Intimate partner violence (IPV) affects 1 in 3 women and has intensified during the COVID-19 pandemic. Although most injuries are to the head, face, and neck, leaving survivors vulnerable to sustaining traumatic brain injury (TBI), the intersection of IPV and TBI remains largely unrecognized. This article reports on COVID-19–related effects, barriers, needs, and priorities to health care and support services for women survivors of IPV-TBI. Using a participatory research model, we engaged 30 stakeholders in virtual meetings drawn from an IPV-TBI Knowledge to Practice Network in two virtual meetings. Stakeholders included women survivors, service providers, researchers, and decision makers across the IPV, TBI, and healthcare sectors. Data were gathered through small group breakout sessions facilitated by the research team using semistructured discussion guides. Sessions were recorded, transcribed verbatim, and analyzed using thematic analysis techniques. Stakeholders were given the opportunity to contribute to the analysis and knowledge transfer through member checking activities. Ethics approval was obtained through the University of Toronto. Stakeholders shared that COVID-19 has increased rates and severity of IPV and barriers to services and help-seeking. These effects have been exacerbated by infrastructure difficulties in rural and remote areas, including limited access to services. They noted the need to carefully consider implications of virtual care such as safety, privacy, and usability. Requests from survivors for peer support have increased significantly, indicating a need for more formalized and better-supported peer roles. Stakeholders further noted that an overwhelming lack of awareness of the intersection of IPV-TBI continues. Increasing education and awareness among health care and IPV service providers, survivors, and the public remains a priority. The COVID-19 pandemic has intensified IPV-TBI, increased challenges for women survivors, and accentuated the continued lack of IPV-TBI awareness. Key recommendations for health care and rehabilitation to address this priority are discussed.

IntroductionTraffic injury is a leading and preventable cause of child death and disability, with child pedestrians and cyclists particularly vulnerable. Examining built environment correlates of child pedestrian and cyclist motor vehicle collisions (PCMVC) in different settings is needed to promote an evidence-based approach to road safety.MethodsWe conducted a cross-sectional study across multiple urban/suburban environments in Canada (Calgary, Toronto, Montreal, Laval, Peel Region). All public elementary schools were included (n=1030). We examined the role of land use/social environments, road environments and traffic safety interventions on the rates of child PCMVC within 1000 m of schools. Multivariable negative binomial regression was conducted for all cities and by individual city. In a subset of schools (n=389), we examined associations when controlling for active school transportation (AST).ResultsMean PCMVC rate per school ranged from 0.13 collisions/year in Peel to 0.35 in Montreal. Child PCMVC were correlated with land use, social and road environments and traffic safety interventions. In fully adjusted models, social and land use features remained the most important correlates. New immigrant population had the largest positive association with child PCMVC (incidence rate ratio (IRR): 1.26, 95% CI 1.06 to 1.50), while old housing (pre-1960) density was most protective (IRR: 0.83, 95% CI 0.77 to 0.90). AST was associated with PCMVC, but it had no effect on the relationships between PCMVC and other social/environmental correlates.ConclusionThe built environment and social factors influence rates of child PCMVC. Opportunities to reduce child PCMVC exist through modifications to city design and road environments and implementing traffic safety interventions.

This chapter will provide a summary of high-level details regarding Vision Zero implementation in Canada, looking specifically at research, strategies, and implementation experiences in British Columbia, Edmonton, Calgary, and Fort Saskatchewan in Alberta, Toronto in Ontario, and Montreal in Quebec. This chapter will speak to the differences between Vision Zero implementation in Canada compared with Sweden, considering the viewpoint and circumstances of the unique governmental structure in Canada and implementation in municipalities versus entire provinces or territories. Priorities for the future of Vision Zero will also be discussed, along with the intersections and role of public health and other applications of Vision Zero.

Road traffic injury and death continue to be a concern in Canada. The built environment (BE) is a contributing factor affecting the health of road users, yet there are significant challenges to making injury-reducing BE changes. This research increases our knowledge of these challenges by investigating the opinions of injury prevention and road safety professionals working in Canada about the barriers and facilitators to BE change. Cross-referencing themes with the CFIR situated our findings within the scope of implementation science, which demonstrated how barriers and facilitators influence BE change project implementation. The prioritization of motor vehicles results in disproportionate injury and health risks for VRUs. Collaboration across sectors, with the support of champions and advocates, can facilitate sharing of resources, data, and expertise, which results in more opportunities to enact BE change. Knowledge of these barriers and facilitators, contextualized by the CFIR, makes a case for policy/decision-makers in Canada to approve BE projects that reduce risk of road traffic injury and death.

