Moving through the landscape with awareness has always been a revolutionary act for humanity, involving a renewed consciousness of our place in space and time. It is no coincidence that a walk is placed at the origin of the modern concept of landscape, and then continues, through some fundamental moments, to characterize a way of relating to the surrounding environment through movement. If in various eras walking has represented a form of aesthetic research, even a desecrating model of rebellion against the status quo, it still remains a fundamental form of primary knowledge, essential for the understanding of everyday landscapes, explored through bodily and related practices to a more inclusive modality involving different actors, who shape and interpret contemporary landscapes.

The living environment has widely recognized consequences on the well-being of city-dwellers. Green streets, integrated in a network of parks and green space, can increase liveability of everyday landscapes and the health of residents, supporting and stimulating physical and psychological benefits. Greenways, in fact, are key infrastructures for the practice of physical activity and consequently have an important function of the reduction of diseases caused by a sedentary lifestyle. Beyond promoting exercise, a number of studies have proved that the perception of green and vegetation may have intrinsic qualities, which enhance psychological well-being. Moreover, green streets can increase social cohesion and social justice in urban contexts, counteracting health inequalities related to income deprivation, and supporting the recreational value of urban contexts.

Green streets can have a great impact on urban stormwater management. As part of a green infrastructure, they can offer adaptable solutions to manage rainwater through minimizing the impact of development on the water cycle. They can use natural processes to manage runoff at source at a lower cost than traditional gray infrastructure’ (pipes and other water control devices). In fact, it is possible to incorporate water management features into streets, plazas, domestic gardens, and urban living spaces. Thus, many cities worldwide are exploiting green streets to control and treat stormwater runoff through the combination of pervious surfaces, vegetation, and soil. This is intended to slow, filter from impurities, slower infiltrate flow into the underground to improve water quality, reduce erosion, and recharge groundwater.

Although heparin-induced thrombocytopenia (HIT) is a known complication of intravenous unfractionated heparin (UFH), its incidence in medical patients treated with subcutaneous UFH is less well defined. To determine the incidence of HIT in this category of patients, the platelet count was performed at baseline and then every 3 ± 1 days in 598 consecutive patients admitted to 2 medical wards and treated with subcutaneous UFH for prophylactic (n = 360) or therapeutic (n = 238) indications. The diagnosis of HIT was accepted in the case of a platelet drop of 50% or more and either the demonstration of heparin-dependent antibodies or (when this search could not be performed) the combination of the following features: (1) the absence of any other obvious clinical explanation for thrombocytopenia, (2) the occurrence of thrombocytopenia at least 5 days after heparin start, and (3) either the normalization of the platelet count within 10 days after heparin discontinuation or the earlier patient's death due to an unexpected thromboembolic complication. HIT developed in 5 patients (0.8%; 95% CI, 0.1%-1.6%); all of them belonged to the subgroup of patients who received heparin for prophylactic indications. The prevalence of thromboembolic complications in patients with HIT (60%) was remarkably higher than that observed in the remaining 593 patients (3.5%), leading to an odds ratio of 40.8 (95% CI, 5.2-162.8). Although the frequency of HIT in hospitalized medical patients treated with subcutaneous heparin is lower than that observed in other clinical settings, this complication is associated with a similarly high rate of thromboembolic events.

The figure of the Nursing Case Manager can be employed in many areas of pediatrics, in particular in chronic situations that require not only a specific care plan while the patient is admitted but also a training program to teach patients how to manage their illness at home. In this context the activities of the Case Manager Nurse in the Division of Pediatric Diabetology are described.

