Abstract

ITER envisages the use of two heating neutral beam injectors plus an optional one as part of the auxiliary heating and current drive system. The 16.5 MW expected neutral beam power per injector is several notches higher than worldwide existing facilities. A Neutral Beam Test Facility (NBTF) was established at Consorzio RFX, exploiting the synergy of two test beds, SPIDER and MITICA. SPIDER is dedicated to developing and characterizing large efficient negative ion sources at relevant parameters in ITER-like conditions: source and accelerator located in the same vacuum where the beam propagates, immunity to electromagnetic interferences of multiple radio-frequency (RF) antennas, avoidance of RF-induced discharges on the outside of the source. Three years of experiments on SPIDER have addressed to the necessary design modifications to enable full performances. The source is presently under a long shut-down phase to incorporate learnings from the experimental campaign. Parallelly, developments on MITICA, the full-scale prototype of the ITER NBI featuring a 1 MV accelerator and ion neutralization, are underway including manufacturing of in-vessel components, while power supplies and auxiliary plants are already under final testing and commissioning. Integration, commissioning and tests of the 1MV power supplies are essential for this first-of-kind system, unparalleled both in research and industry field. The integrated test to confirm 1MV output by combining invertor systems, DC generators and transmission lines extracted errors/accidents in some components. To realize a concrete system for ITER, solutions for the repair and the improvement of the system were developed. Hence, NBTF is emerging as a necessary facility, due to the large gap with existing injectors, effectively dedicated to identify issues and find solutions to enable successful ITER NBI operations in a time bound fashion.

Abstract

The design for an inductive superconducting quantum interference proximity transistor with enhanced performance, the L-SQUIPT, is presented and analyzed. The interferometer is based on a double-loop structure, where each ring comprises a superconductor-normal metal-superconductor mesoscopic Josephson weak-link and the read-out electrode is implemented in the form of a superconducting tunnel probe. Our design allows both to improve the coupling of the transistor to the external magnetic field and to increase the characteristic magnetic flux transfer functions, thereby leading to an improved ultrasensitive quantum limited magnetometer. The L-SQUIPT behavior is analyzed in both the dissipative and the dissipationless Josephson-like operation modes, in the dissipative or in the dissipationless Josephson-like operation mode in the latter case by exploiting both an inductive and a dispersive readout scheme. The improved performance makes the L-SQUIPT promising for magnetic field detection as well as for specific applications in quantum technology, where a responsive dispersive magnetometry at milliKelvin temperatures is required., Comment: 11 pages, 8 figures

Abstract

Josephson junctions, in appropriate configurations, can be excellent candidates for detection of single photons in the microwave frequency band. Such possibility has been recently addressed in the framework of galactic axion detection. Here are reported recent developments in the modelling and simulation of dynamic behaviour of a Josephson junction single microwave photon detector. For a Josephson junction to be enough sensitive, small critical currents and operating temperatures of the order of ten of mK are necessary. Thermal and quantum tunnelling out of the zero-voltage state can also mask the detection process. Axion detection would require dark count rates in the order of 0.001 Hz. It is, therefore, is of paramount importance to identify proper device fabrication parameters and junction operation point., Comment: 7 pages, 9 figures

Abstract

Superconducting computing promises enhanced computational power in both classical and quantum approaches. Yet, scalable and fast superconducting memories are not implemented. Here, we propose a fully superconducting memory cell based on the hysteretic phase-slip transition existing in long aluminum nanowire Josephson junctions. Embraced by a superconducting ring, the memory cell codifies the logic state in the direction of the circulating persistent current, as commonly defined in flux-based superconducting memories. But, unlike the latter, the hysteresis here is a consequence of the phase-slip occurring in the long weak link and associated to the topological transition of its superconducting gap. This disentangle our memory scheme from the large-inductance constraint, thus enabling its miniaturization. Moreover, the strong activation energy for phase-slip nucleation provides a robust topological protection against stochastic phase-slips and magnetic-flux noise. These properties make the Josephson phase-slip memory a promising solution for advanced superconducting classical logic architectures or flux qubits., Comment: 14 pages, 10 color figures

