How do attentional networks influence conscious perception? To answer this question, we used magnetoencephalography (MEG) in human participants, and assessed the effects of spatially nonpredictive or predictive supra-threshold peripheral cues on the conscious perception of near-threshold Gabors. Three main results emerged. (1) As compared with invalid cues, both nonpredictive and predictive valid cues increased conscious detection. Yet, only predictive cues shifted the response criterion towards a more liberal decision (i.e., willingness to report the presence of a target under conditions of greater perceptual uncertainty) and affected target contrast leading to 50% detections. (2) Conscious perception following valid predictive cues was associated to enhanced activity in frontoparietal networks. These responses were lateralized to the left hemisphere during attentional orienting, and to the right hemisphere during target processing. The involvement of frontoparietal networks occurred earlier in valid than in invalid trials, a possible neural marker of the cost of re-orienting attention. (3) When detected targets were preceded by invalid predictive cues, and thus reorienting to the target was required, neural responses occurred in left hemisphere temporo-occipital regions during attentional orienting, and in right hemisphere anterior insular and temporo-occipital regions during target processing. These results confirm and specify the role of frontoparietal networks in modulating conscious processing, and detail how invalid orienting of spatial attention disrupts conscious processing.Significance StatementDo we need to pay attention to external objects in order to become aware of them? Characterizing the spatiotemporal dynamics of attentional effects on visual perception is critical to understand how humans process and select relevant information. Participants detected near-threshold visual targets preceded by supra-threshold spatial cues with varying degrees of predictivity, while their brain activity was recorded using magnetoencephalography. Results demonstrated that valid predictive cues biased participants’ conscious perception through an early recruitment of frontoparietal regions, and that attentional costs associated to invalid predictive cues were related to activation of the right hemisphere ventral network. This work characterizes the neural dynamics associated with the cost of attentional reorienting on conscious processing.

How do individual variations in the vividness of visual mental imagery impact different imagery domains, such as object shape, color, written words, faces, and spatial relationships? To address this question, we conducted a study with 117 healthy participants who reported different levels of imagery vividness, to examine how individual variations in the vividness of visual mental imagery affect different imagery domains, such as object shape, color, written words, faces, and spatial relationships. Of these participants, 44 reported experiencing absent or nearly absent visual imagery, a condition known as "aphantasia". These individuals were compared to those with typical (N = 42) or unusually vivid (N = 31) imagery ability. We used an online version of the French-language Battérie Imagination-Perception (eBIP), which consists of tasks tapping each of the above-mentioned domains in visual imagery and in visual perception. We recorded the accuracy and response times (RTs) of participants' responses. Bayesian analyses showed that there was a very high level of confidence that aphantasic participants had similar accuracy on all tasks compared to the other groups (BF = 0.02). However, their RTs were slower in both imagery and perceptual tasks (BF = 266), and they had lower confidence in their responses on perceptual tasks (BF = 7.78e5). A Bayesian regression analysis revealed that there was an inverse correlation between subjective vividness and RTs for the entire participant group. The pattern was similar in all the explored domains. The findings suggest that individuals with congenital aphantasia experience a slowing in processing visual information in both imagery and perception, but the precision of their processing remains unaffected. The observed performance pattern lends support to the ideas that congenital aphantasia is primarily a deficit of phenomenal consciousness, or that it employs alternative strategies other than visualization to access preserved visual information.

The ventral temporal cortex hosts key regions for the high-level visual processing of object shape and color. These areas represent nodes of large-scale neural circuits dedicated to object recognition. In the language-dominant hemisphere, some of these regions communicate with the language systems; by assigning verbal labels to percepts, these circuits speedup stimulus categorization, and permit fast and accurate interindividual communication. By impairing the functioning of these circuits, neurological damage may provoke disabling disorders of the processing of visual objects and of their colors. Brain damage of vascular, degenerative, toxic, or traumatic origin can induce deficits at different levels of visual processing, from the building of shape- or wavelength-invariant percepts, to their connections with semantic knowledge and with the appropriate lexical entry. After an overview of the neuroimaging of domain-preferring regions for object shape and color in the ventral temporal cortex, this chapter reviews evidence from historical and recent cases of acquired visual agnosia and color processing deficits. A recurrent motif emerging from patients' patterns of performance and lesion locations is the existence of caudo-rostral gradients in the ventral occipito-temporal cortex, spanning from more perceptual to more cognitive stages of processing.

