(1) Background: Cardiovascular prevention was left in second place during the COVID-19 pandemic and the use of telemedicine turned out to be very useful. We aimed to evaluate the effectiveness of a telemedicine application for remote monitoring and treatment adjustments in terms of improving cardiovascular prevention. (2) Methods: A prospective study of 3439 patients evaluated between the 1st of March 2019 and the 1st of March 2022, in the pre-pandemic period by face-to-face visits and during the pandemic by teleconsultations or hybrid follow-up. We compared four periods: pre-pandemic—Pre-P (1 March 2019–1 March 2020), lockdown—Lock (1 March–1 September 2020), restrictive-pandemic—Restr-P (1 September 2020–1 March 2021), and relaxed–pandemic—Rel-P (1 March 2021–1 March 2022). (3) Results: The average values of total cholesterol (TC), LDL cholesterol, triglycerides, uric acid, and glucose had an increasing trend during Lock and Restr-P, and they decreased close to the baseline level during the Rel-P, with the exception of glucose which remained elevated in Rel-P. The number of patients with newly discovered DM increased significantly in the Rel-P, and 79.5% of them had mild/moderate forms of COVID-19. During Lock and Res-P, the percentage of obese, smoking, or hypertensive patients increased, but probably through the use of telemedicine, we managed to reduce it, although it remained slightly higher than the pre-pandemic level. Physical activity decreased in the first year of the pandemic, but in Rel-P people became more active than before the pandemic. (4) Conclusions: The use of telemedicine for cardiovascular prevention seems to yield favorable results, especially for secondary prevention in the very high-risk group and during the second year. (1) Background: The COVID-19 pandemic has introduced a major disruption to the delivery of secondary prevention measures in patients with established atherosclerotic cardiovascular disease (CVD). It required a rapid and widespread adoption of new medical services, including the use of telemedicine. This study aimed to examine the impact of COVID-19 on secondary prevention in patients with coronary artery bypass grafting (CABG) and to evaluate the effectiveness of the telemedicine application for the implementation of lifestyle change measures, remote monitoring, and treatment regimen adjustment; (2) Methods: This prospective study on 194 CABG patients evaluated three consecutive years between 2019 and 2022 in the pre-pandemic period by face-to-face visits and during the pandemic by teleconsultations or hybrid follow-up. Variables of interest were compared between four periods: pre-pandemic—pre-P (1 March 2019–29 February 2020), lockdown—Lock (1 March–31 August 2020), restrictive-pandemic—Restr-P (1 September 2020–28 February 2021), and relaxed–pandemic—Rel-P (1 March 2021–1 March 2022). (3) Results: The average values of the lipidogram, blood sugar, and uric acid increased during Lock and Restr-P, but, through the use of teleprevention, they returned to the pre-pandemic level or even below this level. The exception was blood sugar, which remained high in Rel-P. The number of newly diagnosed patients with diabetes also increased, with most of them having moderate forms of COVID. During Lock and Res-P, the percentage of obese, smoking, or hypertensive patients increased, but, through the use of teleprevention, we managed to reduce it, although it remained slightly higher than the pre-pandemic level. Physical activity decreased in the first year of the pandemic, but, in Rel-P, CABG patients became more active than before the pandemic (4) Conclusions: The use of telemedicine for cardiovascular secondary prevention allowed us to not only continue seeing CABG patients but, also, to adjust their medication and to expand cardiovascular preventive counseling and testing with favorable results, especially during the second year of the pandemic. Cathelicidina LL37 este un peptid antimicrobicin aparţinând imunităţii înnăscute, implicat în modularea IFN-y, citokina cheie a răspunsului T helper de tip 1. Rolul LL37 în răspunsul inflamator din cursul hepatitelor virale este necunoscut\ Am evaluat variaţiile serice ale LL37 şi răspunsul Thl în infecţiile cu virus hepatitic C (HCV), cu virus hepatitis B (HBV) şi cu virus hepatitic D (HDV). Nivelul LL37 (analizat prin test ELISA) şi răs- punsul Thl (definit prin nivelul IFN-y şi numărul de celule CD4+ şi CD8+) au fost analizate la 87 pacienţi (65 cu hepatită (34 HCV, 18 HBV, 13 HDV) şi 22 martori sănătoşi. Subiecţii, 33 bărbaţi/54 femei, între 20-64 ani, au fost selectaţi la Institutul Matei Balş, Bucureşti, România, în septembrie 2011 -martie 2012. Pacienţii cu he- patită au fost analizaţi în funcţie de etiologie şi de activitatea virală (activi vs inactivi). S-a folosit testul t student şi Mann Whitney. In grupul total de hepatite au fost detectate nivele ridicate de LL37 (138,09±88,45ng/ml, p=0,045) şi IFN-y (69,82pg/ml, p=0,005). în hepatita C activă nivelul de LL37 a fost semnificativ crescut (155,15±78,84ng/ml, p=0,014) şi răspunsul Thl a fost predominant în raport cu forma inactivă. Dimpotrivă, pa- cienţii cu hepatită B activă au prezentat LL37 scăzut (76,75ng/ml, p=0,009), fără răspuns Thl dominant compa- rativ cu forma inactivă. în hepatita D s-au întâlnit nivele de LL37 crescute până la 171,01 ±72,08ng/ml şi un răs- puns moderat Thl. Rezultatele indică un răspuns imun puternic şi concordant în hepatita C activă şi nivele cres- cute de LL37 în toate hepatitele virale.

