Microplastic (MP) contamination is an urgent environmental issue to address. Fibrous microplastics (FMPs) are the principal MP type in the air and have already been found in human stool and lung tissues. FMPs are generated from the lifecycle of synthetic and blended textiles and are expected to increase due to fast fashion. Among textile processes, the finishing of fabrics is estimated to generate 5000 t/year of textile waste fibers in Italy, including FMPs. To limit FMPs spread, this paper suggests, for the first time, the direct collection of blended finishing textile waste microfibers and reuse in designing thermal-insulating and mechanical-performing fiber-reinforced cementitious composites (FRCs). The microfibers were thoroughly characterized (size, morphology, composition, and density), and their use in FRCs was additionally evaluated by considering water absorption and release capacity. Untreated, water-saturated, and NaOH-treated microfibers were considered in FRCs up to 4 wt%. Up to a +320% maximum bending load, +715% toughness, −80% linear shrinkage, and double-insulating power of Portland cement were observed by increasing microfiber contents. NaOH-treated and water-saturated microfibers better enhanced toughness and linear shrinkage reduction. Therefore, green and performant composite construction materials were obtained, allowing for the mitigation of more than 4 kg FMPs per ton of cement paste. This is a great result considering the FMP contamination (i.e., 2–8 kg/day fallout in Paris), and that FRCs are promising and shortly-widely used construction materials.

Microplastics (MPs) are plastic particles of ≤5 mm with a ubiquitous presence in aquatic ecosystems. MPs harm marine life and can cause severe health problems for humans. Advanced oxidation processes (AOPs) that involve the in-situ generation of highly oxidant hydroxyl radicals can be an alternative to fight MPs pollution. Of all the AOPs, photocatalysis has been proven a clean technology to overcome microplastic pollution. In this work, we propose novel C,N-TiO2/SiO2 photocatalysts with proper visible-active properties to degrade polyethylene terephthalate (PET) MPs. Photocatalysis was performed in an aqueous medium and at room temperature, evaluating the influence of two pH values (pH 6 and 8). The obtained results demonstrated the degradation of the PET MPs by C,N-TiO2/SiO2 semiconductors and achieved mass loss between 9.35 and 16.22%, carbonyl indexes up to 28.7% lower, and a significant decrease in crystallinity compared to the original PET MPs.

We assessed the causes of polyserositis in pigs, categorized by causative agents and ages of animals affected. In a 3-y study, 246 pigs from 80 different farms with recurrent problems of polyserositis, in a high-density breeding area, were submitted for autopsy; 154 pigs with typical fibrinous serosal lesions were sampled for further bacterial and viral investigation. The most common gross lesions were pleuritis and pericarditis (141 of 154; 92%). The animals most affected were weaned pigs (139 of 154; 90%). Haemophilus parasuis and Mycoplasma hyorhinis were the most common bacteria detected and were present at the same rate (85 of 154; 55%). Other bacteria isolated were Streptococcus sp. (44 of 154; 29%), Pasteurella multocida (21 of 154; 14%), Escherichia coli (19 of 154; 12%), Actinobacillus pleuropneumoniae (7 of 154; 5%), and Trueperella pyogenes (4 of 154; 3%). Porcine reproductive and respiratory syndrome virus (PRRSV; 119 of 154; 77%) predominated among the viruses detected, followed, with lesser prevalence, by porcine circovirus 2 (40 of 154; 26%) and swine influenza A virus (19 of 154; 12%). Bacterial coinfection and coinfection of bacteria and viruses were common (128 of 154; 83%). A strong positive correlation was found between coinfection by H. parasuis and M. hyorhinis and also by H. parasuis with PRRSV.

We assessed the bacterial agents found in 8–12-wk-old post-weaning pigs with arthritis. The bodies of 178 post-weaning pigs from 90 farms (average of 2 pigs/farm) with recurrent problems of lameness and swollen joints in a high-density breeding area were submitted for autopsy and sampled for further bacterial investigation. The most common articular gross lesions and histopathologic findings were serofibrinous (95 of 178; 53%) or serous (65 of 178; 37%) arthritis; suppurative lesions were less frequent (18 of 178; 10%). In 133 of 178 (74.7%) cases, a bacterial agent was detected in joints. Mycoplasma hyorhinis was the most common bacterium detected (82 of 133; 61.6%). Haemophilus parasuis and Streptococcus spp. were observed in 27 of 133 (20.3%) and 24 of 133 (18.0%) cases, respectively. Other bacteria in the 113 cases, considered less important, in order of their low frequency, were Mycoplasma spp. (13; 9.8%), Trueperella pyogenes (11; 8.2%), Mycoplasma hyosynoviae (4; 3.0%), Staphylococcus spp. (3; 2.2%), Escherichia coli (2; 1.5%), and Actinobacillus spp. (2; 1.5%). Our results highlight the primary role of M. hyorhinis compared to other microorganisms involved in young pigs with arthritis.