Abstract

Quantum Sensing is a rapidly expanding research field that finds one of its applications in Fundamental Physics, as the search for Dark Matter. Devices based on superconducting qubits have already been successfully applied in detecting few-GHz single photons via Quantum Non-Demolition measurement (QND). This technique allows us to perform repeatable measurements, bringing remarkable sensitivity improvements and dark count rate suppression in experiments based on high-precision microwave photon detection, such as for Axions and Dark Photons search. In this context, the INFN Qub-IT project goal is to realize an itinerant single-photon counter based on superconducting qubits that will exploit QND for enhancing Axion search experiments. In this study, we present Qub-IT's status towards the realization of its first superconducting qubit device, illustrating design and simulation procedures and the characterization of fabricated Coplanar Waveguide Resonators (CPWs) for readout. We match target qubit parameters and assess a few-percent level agreement between lumped and distributed element simulation models. We reach a maximum internal quality factor of 9.2×105 for -92 dBm on-chip readout power.

10. Metallic supercurrent field-effect transistor

Abstract

In their original formulation of superconductivity, the London brothers predicted the exponential suppression of an $electrostatic$ field inside a superconductor over the so-called London penetration depth, $\lambda_L$. Despite a few experiments indicating hints of perturbation induced by electrostatic fields, no clue has been provided so far on the possibility to manipulate metallic superconductors via field-effect. Here we report field-effect control of the supercurrent in $all$-metallic transistors made of different Bardeen-Cooper-Schrieffer (BCS) superconducting thin films. At low temperature, our field-effect transistors (FETs) show a monotonic decay of the critical current under increasing electrostatic field up to total quenching for gate voltage values as large as $\pm 40$V in titanium-based devices. This $bipolar$ field effect persists up to $\sim 85\%$ of the critical temperature ($\sim 0.41$K), and in the presence of sizable magnetic fields. A similar behavior was observed in aluminum thin film FETs. A phenomenological theory accounts for our observations, and points towards the interpretation in terms of an electric-field-induced perturbation propagating inside the superconducting film. In our understanding, this affects the pairing potential and quenches the supercurrent. Abstract

Solid mixed conductors with significant ionic as well as electronic conduction play a pivotal role for mass transfer and storage as required in battery electrodes. Single-phase materials with simultaneously high electronic and ionic conductivity at room temperature are hard to come by and therefore multi-phase systems with separate ion and electron channels have been put forward instead. Here, we explore bilayer graphene as a true single phase mixed conductor and demonstrate ultrafast lithium diffusion exceeding diffusion in bulk graphite by an order of magnitude and even surpassing diffusion of sodium chloride in liquid water. To this end, an innovative electrochemical cell architecture has been developed where the redox-reaction forcing lithium intercalation is localized at a protrusion of the device only. Its remainder consists of pristine bilayer graphene unperturbed by an electrolyte. The geometry lends itself to the use of magnetotransport machinery known from mesoscopic low-dimensional physics. Time dependent Hall measurements across spatially displaced Hall probes deliver a direct view on the in-plane diffusion kinetics. Abstract

The temperature dependence of electric transport properties of single-layer and few-layer graphene at large charge doping is of great interest both for the study of the scattering processes dominating the conductivity at different temperatures and in view of the theoretically predicted possibility to reach the superconducting state in such extreme conditions. Here we present the results obtained in 3-, 4- and 5-layer graphene devices down to 3.5 K, where a large surface charge density up to about 6.8x10^14 cm^(-2) has been reached by employing a novel polymer electrolyte solution for the electrochemical gating. In contrast with recent results obtained in single-layer graphene, the temperature dependence of the sheet resistance between 20 K and 280 K shows a low-temperature dominance of a T^2 component - that can be associated with electron-electron scattering - and, at about 100 K, a crossover to the classic electron-phonon regime. Unexpectedly this crossover does not show any dependence on the induced charge density, i.e. on the large tuning of the Fermi energy., Comment: 13 pages, 6 color figures

