Abstract

This paper presents a computational methodology for tracing the locations of Blade Vortex Interaction (BVI) phenomena on a helicopter rotor disk. This methodology utilizes Vortex Element Method for rotor free wake computations. Wake vortices are modeled by a series of discrete vortex elements and induced velocity on rotor disk is calculated for the distorted wake geometry, integrating Biot-Savart law in closed form over each one of them. Bound circulation variations and unsteady blade airloading as a result of the nonuniform induced downwash are computed by blade element method. Wake roll up process, vortex core modeling and elastic blade motion are some of the aerodynamic topics modeled with special care in the developed procedure. BVI locations are detected and the phenomena are categorized as parallel, perpendicular and oblique regarding their orientation relative to rotor blade. The corresponding intensity and locus for each category are calculated. By these means, specific BVI phenomena can be isolated and their influence on blade downwash and airloads distribution can be demonstrated. The computational results of BVIs effects on blade airloading are compared with experimental data. These data are derived from model-rotor wind tunnel tests, performed in the duration of joined European research programs. [ABSTRACT FROM AUTHOR]

Abstract

Aims: This international study aimed to assess: 1) the prevalence of preoperative and postoperative COVID-19 among patients with hip fracture, 2) the effect on 30-day mortality, and 3) clinical factors associated with the infection and with mortality in COVID-19-positive patients. Methods: A multicentre collaboration among 112 centres in 14 countries collected data on all patients presenting with a hip fracture between 1st March-31st May 2020. Demographics, residence, place of injury, presentation blood tests, Nottingham Hip Fracture Score, time to surgery, management, ASA grade, length of stay, COVID-19 and 30-day mortality status were recorded. Results: A total of 7090 patients were included, with a mean age of 82.2 (range 50–104) years and 4959 (69.9%) being female. Of 651 (9.2%) patients diagnosed with COVID-19, 225 (34.6%) were positive at presentation and 426 (65.4%) were positive postoperatively. Positive COVID-19 status was independently associated with male sex (odds ratio (OR) 1.38, p = 0.001), residential care (OR 2.15, p < 0.001), inpatient fall (OR 2.23, p = 0.003), cancer (OR 0.63, p = 0.009), ASA grades 4 (OR 1.59, p = 0.008) or 5 (OR 8.28, p < 0.001), and longer admission (OR 1.06 for each increasing day, p < 0.001). Patients with COVID-19 at any time had a significantly lower chance of 30-day survival versus those without COVID-19 (72.7% versus 92.6%, p < 0.001). COVID-19 was independently associated with an increased 30-day mortality risk (hazard ratio (HR) 2.83, p < 0.001). Increasing age (HR 1.03, p = 0.028), male sex (HR 2.35, p < 0.001), renal disease (HR 1.53, p = 0.017), and pulmonary disease (HR 1.45, p = 0.039) were independently associated with a higher 30-day mortality risk in patients with COVID-19 when adjusting for confounders. Conclusion: The prevalence of COVID-19 in hip fracture patients during the first wave of the pandemic was 9%, and was independently associated with a three-fold increased 30-day mortality risk.

Abstract

Real gas effects in high entry temperatures are taken into consideration combining flight conditions during hypersonic flight into Earth's atmosphere. Convective stagnation heating loads are estimated from the original full Fay-Riddell formula and compared with other engineering analytical methods. A new engineering correlation formula is proposed for wall stagnation temperature distribution during reentry motion. The efficiency of the applied analysis gives satisfactory results compared with published data of verified experiments and computational codes for the lifting Space Shuttle STS-40 and ballistic Apollo4 capsule missions. Abstract

Background: Cognitive impairment is common in multiple sclerosis (MS), but deficits tend to be more pronounced in progressive MS, negatively impacting daily functional capacity. Despite this, most cognitive rehabilitation (CR) interventions to date have focused on relapsing-remitting MS (RRMS). Moreover, information on the efficacy of CR in progressive MS is limited and controversial. The present study investigated the efficacy of a home based, computer assisted cognitive rehabilitation (HBCACR) intervention (RehaComTM software) exclusively in a Secondary Progressive Multiple Sclerosis (SPMS) sample. Methods: This was a randomized, multi site, sham controlled trial. Thirty six (36) individuals with SPMS, naïve to the RehaCom software, with cognitive deficits were randomized to the treatment (IG; n= 19) or control group condition (CG; n=17). Treatment with the RehaCom modules consisted of 24 domain and task specific, 45 minute session's over an 8-week period, three sessions per week, applied by each patient at home. The CG completed non specific computer based activities at home with the same frequency and duration. Primary cognitive outcome measures included the Brief International Cognitive Assessment for Multiple Sclerosis (BICAMS) battery, and secondary outcome measures for depression (BDI-FS), fatigue (MFIS), and quality of life (EuroQol EQ-5D) visual analogue scale (VAS). Abstract

