Abstract

The ongoing severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic and heterologous immunization approaches implemented worldwide for booster doses call for diversified vaccine portfolios. GRAd-COV2 is a gorilla adenovirus-based COVID-19 vaccine candidate encoding prefusion-stabilized spike. The safety and immunogenicity of GRAd-COV2 is evaluated in a dose- and regimen-finding phase 2 trial (COVITAR study, ClinicalTrials.gov: NCT04791423) whereby 917 eligible participants are randomized to receive a single intramuscular GRAd-COV2 administration followed by placebo, or two vaccine injections, or two doses of placebo, spaced over 3 weeks. Here, we report that GRAd-COV2 is well tolerated and induces robust immune responses after a single immunization; a second administration increases binding and neutralizing antibody titers. Potent, variant of concern (VOC) cross-reactive spike-specific T cell response peaks after the first dose and is characterized by high frequencies of CD8s. T cells maintain immediate effector functions and high proliferative potential over time. Thus, GRAd vector is a valuable platform for genetic vaccine development, especially when robust CD8 response is needed.

Abstract

Background and purpose: The objective of this study was to assess the neurological manifestations in a series of consecutive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-positive patients, comparing their frequency with a population hospitalized in the same period for flu/respiratory symptoms, finally not related to SARS-CoV-2. Methods: Patients with flu/respiratory symptoms admitted to Fondazione Policlinico Gemelli hospital from 14 March 2020 to 20 April 2020 were retrospectively enrolled. The frequency of neurological manifestations of patients with SARS-CoV-2 infection was compared with a control group. Results: In all, 213 patients were found to be positive for SARS-CoV-2, after reverse transcriptase polymerase chain reaction on nasal or throat swabs, whilst 218 patients were found to be negative and were used as a control group. Regarding central nervous system manifestations, in SARS-CoV-2-positive patients a higher frequency of headache, hyposmia and encephalopathy always related to systemic conditions (fever or hypoxia) was observed. Furthermore, muscular involvement was more frequent in SARS-CoV-2 infection. Conclusions: Patients with COVID-19 commonly have neurological manifestations but only hyposmia and muscle involvement seem more frequent compared with other flu diseases.

