Residual inhibition is a well-known phenomenon defined as a temporary reduction of tinnitus loudness after a prolonged exposure to an acoustic stimulus Recently it has been suggested that modulated (in amplitude - AM and frequency - FM) sounds are also effective in reducing the tinnitus loudness. The aim of our study was to test weather amplitude modulated white noise may be more effective in masking tinnitus and producing residual inhibition (RI) than standard, non-modulated stimuli (white noise). We tested 57 patients from two ENT centers, namely the IMERTA Clinic in Marseille (22 patients) and the ENT Clinic in Poznan (35 patients). The mean age of patients in tested population was 51.04 years (SD=16.0), and the mean duration of tinnitus 5.53 years (SD=7.71). We studied the effects of three frequencies of modulation (100% Departmenth), namely 4, 32 and 64 Hz. The minimum masking level (MML) and the minimum residual inhibition level (MRIL) have been assessed from these stimuli. The results indicate that that there is a dependency of the modulation frequency on the MRIL (the higher the frequency of AM, the lower MRIL is), which cannot be inferred from the data for the MML. On average, relatively high frequency modulated signals (64 Hz) are more efficient than unmodulated or low-frequency related signal to mask and produce residual inhibition of tinnitus. Middle ear muscle (MEM) abnormalities have been proposed to be involved in the development of ear-related symptoms such as tinnitus, hyperacusis, ear fullness, dizziness and/or otalgia. This cluster of symptoms have been called the Tonic Tensor Tympani Syndrome (TTTS) because of the supposed involvement of the tensor tympani muscle (TTM). However, the putative link between MEM dysfunction and the symptoms has not been proven yet and the detailed mechanisms (the causal chain) of TTTS are still elusive. It has been speculated that sudden loud sound (acoustic shock) may impair the functioning of the MEM, specifically the TTM, after an excessive contraction. This would result in inflammatory processes, activation of the trigeminal nerve and a change of the MEMs state into a hypersensitive one, that may be associated to the cluster of symptoms listed above. The goal of this study is to provide further insights into the mechanisms of TTTS. The middle ear function of 11 patients who reported TTTS symptoms has been investigated using either admittancemetry and/or measurement of air pressure in the sealed external auditory canal. While the former method measured the middle ear stiffness the latter provides an estimate of the tympanic membrane displacement. Most patients displayed results consistent with phasic contractions of the TTM (n = 9) and/or Eustachian Tube (ET) dysfunction (n = 6). The MEM contraction or ET dysfunction could be evoked by acoustic stimulation (n = 3), somatic maneuvers (n = 3), or pressure changes in the ear canal (n = 3). Spontaneous TTM contraction (n = 1) or ET opening (n = 1) could also be observed. Finally, voluntary contraction of MEM was also reported (n = 5). On the other hand, tonic contraction of the TTM could not be observed in any patient. The implications of these results for the mechanisms of TTTS are discussed., (Copyright © 2022 Elsevier B.V. It has been suggested that tensor tympani muscle (TTM) contraction may be involved in the development of ear-related pathologies such as tinnitus, hyperacusis and otalgia, called the tonic tensor tympani syndrome (TTTS). However, as there is no precise measure of TTM function under normal and pathological states, its involvement remains speculative. When the TTM or the stapedius muscle (SM) contracts, they both generate an increase of middle ear stiffness that can be measured through middle ear admittance. However, this technique cannot differentiate the contraction between the two muscles. On the other hand, the air pressure measured in a sealed external auditory canal can provide a measure of the eardrum displacement that may be able to differentiate SM from TTM contraction. TTM is attached to the malleus, and its contraction causes a retraction of the eardrum inside the middle ear cavity, while SM can have