Abstract

Invasive pulmonary aspergillosis (IPA) is a life‐threatening complication in patients with severe fever with thrombocytopenia syndrome (SFTS), yet SFTS‐associated IPA (SAPA)'s risk factors remain undefined. A multicenter retrospective cohort study across Hubei and Anhui provinces (May 2013–September 2022) utilized least absolute shrinkage and selection operator (LASSO) regression for variable selection. Multivariable logistic regression identified independent predictors of SAPA, Cox regression highlighted mortality‐related risk factors. Of the 1775 screened SFTS patients, 1650 were included, with 169 developing IPA, leading to a 42‐day mortality rate of 26.6% among SAPA patients. Multivariable logistic regression revealed SAPA risk factors including advanced age, petechia, hemoptysis, tremor, low albumin levels, elongated activated partial thromboplastin time (APTT), intensive care unit (ICU) admission, glucocorticoid usage, intravenous immunoglobulin (IVIG) and prolonged hospital stays. Cox regression identified predictors of 42‐day mortality, including ecchymosis at venipuncture sites, absence of ICU admission, elongated prothrombin time (PT), vasopressor and glucocorticoid use, non‐antifungals. Nomograms constructed on these predictors registered concordance indexes of 0.855 (95% CI: 0.826–0.884) and 0.778 (95% CI: 0.702–0.854) for SAPA onset and 42‐day mortality, respectively. Lower survival rates for SAPA patients treated with glucocorticoids (p < 0.001) and improved 14‐day survival with antifungal therapy (p = 0.036). 27. Analysis of monthly variability of thermocline in the South China Sea

Abstract

This study analyzes monthly variability of thermocline and its mechanism in the South China Sea (SCS). The study is based on 51-year (1960–2010) monthly seawater temperature and surface wind stress data from Simple Ocean Data Assimilation (SODA), together with heat flux, precipitation and evaporation data from the National Centers for Environmental Prediction (NCEP), the National Oceanic and Atmospheric Administration (NOAA) and the Woods Hole Oceanographic Institution, respectively. The results reveal that the upper boundary depth (Zup), lower boundary depth (Zlow), thickness (ΔZ) and intensity (Tz) of thermocline in the SCS show remarkable monthly variability. Being averaged for the deep basin of SCS, Zupdeepens gradually from May to the following January and then shoals from February to May, while Zlowvaries little throughout the whole year. Further diagnostics indicates that the monthly variability of Zupis mainly caused by the buoyancy flux and wind stress curl. Using a linear method, the impacts of the buoyancy flux and wind stress curl on Zupcan be quantitatively distinguished. The results suggest that Zuptends to deepen about 4.6 m when the buoyancy flux increases by 1×10-5kg/(m∙s3), while it shoals about 2.5 m when the wind stress curl strengthens by 1×10-7N/m³.

28. A study of response of thermocline in the South China Sea to ENSO events

Abstract

This paper investigates the response of the thermocline depth (TD) in the South China Sea (SCS) to the El Niño-Southern Oscillation (ENSO) events using 51-year (from 1960 to 2010) monthly seawater temperature and surface wind stress data acquired from the Simple Ocean Data Assimilation (SODA), together with heat flux data from the National Centers for Environmental Prediction (NCEP), precipitation data from the National Oceanic and Atmospheric Administration (NOAA) and evaporation data from the Woods Hole Oceanographic Institution (WHOI). It is indicated that the response of the SCS TD to the El Niño or La Niña events is in opposite phase. On one hand, the spatial-averaged TDs in the SCS (deeper than 200 m) appear as negative and positive anomalies during the mature phase of the El Niño and La Niña events, respectively. On the other hand, from June of the El Niño year to the subsequent April, the spatial patterns of TD in the north and south of 12°N appear as negative and positive anomalies, respectively, but present positive and negative anomalies for the La Niña case. However, positive and negative TD anomalies occur almost in the entire SCS in May of the subsequent year of the El Niño and La Niña events, respectively. It is suggested that the response of the TD in the SCS to the ENSO events is mainly caused by the sea surface buoyancy flux and the wind stress curl.

Abstract

Abstract: Diurnal evolution of the barrier layer (BL) and its local feedback features in the central Taiwan Strait (119.2°E, 24.3°N) during summertime monsoon are investigated using in situ moored observations conducted by the “Yan-Ping 2” research vessel in late June 2005. During the initiation phase, for the non-solar radiation tends to be trapped in the upper mixed layer, whereas the solar radiation can penetrate deeply through the mixed layer approaching the thermocline, most heat is accumulated inside the BL inducing an inverse-thermal layer. Along with heat convergence inside the BL, thermal exchange increases between the BL and the overlaying mixed layer and finally, a prominently warming mixed layer is formed. Moreover, the BL is associated with a buoyancy frequency minimum with mild stability. Further analysis reveals that the BL’s local feedbacks can be divided into two aspects, on one hand, the BL can generate dramatic changes in the local sensible and latent heat fluxes; on the other hand, the sub-halocline and the thermocline serve as two interfaces during the downward transmission of the wind stirring turbulent kinetic energy (TKE) and as a result, most TKE is retarded by the shallow halocline and being trapped above the upper mixed layer, while the residual pierced through the base of the mixed layer is likewise blocked by the thermocline.

However, whether these patients ultimately benefit from this improvement remains unknown.Methods: In this retrospective, nested, case–control study, patients with severe hospital-acquired pneumonia (HAP) who had undergone mNGS of bronchoalveolar lavage fluid while in our intensive care unit from March 2017 to December 2020 (n = 33) were matched in a ratio of 1 to 2 (n = 66) by sex, age, comorbidities, immune status, Acute Physiology and Chronic Health Evaluation II score, severity of pulmonary infection, and use of extracorporeal life support with patients who had undergone conventional microbiological testing only. The primary outcome was 90-day mortality; secondary outcomes being length of intensive care unit stay, duration of mechanical ventilation support, 7-day and 28-day mortality, and efficacy of treatment of pulmonary infection.Results: In the CMT group, 17 patients (25.8%) had negative results, whereas only one (3.0%) had n

Abstract

Organ transplantation is an effective treatment for end-stage organ failure. However, organ shortage has always been a common problem faced by countries around the world. The recognition and active participation of intensive care unit (ICU) medical staff in organ donation contributes to promoting the development of organ donation, thereby alleviating the shortage of donor organ. In this article, the key strategies of ICU donor management to promote organ donation and the key strategies of ICU medical staff management to promote organ donation were summarized, aiming to provide reference for organ donation practitioners (especially ICU medical staff) and jointly facilitate the professional development of organ donation.