The optimal treatment of superficial thrombophlebitis of the leg is undefined. The main study objective was to assess the efficacy and safety of unmonitored high doses as compared to low doses of unfractionated heparin (UFH) for prevention of venous thromboembolic complications in patients with superficial thrombophlebitis of the thigh.Sixty consecutive patients with acute thrombophlebitis of the great saphenous vein, as assessed by ultrasonography, were randomized to subcutaneous injection twice daily of UFH in high unmonitored doses (12,500 IU for one week followed by 10,000 IU) or prophylactic doses (5,000 IU) for four weeks. The rate of asymptomatic involvement of the deep venous system and/or symptomatic thromboembolic events during a six-month follow-up period was assessed and compared between the two study groups.Six of the 30 patients (20.0 %; 95% CI, 7.7 to 38.6) randomized to low-dose UFH developed symptomatic or asymptomatic events as compared to 1 of the 30 patients (3.3%; 95% CI, 0.07 to 17.2) who received high-dose UFH (p=0.05 by one-sided Fisher's exact test). No patient experienced major bleeding complications in either group.The results of this study suggest that in patients with acute thrombophlebitis of the thigh unmonitored high doses of UFH are more effective than prophylactic doses of UFH for prevention of venous thromboembolic complications and do not enhance the risk of bleeding complications.

A small proportion of patients with deep vein thrombosis develop recurrent venous thromboembolic complications or bleeding during anticoagulant treatment. These complications may occur more frequently if these patients have concomitant cancer. This prospective follow-up study sought to determine whether in thrombosis patients those with cancer have a higher risk for recurrent venous thromboembolism or bleeding during anticoagulant treatment than those without cancer. Of the 842 included patients, 181 had known cancer at entry. The 12-month cumulative incidence of recurrent thromboembolism in cancer patients was 20.7% (95% CI, 15.6%-25.8%) versus 6.8% (95% CI, 3.9%- 9.7%) in patients without cancer, for a hazard ratio of 3.2 (95% CI, 1.9-5.4) The 12-month cumulative incidence of major bleeding was 12.4% (95% CI, 6.5%-18.2%) in patients with cancer and 4.9% (95% CI, 2.5%-7.4%) in patients without cancer, for a hazard ratio of 2.2 (95% CI, 1.2-4.1). Recurrence and bleeding were both related to cancer severity and occurred predominantly during the first month of anticoagulant therapy but could not be explained by sub- or overanticoagulation. Cancer patients with venous thrombosis are more likely to develop recurrent thromboembolic complications and major bleeding during anticoagulant treatment than those without malignancy. These risks correlate with the extent of cancer. Possibilities for improvement using the current paradigms of anticoagulation seem limited and new treatment strategies should be developed.

Although pharmacologic prophylaxis against venous thromboembolism has become the standard of care following total hip and knee replacement, prophylaxis among patients undergoing surgery for hip fracture and other lower extremity trauma remains underutilized. Available experience consistently supports the view that low-molecular-weight heparins are more effective than unfractionated heparin for prevention of proximal deep vein thrombosis (DVT) with no additional hemorrhagic risk and more effective than oral anticoagulants for prevention of in-hospital (mostly distal) venous thrombosis at the price of a higher surgical site bleeding and wound hematoma. The choice between low-molecular-weight heparin and warfarin should be tailored to the individual patients based on the clinical assessment of postoperative thrombosis and bleeding risk as well as the prophylaxis-specific cost and convenience. Whether thromboprophylaxis should be continued for a few additional weeks after hospital discharge is controversial. The overall incidence of postoperative DVT in patients with cancer is about twice as high as that of patients free of malignancy. Accordingly, they require prophylactic measures comparable with those usually recommended for major orthopedic surgery. In this setting, dermatan sulfate shows promise. In contrast to surgical patients, prevention of venous thromboembolism is less well studied in hospitalized medical patients. In a recent controlled randomized trial, enoxaparin in high prophylactic doses was an effective and safe measure of thromboprophylaxis in ordinary bedridden patients.

