from the McGill Pain Questionnaire (MPQ), and the 'Average Pain' and 'Worst Pain' items of the Brief Pain Inventory Short-Form (BPI-SF) was also assessed. Results Patients with CRPC and bone metastases present with a constellation of symptoms that can have a profound effect on HRQL. For patients in this study, bone pain was the most prominent and debilitating symptom associated with their condition. Bone pain was chronic and, despite being generally well-managed by analgesic medication, instances of breakthrough cancer pain (BTcP) were common. Cognitive debriefing of the selected PRO measures of pain severity highlighted difficulties among patients in understanding the verbal response scale (VRS) of the MPQ PPI scale. There were also some inconsistencies in the way in which the BPI-SF 'Average Pain' item was interpreted by patients. In contrast, the BPI-SF 'Worst Pain' item was well understood and interpreted consistently among patients. Conclusions Study findings support the importance of PRO measures of pain severity as key endpoints for evaluating the efficacy of treatments for CRPC, particularly for patients with bone metastases where episodes of BTcP are common. Qualitative evidence from CRPC patients supports the content validity of the BPI-SF ''Worst Pain' item and promotes use of this item for measuring pain severity in this population.

Organic pollutants such as dyes and pharmaceutical drugs have become a significant environmental problem due to their unrestricted discharge, especially in water bodies. As a result, an economically viable and environmentally friendly approach to their degradation in water bodies is required and the incorporation of metal tungstate with single metal oxide has attracted attention due to its potential ability towards the photocatalytic degradation of pollutants. The work demonstrates a WO 3 /g-C 3 N 4 /V 2 O 5 nanocomposite synthesized using a facile route wet impregnation method. The results revealed that WO 3 /g-C 3 N 4 /V 2 O 5 nanocomposites are suitable, mainly for their better surface properties, enhanced visible-light absorption, and preferred band positions. Besides that, the degradation of methylene blue (MB) dye is carried out and demonstrated that the complete degradation occurs over 120 min using 10 mg L -1 of WO 3 /g-C 3 N 4 /V 2 O 5 nanocomposite under UV-visible-light irradiation. The scavenger experimental result implies that the photogenerated free electrons and superoxide radials are important role in MB dye degradation. In addition, a possible mechanism is proposed for the photocatalytic activity of WO 3 /g-C 3 N 4 /V 2 O 5 nanocomposite. Moreover, the stability analysis demonstrated that the WO 3 /g-C 3 N 4 /V 2 O 5 nanocomposite can be recycled multiple times., (© 2023. Copper oxide nanoparticles (CuO NPs) are among the most commonly employed nanoparticle materials owing to their antibacterial qualities, although their primary mechanism of action (MOA) is still not completely understood. CuO NPs are synthesized in this study using leaf extract of Tabernaemontana divaricate (TDCO3), and they are then analyzed using XRD, FT-IR, SEM, and EDX analysis. The zone of inhibition of TDCO3 NPs against both gram-positive (G + ) B. subtilis and gram-negative (G - ) K. pneumoniae bacteria was 34 mm and 33 mm, respectively. Furthermore, Cu 2+ /Cu + ions promote reactive oxygen species and electrostatically bind with the negatively charged teichoic acid in the bacterial cell wall. The anti-inflammatory and anti-diabetics analysis was conducted using standard BSA denaturation and α-amylase inhibition technique with cell inhibition values of 85.66 and 81.18% for TDCO3 NPs. Additionally, the TDCO3 NPs delivered prominent anticancer activity with the lowest IC 50 value 18.2 μg/mL in the MTT assay technique against HeLa cancer cells., (© 2023. The foliar application of nutrients and plant growth regulators (PGRs) at critical crop growth periods can improve the yield of field crops. Hence, the present study was conducted to quantify the effects of the combined application of nutrients and PGRs (crop-specific formulation) on maize, blackgram, greengram, groundnut, cotton, sugarcane, and coconut yield. In all the crops except coconut, the treatments included (i) a foliar spray of crop-specific nutrients and PGR combinations and (ii) an unsprayed control. In coconut, the treatments included (i) the root feeding of coconut-specific nutrients and PGR combinations and (ii) an untreated control. Crop-specific nutrient and PGR formulations were sprayed, namely, Tamil Nadu Agricultural University (TNAU) maize maxim 1.5% at the tassel initiation and grain-filling stages of maize, TNAU pulse wonder 1.0% at the peak flowering stage of green gram and black gram, TNAU groundnut-rich 1.0% at the flowering and pod-filling stages of groundnut, TNAU cotton plus 1.25% at the flowering and boll development stages of cotton, and TNAU sugarcane booster 0.5% at 45 days after planting (DAP), 0.75% at 60 DAP, and 1.0% at 75 DAP of sugarcane. The results showed that the foliar application of TNAU maize maxim, TNAU pulse wonder, TNAU groundnut-rich, TNAU cotton plus and TNAU sugarcane booster and the root feeding of TNAU coconut tonic increased the yield of maize, pulses, groundnut, cotton, sugarcane, and coconut, resulting in higher economic returns.