Optical aberrations affect the quality of light propagating through a turbid medium, where refractive index is spatially inhomogeneous. In multiphoton optical applications, such as two-photon excitation fluorescence imaging and optogenetics, aberrations non-linearly impair the efficiency of excitation. We demonstrate a sensorless adaptive optics technique to compensate aberrations in holograms projected into turbid media. We use a spatial light modulator to project custom three dimensional holographic patterns and to correct for local (anisoplanatic) distortions. The method is tested on both synthetic and biological samples to counteract aberrations arising respectively from misalignment of the optical system and from samples inhomogeneities. In both cases the anisoplanatic correction improves the intensity of the stimulation pattern at least two-fold.

We propose a sensor-less adaptive optics approach to correct local aberrations in holograms used for two-photon stimulation. Our method showed intensity enhancement of about 60 % in holograms projected into fixed zebrafish tissue.

In this Letter, we present a solution for simple implementation of adaptive optics in any existing laser scanning fluorescence microscope. Adaptive optics are implemented by the introduction of a multiactuator adaptive lens between the microscope body and the objective lens. Correction is performed with a sensorless method by optimizing the quality of the images presented on screen by the microscope software. We present the results acquired on both a commercial linear excitation confocal microscope and a custom-made multiphoton excitation microscope.

The virus responsible for the pandemic that has affected 152 countries worldwide is a new strain of coronavirus (CoV), which belongs to a family of viruses widespread in many animal species, including birds, and mammals including humans. Indeed, CoVs are known in veterinary medicine affecting several species, and causing respiratory and/or enteric, systemic diseases and reproductive disease in poultry. Animal diseases caused by CoV may be considered from the following different perspectives: livestock and poultry CoVs cause mainly “population disease”; while in companion animals they are a source of mainly “individual/single subject disease”. Therefore, respiratory CoV diseases in high-density, large populations of livestock or poultry may be a suitable example for the current SARS-CoV-2/COVID-19 pandemic. In this review we describe some strategies applied in veterinary medicine to control CoV and discuss if they may help to develop practical and useful strategies to control the SARS-CoV-2/COVID-19 pandemic.

Background: The work wants to see if the Lung Sonography (LUS) can be helpful in the evaluation of a new device before and after a respiratory treatment. Aim: Simeox is a new device for airway clearway. The patient activates it using a button during the expiration phase that creates a negative vibratory suction of small volumes of air.The action is directed to the mucus rheology, improves the drainage from the periphery of the lung. Methods: 8 patients (4 female and 4 male), were examined by comparing LUS with traditional parameters such as SpO2 , FC, scales before and after a week’s use of the Simeox device and gym exercises. The ecography machine was set in B-mode with a convex probe oriented horizontally to have a greater view of the pleural line in the four Volpicelli’s zones. We took note on a map of number of the vertical artifacts B lines starting from the pleural line with a score from 0 to 4. Results: Lus and all parameters have improved but some patients, little compliant, were unable to tell if the amount of secretions had changed. The usual exams give the state of health but not the specific state of the lung such as LUS that represents a third eye, inner, in addition to the clinic exam. Conclusion: Our study suggests that LUS is a patient bedside exam modality, safe, reliable, fast to evaluate the alveolar drainage. Further studies are needed to understand the interaction of LUS with the respiratory treatments.

