Abstract

Indirect Drive Inertial Confinement Fusion Experiments on the National Ignition Facility (NIF) have achieved a burning plasma state with neutron yields exceeding 170 kJ, roughly 3 times the prior record and a necessary stage for igniting plasmas. The results are achieved despite multiple sources of degradations that lead to high variability in performance. Results shown here, for the first time, include an empirical correction factor for mode-2 asymmetry in the burning plasma regime in addition to previously determined corrections for radiative mix and mode-1. Analysis shows that including these three corrections alone accounts for the measured fusion performance variability in the two highest performing experimental campaigns on the NIF to within error. Here we quantify the performance sensitivity to mode-2 symmetry in the burning plasma regime and apply the results, in the form of an empirical correction to a 1D performance model. Furthermore, we find the sensitivity to mode-2 determined through a series of integrated 2D radiation hydrodynamic simulations to be consistent with the experimentally determined sensitivity only when including alpha-heating.

Abstract

An experimental program is currently underway at the National Ignition Facility (NIF) to compress deuterium and tritium (DT) fuel to densities and temperatures sufficient to achieve fusion and energy gain. The primary approach being investigated is indirect drive inertial confinement fusion (ICF), where a high-Z radiation cavity (a hohlraum) is heated by lasers, converting the incident energy into x-ray radiation which in turn drives the DT fuel filled capsule causing it to implode. Previous experiments reported DT fuel gain exceeding unity [O.A. Hurricane et al., Nature 506, 343 (2014)] and then exceeding the kinetic energy of the imploding fuel [S. Le Pape et al., Phys. Rev. Lett. 120, 245003 (2018)]. We report on recent experiments that have achieved record fusion neutron yields on NIF, greater than 100 kJ with momentary fusion powers exceeding 1PW, and have for the first time entered the burning plasma regime where fusion alpha-heating of the fuel exceeds the energy delivered to the fuel via compression. This was accomplished by increasing the size of the high-density carbon (HDC) capsule, increasing energy coupling, while controlling symmetry and implosion design parameters. Two tactics were successful in controlling the radiation flux symmetry and therefore the implosion symmetry: transferring energy between laser cones via plasma waves, and changing the shape of the hohlraum. In conducting these experiments, we controlled for known sources of degradation. Herein we show how these experiments were performed to produce record performance, and demonstrate the data fidelity leading us to conclude that these shots have entered the burning plasma regime.

Non-invasive ventilation may have deleterious effects, especially in patients exerting strong inspiratory efforts, and no current recommendations support its use in this setting. Protective non-invasive ventilation using higher levels of positive-end expiratory pressure, more prolonged sessions and other interfaces such as the helmet may have beneficial physiological effects leading to it being proposed as alternative to high-flow nasal oxygen in acute hypoxemic respiratory failure. By contrast, non-invasive ventilation is the first-line strategy of oxygenation in patients with acute exacerbation of chronic lung disease, while high-flow nasal oxygen could be an alternative to non-invasive ventilation after partial reversal of respiratory acidosis. Questions remain about the target populations and non-invasive oxygen strategy representing the best alternative to standard oxygen in acute hypoxemic respiratory failure. Abstract

Creating non-equilibrium states of matter with highly unequal electron and lattice temperatures allows unsurpassed insight into the dynamic coupling between electrons and ions through time-resolved energy relaxation measurements. Recent studies on low-temperature laser-heated graphite suggest a complex energy exchange when compared to other materials. To avoid problems related to surface preparation, crystal quality and poor understanding of the energy deposition and transport mechanisms, we apply a different energy deposition mechanism, via laser-accelerated protons, to isochorically and non-radiatively heat macroscopic graphite samples up to temperatures close to the melting threshold. Using time-resolved x ray diffraction, we show clear evidence of a very small electron-ion energy transfer, yielding approximately three times longer relaxation times than previously reported. Abstract