Herein, a Bi 2 S 3 /Cu 2 S was successfully synthesized via a simple one-step wet impregnation process. The compositional behavior and electrical and optical properties of photocatalysts were investigated in detail. Photocatalytic technology has shown great promise in wastewater treatment, splitting water to hydrogen, and converting CO 2 to fuel. Researchers or scientist are attempting to design sulfate-based heterojunction photocatalytic systems in order to develop novel photocatalysts with excellent performance. Photodegradation of methylene blue (MB) dye and tetracycline (TC) drug under visible light irradiation was used to assess the photocatalytic activity of as-prepared samples. As a result, 2:1% wt of Bi 2 S 3 /Cu 2 S heterostructure composite revealed superior visible light degradation performing of MB dye, and TC drug efficiency as 90.2% and 87.5%, respectively. The prepared hybrid photocatalyst has demonstated a potential for use in the photocatalytic degradation of antibiotic durgs and dyes, indicating a promissing future for its application., (© 2023. A facile and cost-effective hydrothermal followed by precipitation method is employed to synthesize visible light-driven ZnS-Ag ternary composites supported on carbon aerogel (CA). Extensive studies were conducted on the structural, morphological, and optical properties, confirming the successful formation of ternary nanocomposites. The obtained results evidently demonstrate the successful loading of ZnS and Ag onto the surface of the CA. High-resolution transmission electron microscopy analysis revealed that ZnS and Ag nanoparticles (AgNPs) were uniformly distributed on the surface of the CA with an average diameter of 18 nm. The biomass-derived CA, containing a hierarchical porous nano-architecture and an abundant number of -NH 2 functional groups on the surface, can greatly prevent the agglomeration, stability and reduce particle size. Brunauer-Emmett-Teller analysis results indicated specific surface areas of 4.62 m 2  g -1 for the CA, 48.50 m 2  g -1 for the CA/ZnS composite, and 62.62 m 2  g -1 for the CA/ZnS-Ag composite. These values demonstrate an increase in surface area upon the incorporation of ZnS and Ag into the CA matrix. Under visible light irradiation, the synthesized CA/ZnS-Ag composites displayed remarkably improved photodegradation efficiency of methylene blue (MB). Among the tested samples, the CA/ZnS-Ag composites exhibited the highest percentage of photodegradation efficiency, surpassing ZnS, CA, and CA/ZnS. The obtained percentages of degradation efficiency for CA, ZnS, CA/ZnS, and CA/ZnS-Ag composites were determined as 26.60%, 52.12%, 68.39%, and 98.64%, respectively. These results highlight the superior photocatalytic performance of the CA/ZnS-Ag composites in the degradation of MB under visible light conditions. The superior efficiency of the CA/ZnS-Ag composite can be attributed to multiple factors, including its elevated specific surface area, inhibition of electron-hole pair recombination, and enhanced photon absorption within the visible light spectrum. The CA/ZnS-Ag composites displayed consistent efficiency over multiple cycles, confirming their stable performance, reusability, and enduring durability, thereby showcasing the robust nature of this composite material., (© 2024. The present study, reports a facile approach for the synthesis of FeWO 4 /FeS 2 nanocomposites were demonstrated through hydrothermal method. The surface morphology, crystalline structure, chemical composition, optical properties of the prepared samples was analysed by different various technique. The result observed analysis indicates that, the formation of heterojunction by 2:1wt% of FeWO 4 /FeS 2 nanohybrid has the lowest recombination rate of electron-hole pairs