a small but reversed effect on TTM displacement. To investigate this issue, we compared the middle ear admittance and air pressure in a sealed external ear canal upon auditory stimulation (sMEMC) and voluntary middle ear muscle contraction (vMEMC). In addition, we assessed the perceptual effect of vMEMC, including pitch and loudness matching of the fluttering noise produced by vMEMC and the threshold shifts, were measured. Out of the 14 ears tested, sMEMC was associated with a decrease of admittance in 93% (mean peak average: -0.06 ml, SD:0.04) and an increase of air pressure in 29% of ears (mean peak average: 8.1 Pa, SD:5.1). No decrease in air pressure was found upon sMEMC. For vMEMC (n = 8 ears), decreases were found for both admittance and air pressure in 100% and 88%, with a mean peak average of -0.38 ml, SD: 0.54 and -149 Pa, SD:156, for admittance and pressure respectively. These results suggest that SM and TTM are involved in sMEMC and vMEMC, respectively. In addition, vMEMC was associated with perceptual effects including a low-frequency sound, pitch-matched at ∼30 Hz (>15 dB SL), and a low-frequency hearing loss of at least 10 dB between 20 and 200 Hz. Tinnitus masking patterns have long been known to differ from those used for masking external sound. In the present study, we compared the shape of tinnitus tuning curves (TTCs) to psychophysical tuning curves (PTCs), the latter using as a target, an external sound that mimics the tinnitus characteristics. A secondary goal was to compare sound levels required to mask tinnitus to those required to mask tinnitus-mimicking sounds. The TTC, PTC, audiometric thresholds, tinnitus pitch, and level matching results of 32 tinnitus patients were analyzed. Narrowband noise maskers were used for both PTC and TTC procedures. Patients were categorized into three groups based on a combination of individual PTC-TTC results. Our findings indicate that in 41% of cases, the PTC was sharp (V shape), but the TTC showed a flat configuration, suggesting that the tinnitus-related activity in that subgroup does not behave as a regular stimulus-induced activity. In 30% of cases, V-shape PTC and TTC were found, indicating that the tinnitus-related activity may share common properties with stimulus-induced activity. For a masker centered at the tinnitus frequency, the tinnitus was more difficult to mask than the mimicking tone in 72% of patients; this was particularly true for the subset with V-shape PTCs and flat TTCs. These results may have implications for subtyping tinnitus and acoustic therapies, in particular those targeting the tinnitus frequency.

Tinnitus masking and residual inhibition (RI) are two well-known psychoacoustic measures of tinnitus. While it has long been suggested that they may provide diagnostic and prognostic information, these measures are still rarely performed in clinics, as they are too time consuming. Given this issue, the main goal of the present study was to validate a new method for assessing these measures. An acoustic sequence made of pulsed stimuli, which included a fixed stimulus duration and interstimulus interval, was applied to 68 tinnitus patients at two testing sites. First, the minimum masking level (MML) was measured by raising the stimulus intensity until the tinnitus was unheard during the stimulus presentation. Second, the level of the stimulus was further increased until the tinnitus was suppressed during the silence interval between the acoustic pulses. This level was called the minimum residual inhibition level (MRIL). The sequential measurement of MML and MRIL from the same stimulus condition offers several advantages such as time efficiency and the ability to compare results between the MRIL and MML. Our study confirms that, from this new approach, MML and MRIL can be easily and quickly obtained from a wide variety of patients displaying either normal hearing or different hearing loss configurations. Indeed, MML was obtained in all patients except one (98.5%), and some level of MRIL was found on 59 patients (86.7%). More so, this approach allows the categorization of tinnitus patients into different subgroups based on the properties of their MRIL.