Abstract

Questo documento riassume le attività svolte nell'ambito del quarto anno del Progetto ASI Detriti Spaziali, dalla IV alla V Riunione di Avanzamento. Oltre a ciò, sono descritte anche le attività del Primo Atto Aggiuntivo al contratto, svolte nello stesso periodo. Va sottolineato che il presente documento coincide con il rapporto finale delle attività contrattuali, escluse quelle relative ai WP130 e WP120b, per il completamento dei quali è stata ottenuta una proroga di sei mesi rispetto alla scadenza del 13 dicembre 2010. Per quanto concerne le misure ottiche (WP110), di responsabilità di UNIRM1, sono proseguite le attività di osservazioni ottiche riepilogate in una tabella e si è migliorata l'automazione dell'osservatorio SpaDE. E' stata analizzata la possibilità di effettuare misure spettrometriche con l'osservatorio di Toppo di Castelgrande. Problemi tecnici hanno impedito la consegna del nuovo telescopio da 50 cm da parte del fornitore che ha quindi messo a disposizione del GAUSS diversi sistemi ottici, tra cui un telescopio da 41 cm di diametro. Riguardo il Contributo di altri osservatori (WP111), di responsabilità di UNIRM1, vengono presentati i risultati delle osservazioni congiunte con gli osservatori amatoriali di Massa d'albe e Anguillara. Vengono riportati i risultati finali relativi all'analisi di immagini di campagne osservative congiunte con CNES e JAXA. Inoltre viene illustrata la rete di osservatori italiani e internazionali (in particolare Toppo di Castelgrande e Morehead (MSU)) coordinati dal GAUSS. Circa il Monitoraggio in-situ (WP130), di responsabilità di UNIRM, sono iniziate le prove di sensibilità dei trasduttori piezoelettrici e la revisione dell'elettronica. Le prove di impatto iperveloce per la calibrazione saranno effettuate congiuntamente a CISAS. Per quanto riguarda le misure radar (WP120a), di responsabilità IRA-OATO dell'INAF,. vengono riassunte le attività svolte tra la quarta e la quinta riunione di avanzamento, che portano al completamento di tutte le azioni previste nell'Allegato Tecnico. Viene presentato lo studio elettromagnetico dell'antenna VLBI di Medicina effettuato allo scopo di valutare la possibilità di installare un sistema ricevente in piggy-back. Tale studio viene interamente allegato al presente documento (vedi allegato 1). Per quanto concerne le campagne di misura, nel periodo in esame è stata effettuata la campagna osservativa con il sistema radar multistatico (Evpatoria-Medicina-Ventspils) e una campagna di test per localizzare delle sorgenti di interferenza. Vengono presentati i risultati di entrambe le sessioni osservative. Infine per quanto riguarda il software di analisi in real-time dei dati radar, è stato ultimata la realizzazione dello spettrometro installato su schede basate su FPGA. Si riportano quindi la descrizione e i risultati dei test di funzionamento di tale spettrometro. Per quanto riguarda le misure radar (WP120b), di responsabilità ISAC-CNR, ha avuto inizio a fine ottobre lo spostamento della stazione nel nuovo campo di Villa Sant'Elia in località Campi Salentina (Le), operazione che verrà conclusa inderogabilmente a fine novembre 2010. Sono proseguite le osservazioni mensili dei detriti al rientro in atmosfera. Per ogni giorno dei periodi mensili di osservazione fino ad ottobre 2010 sono stati calcolati i rapporti percentuali dei flussi dei debris rispetto ai flussi totali, i flussi medi dei debris su ogni periodo, e infine le variazioni giornaliere dei flussi totali e dei debris sull'arco temporale