Abstract

ITER envisages the use of two heating neutral beam injectors plus an optional one as part of the auxiliary heating and current drive system, to reach the desired performances during its various phases of operation. The 16.5 MW expected neutral beam power per injector is several notches higher than worldwide existing facilities. In order to enable such development, a Neutral Beam Test Facility (NBTF) was established at Consorzio RFX, exploiting the synergy of two test beds, called SPIDER and MITICA. SPIDER is dedicated developing and characterizing large efficient negative ion sources at relevant parameters in ITER-like conditions: source and accelerator located in the same vacuum where the beam propagates, immunity to electromagnetic interferences of multiple radio-frequency (RF) antennas, avoidance of RF-induced discharges on the outside of the source. Three years of experiments on SPIDER have addressed to the necessary design modifications to enable full performances. The source is presently under a long shut-down phase to incorporate learnings from the experimental campaign, in particular events/issues occurred during operation, which led to the identification of improvement opportunities/necessities (e.g. RF discharges, local burns, water leaks, other damages, configuration/design upgrades to maximize chances/margin to quest target parameters). Parallelly, developments on MITICA, the full-scale prototype of the ITER Neutral Beam Injector (NBI) featuring a 1 MV accelerator and ion neutralization, are underway including manufacturing of the beam source, accelerator and the beam line components, while power supplies and auxiliary plants, already installed, are under final testing and commissioning. Abstract

The field-effect technique, popular thanks to its application in common field-effect transistors, is here applied to metallic thin films by using as a dielectric a novel polymer electrolyte solution. The maximum injected surface charge, determined by a suitable modification of a classic method of electrochemistry called double-step chronocoulometry, reached some units in 10^15 charges/cm^2. At room temperature, relative variations of resistance up to 8%, 1.9% and 1.6% were observed in the case of gold, silver and copper, respectively and, if the films are thick enough (> 25 nm), results can be nicely explained within a free-electron model with parallel resistive channels. The huge charge injections achieved make this particular field-effect technique very promising for a vast variety of materials such as unconventional superconductors, graphene and 2D-like materials., Comment: 6 pages, 6 figures. Abstract

By using an electrochemical gating technique with a new combination of polymer and electrolyte, we were able to inject surface charge densities n_2D as high as 3.5 \times 10^15 e/cm^2 in gold films and to observe large relative variations in the film resistance, DeltaR/R', up to 10% at low temperature. DeltaR/R' is a linear function of n_2D - as expected within a free-electron model - if the film is thick enough (> 25 nm), otherwise a tendency to saturation due to size effects is observed. The application of this technique to 2D materials will allow extending the field-effect experiments to a range of charge doping where giant conductance modulations and, in some cases, even the occurrence of superconductivity are expected., Comment: 5 pages, 5 figures, RevTeX

Abstract

This work describes the early stage of an interactive and accelerated AI-driven framework for Practical Driving Courses and Driving Licence Exams. The core of the project is an innovative multi-parameter AI-assisted telemetry system able to compute test scores and outcome, useful for human-neutral auditability of Driving Licence Exams. The distributed Artificial Intelligence (AI) system available at the Track Testbed will be able to perform driving behaviour classifications and will suggest specific improvements based on the analysis of vehicle trajectories acquired during the driving test. Finally, the project will target the creation of a large dataset for driving test classification of key performance parameters. The system is envisioned to have a relevant impact on all the certification, driving licence operators and regulator entities.