Classical theoretical models suggest that visual short-term memory can be divided in two main memory systems: sensory memory, a short-lasting but high-capacity memory storage and working memory, a long-lasting but low-capacity memory store. Whilst, previous research has systematically shown a strong interplay between attentional mechanisms and working memory, less clear is the role of attention in sensory memory. In the present study we approach this issue by asking whether withdrawing attentional resources by a dual task (Experiment 1) or by presenting task irrelevant information during memory maintenance (Experiment 2 and 3) similarly or differently affect sensory and working memory. Overall, results showed that sensory memory content was undermined not only by a simultaneous high-demanding cognitive task but even when purely task-irrelevant and non-masking visual distractors were presented during maintenance. Our data provide support against theories that consider sensory memories as a case of visual awareness free of attention.

Spatial neglect usually concerns left-sided events after right hemisphere damage. Its anatomical correlates are debated, with evidence suggesting an important role for fronto-parietal white matter disconnections in the right hemisphere. Here we describe the less frequent occurrence of neglect for right-sided events, observed in three right-handed patients after a focal stroke in the left hemisphere. Patients were tested one month and three months after stroke. They performed a standardised paper-and-pencil neglect battery and underwent brain MRI with both structural and Diffusion Tensor (DT) sequences, in order to assess both grey matter and white matter tracts metrics. Lesions were manually reconstructed for each patient. Patients presented signs of mild right-sided neglect during visual search and line bisection, as well as pathological performance in real-life situations. Structural MRI demonstrated left parietal strokes in two patients, in the region extending from the postcentral gyrus to the temporo-parietal junction. One of these two patients also had had a previous occipital stroke. The remaining patient had a left frontal stroke, affecting the precentral, the postcentral gyri and the basal ganglia. DT MRI tractography showed disconnections in the fronto-parietal regions, concerning principally the superior longitudinal fasciculus (SLF). These results suggest an important role for left SLF disconnection in right-side neglect, which complements analogous evidence for right SLF disconnection in left-side neglect.

Most of us can use their “mind’s eye” to mentally visualize things that are not in their direct line of sight, an ability known as visual mental imagery. Extensive left temporal damage can impair patients’ visual mental imagery experience, but the critical locus of lesion is unknown. Our recent meta-analysis of 27 fMRI studies of visual mental imagery highlighted a well-delimited region in the left lateral midfusiform gyrus, which was consistently activated during visual mental imagery, and which we called the Fusiform Imagery Node (FIN). Here we describe the connectional anatomy of FIN in neurotypical participants and in RDS, a right-handed patient with an extensive occipitotemporal stroke in the left hemisphere. The stroke provoked right homonymous hemianopia, alexia without agraphia, and color anomia. Despite these deficits, RDS had normal subjective experience of visual mental imagery and reasonably preserved behavioral performance on tests of visual mental imagery of object shape, object color, letters, faces, and spatial relationships. We found that the FIN was spared by the lesion. We then assessed the connectional anatomy of the FIN in the MNI space and in the patient’s native space, by visualizing the fibers of the inferior longitudinal fasciculus (ILF) and of the arcuate fasciculus (AF) passing through the FIN. In both spaces, the ILF connected the FIN with the anterior temporal lobe, and the AF linked it with frontal regions. Our evidence is consistent with the hypothesis that the FIN is a node of a brain network dedicated to voluntary visual mental imagery. The FIN could have a specific role in integrating high-level visual representations in domain-preferring regions in the ventral temporal lobe with elements of semantic knowledge stored in the anterior temporal lobe and in the language circuits.