Subtrochanteric fractures can result from high-energy trauma in young patients or from a fall or minor trauma in the elderly. Intramedullary nails are currently the most commonly used implants for the stabilization of these fractures. However, the anesthetic procedure for the patients, the surgical reduction and osteosynthesis for the fractures are challenging. The anesthetic management of orthopedic trauma patients should be based upon various parameters that must be evaluated before the implementation of any anesthetic technique. Surgery- and patient-related characteristics and possible comorbidities must be considered during the pre-anesthetic evaluation. Adequate fracture reduction and proper nail entry point are critical. Understanding of the deforming forces acting on various fracture patterns and knowledge of surgical reduction techniques are essential in obtaining successful outcomes. 31. Depressive mood in individuals with spinal cord injury (SCI) living in Greece

Abstract

Study design: Cross-sectional study. Objectives: To provide data on depressive symptoms rates in the Greek SCI population and to investigate their association with demographic and clinical variables. Setting: Greek territory. Methods: One hundred and sixty-four individuals with SCI living in the community for at least 1 year after the completion of the primary inpatient rehabilitation program were included in the study. Our group of participants were enrolled from multiple rehabilitation centers throughout Greece and were evaluated for probable depression according to the Patient Health Questionnaire (PHQ-9). Univariable and multiple linear regression analyses were performed to assess the possible association of risk factors with the occurrence of depression. We had also examined the correlation between PHQ-9 and scales measuring social reintegration (Craig Handicap Assessment and Reporting Technique (CHART)), quality of life (World Health Organization Quality of Life BREF (WHOQOL-BREF)), and independence (Spinal Cord Independence Measure (SCIM)). Results: It was found that 18.2% of the sample had probable depression. The mean (SD) PHQ-9 score was 5.7 (4.9). The multiple linear regression analysis showed that high pain scores (P = 0.001) and suffering from both nociceptive and neuropathic pain (P = 0.005) were associated with depressive mood, while pressure ulcers had a significant effect (P = 0.049) only in the univariable analysis. Participant’s PHQ-9 scores had also a negative correlation with almost all CHART, WHOQOL-BREF, and SCIM subscales’ scores. Conclusions: This study documents relatively low rates of probable depression among individuals with SCI in Greece. Severe pain and pressure ulcers were the main identified predictors of depressive mood. © 2018, International Spinal Cord Society.

Abstract

Nerve injuries around the shoulder are common. They usually occur as a result of motor vehicle acci-dents, gunshot or athletic injuries, or overuse and chronic repetitive stress often observed in sports involving repetitive overhead or throwing activities. The mechanisms of nerve injury include direct pressure, repetitive microtrauma, and compression-or stretch-induced ischemia. The severity and extent of compression is related to the degree of the nerve injury. Several nerve injury grading systems have been developed, allowing for the correlation between microscopic changes occurring after the injury and the patient’s symptoms; recovery and prognosis is related to the type and severity of the injury. Distinct clinical syndromes involving the shoulder joint area can develop after injuries to the axillary, musculocutaneous, suprascapular, long thoracic, and spinal accessory nerves, as well as the proximal brachial plexus trunks and the lower cervical nerve roots. When a patient complains of pain, weakness, or paresthesia in the absence of known bone, soft tissue, or vascular injury, nerve injury must be suspected. This article summarizes the most common nerve injuries around the shoulder and discusses the classification, mechanism, treatment, and prognosis for these injuries. © 2017 by Begell House, Inc.

34. Posterior shoulder fracture–dislocation: an update with treatment algorithm

Abstract

Posterior shoulder fracture–dislocation is a rare injury accounting for approximately 0.9 % of shoulder fracture–dislocations. Impression fractures of the articular surface of the humeral head, followed by humeral neck fractures and fractures of the lesser and grater tuberosity, are the more common associated fractures. Multiple mechanisms have been implicated in the etiology of this traumatic entity most commonly resulting from forced muscle contraction as in epileptic seizures, electric shock or electroconvulsive therapy, major trauma such as motor vehicle accidents or other injuries involving axial loading of the arm, in an adducted, flexed and internally rotated position. Despite its’ scarce appearance in daily clinical practice, posterior shoulder dislocation is of significant diagnostic and therapeutic interest because of its predilection for age groups of high functional demands (35–55 years old), in addition to high incidence of missed initial diagnosis ranging up to 79 % in some studies. Several treatment options have also been proposed to address this type of injury, ranging from non-surgical methods to humeral head reconstruction procedures or arthroplasty with no clear consensus over definitive treatment guidelines, reflecting the complexity of this injury in addition to the limited evidence provided by the literature. To enhance the literature, this article aims to present the current concepts for the diagnosis, evaluation and treatment of the patients with posterior fracture–dislocation shoulder, and to present a treatment algorithm based on the literature review and our own experience. © 2016, Springer-Verlag France.