Our objective was to evaluate whether the severity of vestibular loss and old-age (>65) affect a patient's ability to benefit from training using head-position based, tongue-placed electrotactile feedback. Seventy-one chronic dizzy patients, who had reached a plateau with their conventional rehabilitation, followed six 1-h training sessions during 4 consecutive days (once on days 1 and 4, twice on days 2 and 3). They presented bilateral vestibular areflexia (BVA), bilateral vestibular losses (BVL), unilateral vestibular areflexia or unilateral vestibular losses and were divided into two age-subgroups (≤65 and >65). Posturographic assessments were performed without the device, 4h before and after the training. Patients were tested with eyes opened and eyes closed (EC) on static and dynamic (passively tilting) platforms. The studied posturographic scores improved significantly, especially under test conditions restricting either visual or somatosensory input. This 4-h retention effect was greater in older compared to younger patients and was proportional to the degree of vestibular loss, patients with increased vestibular losses showing greater improvements. In bilateral patients, who constantly fell under dynamic-EC condition at the baseline, the therapy effect was expressed by disappearance of falls in BVL and significant prolongation in time-to-fall in BVA subgroups. Globally, our data showed that short training with head-position based, tongue-placed electrotactile biofeedback improves balance in chronic vestibulopathic patients some 16.74% beyond that achieved with standard balance physiotherapy. Further studies with longer use of this biofeedback are needed to investigate whether this approach could have long-lasting retention effect on balance and quality of life., (Copyright © 2013 Elsevier B.V. Introduction: Tinnitus is a subjective phenomenon and in the majority of cases it is impossible to objectify it. The patient's self-evaluation using questionnaires and rating scales is the only way to quantify this symptom. Among the most widely used tools is the Tinnitus Handicap Inventory (THI). This questionnaire permits the practitioner to quantify the severity of tinnitus and measure the psycho-emotional and functional components of the tinnitus related handicap. It has been translated and validated in many languages. However, there is no French version of the THI. The reliability of the THI as a powerful clinical tool for tinnitus evaluation, and the lack of a French version of this questionnaire led us to translate it into French. The goal of this study was to validate our French adaptation of the THI (fTHI) for the management of French-speaking tinnitus sufferers. This is also essential for an effective international cooperation., Methods: The fTHI was presented to a sample of 174 patients (63% men and 34% women) who had consulted our IMERTA Tinnitus Centre from June 2007 to July 2008. All patients reported tinnitus that had persisted for at least six months (average duration: 5.9 ± 4.3 years) as their primary complaint. The mean age of the sample was 54.2 ± 16.4 years, ranging from 22 to 84. The patients did not present any linguistic or mental problems likely to compromise their capacities to answer French questionnaires. During the first visit, before the consultation with the doctor, the patients were asked to complete the fTHI, and to evaluate the clearness of items and the ease of answering the questionnaire. Using rating scales, the patients were also asked to evaluate the severity of tinnitus and its impact on their quality of life. Internal consistency reliability of the fTHI and each sub-scale was evaluated using Cronbach's alpha coefficient (α). The average values, standard errors and correlations between the items, the scores of fTHI and those of the rating scales were calculated using "Statistica 6" software. The level of significance was set at 0.05. The homogeneity was considered to be satisfactory for α greater than 0.8., Results: Ninety-seven percent of the patients answered the totality of the fTHI. Ninety-six percent of them evaluated it as simple, comprehensible and easy to answer. Cronbach's α for the fTHI was 0.90 with item/total correlations (p<0.001) ranging from r=0.21 (item n(o) 19) to r=0.71 (items n(o) 17 and 21). Cronbach's α for the functional sub-scale was 0.80 with item/total correlations (p<0.001) ranging from r=0.38 (item n(o) 24) and 0.69 (item n(o) 4). Cronbach's α for the emotional sub-scale was 0.85 with item/total correlations (p<0.001) ranging from r=0.52 (item n(o) 6) and r=0.78 (item n(o) 21). Cronbach's α for the catastrophic sub-scale was 0.49 with item/total correlations (p<0.001) ranging from r=0.51 (item n(o) 19) and 0.63 (item n(o) 23). The total score of the fTHI is correlated significantly (p<0.001) with the functional (r=0.91), emotional (r=0.90) and catastrophic (r=0.69) sub-scales. Correlations between the fTHI and the rating scales vary between 0.26 (p<0.001) and 0.61 (p<0.001). The highest coefficient of correlation is obtained for the "Total score of fTHI"/"Tinnitus impact on quality of life"., Conclusions: Our results show that the French adaptation of the THI and its sub-scales has good internal consistency reliabilities, comparable to those of the original version. Significant inter-correlations are found between the sub-scales items, even if they represent three distinct factors. In addition, the fTHI is correlated with the self-report rating scales of the tinnitus's impact on the patient's quality of life and the severity of tinnitus. Thus, the fTHI is a psychometrically robust, reliable and valid measure of tinnitus related handicaps and can be used in a clinical setting to quantify the impact of tinnitus on French-speaking patients' quality of life, and to monitor their progress with specific therapies., (Copyright © 2010 L'Encéphale, Paris. Twenty-one patients with high risk non-Hodgkin's lymphoma (NHL) or Hodgkin's disease (HD) underwent autologous bone marrow transplantation (ABMT). Nine out of 21 patients received in addition to bone marrow (BM) cells also peripheral blood (PB) cells collected by leucapheresis performed in the recovery phase after high-doses of either cyclophosphamide or etoposide (7 patients), or after ara-C (2 patient). All patients receiving BM + PB cells had ABMT as salvage therapy following extensive relapse or progression of their disease. The addition of PB cells to BM cells allowed a significantly faster recovery after ABMT. Median time to reach WBC greater than 1,000/microL was 14 days versus 20.5 days for patients receiving BM only. Furthermore, a reduced requirement of supportive care and a shorter period of isolation was observed. These results confirm that PB cells combined with BM cells allow a prompt hematopoietic reconstitution after myeloablation. In addition, the data demonstrate the efficacy of PB cells collected after various cytotoxic drugs, even in patients previously exposed to cytoreductive regimens.