Objectives This study investigates the electrocardiographic-electrophysiological effects of administration of anesthetic drugs for general anesthesia (GA) in patients with BrS at high risk of sudden cardiac death (SCD). Background The safety of anesthetic agents in Brugada syndrome (BrS) is under debate. Methods All consecutive patients with spontaneous type 1 BrS electrocardiographic (ECG) patterns undergoing epicardial ablation of the arrhythmogenic substrate (AS) under GA were enrolled. Anesthesia was induced with single bolus of propofol and maintained with sevofluorane. ECG measurements were collected before, immediately after, and 20 min after induction of GA. Three-dimensional maps during GA and after ajmaline indicated the epicardial AS before ablation. Results Thirty-six patients with BrS (32 male, 88.9%; mean age 38.8 ± 12.0 years) with a spontaneous type 1 ECG pattern underwent GA. Induction was performed using propofol at mean dose of 1.6 to 2.6 mg/kg (2.1 ± 0.3 mg/kg). Twenty-eight (28 of 36, 77.8%) patients showed a reversion to a nondiagnostic pattern. ST-segment elevation (0.32 ± 0.01 mV vs. 0.19 ± 0.02 mV, p 2 vs. 20.3 ± 0.8 cm 2 ). No patient developed malignant arrhythmias during GA induction and maintenance. Conclusions This study shows that GA using single-bolus propofol and volatile anesthetics is safe in high-risk patients with BrS, and it may exert a modulating effect by reducing the manifestation of type 1 BrS pattern and AS in the form of epicardial abnormal ECGs. Polymer concrete (PC) is a composite construction material that boasts several advantages, such as lightness, low water permeability, high resistance to corrosive environments, and chemical degradation. Consequently, it has recently attracted interest as an alternative material to the traditional ones for several civil applications. In this study, unsaturated polyester resin was considered the matrix phase of PC. Aimed to produce green PC, the commonly dispersed phase of natural aggregate was totally replaced by recycled glass aggregate (RGA) deriving from cathode ray tube (CRT) glass waste. Fine and coarse fractions of non-hazardous CRT glass were considered in different ratios. Chemical and physical analyses were carried out through XRF, particle size distribution and microstructural analysis to characterize RGA. The influence of RGA particle size and percentage on PC performance was investigated by microstructural analysis and aggregate packing, chemical resistance, water absorption, and mechanical analyses, such as bending, impact, and scratch test. Using solely the coarse fraction of RGA led to the manufacturing of a green PC with similar performance to the traditional PC and in addition lower in density. The PC quality mainly depended on the matrix crosslinking which, for PC containing fine RGA, was promoted by adding 4 wt% of silane coupling agent.

Inhomogeneities in the refractive index of a biological microscopy sample can introduce phase aberrations, severely impairing the quality of images. Adaptive optics can be employed to correct for phase aberrations and improve image quality. However, conventional adaptive optics can only correct a single phase aberration for the whole field of view (isoplanatic correction) while, due to the highly heterogeneous nature of biological tissues, the sample induced aberrations in microscopy often vary throughout the field of view (anisoplanatic aberration), limiting significantly the effectiveness of adaptive optics. This paper reports on a new approach for aberration correction in laser scanning confocal microscopy, in which a spatial light modulator is used to generate multiple excitation points in the sample to simultaneously scan different portions of the field of view with completely independent correction, achieving anisoplanatic compensation of sample induced aberrations, in a significantly shorter time compared to sequential isoplanatic correction of multiple image subregions. The method was tested in whole Drosophila brains and in larval Zebrafish, each showing a dramatic improvement in resolution and sharpness when compared to conventional isoplanatic adaptive optics.

Background: The relationship between the typical electrocardiographic pattern and electromechanical abnormalities has never been systematically explored in Brugada syndrome (BrS). Objectives: The aims of this study were to characterize the electromechanical substrate in patients with BrS and to evaluate the relationship between electrical and mechanical abnormalities. Methods: We enrolled 50 consecutive high-risk patients with BrS (mean age 42 ± 7.2 years), with implantable cardioverter-defibrillator implantation for primary or secondary prevention of ventricular tachyarrhythmias (ventricular tachycardia/ventricular fibrillation [VT/VF]), undergoing substrate mapping and ablation. Patients underwent 3-dimensional (3D) echocardiography with 3D wall motion/deformation quantification and electroanatomic mapping before and after ajmaline administration (1 mg/kg in 5 minutes); 3D mechanical changes were compared with 50 age- and sex-matched controls. The effect of substrate ablation on electromechanical abnormalities was also assessed. Char coming from gasification of biomasses was mixed with red clay to create both lightweight aggregates (LWAs) for green roofs applications and bricks and with polyoils and other reactants to obtain polyurethane panels for insulating purpose. Application in LWAs by substituting the clay with char leads to a weight-lightening of the material. To optimize LWAs pH, spent coffee grounds (SCG) were added. A greater decrease in weight and pH values in the neutrality range were observed. Adding biochar on bricks led to a significant reduction of materials bulk density (from 2 to 1.5g/cm3). Polyurethane panels show a decrease of thermal conductivity by increasing the content of char. The addition of char increases the maximum applicable load related to stress relaxation tests and improves the recovery of the load after 10 minutes. Higher elastic modulus values are seen after compressive tests. Electrical conductivity increases in samples containing char.