In everyday life, information from different cognitive domains—such as visuospatial attention, alertness and inhibition—needs to be integrated between different brain regions. Early models suggested that completely segregated brain networks control these three cognitive domains. However, more recent accounts, mainly based on neuroimaging data in healthy participants, indicate that different tasks lead to specific patterns of activation within the same, higher-order and ‘multiple-demand’ network. If so, then a lesion to critical substrates of this common network should determine a concomitant impairment in all three cognitive domains. The aim of the present study was to critically investigate this hypothesis, i.e. to identify focal stroke lesions within the network that can concomitantly affect visuospatial attention, alertness and inhibition. We studied an unselected sample of 60 first-ever right-hemispheric, subacute stroke patients using a data-driven, bottom-up approach. Patients performed 12 standardized neuropsychological and oculomotor tests, four per cognitive domain. A principal component analysis revealed a strong relationship between all three cognitive domains: 10 of 12 tests loaded on a first, common component. Analysis of the neuroanatomical lesion correlates using different approaches (i.e. voxel-based and tractwise lesion-symptom mapping, disconnectome maps) provided convergent evidence on the association between severe impairment of this common component and lesions at the intersection of superior longitudinal fasciculus II and III, frontal aslant tract and, to a lesser extent, the putamen and inferior fronto-occipital fasciculus. Moreover, patients with a lesion involving this region were significantly more impaired in daily living cognition, which provides an ecological validation of our results. A probabilistic functional atlas of the multiple-demand network was performed to confirm the potential relationship between patients’ lesion substrates and observed cognitive impairments as a function of the multiple-demand network connectivity disruption. These findings show, for the first time, that a lesion to a specific white matter crossroad can determine a concurrent breakdown in all three considered cognitive domains. Our results support the multiple-demand network model, proposing that different cognitive operations depend on specific collaborators and their interaction, within the same underlying neural network. Our findings also extend this hypothesis by showing (i) the contribution of superior longitudinal fasciculus and frontal aslant tract to the multiple-demand network; and (ii) a critical neuroanatomical intersection, crossed by a vast amount of long-range white matter tracts, many of which interconnect cortical areas of the multiple-demand network. The vulnerability of this crossroad to stroke has specific cognitive and clinical consequences; this has the potential to influence future rehabilitative approaches.

This year marks the 130th anniversary of the untimely death of Heinrich Lissauer (1861–91). Paolo Bartolomeo explores how the anatomical account of dissociations between perception and imagery deficits proposed by Lissauer in his 1890 article on visual agnosia, resonates with the present debate on the neural bases of visual mental imagery.

Naming a color can be understood as an act of categorization, i.e. identifying it as a member of category of colors that are referred to by the same name. But are naming and categorization equivalent cognitive processes, and consequently rely on same neural substrates? Here, we used task and resting-state fMRI, as well as behavioral measures to identify functional brain networks that modulated naming and categorization of colors. Color naming and categorization response times were modulated by different resting state connectivity networks spanning from the color-sensitive regions in the ventro-occipital cortex. Color naming correlated with the connectivity between the left posterior color region, the left medial temporal gyrus, and the left angular gyrus; whereas color categorization involved the connectivity between the bilateral posterior color regions, the left frontal, right temporal and bilateral parietal areas. The networks supporting naming and categorization did not overlap, suggesting that the two processes rely on different neural mechanisms.SignificanceWhen we name a color, we also identify it as a member of a color category, e.g. blue or yellow. Are neural processes underlying color categorization equivalent to those of color naming? Here, we address this question by measuring how individual differences in color categorization and naming response times relate to the strength of functional connections in the brain. Color naming speed correlated with left-hemispheric connectivity between the color-sensitive visual regions and the anterior temporal lobe. Color categorization speed was modulated by a different brain network, encompassing bilateral color-sensitive visual areas, and high-level executive and semantic regions. Thus, color categorization and naming performance involved distinct, non-overlapping brain networks, suggesting that the two processes depend on different neural mechanisms.