delle osservazioni. Dall'analisi dei dati storici relativi agli eventi del test ASAT Cinese del febbraio 2007, del test ASAT USA del febbraio 2008 e della collisione Iridium 333/Cosmos 2251 e inoltre dal successivo confronto con i dati del radar BL, si è osservato un generale incremento delle percentuali dei debris nei mesi del 2010 rispetto ai corrispondenti mesi del 2009. Questo è stato messo in relazione al notevole numero dei frammenti aggiuntivi prodotti dai citati eventi catastrofici presenti in orbite LEO. Per febbraio-aprile 2009 e per aprile 2010 sono stati analizzate le variazioni orarie dei debris in avanzato rientro, al fine di verificare possibili anomalie nei flussi orari. Si sta sviluppando il software per l'accquisizione dei dati da 5 canali che rappresentano le uscite dei ricevitori dell'interferometro. Per quanto riguarda la modellazione (WP200) e la mitigazione (WP400), di responsabilità ISTI/CNR, le ultime attività previste dal contratto originario (non essendo ISTI incluso nell'Atto Aggiuntivo) sono state completate in occasione dello IADC-28 e presentate alla RA4. Comunque, ISTI ha assicurato il proprio supporto alle attività IADC e contrattuali, fino alla chiusura naturale del presente contratto, per evitare ricadute negative sui task IADC in corso. Successivamente alla RA4 è continuata la caratterizzazione delle nubi di detriti collisionali di Fengyun 1C, Cosmos 2251 e Iridium 33. Sono inoltre ripartite le attività connesse con l'AI 27.1, per la valutazione della necessità di un Active Debris Removal in LEO, e si è svolta la programmata campagna test di rientro 2010-1, con esiti molto positivi. Per quanto riguarda specificamente le collaborazioni e la mitigazione, sono stati riveduti e commentati diversi importanti documenti IADC, è stato analizzato il contenuto di un database di informazioni spaziali su richiesta dell'ASI ed è proseguita l'attività nell'ambito degli AI 22.2, 26.2 e 27.2, relativamente all'aggiornamento e al supporto delle IADC Mitigation Guidelines e alla valutazione del rischio rappresentato dai rientri. Infine, per quanto concerne le problematiche di protezione (WP310 e 330), di responsabilità CISAS, vengono presentati i risultati degli ultimi esperimenti di impatto effettuati a supporto dell'AI 26-1 IADC per la caratterizzazione di ejecta da satellite. In particolare, è riportato l'esito della valutazione di diversi sensori per la caratterizzazione degli ejecta, oltre che lo studio del fenomeno di produzione di frammenti secondari in seguito ad impatti obliqui. I dati sperimentali raccolti hanno anche permesso di formulare delle procedure semi-empiriche per la previsione della distribuzione di massa e velocità delle particelle entro le nuvole di ejecta, producendo dunque un'informazione utile per il calcolo del flusso detritico secondario originato in seguito ad impatti primari su superfici satellitari. Per quanto concerne le attività numeriche, sono state affinate le tecniche di simulazione della generazione di ejecta mediante hydrocode, ottenendo buoni risultati a spese però di tempi di calcolo estremamente significativi. Infine, è stato predisposto un software specifico per il Risk Assessment, che consente di utilizzare MASTER e ORDEM come modelli di ambiente e implementa varie equazioni di danno per il calcolo della resistenza balistica del sistema. Lo stesso software consente di stimare il flusso detritico secondario originato a seguito della produzione di ejecta da impatto iperveloce su superfici di veicoli spaziali.