Hyaluronic Acid (HA), or hyaluronan, is a polysaccharide composed of a repeated disaccharide unit of β-1,4-D-glucuronic acid - β-1,3-N-acetyl-D-glucosamine linked by glycosidic bonds.

Reproductive failure in sows is one of the most important factors affecting pig breeding. Many reproductive disorders are linked to both environmental factors and infectious agents. The goal of our study was to determine the presence of pathogens that are known to cause abortion, considering a set of conditioning factors, such as seasonality and pregnancy period. A large number of aborted fetuses (1,625 fetuses from 140 farms) from a high-density breeding area in northern Italy was analyzed for a period of 3 years. The pigs were diagnosed based on direct (culture, PCR) or indirect (enzyme-linked immunosorbent assay) evidence. An infectious etiologic agent was found in 323 of 549 cases of abortion (58.8%). These included viral agents ( Porcine circovirus-2, 138/323; Porcine reproductive and respiratory syndrome virus, 108/323; porcine parvovirus, 20/323; pseudorabies virus, 6/323; and Encephalomyocarditis virus, 3/323) and bacteria ( Escherichia coli, 64/323; Streptococcus sp., 63/323; Staphylococcus sp., 5/323; Pasteurella sp., 3/323; Shigella sp., 1/323; and Yersinia sp., 1/323). This study describes the prevalence of infectious agents involved in reproductive failure in a high-density swine population. The data can be useful to swine breeders, practitioners, and medical specialists in monitoring animal health and in supervising the breeding process.

We report the case of a 68-year-old man who presented with ataxia, insomnia, rapidly developing cognitive decline, seizures and small vessel vasculitis. Both serum and cerebro-spinal fluid samples showed positive titre of anti-CASPR2 antibodies. Limbic encephalitis was diagnosed and immunomodulatory therapy was started with benefit. After one-year follow-up, the patient relapsed with a difficult-to-treat respiratory failure, brainstem involvement, neuropathic pain and severe dysautonomia with esophageal dysfunction. We discuss here the occurrence of life-threating complication such as respiratory dysfunction in CASPR2 limbic encephalitis. Furthermore, we showed different phenotype and treatment response during disease onset compared to relapse. This case expands the clinical spectrum of anti-CASPR2 associated disease, underlying the need for respiratory and sleep evaluation.

Data on the prevalence of major contagious pathogens in bulk tank milk (BTM) in Italy are generally not available. The availability of Real-Time PCR procedures (qPCR) to perform BTM analysis by represents an important step to define herd health status. Therefore, a cross-sectional epidemiological study was designed to assess the prevalence of contagious pathogens and Prototheca spp in BTM samples. The study was performed on 581 herds from four districts in the west Lombardy region of Italy. Additionally, the relationship between pathogens in BTM and SCC or milk yield; the presence of an association between four risk factors (district, herd size, average milk yield and SCC) with pathogens in BTM were assessed. The overall data showed that S. aureus was recovered in 42% of the herds, Str. agalactiae in 10%, Prototheca spp in 11% and M. bovis in 1.5% of the herds. The GLM model applied showed a significant influence of BTM results, district, herd size and their interactions on SCC and on milk yield variance. Particularly, S. aureus or Str. agalactiae have a significant effect on milk yield variability and, in a lesser extent, on SCC. The very high prevalence of contagious pathogens significantly affects milk characteristics and yield, thus affecting economic sustainability of the herds, and suggests the need to implement control programmes to decrease the prevalence of contagious pathogens, This will also allow to decrease the use of antimicrobials and to improve cow welfare.HighlightsFirst study on a large sample of Italian dairy herds on the prevalence of contagious pathogens in bulk tank milk samples. Introduction The zoonotic hepatitis E virus (HEV) genotype 3 (HEV-G3) has become a common cause of acute and chronic hepatitis among humans worldwide. In Israel, while HEV-3 sequences have previously been detected in sewage, only the non-zoonotic HEV-G1 genotype has been found in samples from human patients. Aim In this pilot study, we aimed to assess the status of HEV in a sample of the swine population and among swine farm workers in Israel. Methods Pig blood (n = 141) and faecal samples (n = 39), pig farm sewage samples (n = 8) and blood from farm workers (n = 24) were collected between February 2016 and October 2017. Anti-HEV IgG was detected using the Wantai assay. HEV RNA was analysed with the RealStar HEV kit. HEV open reading frame 1 fragments amplified from representative HEV RNA-positive samples were used for phylogenetic analysis. Results Overall prevalence of HEV antibodies in pigs was 75.9% (107/141). HEV RNA was detected in plasma (2.1%, 3/141), faecal (22.8%, 18/79) and pig sewage (4/8) samples. Pig and sewage-derived viral sequences clustered with previously identified human sewage HEV-G3 sequences. Most pig farms workers (23 of 24) were HEV-seropositive; none was viraemic or reported previous clinical signs. Conclusions This study showed that domestic pigs in Israel are infected with HEV-G3. The high HEV seropositivity of the farm workers together with the previous identification of this virus in human sewage suggests circulation to humans. The clinical impact of these findings on public health should be further explored.