Background & Aims: Autoimmune hepatitis (AIH) has been well characterised and codified through the development of diagnostic criteria. These criteria have been adapted and simplified and are widely used in clinical practice. However, there is a need to update and precisely define the criteria for both treatment response and treatment. Methods: A systematic review was performed and a modified Delphi consensus process was used to identify and redefine the response criteria in autoimmune hepatitis. Results: The consensus process initiated by the International Autoimmune Hepatitis Group proposes that the term ‘complete biochemical response’ defined as ‘normalization of serum transaminases and IgG below the upper limit of normal’ be adopted to include a time point at 6 months after initiation of treatment. An insufficient response by 6 months was a failure to meet the above definition. Non-response was defined as ‘<50% decrease of serum transaminases within 4 weeks after initiation of treatment’. Remission is defined as liver histology with a Hepatitis Activity Index <4/18. Intolerance to treatment was agreed to stand for ‘any adverse event possibly related to treatment leading to potential drug discontinuation’. Conclusions: These definitions provide a simple and reproducible framework to define treatment response and non-response, irrespective of the therapeutic intervention. A consensus on endpoints is urgently required to set a global standard for the reporting of study results and to enable inter-study comparisons. Abstract

This report describes the Key Comparison of the Consultative Committee for Photometry and Radiometry CCPR-K1.a.2017 for Spectral Irradiance in the wavelength range of 250 nm to 2500 nm. Tungsten quartz halogen lamps of the FEL type (1000 W) were used as artefacts. Each participant used its own set of lamps. The star scheme of measurements was applied with the sequence participant - pilot - participant. Twelve National Metrology Institutes participated in the comparison. Each participant's measurement data were based on a totally independent spectral irradiance scale realisation. VNIIOFI (the Russian Federation) served as a pilot. The measurements were carried out from 2017 - 2020. The analysis of the comparison was performed following the step-by-step approach described in Appendix B of the Guidelines for CCPR Key Comparison Report Preparation (CCPR-G2 Rev.4). The key comparison reference values (KCRV) were calculated independently at each wavelength as the weighted means with cut-off. The measurement data of all participants were used for the KCRV excluding just a few outliers in the wavelength range of 1700 nm to 2500 nm. The relative uncertainties of the KCRV were minimal, 0.08 %, at the wavelengths of 900 nm to 1500 nm and increasing towards the limits of the spectral range up to 0.28 % at 250 nm and 0.20 % at 2500 nm. Consistency checks were satisfied at 17 of the 28 measured wavelengths, but failed at 11 wavelengths, mostly in the infrared range. For the latter wavelengths the Mandel-Paule method was applied resulting in an additional uncertainty that varied depending on wavelength from 0.11 % to 0.89 %. In general, the measurement uncertainties submitted by the participants in this comparison were less than those submitted in the previous CCPR-K1.a comparison. This is evidenced by the values of the cut-off, which had decreased at all wavelengths by a factor of 1.2 to 3.0 depending on wavelength. Abstract

Background Currently, the standard therapy for autoimmune hepatitis (AIH) consists of a combination of prednisolone and azathioprine. However, 15% of patients are intolerant to azathioprine which necessitates cessation of azathioprine or changes in therapy. In addition, not all patients achieve complete biochemical response (CR). Uncontrolled data indicate that mycophenolate mofetil (MMF) can induce CR in a majority of patients. Better understanding of first-line treatment and robust evidence from randomised clinical trials are needed. The aim of this study was to explore the potential benefits of MMF as compared to azathioprine, both combined with prednisolone, as induction therapy in a randomised controlled trial in patients with treatment-naive AIH. Methods CAMARO is a randomised (1:1), open-label, parallel-group, multicentre superiority trial. All patients with AIH are screened for eligibility. Seventy adult patients with AIH from fourteen centres in the Netherlands and Belgium will be randomised to receive MMF or azathioprine. Both treatment arms will start with prednisolone as induction therapy. The primary outcome is biochemical remission, defined as serum levels of alanine aminotransferase and immunoglobulin G below the upper limit of normal. Secondary outcomes include safety and tolerability of MMF and azathioprine, time to remission, changes in Model For End-Stage Liver Disease (MELD)-score, adverse events, and aspects of quality of life. The study period will last for 24 weeks. Discussion The CAMARO trial investigates whether treatment with MMF and prednisolone increases the proportion of patients in remission compared with azathioprine and prednisolone as the current standard treatment strategy. Abstract

Tuberculosis is a global public health problem with 10 million people developing an active form each year. The exact incidence of the pelvic form of tuberculosis is not known due to under-reporting of asymptomatic cases, vague symptomatology and lack of reliable diagnosis. We report a case of ovarian tuberculosis mimicking cancer and peritoneal carcinosis. The case involved a 15-year-old girl, hospitalized for severe pelvic pain and secondary amenorrhea. The examination showed an altered general state, a fever, a sensitive distended abdomen. The biology showed an elevation of Ca125 to 357 IU/ml. The CT scan showed a latero-uterine mass taking contrast in a heterogeneous way with an irregular and nodular thickening of the peritoneum making evoke a peritoneal carcinosis. Exploratory laparotomy found a bilateral ovarian mass with ileo-ileal, ileo-parietal and ileo-ovarian adhesions. Biopsy of the mass and the peritoneum showed a caseo-follicular gigantocellular epitheloid granulomatosis.