BackgroundSchool safety zones were created in 2017 under the City of Toronto’s Vision Zero Road Safety Plan. This pilot study examined the effect of built environment interventions on driver speeds, active school transportation (AST) and dangerous driving.MethodsInterventions were implemented at 34 schools and 45 matched controls (2017–2019). Drivers travelling over the speed limit of >30 km/hour and 85th percentile speeds were measured using pneumatic speed tubes at school frontages. Observers examined AST and dangerous driving at school arrival times. Repeated measures beta and multiple regression analyses were used to study the intervention effects.ResultsMost schools had posted speed limits of 40 km/hour (58%) or ≥50 km/hour (23%). A decrease in drivers travelling over the speed limit was observed at intervention schools post-intervention (from 44% to 40%; OR 0.79, 95% CI 0.66 to 0.96). Seventy-one per cent of drivers travelled >30 km/hour and the 85th percentile speed was 47 km/hour at intervention schools, with no change in either postintervention. There were no changes in speed metrics in the controls. AST increased by 5% (OR 1.22, 95% CI 0.97 to 1.54) at intervention schools. Reductions in dangerous driving were observed at all schools.ConclusionsPosted speed limits were >30 km/hour at most schools and high proportions of drivers were travelling above the speed limits. There were reductions in drivers exceeding the speed limit and in dangerous driving, and modest increased AST post intervention. Bolder interventions to slow traffic are required to effectively reduce speeding around schools, which may increase safe AST.

COVID-19 restrictions led to reduced levels of physical activity, increased screen usage, and declines in mental health in youth, however, in-depth understandings of the experiences of high school student-athletes have yet to be explored. To describe the experiences of the COVID-19 pandemic on student-athletes’ physical activity, social connection, and mental health, 20 high school student-athletes living in Calgary, Alberta participated in semi-structured interviews, designed using phenomenography. Participants reported variations in physical activity, social connections, and mental health which were influenced by stay-at-home restrictions and weather. Access to resources, changes to routines, online classes, and social support all influenced engagement in physical activity. School and sports provided opportunities for in-person social connections, impacted by the onset of the pandemic. Participants reported their mental health was influenced by social connections, online classes, and physical activity. Findings from this study will inform the development of resources for high school student-athletes amidst COVID-19.

To reflect on the role of risk-taking and risky play in child development and consider recommendations for the injury prevention field, a symposium was held prior to the November 2013 Canadian Injury Prevention and Safety Promotion Conference. Delegates heard from Canadian and international researchers, practitioners and play safety experts on child development, play space design and playground safety, provision of recreation, and legal and societal perceptions of risk and hazard. The presenters provided multidisciplinary evidence and perspectives indicating the potential negative effect on children's development of approaches to injury prevention that prioritise safety and limit children's opportunities for risky play. Delegates considered the state of the field of injury prevention and whether alternative approaches were warranted. Each presenter prepared a discussion paper to provide the opportunity for dialogue beyond attendees at the symposium. The resulting discussion papers provide a unique opportunity to consider and learn from multiple perspectives in order to develop a path forward.

AimTo undertake a comprehensive review of the best available evidence related to risk factors for child pedestrian motor vehicle collision (PMVC), as well as identification of established and emerging prevention strategies.MethodsArticles on risk factors were identified through a search of English language publications listed in Medline, Embase, Transport, SafetyLit, Web of Science, CINHAL, Scopus and PsycINFO within the last 30 years (~1989 onwards).ResultsThis state-of-the-art review uses the road safety Safe System approach as a new lens to examine three risk factor domains affecting child pedestrian safety (built environment, drivers and vehicles) and four cross-cutting critical issues (reliable collision and exposure data, evaluation of interventions, evidence-based policy and intersectoral collaboration).ConclusionsResearch conducted over the past 30 years has reported extensively on child PMVC risk factors. The challenge facing us now is how to move these findings into action and intervene to reduce the child PMVC injury and fatality rates worldwide.

Walking and bicycling to school (active school transportation, AST) has been in decline for decades in North America and globally with the rise of automobility. This cross-sectional study estimated associations between the built environment and AST in seven Canadian communities. We observed the travel behaviours of almost 118,000 students at 552 schools. Using beta regression, we modeled the proportion of children using AST, considering built environment and social environment factors around schools. Across all schools, the average proportion of children using AST was 54.3% (SD 18.9%), with variability among cities from a low of 39.5% (SD 22.1%) in Laval, Quebec to 69.7% (SD 18.1%) in Montreal, Quebec. Overall, several modifiable road design features were associated with AST, including the presence of school crossing guards, cycling infrastructure, Walk Score® and traffic signal density. There was variability in the directionality and statistical significance of associations with design variables across cities, suggesting that the local context and directed local interventions are important to support AST. Natural experiment studies are necessary to examine local approaches related to the built environment to increase AST and ensure appropriate new policy and program interventions are developed.