International audience; Visuospatial neglect is a frequent and disabling consequence of injuries to the right hemisphere. Patients with neglect show signs of impaired attention for left-sided events, which depends on dysfunction of fronto-parietal networks. After unilateral injury, such as stroke, these networks and their contralateral homologs can reorganize following multiple potential trajectories, which can be either adaptive or maladaptive. This article presents possible factors influencing the profile of evolution of neglect towards recovery or chronicity, and highlights potential mechanisms that may constrain these processes in time and space. The integrity of white matter pathways within and between the hemisphere appears to pose crucial connectivity constraints for compensatory brain plasticity from remote brain regions. Specifically, the availability of a sufficient degree of inter-hemispheric connectivity might be critical to shift the role of the undamaged left hemisphere in spatial neglect, from exerting maladaptive effects, to promoting compensatory activity.

International audience; Visual mental imagery is the faculty whereby we can "visualize" objects that are not in our line of sight. Longstanding evidence dating back over thirty years has shown that unilateral brain lesions, especially in the left temporal lobe, can impair aspects of this ability. Yet, there is currently no attempt to identify analogies between these neuropsychological findings of hemispheric asymmetry and those from other neuroscientific approaches. Here, we present a critical review of the available literature on the hemispheric laterality of visual mental imagery, by looking at cross-method patterns of evidence in the domains of lesion neuropsychology, neuroimaging, and direct cortical stimulation. Results can be summarized under three main axes. First, frontoparietal networks in both hemispheres appear to be associated with visual mental imagery. Second, lateralization patterns emerge in the temporal lobes, with the left inferior temporal lobe being the most common finding in the literature for endogenously generated images, especially, but not exclusively, when orthographic material is used to ignite imagery. Third, an opposite pattern of hemispheric laterality emerges when visual mental images are induced by exogenous stimulation; direct cortical electrical stimulation tends to produce visual imagery experiences predominantly when applied to the right temporal lobe. These patterns of hemispheric asymmetry are difficult to reconcile with the dominant model of visual mental imagery, which emphasizes the implication of early sensory cortices. They suggest instead that visual mental imagery relies on large-scale brain networks, with a crucial participation of high-level visual regions in the temporal lobes.

Background and Purpose: We used differential actigraphy as a novel, objective method to quantify motor neglect (a clinical condition whereby patients mimic hemiplegia even in the absence of sensorimotor deficits), whose diagnosis is at present highly subjective, based on the clinical observation of patients’ spontaneous motor behavior. Methods: Patients wear wristwatch-like accelerometers, which record spontaneous motor activity of their upper limbs during 24 hours. Asymmetries of motor behavior are then automatically computed offline. On the basis of normal participants’ performance, we calculated cutoff scores of left/right motor asymmetry. Results: Differential actigraphy showed contralesional motor neglect in 9 of 35 patients with unilateral strokes, consistent with clinical assessment. An additional patient with clinical signs of motor neglect obtained a borderline asymmetry score. Lesion location in a subgroup of 25 patients was highly variable, suggesting that motor neglect is a heterogenous condition. Conclusions: Differential actigraphy provides an ecological measure of spontaneous motor behavior, and can assess upper limb motricity in an objective and quantitative manner. It thus offers a convenient, cost-effective, and relatively automatized procedure for following-up motor behavior in neurological patients and to assess the effects of rehabilitation.