Abstract

To reach fusion conditions and control the plasma configuration in ITER, the next step in tokamak fusion research, two neutral beam injectors (NBIs) will supply 16.5 MW each, by neutralizing accelerated negative hydrogen or deuterium ions. The requirements of ITER NBIs (40A/1 MeV D- ions for ≤1 h, 46A/870 keV H- ions for ≤1000 s) have never been simultaneously attained. So in the Neutral Beam Test Facility (NBTF, Consorzio RFX, Italy) the operation of the full-scale ITER NBI prototype (MITICA) will be tested and optimised up to full performances, focussing on accelerator (including voltage holding), beam optics, neutralisation, residual ion removal. The NBTF includes also the full-scale prototype of the ITER NBI source with 100 keV particle energy (SPIDER), for early investigation of: negative ion production and extraction, source uniformity, negative ion current density and beam optics. This paper will describe the main results of the first two years of SPIDER operation, devoted to characterizing plasma and beam parameters, including investigation of RF-plasma coupling efficiency and magnetic filter field effectiveness in reducing co-extracted electrons. SPIDER is progressing towards the first caesium injection, which aims at increasing the negative ion density. A major shutdown, planned for 2021, to solve the issues identified during the operation and to carry out programmed modifications, will be outlined. The installation of each MITICA power supply and auxiliary system is completed; in-vessel mechanical components are under procurement by Fusion for Energy (F4E). Integration, commissioning and test of the power supplies, procured by F4E and QST, as the Japanese Domestic Agency (JADA), will be presented. 33. Patient and clinician characteristics and preferences for increasing participation in placebo surgery trials: a scoping review of attributes to inform a discrete choice experiment

Abstract

BACKGROUND: Orthopaedic surgeries include some of the highest volume surgical interventions globally; however, studies have shown that a significant proportion of patients report no clinically meaningful improvement in pain or function after certain procedures. As a result, there is increasing interest in conducting randomised placebo-controlled trials in orthopaedic surgery. However, these frequently fail to reach recruitment targets suggesting a need to improve trial design to encourage participation. The objective of this study was to systematically scope the available evidence on patient and clinician values and preferences which may influence the decision to participate in placebo surgery trial. METHODS: A systematic review was conducted via a literature search in the MEDLINE, Embase, PsycInfo, CINAHL, and EconLit databases as of 19 July 2021, for studies of any design (except commentaries or opinion pieces) based on two key concepts: patient and clinician characteristics, values and preferences, and placebo surgery trials. RESULTS: Of 3424 initial articles, we retained 18 eligible studies. Characteristics, preferences, values, and attitudes of patients (including levels of pain/function, risk/benefit perception, and altruism) and of clinicians (including concerns regarding patient deception associated with placebo, and experience/training in research) influenced their decisions to participate in placebo-controlled trials. Furthermore, some aspects of trial design, including randomisation procedures, availability of the procedure outside of the trial, and the information and consent procedures used, also influenced decisions to participate. CONCLUSION: Participant recruitment is a significant challenge in placebo surgery trials, and individual decisions to participate appear to be sensitive to preferences around treatment. Abstract

BACKGROUND AND OBJECTIVE: Waste in clinical trials remains rife. We developed an economic model to predict the cost of trials based on input costs, duration, power, number of sites, recruitment eligibility and consenting rates. METHODS: We parameterised the model for three proxy placebo-controlled surgical trials using data from a systematic review, a bespoke cost survey, and from the literature. We used the model to compare target and actual trial performance for (i) a trial that was completed on time but with more sites, (ii) a trial that completed after a time extension, and (iii) an incomplete trial. RESULTS: Successful trials more accurately anticipated the true recruitment rate that they achieved and those that overestimated this were most likely to fail. The costs of overestimating recruitment rates were dramatic: all proxy trials had significantly higher costs than planned, with additional funding of at least AUD$600,000 (50% above budget) required for trials that completed after adding more sites or more time, and over AUD$2 million (260% above budget) for incomplete trials. CONCLUSIONS: This model shows the trade-offs between time and cost, or both, when recruitment is lower than anticipated. Greater consideration is needed to improve trial planning, reviewing, and funding of these trials to avoid costly overruns and incomplete trials.