Abstract

Questo documento rappresenta un sunto delle attività svolte nell'ambito del terzo anno del Progetto ASI Detriti Spaziali, dalla Seconda alla Terza Riunione di Avanzamento. Oltre a ciò, sono descritte anche le attività del Primo Atto Aggiuntivo al contratto, svolte nello stesso periodo. Per quanto concerne le misure ottiche (WP110), di responsabilità di UNIRM1, l'attività dell'osservatorio SPADE è stata dedicata all'osservazione di satelliti in orbita bassa. Tali satelliti sono stati scelti sulla base delle informazioni fornite dal servizio "SOCRATES" del sito Celestrak che individua tra i satelliti catalogati quelli coinvolti in "close approach". Tra le coppie identificate dal sito, sono state osservate quelle con la maggiore probabilità di collisione. E' stato poi necessario realizzare un software in MATLAB capace di passare dai TLE alle effemeridi dei satelliti da tracciare in maniera di realizzare un inseguimento di satelliti veloci. Nell'ambito delle cooperazioni internazionali finalizzate al miglioramento del sistema di osservazione dei detriti spaziali comprendente l'osservatorio SPADE si è dato luogo ad una collaborazione tra GAUSS/ASI, CNES e JAXA nel campo dell'image processing. Per quanto riguarda il sensore di impatti è stato realizzato l'hardware di acquisizione dati ed il software di interfaccia, deve iniziare la fase di scelta del sensore. Per quanto riguarda le misure radar (WP120a), di responsabilità IRA-OATO dell'INAF, viene data una sintesi delle attività svolte nel primo semestre dal KOM. Dopo avere illustrato brevemente la metodologia del piggy-back nei radar bistatici, si descrive in dettaglio l'algoritmo, sviluppato nell'ambito di questo contratto, per calcolare le coordinate di puntamento delle antenne slave nei sistemi piggy-back. La sincronizzazione (nella registrazione dei dati e nelle operazioni di puntamento) è già disponibile sulle antenneappartenenti alla rete VLBI (Very Long Baseline Interferometry). Per quanto concerne le attività di misura, sono state effettuate duecampagne osservative in modalità bistatica utilizzando l'antenna ricevente VLBI di Medicina e la trasmittente RT-70 di Evpatoria. Talicampagne hanno riguardato l'osservazione di detriti noti (prodotti dalla collisione Iridium 33 - Cosmos 2251), la ricerca in beam-parking di oggetti non correlati in orbita LEO e l'osservazione di calibratori radar, che saranno utilizzati nella stima delle RCS. Durante le osservazioni è stato anche testato un nuovo sistema di preview in real-time degli echi, implementato sullo spettrometro SPECTRA-1. Per quanto riguarda le misure radar (WP120b), di responsabilità ISAC-CNR, sono state condotte osservazioni mensili dei detritial rientro in atmosfera, discriminandolli dai meteoroidi in base ai tempi di salita dei corrispondenti radioechi. E' stato applicato unfattore correttivo nel calcolo della velocità del debris tenendo conto dell'effetto dell'ablazione fino a quote di 70-80km dove èmassima la concentrazione dei debris. Si è discussa l'esistenza di una possibile evoluzione temporale (stagionale) dei flussi deidebris connessa alla radiazione solare. Sono stati messi a confronto i flussi cumulativi dei debris e dei meteoroidi in funzione delleloro dimensioni e analizzato il comportamento delle curve teoriche applicate a tali flussi, estrapolate a particelle micrometriche. E'stato effettuato un primo confronto di questi risultati con quelli ottenuti dall'analisi dei crateri da impatto su strutture orbitanti e daaltre misure radar dell'ambiente detritico in orbite LEO. Per quanto riguarda le attività di modellazione (WP200) e mitigazione (WP400), di responsabilità ISTI/CNR, è stata investigatal'evoluzione delle nubi di detriti generate dalla collisione tra i satelliti Cosmos 2251 e Iridium 33, è stato completato l'Internal Task del WG2 per la valutazione dei benefici per l'ambiente LEO di un Active Debris Removal, è stato completato uno studio a supporto dello IADC AI 22.2 "Long term presence in GEO region of GTO objects", è stata garantita la partecipazione attiva alla campagna di rientro IADC-2009-1, sono state esaminate le problematiche del "conjunction assessment" e della "collision avoidance" per la costellazione di satelliti COSMO-SkyMed, è stata presentata la situazione degli oggetti italiani in orbita nel contesto della popolazionedei detriti orbitali e sono state delineate la attività svolte in ambito IADC, COPUOS, ISO e European NoC SD. Infine, per quanto concerne le problematiche di protezione (WP310 e 330), di responsabilità CISAS, viene presentata la selezionedelle tipologie di target e la definizione della relativa Test Matrix per gli ultimi 40 test HVI previsti dal contratto: tali esperimenti rispondono alle responsabilità italiane relative all'AI26-1 IADC "Characterization of ejecta from HVI on spacecraft outer surfaces" e alla necessità di calibrare il sensore in-situ sviluppato nell'ambito del WP130. Abstract