Background Injuries are the leading cause of death for British Columbians during their first four decades of life costing $3.7 billion annually. Lack of transfer of national and provincial injury prevention efforts to the grass-root level and underutilization of proven preventive strategies at this level has been identified as one of the barriers for injury prevention in BC. Objective To implement a community mobilization outreach program to strengthen injury prevention efforts at the grass roots level. Methods A rapid environmental assessment was completed together with the development of an online directory mapping existing and potential injury prevention stakeholders and community groups working on injury prevention priorities across BC. This information was used to facilitate consistent communication, networking and collaboration, and to increase the efficiency and effectiveness of injury prevention initiatives. Findings The online directory served as a hub for forging new connections and greater collaboration among the stakeholder and community groups. Connecting and activating these channels enhanced community involvement in injury prevention initiatives. Provincial health authorities and community involvement in injury prevention programs such as, Safe Kids Week and National Teen Driver Safety Week increased significantly, ranging from 2- to 6-fold increases during the implementation of this grass-roots initiative. As well, a number of new organizations joined to support the effort. Conclusion Coordinating injury prevention efforts at the community level throughout the province has allowed stakeholders to co-build programs that are mutually supportive, reducing duplication of projects and programs and which facilitated the efficient allocation of limited resources. Policy implications More efforts should be taken to strengthen community network engagement and stakeholder relations. The evidence from this project indicates that a community mobilization outreach program can have significant positive effect on injury prevention efforts.

Concussion has emerged as an important public health issue affecting thousands of Canadians annually. Health care providers including paediatricians, family and emergency medicine physicians, nurses, and nurse practitioners are commonly tasked with the responsibility of providing primary care to patients with acute concussion and those with persistent post-concussion symptoms. In July 2017, Parachute, in collaboration with the Public Health Agency of Canada and Sport Canada released the Canadian Guideline on Concussion in Sport that outlines a standardized and evidence-based approach to the recognition, diagnosis, and management of youth and adults with suspected concussion. In this report, we provide a brief overview of the important roles of primary care providers in the medical assessment, management, and prevention of concussion as outlined in this national best practice guideline.

Child and youth injury prevention research in Canada has lagged behind other Organisation for Economic Co-operation and Development nations, despite existing surveillance systems and longitudinal data. A critical need to improve access to the available data, as well as need to tailor its display and interpretation, was identified by injury prevention stakeholders involved in research, policy, and practice. The Canadian Atlas of Child and Youth Injury Prevention (“the Atlas”) was developed to address this need. Following a series of iterative consultation meetings and a pilot testing session, the Atlas was scaled up with national data. Two testing sessions were held to evaluate the tools. The Atlas is comprised of three main components: data, indicators, and visualizations. The accessibility of the dashboard is enhanced by customization of data visualizations and data outputs to suit the user’s needs. Overall feedback indicated that the tools were easy to use, and that the interface was intuitive and visually appealing. The Canadian Atlas of Child and Youth Injury Prevention provides readily accessible information to injury prevention practitioners, policy makers and researchers, helping to chart pathways to success in improving the child and youth injury prevention system in Canada.

Background and Purpose Child safety campaigns play an important role in disseminating injury prevention information to families. A critical discourse analysis of gender bias in child safety campaign marketing materials can offer important insights into how families are represented and the potential influence that gender bias may have on uptake of injury prevention information.Methods Our approach was informed by poststructural feminist theory, and we used critical discourse analysis to identify discourses within the poster materials. We examined the national Safe Kids Canada Safe Kids Week campaign poster material spanning twenty years (1997-2016). Specifically, we analyzed the posters’ typeface, colour, images, and language to identify gender bias in relation to discourses surrounding parenting, safety, and societal perceptions of gender.Results The findings show that there is gender bias present in the Safe Kids Week poster material. The posters represent gender as binary, mothers as primary caregivers, and showcase stereotypically masculine sporting equipment among boys and stereotypically feminine equipment among girls. Interestingly, we found that the colour and typeface of the text both challenge and perpetuate the feminization of safety.Discussion It is recommended that future child safety campaigns represent changing family dynamics, include representations of children with non-traditionally gendered sporting equipment, and avoid the representation of gender as binary. This analysis contributes to the discussion of the feminization of safety in injury prevention research and challenges the ways in which gender is represented in child safety campaigns.