34. Developing Mathematical Thinking and Self-Regulated Learning: A Teaching Experiment in a Seventh-Grade Mathematics Classroom

Abstract

Mathematics educators have found sociocultural models of teaching and learning to be powerful in their ability to describe and support the pursuit of instruction based on recent standards documents (e.g., National Council of Teachers of Mathematics [NCTM], 1989, 2000). These models of instruction, however, have been criticized for their lack of explicitness. Detailed descriptions of cognitive processes within self-regulated learning (SRL) and attribution theories (e.g., Borkowski et al., 1988; Butler, 2002; Zimmerman, 2000) lend support for and provide examples of explicit instruction embedded within sociocultural models of mathematics instruction. Self-regulated learners are active participants in their own learning, are able to select from a repertoire of strategies and to monitor their progress in using these strategies toward a goal. In this article, we describe the collaborative efforts of a seventh-grade mathematics teacher and a university faculty member to develop students' mathematical thinking and self-regulation within one middle school classroom. We argue that implementing mathematics instruction consonant with NCTM (1989, 2000) standards documents makes the development of self-regulated learners possible and is at the same time dependent upon some degree of self-regulation within the community of practice. Several factors are crucial to this development: multiple representations and rich mathematical tasks; classroom discourse; environmental scaffolding of strategic behavior; and varying needs for explicitness and support. This work constitutes beginning efforts to describe contexts that support the development of mathematical thinking and SRL in the middle school mathematics classroom.

Abstract

For more than half a century, researchers around the world have been engaged in attempts to achieve fusion ignition as a proof of principle of various fusion concepts. Following the Lawson criterion, an ignited plasma is one where the fusion heating power is high enough to overcome all the physical processes that cool the fusion plasma, creating a positive thermodynamic feedback loop with rapidly increasing temperature. In inertially confined fusion, ignition is a state where the fusion plasma can begin "burn propagation" into surrounding cold fuel, enabling the possibility of high energy gain. While "scientific breakeven" (i.e., unity target gain) has not yet been achieved (here target gain is 0.72, 1.37 MJ of fusion for 1.92 MJ of laser energy), this Letter reports the first controlled fusion experiment, using laser indirect drive, on the National Ignition Facility to produce capsule gain (here 5.8) and reach ignition by nine different formulations of the Lawson criterion.