Space debris are a concern for operative satellites and human missions. A large amount of debris is located in Low Earth Orbit and in Geostationary Orbit, and several institutes all around the world monitor the growth of the debris population. Since a few years, GAUSS (Group of Astrodynamics of the University of Rome “La Sapienza”) has been carrying out space debris observation campaigns exploiting amateur astronomers’ facilities. Presently GAUSS is making up, in the frame of a contract with Italian Space Agency (ASI), the first Italian optical observa-tory completely addressed to space debris detection. During the optical campaigns, a large number of images is expected to be archived. This is the reason why a software is needed to process the images in order to automatically detect the objects and correlate them with the catalogue. The algorithm is based mainly on single-frame handling to make the noise level as flat as possible, with a successive two-image comparison which allows removing stars and bi-ases. In the object detection procedure a shape identification algorithm has been avoided, in order to reduce calculation time. The last step is the object identification by comparison with the Two-Line Elements catalogue. The actual position of the detected objects is compared with the propagated positions of the catalogued ones. In conclusion, both the image processing itself and the object identification are performed automatically. The paper describes the logic of the developed algorithm as well as the preliminary results of the software.

Abstract

Highly active antiretroviral therapy (HAART) greatly reduces the risk of developing tuberculosis for HIV-infected persons. Nonetheless, HIV-associated tuberculosis continues to occur in countries where HAART is widely used. To identify the characteristics of HIV-infected persons who develop tuberculosis in the context of the availability of HAART, the current authors analysed data taken from 271 patients diagnosed, in Italy, during 1999-2000. These patients represent 0.7% of the 40,413 HIV-infected patients cared for in the clinical units participating in this current study. From the data it was observed that 20 patients (7.4%) had a previous episode of tuberculosis whose treatment was not completed. Eighty-one patients (29.9%) were diagnosed with HIV at tuberculosis diagnosis, 108 (39.8%) were aware of their HIV status but were not on antiretroviral treatment and 82 (30.3%) were on antiretroviral treatment. Patients on antiretroviral treatment were significantly less immunosuppressed than patients with HIV diagnosed concurrently with tuberculosis, or other patients not on antiretrovirals (median CD4 lymphocytes count: 220 cells x mm(-3) versus 100 cells x mm(-3), and 109 cells x mm(-3), respectively). No significant differences in clinical presentation of tuberculosis according to antiretroviral therapy status were recorded. Failure of tuberculosis control interventions (e.g. noncompletion of treatment) and of HIV care (delayed diagnosis of HIV infection and suboptimal uptake of therapy) may contribute to continuing occurrence of HIV-associated tuberculosis in a country where highly active antiretroviral therapy is largely available. However, a significant proportion of cases occur in patients who are on antiretroviral treatment.

Abstract

Objective To evaluate the effects of infrared-light-emitting diode (LED) during treadmill training on functional performance. Methods Thirty postmenopausal women aged 50-60 years were randomly assigned to one of three groups and successfully completed the full study. The three groups were: (1) the LED group, which performed treadmill training associated with phototherapy ( n = 10); (2) the exercise group, which carried out treadmill training only ( n = 10); and (3) the sedentary group, which neither performed physical training nor underwent phototherapy ( n = 10). Training was performed over a period of 6 months, twice a week for 45 min per session at 85-90% of maximal heart rate, which was obtained during progressive exercise testing. The irradiation parameters were 100 mW, 39 mW/cm2 and 108 J/cm2 for 45 min. Quadriceps performance was measured during isokinetic exercise testing at 60°/s and 300°/s. Results Peak torque did not differ amongst the groups. However, the results showed significantly higher values of power and total work for the LED group (∆ = 21 ± 6 W and ∆ = 634 ± 156 J, p < 0.05) when compared to both the exercise group (∆ = 13 ± 10 W and = 410 ± 270 J) and the sedentary group (∆ = 10 ± 9 W and ∆ = 357 ± 327 J). Fatigue was also significantly lower in the LED group (∆ = −7 ± 4%, p < 0.05) compared to both the exercise group (∆ = 3 ± 8%) and the sedentary group (∆ = −2 ± 6%). Conclusions Infrared-LED during treadmill training may improve quadriceps power and reduce peripheral fatigue in postmenopausal women. [ABSTRACT FROM AUTHOR]

Abstract

In healthy subjects the administration of an amino acid mixture devoid of tryptophan causes a marked decrease of plasma tryptophan. This is because amino acid mixtures induce protein synthesis and tryptophan in blood is incorporated into newly synthesized proteins. We hypothesized that a tryptophan-free mixture could differently affect plasma tryptophan levels in subjects with an impaired protein synthesis such as chronic liver patients. We studied tryptophan levels after a tryptophan-free amino acid solution in controls and cirrhotics fasting 12 hours. Plasma total tryptophan fell to 91% of the initial level 60 minutes after the administration of the diet, to 71% after 120 ′, and to 50% after 210′ in controls. In cirrhotics the solution caused a decrease of plasma tryptophan that began significantly later than in controls, the delay being proportional to the severity of the disease. Cirrhotics were subdivided into two groups in accordance to the Pugh modification of the Child-Turcotte criteria. Total plasma tryptophan was 100% of base line levels after 60′, 88% after 120′, and 65% after 210′ in less severe clinical condition; total plasma tryptophan was 102% of base line levels after 60′, 98% after 120′, and 75% after 210′ in more severe clinical condition.