The use of spatial light modulators to project computer generated holograms is a common strategy for optogenetic stimulation of multiple structures of interest within a three-dimensional volume. A common requirement when addressing multiple targets sparsely distributed in three dimensions is the generation of a points cloud, focusing excitation light in multiple diffraction-limited locations throughout the sample. Calculation of this type of holograms is most commonly performed with either the high-speed, low-performance random superposition algorithm, or the low-speed, high performance Gerchberg&ndash, Saxton algorithm. This paper presents a variation of the Gerchberg&ndash, Saxton algorithm that, by only performing iterations on a subset of the data, according to compressive sensing principles, is rendered significantly faster while maintaining high quality outputs. The algorithm is presented in high-efficiency and high-uniformity variants. All source code for the method implementation is available as Supplementary Materials and as open-source software. The method was tested computationally against existing algorithms, and the results were confirmed experimentally on a custom setup for in-vivo multiphoton optogenetics. The results clearly show that the proposed method can achieve computational speed performances close to the random superposition algorithm, while retaining the high performance of the Gerchberg&ndash, Saxton algorithm, with a minimal hologram quality loss.

Three-dimensional microscopy suffers from sample-induced aberrations that reduce the resolution and lead to misinterpretations of the object distribution. In this paper, the resolution of a three-dimensional fluorescent microscope is significantly improved by introducing an amplitude diversity in the form of a binary amplitude mask positioned in several di erent orientations within the pupil, followed by computer processing of the diversity images. The method has proved to be fast, easy to implement, and cost-e ective in high-resolution imaging of casper fli:GFP zebrafish.

Background Oral mucositis (OM)-related pain affects most patients with head and neck cancer during treatments, but its management is not standardized. Methods We retrospectively collected data about the opioid therapy used for OM-induced pain in all patients with oropharyngeal cancer treated with chemoradiotherapy (CRT) between 2009 and 2013. To compare the different opioids, a conversion into oral morphine equivalent daily dose (OMEDD) was performed. The highest OMEDD (h-OMEDD) and the opioids' weekly increase were associated with patient, tumor, or treatment-related characteristics in order to identify predictive factors of opioid consumption. Results Ninety-seven percent of patients received opioids. The h-OMEDD was significantly correlated with a higher OM-grade and a lower smoking history. The weekly opioids' increase was higher in patients with lower smoking history and human papillomavirus (HPV) positivity. Conclusion Opioid therapy remains the mainstay for OM-related pain management during CRT. The role of previous smoking and HPV on opioid use needs further confirmations. © 2016 Wiley Periodicals, Inc. Head Neck, 2016

In this report we describe for the first time the detection of Sarcoptes scabiei type suis mites on domestic pigs in Israel and examine its genetic variation compared with S. sabiei from other hosts. Microscopic examination of skin samples from S. scabiei-infested pigs (Sus scrofa domesticus) revealed all developmental stages of S. scabiei. To detect genetic differences between S. scabiei from different hosts, samples obtained from pig, rabbits (Orictolagus cuniculus), fox (Vulpes vulpes), jackal (Canis aureus) and hedgehog (Erinaceus concolor) were compared with GenBank-annotated sequences of three genetic markers. Segments from the following genes were examined: cytochrome C oxidase subunit 1 (COX1), glutathione-S-transferase 1 (GST1), and voltage-sensitive sodium channel (VSSC). COX1 analysis did not show correlation between host preference and genetic identity. However, GST1 and VSSC had a higher percentage of identical sites within S. scabiei type suis sequences, compared with samples from other hosts. Taking into account the limited numbers of GST1 and VSSC sequences available for comparison, this high similarity between sequences of geographically-distant, but host-related populations, may suggest that different host preference is at least partially correlated with genetic differences. This finding may help in future studies of the factors that drive host preferences in this parasite.