and the least electron transfer resistance. Due to its the broad absorption spectral range and preferable energy band gap, the (2:1) FeWO 4 /FeS 2 nanohybrid photocatalyst exhibits an excellent ability to remove MB dye when exposed to UV-Vis. Light irradiation. Its photocatalytic activity of (2:1) FeWO 4 /FeS 2 nanohybrid is higher than other as prepared samples due to its synergistic effects, enhanced light absorption and high charge carrier separation. Radical trapping experimental result implies that the photo-generated free electrons and hydroxyl radials are essential to degrade the MB dye. Furthermore, a possible future mechanism for FeWO 4 /FeS 2 nanocomposites photocatalytic activity was discussed. Moreover, the recyclability analysis demonstrated that the FeWO 4 /FeS 2 nanocomposites can be recycled multiple times. BACKGROUND: Transthyretin amyloid cardiomyopathy (ATTR-CM) is an underdiagnosed, life-threatening condition that mostly affects older persons. In May 2019, regulatory approval of tafamidis provided the first pharmacologic treatment of ATTR-CM. In the pivotal phase 3 Transthyretin Amyloidosis Cardiomyopathy Clinical Trial (ATTR-ACT), 97.2% of patients were classified as adherent (defined as taking ≥ 80% of scheduled doses). Given its recent approval, there is limited real-world evidence examining patient adherence to tafamidis. OBJECTIVE: To evaluate adherence patterns, demographics, and clinical characteristics of patients in the United States receiving tafamidis prescriptions through Medicare. Secondarily, we aimed to evaluate concomitant medications filled by this patient population. METHODS: We conducted a retrospective cohort study of US Medicare claims data, limited by the Health Insurance Portability and Accountability Act of 1996, in adult patients with an adjudicated pharmacy claim for tafamidis (tafamidis free acid 61-mg capsule once daily or tafamidis meglumine four 20-mg capsules once daily) between May 1, 2019, and June 30, 2021. Gaps in therapy were measured using day gaps between prescription refills and continuous measure of medication gaps. Implementation adherence was assessed through modified medication possession ratio (MPRm), medication refill adherence (MRA), and proportion of days covered (PDC). Patients were grouped based on Medicare coverage. Patients were analyzed by subgroups based on age and at the zip code level, via distressed communities index quartiles and rural-urban tiers. RESULTS: A total of 3,558 patients who received a prescription fill of a tafamidis formulation were identified using Medicare Fee-for-Service (FFS) and Medicare Advantage (MA) claims data from May 1, 2019, to June 30, 2021. The characteristics of this patient population were consistent with published literature, as 98.6% were older than 65 years, 53.4% were between 75 years and 84 years, and 81.5% were male. In the patient population receiving tafamidis refills, adherence was high across all 3 measures, with mean MPRm greater than 90% and mean MRA greater than 80%, across all age groups. Mean PDC adherence rates were 79% or more across all age groups. Concomitant medications were generally indicated for heart failure and thrombosis. Among monotherapy groups with similar demographic makeup, adherence was significantly higher among users of tafamidis free acid vs tafamidis meglumine ( P < 0.0001 across all mean adherence measures). CONCLUSIONS: Our results demonstrate that real-world adherence to tafamidis in the Medicare population is high, regardless of age, zip code-level socioeconomic quartile, or geography. Adherence was higher among patients receiving tafamidis free acid, suggesting that the enhanced convenience of a single capsule once daily may positively contribute to adherence among patients with ATTR-CM.