Item does not contain fulltext, BACKGROUND & AIMS: Autoimmune hepatitis (AIH) has been well characterised and codified through the development of diagnostic criteria. These criteria have been adapted and simplified and are widely used in clinical practice. However, there is a need to update and precisely define the criteria for both treatment response and treatment. METHODS: A systematic review was performed and a modified Delphi consensus process was used to identify and redefine the response criteria in autoimmune hepatitis. RESULTS: The consensus process initiated by the International Autoimmune Hepatitis Group proposes that the term 'complete biochemical response' defined as 'normalization of serum transaminases and IgG below the upper limit of normal' be adopted to include a time point at 6 months after initiation of treatment. An insufficient response by 6 months was a failure to meet the above definition. Non-response was defined as '<50% decrease of serum transaminases within 4 weeks after initiation of treatment'. Remission is defined as liver histology with a Hepatitis Activity Index <4/18. Intolerance to treatment was agreed to stand for 'any adverse event possibly related to treatment leading to potential drug discontinuation'. CONCLUSIONS: These definitions provide a simple and reproducible framework to define treatment response and non-response, irrespective of the therapeutic intervention. A consensus on endpoints is urgently required to set a global standard for the reporting of study results and to enable inter-study comparisons. Contains fulltext : 252186.pdf (Publisher’s version ) (Open Access), BACKGROUND AND AIMS: Detection of autoantibodies is a mainstay of diagnosing autoimmune hepatitis (AIH). However, conventional autoantibodies for the workup of AIH lack either sensitivity or specificity, leading to substantial diagnostic uncertainty. We aimed to identify more accurate serological markers of AIH with a protein macroarray. APPROACH AND RESULTS: During the search for more-precise autoantibodies to distinguish AIH from non-AIH liver diseases (non-AIH-LD), IgG antibodies with binding capacities to many human and foreign proteins were identified with a protein macroarray and confirmed with solid-phase ELISAs in AIH patients. Subsequently, polyreactive IgG (pIgG) was exemplarily quantified by reactivity against human huntingtin-interacting protein 1-related protein in bovine serum albumin blocked ELISA (HIP1R/BSA). The diagnostic fidelity of HIP1R/BSA binding pIgG to diagnose AIH was assessed in a retrospective training, a retrospective multicenter validation, and a prospective validation cohort in cryoconserved samples from 1,568 adults from 10 centers from eight countries. Reactivity against HIP1R/BSA had a 25% and 14% higher specificity to diagnose AIH than conventional antinuclear and antismooth muscle antibodies, a significantly higher sensitivity than liver kidney microsomal antibodies and antisoluble liver antigen/liver pancreas antigen, and a 12%-20% higher accuracy than conventional autoantibodies. Importantly, HIP1R/BSA reactivity was present in up to 88% of patients with seronegative AIH and in up to 71% of AIH patients with normal IgG levels. Under therapy, pIgG returns to background levels of non-AIH-LD. CONCLUSIONS: pIgG could be used as a promising marker to improve the diagnostic workup of liver diseases with a higher specificity for AIH compared to conventional autoantibodies and a utility in autoantibody-negative AIH. Likewise, pIgG could be a major source of assay interference in untreated AIH.

Abstract

In this presentation we provide an overview of the commissioning and first user experiment of ReLaX in combination with the XFEL beam. We will introduce the setup and laser parameters. We will show the established standard setup combining small-angle x-ray scattering (SAXS), phase-contrast imaging (PCI) and spectroscopy techniques. Finally we will show examples of the data obtained in the interaction of the ReLaX laser with targets of interested for the short-pulse laser community.

Abstract

The High Energy Density (HED) instrument at the European XFEL provides a platform to study hot and warm dense matter. The Helmholtz International Beamline for Extreme Fields (HiBEF) is a User Consortium supplying HED with two laser systems (the high-intensity ReLaX laser, by Amplitude Technologies, and the high-energy Dipole-100X laser, by STFC), Diamond Anvil Cells setup and high-pulsed magnetic fields. These tools in combination with the XFEL beam enable the investigation of relativistic laser plasmas, strong-field QED phenomena, high-pressure astro- and planetary physics as well as magnetic phenomena in condensed matter. The successful commissioning of the ultra-short pulse high-intensity ReLaX laser, provides new unique opportunities in the plasma and high-field physics fields [1]. ReLaX is a double CPA Ti:Sa laser capable of delivering up to 300 TW pulses on target. In the first commissioning phase, 100 TW pulses were used, reaching intensities up to 1020 W/cm2. Small-Angle X-Ray Scattering (SAXS) without the need of a beamstop was first commissioned at HED in September 2019. Two highly annealed pyrolytic graphite (HAPG) crystals were used to reflect the SAXS photons onto a detector while allowing the main XFEL beam to go through [2]. In April and May 2021, Small Angle X-Ray Scattering and Phase Contrast Imaging (PCI) were simultaneously demonstrated in pump-probe experiments at HED in a community experiment involving 15 institutions from all over the world. In this talk we will present the preliminary results of this community experiment probing ultrafast phenomena in a wide array of target configurations: hole boring in wires, shockwave generation in CH blocks, buried heating of a wire inside a CH medium, foam ionization and collective effects in heated foils. Abstract

Ignition is needed to make fusion energy a viable alternative energy source, but has yet to be achieved (1). A key step on the way to ignition is to have the energy generated through fusion reactions in an inertially confined fusion plasma exceed the amount of energy deposited into the deuterium--tritium fusion fuel and hotspot during the implosion process, resulting in a fuel gain greater than unity. Here we report the achievement of fusion fuel gains exceeding unity on the US National Ignition Facility using a 'high-foot' implosion method (2,3), which is a manipulation of the laser pulse shape in a way that reduces instability in the implosion. These experiments show an order-of-magnitude improvement in yield performance over past deuterium--tritium implosion experiments.