Background: Guidelines recommend the use of implanted cardioverter-defibrillators in patients with Brugada syndrome and induced ventricular tachyarrhythmias, but there is no evidence supporting it. Objectives: This prospective registry study was designed to explore clinical and electrophysiological predictors of malignant ventricular tachyarrhythmia inducibility in Brugada syndrome. Methods: A total of 191 consecutive selected patients with (group 1; n = 88) and without (group 2; n = 103) Brugada syndrome–related symptoms were prospectively enrolled in the registry. Patients underwent electrophysiological study and substrate mapping or ablation before and after ajmaline testing (1 mg/kg/5 min). Results: Overall, before ajmaline testing, 53.4% of patients had ventricular tachyarrhythmia inducibility, which was more frequent in group 1 (65.9%) than in group 2 (42.7%; p < 0.001). Regardless of clinical presentation, larger substrates with more fragmented long-duration ventricular potentials were found in patients with inducible arrhythmias than in patients without inducible arrhythmias (p < 0.001). One extrastimulus was used in more extensive substrates (median 13 cm2; p < 0.001), and ventricular fibrillation was the more frequently induced rhythm (p < 0.001). After ajmaline, patients without arrhythmia inducibility had arrhythmia inducibility without a difference in substrate characteristics between the 2 groups. The substrate size was the only independent predictor of inducibility (odds ratio: 4.51; 95% confidence interval: 2.51 to 8.09; p < 0.001). A substrate size of 4 cm2 best identified patients with inducible arrhythmias (area under the curve: 0.98; p < 0.001). Substrate ablation prevented ventricular tachyarrhythmia reinducibility. Conclusions: In Brugada syndrome dynamic substrate variability represents the pathophysiological basis of lethal ventricular tachyarrhythmias. Substrate size is independently associated with arrhythmia inducibility, and its determination after ajmaline identifies high-risk patients missed by clinical criteria. Substrate ablation is associated with electrocardiogram normalization and not arrhythmia reinducibility. We report on a universal sample-independent sensorless adaptive optics method, based on modal optimization of the second moment of the fluorescence emission from a point-like excitation. Our method employs a sample-independent precalibration, performed only once for the particular system, to establish the direct relation between the image quality and the aberration. The method is potentially applicable to any form of microscopy with epifluorescence detection, including the practically important case of incoherent fluorescence emission from a three dimensional object, through minor hardware modifications. We have applied the technique successfully to a widefield epifluorescence microscope and to a multiaperture confocal microscope.

Aims and Background The time of lung cancer screening may provide the ideal setting to discuss and initiate a smoking cessation plan that includes pharmacologic aids. No studies to date have fully investigated the potential effectiveness of such combined approach. Methods We prospectively evaluated the biochemically verified 1-year continuous abstinence rate from smoking of 187 persistent smokers enrolled within the Multicentric Italian Lung Detection Trial (MILD), who received a pharmacologic aid to quit smoking with varenicline along with behavioral counseling. The propensity of study subjects to succeed in smoking cessation was also monitored. Results At 12 months, the continuous abstinence rate from smoking was achieved in 37 out of 187 patients (19.8%), with a propensity to succeed in smoking cessation for the assisted attempt equal to 1.43, as compared to an unassisted MILD patient. At the end of the third month of therapy, 48.7% of subjects showed a continuous abstinence rate, while only 33.7% of patients were abstinent from smoking at 6 months. At baseline, the subgroup of MILD participants who were originally allocated to lung tomography showed higher smoking intensity than those allocated to no screening. Conclusions A combined smoking cessation intervention can be implemented with satisfactory results within a lung cancer screening program; this preliminary observation needs to be replicated in a prospective investigation. Clinicians should consider that lung cancer screening may be falsely reassuring for persistent smokers; therefore it should always be coupled with a smoking cessation program.