Abstract

Heating neutral beam (HNB) injectors, necessary to achieve burning conditions and to control plasma instabilities in ITER, are characterized by such demanding parameters that a neutral beam test facility (NBTF) dedicated to their development and optimization is being realized in Padua (Italy) with direct contributions from the Italian government (through Consorzio RFX as the host entity) and the ITER international organization (with kind contributions from the ITER domestic agencies of Europe, Japan and India) and technical and scientific support from various European laboratories and universities. The NBTF hosts two experiments: SPIDER, devoted to ion source optimization for the required source performance, and MITICA, the full-size prototype of the ITER HNB, with an ion source identical to SPIDER. This paper gives an overview of the progress towards NBTF realization, with particular emphasis on issues discovered during this phase of activity and on solutions adopted to minimize the impact on the schedule and maintain the goals of the facilities. The realization of MITICA is well advanced; operation is expected to start in 2023 due to the long procurement time of the in-vessel mechanical components. The beam source power supplies, operating at 1 MV, are in an advanced phase of realization; all high-voltage components have been installed and the complex insulation test phase began in 2018. At the same time, construction and installation of SPIDER plant systems was successfully completed with their integration into the facility. The mechanical components of the SPIDER ion source were installed inside the vessel and connected to the plants. Integrated commissioning with the control, protection and safety systems ended positively and the first experimental phase has begun. The first results of the SPIDER experiment, with data from operational diagnostics, and the plans for the 1 MV insulation tests on the MITICA high-voltage components are presented.

Abstract

The SPIDER experiment (Source for the Production of Ions of Deuterium Extracted from a Radio frequency plasma) is a prototype devoted to the heating and diagnostic neutral beam studies in operation at the ITER Neutral Beam Test Facility (NBTF) at Consorzio RFX, Padova. SPIDER is the full-size ITER ion source prototype and the largest negative ion source in operation in the world. In view of ITER heating requirements to realize plasma burning conditions and instabilities control, SPIDER aims at achieving long-time operation (3600 s) with beam energy up to 100 keV, high extracted current density (above 355 A.m-2 for H- and above 285 A.m-2 for D-) at maximum beam source pressure of 0.3 Pa. Moreover, the maximum deviation from uniformity must be kept under 10%.[1] The SPIDER pulse preparation follows a strict procedure of approval of the operation parameters, in view of safety, machine protection and efficiency. In a simplified description, the session leader (SL) defines the parameters according with the best implementation of the science program. The technical responsible (RT) verifies all parameters for approval and only after agreeing with the setup, sends the configuration to the technical operator (OT) to load the configuration into SPIDER instrumentation. The current tools used in SPIDER integrated commissioning and initial SPIDER campaign [2] permit the SL to design a new pulse and program the set of parameters directly into a temporary MDSplus pulse file. This information is then passed to the RT for approval. The same pulse file can be visualised by the RT but there is no indication of what parameters were changed since the previous pulse, or from a pre-set pulse file taken from a previous run. In consequence, the RT must go through a tedious and error prone procedure of checking all the parameters, even if the set of parameters have already been approved for a previous pulse. Moreover, the current set of tools does not foresee an automated load of previously set configurations, except for the possibility of using a command line to load previous setups from an executed or reference pulse. Aiming at the automated implementation of rules and procedures, as well as improving the usability, safety and interoperability between the SL and RT, a new configuration tool for setting the SPIDER pulse parameters is under development, using the integration of two relevant tools in the fusion I&C community: MDSplus and Epics. This contribution will emphasize on (i) the present solution that has been used during initial SPIDER campaigns; (ii) the requirements of the configuration tool according to the set of procedures to be implemented in the SPIDER pulse preparation; (iii) the set of development tools available for implementing the necessary application(s); (iv) the design options and application architecture; (v) the implementation details and preliminary tests of the alpha release application.