Multicentre, retrospective cohort study with multivariable Cox proportional-hazards modelling and survival-time inverse-probability-weighting, evaluating the impact of different treatments on survival of proven COVID-19 patients admitted to two Hospitals in the province of Piacenza, Italy. Use of tocilizumab and of high doses of low molecular weight heparin, but not of antivirals (either alone or in combination), azithromycin, and any corticosteroid, was independently associated with lower mortality. Abstract

Background: Piacenza is the closest city to the first coronavirus disease 2019 (COVID-19) cluster in Italy and has the highest national COVID-19 death rates per population. The objective of this study is to present characteristics and outcomes of patients admitted to medical departments of the Hospital of Piacenza during the first wave of the epidemic., Methods: A total of 218 patients with confirmed or suspect COVID-19 and severe pneumonia were included from February 21st to May 15th, 2020. Routinely-collected clinical and laboratory data were retrospectively retrieved from electronic medical files. A Cox proportional-hazards model was fit to assess the association of treatment and other variables with death., Results: Median age of patients was 68 years; 150 patients (69%) had comorbidities, mainly hypertension (107, 49%). Overall, 185 (85%) patients had acute respiratory distress syndrome (ARDS) on admission, including 103 (47%) with moderate or severe ARDS. Chest computed tomography scan showed bilateral disease in 201 (98%) and extensive lung involvement in 79 (50%) patients. Most patients received antiviral treatment (187, 86%) and corticosteroids (134, 61%). All patients received respiratory support and 64 (29%) were admitted to intensive care unit. As of June 30th, 100 patients (46%) died, 109 patients (50%) were discharged, and 9 patients (4%) were still hospitalized. In multivariable Cox analysis, age above 65 years, having more than one comorbidity, severe ARDS, low platelet counts, and high LDH levels at admission were associated with mortality, while having diarrhea at admission was associated with survival. The use of antivirals or corticosteroids was not associated with survival., Conclusions: Overall case fatality rates were high and associated with comorbidities, extensive lung involvement, ARDS at admission, and advanced age. Abstract

Severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) infects humans through the angiotensin converting enzyme-2 (ACE-2) receptor expressed on many cells, including lymphocytes. In Covid-19 patients IL-6 is overexpressed, and hyperactivated plasmacytoid lymphocytes are detected in peripheral blood film. We hypothesize that, due to the unpredictable interaction between the new virus and the B cell lineage of infected patients, a cascade of out of control events can ensue, capable of determining unexpected pathologic disorders involving such lineage. Here we report two cases of autoimmune hemolytic anemia (AIHA) and two cases of B-cell hematological malignancies developed or reactivated during acute SARS-CoV-2 infection. The temporal relationship of the events may suggest a potential causal relationship between SARS-CoV-2 infection and the hematopoietic disorders. Abstract

The Authors present a case of infestation by Toxocara canis occurring in a eighteen months' child. A first time he was hospitalised for wheezing, reported hypereosinophilia and a thoracic X-ray with a soften congestion in the right lung's base. Two months later he was admitted to hospital for the persistence of high level of eosinocyte. The clinical tests and the patient's history explain the first symptoms like the passage of the Toxocara's larvae in the lungs, but don't justify the anomalous behaviour of eosinocytes. In fact the hypereosinophilia was present two months before the admission in our Department, but two tests, executed during the first stay in hospital, didn't show abnormality. A few months after, another blood's sample showed an high level of eosinocytes and, in the same time, a new control of thoracic X-ray, didn't show any change in the image.

The need for accurate and noninvasive evaluation of liver iron stores prompted us to evaluate the reliability of high-field magnetic resonance imaging equipment in liver patients with low or moderate siderosis, given the poor results obtained using systems operating at low field strength in such cases. Twenty patients with sporadic porphyria cutanea tarda and 28 with comparable chronic liver diseases (chronic hepatitis or cirrhosis) and moderate siderosis were compared with 10 patients with idiopathic or secondary hemochromatosis and 10 healthy controls. Plasma iron profile, ferritin concentration and liver iron concentration, determined with atomic absorption spectroscopy, were matched with the magnetic resonance parameters-namely, transverse relaxation time and the signal intensity for a given proton amount, obtained with equipment operating at a field strength of 1.5 T. Hemochromatosis patients with mean liver iron concentrations of 550 mumol/gm dry wt (vs. 10 mumol of controls) exhibited an impressive reduction in the signal intensity with respect to the other three groups, and this reduction prevented any further comparison with the same porphyria cutanea tarda and chronic liver disease groups, whose liver iron level was twice that of the controls. The signal intensity remained almost unchanged in the latter groups, whereas the transverse relaxation time was significantly reduced. Moreover, correlation with liver iron was significantly inverse in the case of the transverse relaxation time (n = 17, r = 0.62, p = 0.008) and direct in the case of the transverse relaxation rate. Abstract