The objective of this study was to survey Canadian parents on their attitudes and beliefs about bicycle helmet legislation and to compare responses from parents living in provinces with and without legislation.A national survey of 1002 parents of children aged under 18 years was conducted. Chi-square tests were used to compare responses from the surveyed parents in the different jurisdictions.Responses from parents living in provinces with legislation (n = 640) and without legislation (n = 362) were as follows: concern for injury (63% vs. 68%, nonsignificant [NS]); believe helmets are effective (98% vs. 98%, NS); child always wears a helmet (74% vs. 69%, NS); support legislation for children (95% vs. 83%, p.001); support legislation for all ages (85% vs. 75%, p.001); support police enforcement (83% vs. 76%, p = .003); believe legislation decreases the amount of time their child bicycles (5% vs. 8%, NS).Parents are highly supportive of bicycle helmet legislation in Canada. They believe that bicycle helmets are effective and that legislation does not decrease the amount of time a child spends bicycling. Canada is consistently ranked as one of the best places to live in the world. A crucial part of this view is based on Canada's approach to public health, which has achieved measurable results in the rate reduction of some leading causes of disease and death. It is therefore surprising to learn that in tackling the leading cause of death for Canadian children and youth, Canada ranks a disappointing 18th of 26 nations in the Organisation for Economic Co-operation and Development (UNICEF 2001). Few are aware that unintentional injury is the leading cause of death for Canadian children and youth between the ages of one and 14. In Canada, injury kills more children and youth than all disease (Canadian Institutes of Health Research 2008). Unintentional injuries are a leading public health issue that directly impacts the health, well-being and quality of life of those injured, as well as their families, communities and greater society. Nevertheless, injury is often neglected, and investment is rarely equal to the magnitude of the problem. The reality is that injury prevention has not kept pace with other public health interventions such as tobacco control or infectious disease prevention programs. Risky outdoor play has been associated with promoting children’s health and development, but also with injury and death. Risky outdoor play has diminished over time, concurrent with increasing concerns regarding child safety and emphasis on injury prevention. We sought to conduct a systematic review to examine the relationship between risky outdoor play and health in children, in order to inform the debate regarding its benefits and harms. We identified and evaluated 21 relevant papers for quality using the GRADE framework. Included articles addressed the effect on health indicators and behaviours from three types of risky play, as well as risky play supportive environments. The systematic review revealed overall positive effects of risky outdoor play on a variety of health indicators and behaviours, most commonly physical activity, but also social health and behaviours, injuries, and aggression. The review indicated the need for additional “good quality” studies; however, we note that even in the face of the generally exclusionary systematic review process, our findings support the promotion of risky outdoor play for healthy child development. These positive results with the marked reduction in risky outdoor play opportunities in recent generations indicate the need to encourage action to support children’s risky outdoor play opportunities. Policy and practice precedents and recommendations for action are discussed.

Background Policies to reduce injury among Canadians can be controversial and there is variability in the enactment of injury prevention laws across the country. In general, laws are most effective when they are based on good research evidence, supported by widespread public awareness and education, and maintained by consistent enforcement strategies. The purpose of this study was to document and compare key informants’ perceptions of the quality, awareness, and enforcement of three evidence-based paediatric injury prevention policies (bicycle helmet legislation, child booster seat legislation, graduated driver licensing) among Canadian provinces and territories. Methods We identified best practices related to each policy, then developed an online survey to ascertain the extent to which each jurisdiction’s policy aligned with best practices, whether experts believed that the public was aware of the policy and whether it was enforced. The survey was distributed using a snowball sampling strategy to key informants across Canada. Results Thirty-eight key informants responded to the bicycle helmet survey, with 73 and 35 key informants for the booster seat and graduated driver licensing surveys, respectively. Respondent’s perceptions of the policies varied substantially. Key informants indicated that residents are not always aware of legislation, and legislation is not consistently enforced. These results suggest that child health policy is not always guided by evidence. Conclusions There was variation between evidence and the policies related to paediatric injury prevention among Canadian provinces and territories. Experts generally rate their policies more highly when they align with evidence and best practice. There is room for improvement and harmonization of injury prevention policies. Unintentional injuries are the leading cause of death for children in Canada. Injury accounts for 30% of all deaths and the largest environmental burden of disease. In other words, approximately 25 children die from an injury every month, the equivalent of one classroom. The good news? Unintentional injuries to children are predictable and preventable. The majority of child and youth injuries are preventable. This statement provides background, direction and a statement of commitment to the issue of child and youth injury prevention in Canada. It acts as a foundation to build upon by focusing first on definitions, scope and priorities for injury prevention. It also describes the burden and patterns of unintentional injury, and the principles of effective intervention for prevention. A list of resources for obtaining data and evidence-based information is included in the full-text version of this statement (www.cps.ca). This statement can also be used for broad-based injury prevention advocacy.