Abstract

SPIDER - the ITER full-size beam source built at the Neutral Beam Test Facility (NBTF) in Padova, Italy - has been in operation since June 2018. SPIDER's mission is to optimize the operation of the beam source so that to reuse the SPIDER experience on the full-size prototype of the ITER Neutral Beam Injector, called MITICA, under advanced construction at the NBTF, and in the ITER heating neutral beam injectors. The exploitation of SPIDER started with short, low-performance pulses lasting up to a few seconds and developed to obtain long pulses lasting up to 3000 seconds. Furthermore, the integration of plant and diagnostic systems has grown over time. The size of the amount of data collected and stored per pulse can provide a simple measure of evolution. In fact, it has gone from a few tens of Mbytes in the first campaign pulses to the current maximum value of over 150 Gbytes, most of which produced by infrared and visible cameras. From the first operation onwards, the control systems have also evolved and consolidated, including components and functions, which were not initially foreseen or developed only in a preliminary form. This includes the progressive integration of plant and diagnostic systems and of protection and safety functions. The paper initially focuses on the architecture of the SPIDER control systems that include CODAS, the system delivering conventional control and data acquisition and management, the central interlock system delivering plant protection, and the central safety system delivering people and environment safety. Since all systems have been developed following the guidelines of ITER for the implementation of control systems, the integrated SPIDER control, interlock and safety systems may provide an interesting example for the ITER plant system developers. The paper then describes how the top-down definition and implementation of operating states and operational scenarios provide the framework for the integration of control, interlock and safety systems and the basic element for successful operation. Finally, the paper reports on the lesson learned during these nearly three years of operation with particular attention to the progressive, continuous evolution and recommissioning of systems.

45. The economic and health impact of rare diseases: A meta-analysis

Abstract

Objective Lack of medical and scientific knowledge on rare diseases (RD) often translates into limited research on them and a subsequent lack of understanding of their economic impact. This meta-analysis aims to fill this gap by evaluating the economic impact of RDs and exploring potential factors associated with the societal burden of RD. Methods Studies published between January 2010 and February 2017 were identified by searches in the PubMed platform. Thirty eligible studies were identified for inclusion, and nineteen studies were included in the meta-analysis and outcomes were explored. The cost categories include direct healthcare costs, direct non-healthcare formal costs, and direct non-healthcare informal costs. The patients’ health-related quality of life (QoL) dimensions examined include EQ-5D scores, VAS scores and Barthel index, and the carers’ utility outcomes include EQ-5d scores, VAS scores and Zarit scale. Random effects meta-regression models were used for modelling the impact of study and societal characteristics on cost. Results Across all RDs, mean direct healthcare (DH) costs ($16,513) account for the majority of direct costs (mainly driven by drug costs), followed by mean direct healthcare informal (€15,557) and mean direct healthcare formal (€4,579) costs. Body system affected by the RD, Gross Domestic Product (GDP) per capita and public health expenditure in country of study were the most significant determinants in predicting cost. In regards to QoL outcomes, patients with musculoskeletal diseases seem to have the lowest quality of life across EQ-5D scores, VAS scores and Barthel index. The burden on caregivers seemed to be associated with Autoimmune, followed by Musculoskeletal and Respiratory conditions. Conclusions This meta-analysis highlights the significant burden of RDs on the health care system and explicitly provides evidence for the magnitude of this impact. Abstract