This study investigates the electrocardiographic-electrophysiological effects of administration of anesthetic drugs for general anesthesia (GA) in patients with BrS at high risk of sudden cardiac death (SCD).The safety of anesthetic agents in Brugada syndrome (BrS) is under debate.All consecutive patients with spontaneous type 1 BrS electrocardiographic (ECG) patterns undergoing epicardial ablation of the arrhythmogenic substrate (AS) under GA were enrolled. Anesthesia was induced with single bolus of propofol and maintained with sevofluorane. ECG measurements were collected before, immediately after, and 20 min after induction of GA. Three-dimensional maps during GA and after ajmaline indicated the epicardial AS before ablation.Thirty-six patients with BrS (32 male, 88.9%; mean age 38.8 ± 12.0 years) with a spontaneous type 1 ECG pattern underwent GA. Induction was performed using propofol at mean dose of 1.6 to 2.6 mg/kg (2.1 ± 0.3 mg/kg). Twenty-eight (28 of 36, 77.8%) patients showed a reversion to a nondiagnostic pattern. ST-segment elevation (0.32 ± 0.01 mV vs. 0.19 ± 0.02 mV; p 0.001) and J-wave amplitude (0.47 ± 0.02 mV vs. 0.31 ± 0.03 mV; p 0.001) decreased after propofol. The AS area during GA, in the absence of BrS pattern, significantly enlarged after administration of ajmaline (3.6 ± 0.5 cmThis study shows that GA using single-bolus propofol and volatile anesthetics is safe in high-risk patients with BrS, and it may exert a modulating effect by reducing the manifestation of type 1 BrS pattern and AS in the form of epicardial abnormal ECGs. Aim: The purpose of this retroactive study was to assess the behavior of diaphragmatic excursion before and after a rehabilitation program in pulmonary diseases. Methods: 234 patients in 2014-15 (COPD=112, OSA=65, neuromuscolar=17, surgical=4, fibrotorax=7, bronchiectasis=7, asthma=5, pneumonia=6, emphysema=4, embolism=7) were investigated with sonographic measurements of the diaphragm motion and compared with 6-minute walk test. The ultrasound machine was a portable one with a 1.3-4 MHz sector phased array transducer with 14x24 mm footprint. The sonography was performed by the same RT therapist at bedside with patient in semirecumber position in anterior subcostal approach. All patients followed a rehabilitation program (the daily average of treatments was of 14 days), based on exercises of mobilization and breathing, retraining through calisthenics exercises, theraband, minibike or cyclette. Results: US diaphragm improved were 76%; WT6 improved 57%; US stationary 20%; WT6 stationary 38%; US worse 4%; WT6 worse 5%. The problems have occurred with critically ill patients for which the wt6 was not effective. Conclusion: Our study suggests that Ultrasound is a safe, reliable modality to set measurement of the diaphragmatic excursion. It can be a strong addition to lung function parameters to control the effects of rehabilitation programs and respiratory muscle training in the evaluation of ventilatory muscle strength.

Consumption of e-cigarettes has increased significantly in recent years, necessitating further research on the potential health risks/benefits of these devices. Recently, Philip Morris International entered the e-cigarette market with the launch of “iQOS”, a “heat-not-burn” cigarette that has been described as a hybrid between traditional and electronic cigarettes. In a recent study (Saffari et al. 2014, Environ. Sci. Process Impacts, 2259-2267), we presented exposure to particulate metals and organic compounds from e-cigarettes. In this study, we have used a similar approach to further expand our previous analyses by characterizing the iQOS. Black carbon (BC) in addition to particle mass and number concentration were measured using continuous monitors, inside of an office environment during e-cigarette and iQOS consumption sessions. Additionally, particle-bound metals and organic compounds as well as gas-phase aldehydes were quantified using time-integrated chemical analysis methods. Moreover, indoor emission rate of these chemical species were quantified by conducting parallel outdoor measurements followed by development of an indoor mass balance model. Our results indicated a substantial decrease in the emission of metals (with the exception of Ni and Cr), BC and organic compounds in e-cigarettes compared to traditional cigarettes. Analysis of the same species in the iQOS side-stream smoke, however, indicated that while PAHs were mostly non-detected in the iQOS smoke, certain n-alkanes, organic acids were still emitted in substantial levels from iQOS.