The ingestion of an amino acid mixture lacking tryptophan causes a rapid fall of plasma tryptophan in healthy subjects. This is because amino acids elicit protein synthesis and endogenous tryptophan is incorporated into new proteins. If protein synthesis is the mechanism through which tryptophan-free solution decrease blood tryptophan, it may be interesting to study tryptophan levels after a tryptophan-free mixture in subjects with impaired protein synthesis. In the present paper we show that in 27 cirrhotics the administration of a tryptophan-free solution caused a fall of total plasma tryptophan that began significantly later than in 14 control subjects, the delay being significantly proportional to the severity of the disease. The difference between control and cirrhotic subjects was due to the bound fraction of plasma tryptophan. The diagnostic and clinical usefulness of our findings are discussed.

Abstract

Edwards (or trisomy 18) syndrome, with a frequency of about 1 over 8000 births, is commonly characterized by a very short survival; those rare cases reported with ages of 12 months or more, always were demonstrated to be chromosomic mosaicisms. Two affected children with completely different outcomes are here described; the first with a classic short-life evolution, in opposition to the second with long survival, the last a good example of all those various biological, psychological and social support interventions in these circumstances, very rare to match with, for this appears to be a trisomy 18 without mosaicism.

Abstract

Perinatal listeriosis is a disease whose incidence has increased constantly in recent years. Like neonatal infection determined by B hemolyticus streptococcus, the disease appears in two forms: the first is the septicaemic form with early onset, the second form is meningoencephalitic with late onset. We describe a case of listeriosis with immediate onset in which, in spite of the prompt therapy and the normality of rachiocentesis, developed a purulent ventriculitis with outcome in hydrocephalus.

Abstract

The role played by carotenoids, retinol and tocopherol in quencing oxidative cellular damage and combatting tumor growth is well documented, but little is known about their activity in human liver cirrhosis (LC), where oxidative damage and tumoral complications are common-place. We investigated 59 patients with LC of different etiology on admission to hospital and compared them with 32 healthy controls, matched for age and sex. Nutritional (cutaneous skinfolds, creatinine-height index) and serum parameters were determined; of these, alpha- and beta-carotene, cryptoxanthin, lycopene, retinol and alpha-tocopherol were detected by an high-performance liquid chromatographic (HPLC) technique, devised in our laboratory, which afforded an accurate and simultaneous resolution of all six compounds. The results point to a significant reduction in almost all the vitamin factors in LC, as well as in total serum lipids. In consequence, the ratio tocopherol/total serum lipids remains almost unchanged: 2.45 +/- 0.08 (m +/- se) in controls and 2.34 +/- 0.16 in patients. The effects of age, sex, nutritional habits, alcohol, malnutrition and the severity of the disease were also evaluated in relation to the vitamin-factor levels. It is suggested that the reduced levels observed in LC patients are due to a number of factors including portal hypertension and lymphatic circulation impairment, and it is concluded that thorough screening and improved diet are beneficial in the follow-up of LC.

Abstract

We describe a case of unilateral diaphragmatic paralysis following a distocic delivery. We think that the rarity of this syndrome in only apparent while, if investigated with fluoroscopy, much more cases can be discovered. Our case is interesting too since the Erb's palsy was contralateral to the diaphragmatic paralysis; that occurs in very few cases.

Abstract

In this study we have tried to examine retrospectively all the babies born to HBsAg + mothers in the past nine years, in order to evaluate which is the real incidence of perinatal transmission of HBsAg, and then if the actual politic to vaccinate only these babies is right and sufficient. The results demonstrate that the perinatal transmission is not a major problem in our country, so we suggest that for improvement of public health is more advised the vaccination of all infants: those born to HBsAg + mothers at the birth, the others in the third month of life.