Abstract Background Weight gain during pregnancy that is outside of recommended levels is associated with a range of adverse outcomes for the mother and child, including gestational diabetes, pre-eclampsia, preterm birth, and obesity. Internationally, 60–80% of pregnant women report gaining weight outside of recommended levels. While guideline recommendations and RCT evidence support the provision of antenatal care that supports healthy gestational weight gain, less than 10% of health professionals routinely weigh pregnant women; discuss weight gain, diet, and physical activity; and provide a referral for additional support. This study aims to determine the effectiveness of an implementation intervention in increasing the provision of recommended gestational weight gain care by maternity services. Methods A stepped-wedge controlled trial, with a staggered implementation intervention, will be conducted across maternity services in three health sectors in New South Wales, Australia. The implementation intervention will consist of evidence-based, locally-tailored strategies including guidelines and procedures, reminders and prompts, leadership support, champions, training, and monitoring and feedback. Primary outcome measures will be the proportion of women who report receiving (i) assessment of gestational weight gain; (ii) advice on gestational weight gain, dietary intake, and physical activity; and (iii) offer of referral to a telephone coaching service or local dietetics service. Measurement of outcomes will occur via telephone interviews with a random sample of women who attend antenatal appointments each week. Economic analyses will be undertaken to assess the cost, cost-consequence, cost-effectiveness, and budget impact of the implementation intervention. 47. Placebo Surgery Controlled Trials Do They Achieve What They Set Out To Do? A Systematic Review

Abstract

OBJECTIVE: To explore whether placebo surgery controlled trials achieve what they set out to do by investigating discrepancy between projected and actual design aspects of trials identified through systematic review methods. SUMMARY BACKGROUND: Interest in placebo surgery controlled trials is growing in response to concerns regarding unnecessary surgery and the societal cost of low-value healthcare. As questions about the justifiability of using placebo controls in surgery have been addressed, attention is now being paid to more practical concerns. METHODS: Six databases were searched from inception - May 2020 (MEDLINE, Embase, Emcare, APA PsycInfo, CINAHL, Cochrane Library). Placebo surgery controlled trials with a published protocol were included. Three authors extracted "projected" design aspects from protocols and "actual" design aspects from main findings papers. Absolute and relative difference between projected and actual design aspects were presented for each trial. Trials were grouped according to whether they met their target sample size ("completed") and were concluded in a timely fashion. Pairs of authors assessed risk of bias. RESULTS: Of 24 trials with data available to analyse; 3 were completed and concluded within target timeframe; 10 were completed and concluded outside the target timeline; 4 were completed without clear target timeframes; 2 were incomplete and concluded within the target framework; 5 were incomplete and concluded outside the target timeline. Trials which reached the recruitment target underestimated trial duration by 88% and number of recruitment sites by 87%. CONCLUSIONS: Trialists need to factor additional time and sites into future placebo surgery controlled trials. A robust reporting framework of projected and actual trial design is imperative for trialists to learn from their predecessors. REVIEW REGISTRATION: PROSPERO (CRD42019133296).

50. Responses to COVID-19 in five Latin American countries

Abstract

Background COVID-19 reached Latin-American countries slightly later than European countries, around February/March, allowing some emergency preparedness response in countries characterized by low health system capacities and socioeconomic disparities. Objective This paper focuses on the first months of the pandemic in five Latin American countries: Brazil, Chile, Colombia, Ecuador and Peru. It analyses how the pre-pandemic context, and the government's responses to contain and mitigate the spread together with economic measures have affected the COVID-19 health outcomes. Methods Extensive qualitative document analysis was conducted focused on publicly-available epidemiological data and federal and state/regional policy documents since the beginning of the pandemic. Results The countries were quick to implement stringent COVID-19 measures and incrementally scaled up their health systems capacity, although tracing and tracking have been poor. All five countries have experienced a large number of cases and deaths due to COVID-19. The analysis on the excess deaths also shows that the impact in deaths is far higher than the official numbers reported to date for some countries. Conclusion Despite the introduction of stringent measures of containment and mitigation, and the scale up of health system capacities, pre-pandemic conditions that characterize these countries (high informal employment, and social inequalities) have undermined the effectiveness of the countries’ responses to the pandemic. The economic support measures put in place were found to be too timid for some countries and introduced too late in most of them. Additionally, the lack of a comprehensive strategy for testing and tracking has also contributed to the failure to contain the spread of the virus.