Background Injury prevention policy is crucial for the safety of Canada’s children; however, legislation is not adopted uniformly across the country. Researchers and policy makers must work together to develop effective legislation that is evidence-based but little is known regarding policy makers views regarding the importance of research in facilitating the legislative process. Methods Purposive snowball sampling identified individuals involved in injury prevention practice and policy throughout Canada. In an online survey, respondents identified injury topics relevant to them and rated the importance of enablers to injury legislation using a 5 point Likert scale. Results There were 57 respondents with representation from all 10 provinces. The most common topics were, bicycle helmets (77%), cell phone-distracted driving (63%), booster seats (49%), ski helmets (42%), and graduated driver’s licensing (37%). The most frequently identified enabler was that research/surveillance was readily available (59%). Other commonly reported research enablers were: research of sufficient quality/quantity that was easy to understand and in a useful format and affiliation of researchers with reputable organisations. Less important was researchers having similar priorities as policy makers and understanding the policy process. The importance of different research enablers varied by injury topic. Conclusions Although policy makers identified the importance that injury prevention research was readily available, it appeared to be less important that researchers had similar priorities or understood the policy process, with variability by topic. This presents a challenge for researchers to conduct timely research and emphasises the need for ongoing relationships with policy makers with discussions early in the research process. This would facilitate the development of common injury prevention priorities to ensure research is used effectively in the legislative process.

Safe Kids Canada launched the Ethno-Cultural Program targeted towards providing injury prevention resources for diverse communities in Canada. Goals To establish a national injury prevention programme that translates knowledge to diverse population in Canada. Identified communities have access to culturally appropriate injury prevention resources for the purpose of influencing behaviour to reduce the burden of injury. To establish collaborative relationships with appropriate key stakeholders, such as ethno-cultural agencies who serve diverse communities. The programme has successfully developed and distributed culturally relevant safety messages for parents and community partners for the purpose of raising awareness and reducing child injuries within ethnic communities in the Greater Toronto Area – Chinese, Portuguese and Punjabi – but can be utilised across Canada. This programme sought to go beyond simple translation from one language to another, by instead engaging the communities who would benefit from injury prevention outreach in the assessment and development of the final resources. Key learnings showed that this method was effective and welcomed by the communities because of their involvement and the collaborative approach we adopted. Extensive cultural media outlets picked up the information and resulted in two million media impressions, 45 000 resources distributed and 100 community leaders engaged who now have increased capacity to provide culturally appropriate resources. This presentation will share the focus group formats, important lessons learned and the resulting resources now available in communities.

With concerns increasing around childhood obesity and inactivity, playgrounds offer a chance for children to be active. But playgrounds also have risks, with injuries from falls being the most common. Research has shown that playground injuries can be reduced by lowering the heights of play equipment and using soft, deep surfaces to cushion falls. The Canadian Standards Association has published voluntary standards for playgrounds to address these risks for several years. Parents can further reduce injury risks by following simple playground strategies. This statement outlines the burden of playground injuries. It also provides parents and health care providers with opportunities to reduce injury incidence and severity through education and advocacy, and to implement evidence-informed safety standards and safer play strategies in local playgrounds. This document replaces a previous Canadian Paediatric Society position statement published in 2002.

The Vancouver Charter on Skiing Safety is based on skiing and snowboarding safety and the Turin Charter (approved in the context of the 2006 Turin Winter Olympic Games) and was brought to Canada by Safe Kids Canada. Originally prepared by a panel of experts and specialists delegated by European Governments who worked under the coordination of TOROC (Turin Organizing Committee), BE.PRA.S.A. (a project co-financed by the European Commission and the health authority of the Italian region of Veneto) and the Italian National Health Institute, the aim of the Charter is to promote a safe, healthy and active sporting and recreational culture, and to make its principles visible to the public, citizens and institutions. Over 30 international, national, provincial and municipal organisations have endorsed the Charter to date. The Charter was introduced at a Helmet Forum in November 2009 in Vancouver and featured experts such as Dr Charles Tator, Dr Richard Stanwick, Dr Brent Hagel and Mr Matt Herman. The forum was attended by injury prevention experts, ski/snowboard industry representatives and media and resulted in extensive national media coverage of the issue, especially the diversity of opinions and debates it has created. This presentation will show Safe Kids Canada9s experience of how the Charter can be leveraged by other countries, review the challenges and successes, present our knowledge translation ‘recipe for success’ and update the audience on activities to date.