Background: Long-term survival after emergency colorectal cancer surgery is low, and its estimation is most frequently neglected, with priority given to the immediate prognosis. This study aimed to propose an effective nomogram to predict overall survival in these patients. Materials and methods: We retrospectively studied 437 patients who underwent emergency surgery for colorectal cancer between 2008 and 2019, in whom we analyzed the clinical, paraclinical, and surgical parameters. Results: Only 30 patients (6.86%) survived until the end of the study. We identified the risk factors through the univariate Cox regression analysis and a multivariate Cox regression model. The model included the following eight independent prognostic factors: age > 63 years, Charlson score > 4, revised cardiac risk index (RCRI), LMR (lymphocytes/neutrophils ratio), tumor site, macroscopic tumoral invasion, surgery type, and lymph node dissection (p < 0.05 for all), with an AUC (area under the curve) of 0.831, with an ideal agreement between the predicted and observed probabilities. On this basis, we constructed a nomogram for prediction of overall survival. Conclusions: The nomogram created, on the basis of a multivariate logistic regression model, has a good individual prediction of overall survival for patients with emergency surgery for colon cancer and may support clinicians when informing patients about prognosis. - Abstract
Surgical practice worldwide has changed rapidly in response to the coronavirus disease 2019 (COVID-19) pandemic. The study aimed to analyze the impact of the COVID-19 pandemic on mortality, in hospitalized patients, in Romanian surgical services. We have developed our research on a national survey of the consecutive records of hospitalizations, surgical interventions and deaths performed in Romania between March and August 2020. Results show that 47 surgical departments responded to the request. The admissions in the period March–August 2020 represented 52% of that from the similar period of 2019. In the studied period, the share of surgical interventions in patients admitted to non-COVID-19 centres was 98.7%, respectively 78.2% in COVID-19 support hospitals (p < 0.05), and emergency interventions of 43.4% in non-COVID-19 hospitals, respectively 84.8% in COVID-19 support hospitals (p < 0.05). Overall mortality in this period was 5.82%, compared to 3.28% in a similar period in 2019, (p < 0.05). Postoperative mortality in COVID-19-positive patients was 19%. In conclusion, in the hospitals in Romania included in the survey, the overall mortality in the studied period was higher than in a similar period in 2019. In patients with COVID-19 positive, the recorded postoperative mortality was higher than overall mortality. [ABSTRACT FROM AUTHOR]

Due to the localization of these tumors at the border between the stomach and the esophagus, the optimal surgical strategy for patients with adenocarcinoma of the esophagus junction is controversial. Evaluation of the influence of surgical approach on immediate and longterm results for patients with adenocarcinoma of esophagogastric junction. Between 2010-2017, 62 patients with locally advanced gastroesophageal junction adenocarcinomas were treated at St. Mary Hospital. The surgical approach was: abdominal in 40 patients -- mainly for patients with Siewert type III - 72.5%. Other types of surgical approach: abdominocervical in 4 patients, abdominothoracic - 5 patients, triple approach in 5 patients were mainly used for patients with Siewert type I (11 patients). Patients with Siewert type II had either abdominal (11 patients) or combined (7 patients). Postoperative morbidity was 46.77%. Anastomotic fistula occurred in 17 patients as follow: after abdominal approach in 10 patients, 2 patients after abdomino cervical approach, 2 patients after abdominothoracic approach, and 3 patients after triple approach. Postoperative mortality was 4.8%. The late complications were anastomotic stenosis in 14 patients. Anastomotic tumor recurrence was diagnosed in 7 patients. Survival analysis shows different survival curves in the way that the abdominocervical approach led to a reduced survival rate: 0% at 24 months, while the triple approach recorded a superior survival, 50% at 5 years. Intermediate curves I noticed after abdominal approach with 52.56% survival rate and 46.2% for abdominothoracic approach. Patients operated using abdominal approach had a significantly different mortality compared to patients operated by combined approach. The surgical approach did not influence the occurrence of postoperative complications - anastomotic fistula, fistula severity or pulmonary complications. [ABSTRACT FROM AUTHOR]

Tuberculosis (TB) is one of the most prevalent infectious diseases in the world, particularly in developing countries. The high incidence and mortality of TB in these countries mainly resides in the limited financial resources, the emergence of multiresistant strains and the spread of HIV infections. The key element for controlling TB is a rapid and early diagnosis which ensures the correct treatment and eradication of the infection source in the community. Current strategies for the development of a rapid and accurate diagnosis address 3 major objectives: improving the present diagnostic tools through better knowledge, developing new tests and increasing the accessibility to such diagnostic possibilities (Who Strategic Directions, 2006). The gold standard of diagnosis in TB is the identification of mycobacteria in the pathologic product using acid-fast smear microscopy and mycobacterial culture. Nevertheless the bacteriologic diagnosis is presently inefficient due to increasing number of smear-negative TB forms. This has led to the development of various complementary diagnostic methods such immunologic assays. The aim of the present chapter is to outline the literature data on the importance of the immunologic diagnosis in neurotuberculosis, a lethal localization of TB. It presents the current immunological assays and their practical value in the diagnosis of neurotuberculosis as complementary methods.
