Pilonidal sinus disease has been described as a hair dressers' or (barbers') disease because of short hairs that penetrate the interdigital spaces of the hand. Pilonidal sinus disease is common around the sacrococcygeal region but rarely seen in the anal canal, umbilicus, chest wall, ear, or on the scalp. Our case involves a 21-year-old man who was doing his military service. He complained of a mass which he had had for several years around the left occipital region of the scalp, the patient's medical history seemed uneventful. We totally excised the lesion and sutured. After the operation, histopathology confirmed our diagnosis as pilonidal sinus of the scalp. After a three-month follow-up, the patient had made a complete recovery. Pilonidal sinus disease involves a hair-containing sinus first described by Herbert Mayo in 1833 [1]. In 1880, Hodge suggested the term 'pilonidal' [2] from the Latin 'pilus' for hair and 'nidus' meaning nest, and later Karydakis [3,4] defined the pathogenesis of this disease. After a long debate the disease is no longer considered a congenital abnormality even though 50-70% of cases have hair shafts in the lesion. The first case of a sinus in a hairdresser was described by Templeton in 1942 [7]. Pilonidal sinus disease is common around the sacrococcygeal region [8] but rarely seen in the anal canal [9], umbilicus [10], chest wall [11], ear [12], or on the scalp [13]. Acute suppurative otitis media rarely involve adjacent dura (posterior and middle) and sigmoid sinus. Wide use of antibiotics is the common cause of decrease incidence of these complications. We are reporting this case because of its presentation. She was presented with symptoms and signs of meningismus with minimal ear symptoms and signs. Mucocele is a rare intracranial pathology. In majority mucoceles arise within the frontal and ethmoid sinuses. Intracranial and intraorbital extensions are uncommon. A 48 year old male with a history of head trauma with multiple craniofacial fructures for 30 years. After 30 years came to first generalized epileptic seizure, also the leftside exophthalmos was observed. The radiological investigations, computed tomography (CT) and magnetic resonanse (MRI), showed large extracerebral pathological mass invading the left frontal and ethmoid sinuses, left orbit and anterior cranial fossa with left frontal lobe compression. The left side fronto--basal craniotomy was performed. The histopathological diagnosis was mucocele. Postoperative neurological examination revealed no deficit. The patient was seizures free and in good condition discharged from the department. Mucocele is a rare, benign pathology arising intracranial from the sinuses. The results are good, the frequency of complication is low. Metallic Foreign bodies in maxillary sinus are rare. Caldwell-Luc (CWL) procedure is used for their removal, but it is associated with complications like oro-antral fistula, gingivolabial fistula, dacrocystitis, and devitalized tooth. Moreover the potential problem of altered facial development as a result surgical treatment in children is well recognized. Functional endoscopic sinus surgery (FESS) has emerged as a better alternative. A case of an 8 year old male child with a metallic pellet due to gun shot injury in maxillary sinus is being presented and the effect of FESS and CWL procedure in pediatric population are discussed.