223 cases with chronic lymphoid leukemia (CLL) were subjected to a prospective study on the presence of adeno- and splenomegaly at diagnosis and their subsequent variation. Subjects with no initial organ involvement were usually female (76%) and of old age (mean 69.2 years). Adenopathy or combined lymph node and spleen enlargement were inversely proportional to the mean age at diagnosis. Subsequent organ enlargement was noted in 11.7% of patients with no initial organomegaly, splenomegaly in 15% of patients with adenopathy only and adenopathy in 15% of patients with splenomegaly only. Except in the few cases with leukopenia, particularly high leuklocyte levels were noted in patients with splenomegaly (with or without adenopathy). Anemia at diagnosis was not related to the degree of organ enlargement. It is suggested that qualitative and also quantitative differences in organomegaly in CLL merit further study to establish their underlying mechanisms. CLL must be seen as something more complex than the simple mechanical expression of progressive lymphocyte accumulation.

Microbial maps were performed taking swabs from nose, pharinx, external auditory meatus, groin, vagina, sputum and urine cultures in 69 cases of acute leukaemia, in order: to assess the germs' incidence in an "open ward" department; to eliminate the most dangerous pathogens with local treatment or with a selective therapy without broad-specturm antibiotics; to check, in the 43 cases followed from onset, the changes occurring during the admission and the disease progression; to collect data for comparison with a "sterile" ward. The local decontamination had only a temporary effect. During the course of the disease new, particularly dangerous, pathogens were cultured. Blood cultures were positive in 15% of the patients with fever at the onset of the disease, and in 36.9% of the patients with fever during the disease progression. These values were virtually the same as those observed in the acute stage of C.M.L. (35.7%). In akute leukaemia E. coli (35%) was the most common, followed by P. aeruginosa (20%), Klebsiella (15%), S. alpha haemolyticus (10%) and others. There was little or no relationship between the germs in the maps and those in the blood cultures, though it must be remembered that no stool cultures were examined.

Three patients with treated acute lymphoblastic leukaemia acquired visual symptoms believed to be caused by ocular infiltration by malignant cells. All three patients had other evidence of systemic disease including bone-marrow involvement at the time. One patient had a previous history of meningeal leukaemia and none had received "prophylactic" cranial irradiation. The ophthalmoscopic appearances were similar in each case and were thought to have been caused by obstruction of axoplasmic flow associated with infiltration of the optic nerve by neoplastic cells. The three patients were treated by local irradiation and two also received intrathecal chemotherapy. Response to treatment was variable but the use of radiotherapy combined with intrathecal cytoxic drugs is probably the best available approach.

Blast cells from peripheral blood of 32 patients with acute leukemia were tested for their ability to react with a monoclonal antibody (D1.B6) specific for HLA-DR surface antigen. In order to evaluate the degree of leukemic cell differentiation a monoclonal antibody (R1.B19) specific for the granulocytopoietic lineage was also employed. The results demonstrated that a considerable proportion of blast cell populations expressed the HLA-DR antigen, while only a small fraction of cells expressed the myeloid antigen.