38. Vibrational bone characteristics versus bone density for the assessment of osteoporosis in ovariectomized rats

Abstract

Our previous research findings suggested this integrated study in order to monitor changes of bone properties and assess bone integrity using vibrational characteristics in osteoporosis. The method is based on measurement of the bone dynamic characteristic modal damping factor (MDF). The experimental animal model is ovariectomized rat followed by alendronate treatment. According to the experimental design, adult female Wistar rats are ovariectomized and 60 days later, with confirmed osteoporosis, the population is divided into two groups. One is administered alendronate and the second is given no treatment. Furthermore, established techniques such as pQCT and histomorphometry are applied at all time points, in order to compare and correlate to MDF. The results indicate induction of osteoporosis due to ovariectomy and render MDF capable of monitoring changes in bone material properties and architecture, with high sensitivity and repeatability. © 2009 Informa UK Ltd.

Abstract

Meniscal tears are attributed to either trauma or degeneration processes. Clinical data suggest that meniscal degeneration (MD) is associated with knee osteoarthritis; however, the molecular events underpinning the pathogenesis of MD in humans remain elusive. Here we immunohistochemically examined the expression of p38 MAPK, its phosphorylated/activated form (p-p38), its target NF-κB (p50-p65 dimer), and COX-2 in ruptured menisci and investigated their involvement in MD development. Our findings demonstrate increased expression of the p38-NF-κB axis elements and COX-2 in disintegrated fibrocartilage, suggesting a role of these molecules in the pathobiochemistry of MD and consequential rupture.

40. Dry reagent dipstick test combined with 23S rRNA PCR for molecular diagnosis of bacterial infection in arthroplasty

Abstract

Periprosthetic joint infections present a challenging problem in orthopaedics. Conventional methods for detection of arthroplasty infections rely on bacterial culture of synovial fluid aspirates. During recent years, however, molecular tests that are based on DNA amplification by the polymerase chain reaction (PCR), followed by electrophoretic analysis of the products, have been introduced. We report a simple and inexpensive assay that allows visual detection and confirmation of the PCR-amplified sequences by hybridization within minutes. The assay is performed in a dry reagent dipstick format (strip) and does not require special instrumentation. Universal primers are used for PCR of the 23S ribosomal RNA (rRNA) gene. The biotinylated amplification product is hybridized with dA-tailed probes that are specific for six pathogens commonly involved in periprosthetic joint infections. The mixture is applied to the strip, which is then immersed in the appropriate buffer. The buffer migrates along the strip by capillary action and rehydrates gold nanoparticles with oligo(dT) strands attached to their surface. The nanoparticles bind to the target DNA through hybridization, and the hybrids are captured by immobilized streptavidin at the test zone of the strip, producing a characteristic red line. Unbound nanoparticles are captured by immobilized oligo(dT) strands at the control zone of the strip, generating a second line. The dipstick test was applied to the detection of Escherichia coli, Staphylococcus aureus, Staphylococcus epidermidis, Streptococcus pneumoniae, Enterococcus faesium, and Haemophilus influenza. Twelve samples of synovial fluids from patients were analyzed for the detection and identification of the infection caused by the six pathogens. Abstract

Quantification of muscle mass is important in clinical practice and several tools are used for its measurement. This is a review and critical appraisal of the muscle mass assessment tools for use with elderly patients in clinical practice. Of the 10 different tools described to measure skeletal muscle mass (SMM), computerized tomography (CT), and magnetic resonance imaging (MRI) are considered the gold standards. Dual-energy X-ray absorptiometry (DXA) is probably the best known method for measuring muscle mass in the elderly but because of the high cost of the equipment and its operation, its use may be limited. Bioelectrical impedance analysis (BIA) could provide a simpler, less expensive alternative, and it is portable. The use of anthropometrics (such as calf circumference and skin-fold thickness measurement) is feasible in the home setting. There is a lack of studies of the reliability of tools for measuring muscle mass in elderly patients. Additional research is needed to investigate how best to optimize measurement and minimize error. 44. Alteration in Serum Leptin Correlates with Alterations in Serum N-Telopeptide of Collagen type I and Serum Osteocalcin During the Progression of Osteoporosis in Ovariectomized Rats

Abstract

The role of leptin during the progression of osteoporosis was investigated in ovariectomized rats by correlation of serum leptin levels with N-telopeptide of collagen type I (NTx) and osteocalcin levels before ovariectomy and 20, 40 and 60 days after the operation. Furthermore, peripheral quantitative computed tomography was used to confirm the development of severe osteoporosis in rats on day 60. The levels of NTx and osteocalcin were significantly increased on day 20 [61.9± 5.4 nM BCE (bone collagen equivalents) and 215.6± 53.3 ng/mL, respectively] in comparison to those before ovariectomy (41.3± 1.7 nM BCE and 60.4± 10.9 ng/mL). Accordingly, leptin was significantly elevated on day 20 (3033± 661 vs. 606± 346 pg/mL before ovariectomy). Bone markers and leptin levels remained constant up to day 40, while a slight, but not statistically significant, decrease was noted for osteocalcin and leptin on day 60. Although leptin and bone markers did not correlate before ovariectomy (r = 0.09 for NTx and r= -0.05 for osteocalcin), strong correlation was observed at all time points after ovariectomy. The data obtained suggest that the alterations in serum leptin levels during the progression of osteoporosis in ovariectomized rats follow the alterations in bone markers. © 2005, by Walter de Gruyter Berlin New York.