The neural bases of visual mental imagery (VMI) are the object of intense debate. Their identification is central to define the brain substrates our conscious experience and can be clinically important to reveal consciousness in non-communicating patients or to alleviate post-traumatic stress disorder. The dominant model of VMI stipulates a functional and anatomical equivalence between visual mental imagery and visual perception. In patients with acquired brain damage, the model predicts a systematic co-occurrence of perceptual and imaginal deficits. However, patients with lesions restricted to the occipital cortex often experience vivid mental images, even in case of cortical blindness resulting from bilateral V1 damage. Instead, patients with extensive left temporal damage are more likely to have impaired VMI. On the other hand, some neuroimaging and neuromodulatory evidence does suggest an implication of striate cortex in VMI. To address this discrepancy, we conducted an activation-likelihood-estimation-based large-scale meta-analysis of 46 functional magnetic resonance imaging experiments to map the extent of cortical activation associated with three contrasts: (1) all studies combined; (2) VMI versus Control, (3) VMI versus Perception, and (4) Motor Mental Imagery versus Control. Results from the VMI versus Control contrast demonstrated an association between VMI and activation increase in the frontoparietal and cingulo-opercular networks, as well as in the left fusiform gyrus. Results from the VMI versus Perception contrast showed the association between VMI and activation increase of the anterior insular and supplementary motor area / anterior cingulate cortices, bilaterally. Conjunction analyses between the VMI versus Control and the VMI Mental Imagery versus Perception contrasts showed the activation of the left anterior insular cortex and in the bilateral supplementary motor area / anterior cingulate cortex. Results from the Motor Mental Imagery versus Control contrast showed that mental motor imagery increases the activation of the cerebellum and of the superior frontal gyri bilaterally, of the left supplementary motor area, of the anterior portion of the left calcarine sulcus, and of the left fusiform / parahippocampal gyri. Conjunction analyses between the VMI versus Control and the Motor Mental Imagery versus Control contrasts showed the activation of the bilateral superior frontal gyrus as well as of the left supplementary motor area / anterior cingulate cortex. Thus, the results stress the importance for VMI of brain networks associated with attentional control and working memory functions, together with rostral portions of the cortical ventral visual stream. Bayesian analysis confirmed the lack of evidence for an activation of the early visual areas in VMI, consistent with the results from brain-damaged patients. Our evidence suggests a revision of the VMI model. A Fusiform Imagery Node (FIN) in the area FG3 of the left fusiform gyrus might act as a hub retrieving visual information from long-term semantic memory in the anterior temporal lobe. FIN connections with the medial temporal lobe provide vivid VMI episodic experiences. Fronto-parietal networks subserving attention and working memory initiate, modulate and maintain activity of the core VMI network in the left temporal lobe. The revised model of VMI reconciles findings from neuroimaging with the reports from patients with brain damage.

Objectives: Limited data is available on HCV directly acting agents (DAAs) in haematopoietic stem cell transplant (HSCT) recipients. This study aimed at reporting the characteristics, treatment practices and treatment efficacy in HSCT recipients with chronic HCV. Methods: Prospective observational study from EBMT Infectious Diseases Working Party (IDWP). Patients with chronic HCV infection were included. Results: Between 12/2015 and 07/2018, 45 patients were included: male in 53%; median age 49 years (range, 8-75); acute leukaemia in 48.9%, lymphoma in 17.7%, non-malignant disorders in 22.3%; allogeneic HSCT in 84%; 77.8% no immunosuppressive treatment. Genotypes 1, 2, 3 and 4 were detected in 54.5%, 20.5%, 13.6% and 11.4%, respectively; advanced fibrosis in 40%, including cirrhosis in 11.4%. Overall, 37 (82.2%) patients received DAAs, at a median of 8.4 years after HSCT (16.2% within 6 months from HSCT). Sofosbuvir-based treatment was given to 62.2%. Thirty-five patients completed planned treatment course, with sustained virological response (SVR) of 89.1%, and 94.3% (33/35) in those who completed the treatment. Side effects possibly related to DAAs were reported in 5 (14%) and did not require treatment discontinuation. Conclusions: DAAs treatment was effective, saf e and feasible in this cohort of mainly allogeneic HSCT recipients with mild/moderate liver damage. (c) 2021 The British Infection Association. SummaryExogenous attention, the process that makes external salient stimuli pop-out of a visual scene, is essential for survival. How attention-capturing events modulate processing dynamics in the human brain remains unclear. We obtained a comprehensive depiction of attentional cortical dynamics at high spatiotemporal resolution, by analyzing brain activity from 1,403 intracortical contacts implanted in 28 individuals, while they performed an exogenous attention task. The timing, location and task-relevance of attentional events defined a spatiotemporal gradient of three neural clusters, which mapped onto cortical core-periphery topography and presented a hierarchy of timescales, providing the scaffolding for attention temporal computations. Visual attributes modulated neural activity at one end of the gradient, while activity at the other end reflected the upcoming response timing, with attentional effects occurring at the intersection of visual and response signals. These findings reveal how the psychological construct of exogenous attention emerges over large-scale cortical gradients in the human brain.