SummaryIn order to investigate the risk of fetal loss in carriers of factor V Leiden who are family members of probands with this mutation, we performed a retrospective cohort study including 109 women who had been pregnant at least once and were family members of 61 probands with venous thromboembolism and a single identified factor V Leiden mutation. The rate of pregnancies ending in unexplained fetal loss, early miscarriage, late miscarriage or stillbirth in women with the factor V Leiden was compared with that of women with normal genotype. In the 65 women who were carriers of factor V Leiden 31 of the 191 pregnancies (16.2% per pregnancy ) resulted in unexplained fetal loss, as compared to 13 of the 121 pregnancies (10.7% per pregnancy) in the 44 non-carriers (relative risk, 1.5; 95% CI, 0.8-3.2). After the first trimester of pregnancy, 25 pregnancies (13.1% per pregnancy) among carriers of factor V Leiden ended in fetal loss, as compared to 7 (5.8% per pregnancy) among females with normal genotype (relative risk, 2.3; 95% CI, 1.01 to 5.1). We conclude that carriers of factor V Leiden who are family members of probands with this mutation have a statistically significant and clinically important risk of late miscarriage or stillbirth. Studies addressing the benefit-to-risk ratio of adopting routinary thromboprophylactic measures following the first trimester of pregnancy in these women are strongly indicated.

Background The optimal duration of thromboprophylaxis after major orthopedic surgery is controversial. Although oral anticoagulants are still widely used for the prevention of venous thromboembolism after hip replacement, to our knowledge no study has assessed the benefit of prolonging anticoagulation beyond the hospital stay. Methods Consecutive patients who had received warfarin sodium prophylaxis after total hip arthroplasty were randomized to stop taking the drug at the time of hospital discharge or to continue taking it for 4 more weeks. The rate of symptomatic and asymptomatic venous thromboembolic events (as shown by compression ultrasonography of the proximal-vein system) occurring during the study period was compared between the 2 groups. The study was prematurely terminated after the inclusion of the first 360 patients because a statistically significant and clinically relevant superiority of extended over short-term thromboprophylaxis was observed. Results Objectively confirmed venous thromboembolic complications were recorded in 10 patients: 9 (5.1%) in the group of 176 control patients, and 1 (0.5%) in the group of 184 patients who continued the warfarin treatment. The absolute difference in the incidence of events was 4.57% (95% confidence interval [CI], 1.15-7.99). The relative risk of venous thromboembolism developing in control patients compared with patients assigned to extended thromboprophylaxis was 9.4 (95% CI, 1.2-73.5). The number needed to treat was 22. Major bleeding developed in 1 patient who was randomized to the extended prophylaxis group (0.5%; 95% CI, 0.02-3.0) compared with none in the control group. Conclusion Extending prophylaxis with warfarin for a few more weeks beyond the hospital stay has the potential to considerably improve the outcome of patients who undergo hip arthroplasty.

Water quality, flooding risk, and water consumption in urban areas are emerging  issues. Urban impervious surfaces increase stormwater runoff,  affecting ecosystems  and leading to hydrogeological instability and flooding risk. Sustainable urban design strategies can contribute to counteract the negative impact of anthropic activities both at city-scale and global scale. Green and Blue Infrastructure (GBI) approaches, in particular, are an alternative to stormwater traditional management. In Europe, Sustainable Drainage System implementation copes with impervious surfaces to achieve water quality, amenity, and biodiversity increase. Best Management Practices, developed mainly in the USA, focus on specific measures for sustainable stormwater treatment. Water Sensitive Urban Design, spread in Australia and England, also aims to minimize the impact of developed areas preventing flood risk, limiting water consumption and enhancing environmental quality. In the USA and Canada, Low Impact Development offers design strategies to manage runoff and deliver structural practices to mimic predevelopment processes of infiltration, filtration and detention.

In order to mainstream NBS projects and secure necessary funding, an evidence-based approach, supported by performance indicators, is necessary for planning and deployment stages. The main aim of this article is to establish an assessment framework, which fosters the urban planning and implementation periods of NBS projects and forms a linkage between adaptation actions with urban sustainability policies by making use of monitoring data that enables predictive estimation of future scenarios. This framework entails definition of relevant environmental indicators through a Material Flow Analysis (MFA)-based urban metabolism (UM) approach and dynamic application of these indicators to be used for monitoring purposes. While the initial application of the environmental assessment framework supports selection of most suitable NBS to tackle urban challenge of concern, the dynamic performance tracking provides critical, strategic and analytical insights to ensure the desired targets are met. Such an assessment methodology can also maximize the contribution of NBS to EU policies including European Green Deal (EGD), EU Biodiversity Strategy and EU Adaptation Strategy.