Background and ObjectiveEye-movement trajectories are rich behavioral data, providing a window on how the brain processes information. We address the challenge of characterizing signs of visuo-spatial neglect from saccadic eye trajectories recorded in brain-damaged patients with spatial neglect as well as in healthy controls during a visual search task.MethodsWe establish a standardized preprocessing pipeline adaptable to other task-based eye-tracker measurements. We use a deep convolutional network, a very successful type of neural network architecture in many computer vision applications, including medical diagnosis systems, to automatically analyze eye trajectories.ResultsOur algorithm can classify brain-damaged patients vs. healthy individuals with an accuracy of 86±5%. Moreover, the algorithm scores correlate with the degree of severity of neglect signs estimated with standardized paper-and-pencil test and with white matter tracts impairment via Diffusion Tensor Imaging (DTI). Interestingly, the latter showed a clear correlation with the third branch of the superior longitudinal fasciculus (SLF), especially damaged in neglect.ConclusionsThe study introduces a new classification method to analyze eyes trajectories in patients with neglect syndrome. International audience; Understanding how few distributed areas can steer large-scale brain activity is a fundamental question that has practical implications, which range from inducing specific patterns of behavior to counteracting disease. Recent endeavors based on network controllability provided fresh insights into the potential ability of single regions to influence whole brain dynamics through the underlying structural connectome. However, controlling the entire brain activity is often unfeasible and might not always be necessary. The question whether single areas can control specific target subsystems remains crucial, albeit still poorly explored. Furthermore, the structure of the brain network exhibits progressive changes across the lifespan, but little is known about the possible consequences in the controllability properties. To address these questions, we adopted a novel target controllability approach that quantifies the centrality of brain nodes in controlling specific target anatomo-functional systems. We then studied such target control centrality in human connectomes obtained from healthy individuals aged from 5 to 85. Main results showed that the sensorimotor system has a high influencing capacity, but it is difficult for other areas to influence it. Furthermore, we reported that target control centrality varies with age and that temporal-parietal regions, whose cortical thinning is crucial in dementia-related diseases, exhibit lower values in older people. By simulating targeted attacks, such as those occurring in focal stroke, we showed that the ipsilesional hemisphere is the most affected one regardless of the damaged area. Notably, such degradation in target control centrality was more evident in younger people, thus supporting early-vulnerability hypotheses after stroke.

Visual neglect is a frequent and disabling consequence of right brain damage. Traditional paper-and- pencil tests of neglect have limitations in sensitivity and ecological validity. The Baking Tray Task (BTT), instead, approaches real-life situations, because it requires participants to place 16 physical objects on a board. The number of objects placed on the left and right portions of the board provides a clinical index of visual neglect. Here we present E-TAN, a technology-enhanced platform which allows patients to perform an enhanced version of the BTT (E-BTT). This platform automatically determines the object locations on the board, and also records the sequence and timing of their placement. We used E-BTT to test 9 patients with right hemisphere damage, and compared their performance with that obtained by 115 healthy participants. To this end, we developed a new method of analysis of participants’ performance, based on the use of the convex hull described by the objects on the board. This measure provides an estimate of the portion of space processed by each participant, and can effectively discriminate neglect patients from patients without neglect. E-TAN allows clinicians to assess visuospatial performance by using a convenient, fast, and relatively automatized procedure, that patients can even perform at home to follow-up the effects of rehabilitation.
