Abstract

Oral inverted ductal papilloma (OIDP) is a rare, nonrecurrent,benign lesion of salivary glands. The etiologyis still poorly understood; the correlation with humanpapilloma virus (HPV) is controversial. Herein wepresent a 74-year-old man with a tumor in lower lip.Incisional biopsy was performed and the histologicaldiagnosis was OIDP. Inno-LiPA assay, based onpolymerase chain reaction and in situ hybridizationwas used to assess for HPV with no detection of viralDNA. Surgical excision was performed without anyrecurrences after two years of follow-up.

Abstract

Background. Torquetenovirus (TTV) is a small DNA virus constituting the human virome. High levels of TTV-DNA have been shown to be associated with immunosuppression and inflammatory chronic disorders. Aim. To assess the possible association between the salivary viral load of TTV-DNA in patients hospitalised due to COVID-19 and disease severity. Methods. Saliva samples collected from 176 patients infected with SARS-CoV-2 were used to investigate the presence of SARS-CoV-2 and TTV-DNA by use of real-time RT-PCR. Results. The majority of patients were male with severe COVID-19. Presence of SARS-CoV-2 was observed in the saliva of 64.77% of patients, showing TTV-DNA in 55.68% of them. Patients with impaired clinical conditions (p < 0.001), which evolved to death (p = 0.003), showed a higher prevalence of TTV-DNA. The median viral load in patients with severe condition was 4.99 log10 copies/mL, in which those who were discharged and those evolving to death had values of 3.96 log10 copies/mL and 6.27 log10 copies/mL, respectively. A statistically significant association was found between the distribution of TTV-DNA viral load in saliva samples and severity of COVID-19 (p = 0.004) and disease outcomes (p < 0.001). Conclusions. These results indicate that TTV-DNA in saliva could be a useful biomarker of COVID-19 severity and prognosis. [ABSTRACT FROM AUTHOR]

Abstract

Gastrointestinal cancers are frequently associated with chronic inflammation and excessive secretion of IL-6 family cytokines, which promote tumorigenesis through persistent activation of the GP130/JAK/STAT3 pathway. Although tumor progression can be prevented by genetic ablation of Stat3 in mice, this transcription factor remains a challenging therapeutic target with a paucity of clinically approved inhibitors. Here, we uncovered parallel and excessive activation of mTOR complex 1 (mTORC1) alongside STAT3 in human intestinal-type gastric cancers (IGCs). Furthermore, in a preclinical mouse model of IGC, GP130 ligand administration simultaneously activated mTORC1/S6 kinase and STAT3 signaling. We therefore investigated whether mTORC1 activation was required for inflammation-associated gastrointestinal tumorigenesis. Strikingly, the mTORC1-specific inhibitor RAD001 potently suppressed initiation and progression of both murine IGC and colitis-associated colon cancer. The therapeutic effect of RAD001 was associated with reduced tumor vascularization and cell proliferation but occurred independently of STAT3 activity. We analyzed the mechanism of GP130-mediated mTORC1 activation in cells and mice and revealed a requirement for JAK and PI3K activity but not for GP130 tyrosine phosphorylation or STAT3. Our results suggest that GP130-dependent activation of the druggable PI3K/mTORC1 pathway is required for inflammation-associated gastrointestinal tumorigenesis. Abstract

Objectives: To evaluate the oral shedding of herpesviruses in patients undergoing hematopoietic stem cell transplantation (HSCT) and correlate it with oral mucositis (OM). Methods: Saliva samples were collected before the HSCT and on day D + 8. Multiplex Polymerse Chain Reaction (PCR) was performed to detect herpes simplex virus (HSV)‐1 and HSV‐2, Epstein‐Barr virus (EBV), Cytomegalovirus (CMV), Variella‐zoster virus (VZV), and human herpesvirus (HHV)‐6, HHV‐7, and HHV‐8. OM was assessed according to WHO criteria. Results: Thirty one patients were enrolled, in which 20 of 31 (64.5%) were males; median age was 50 (21–70) years; 16 of 31 (51.6%) underwent allo‐HSCT; and 15 of 31 (48.4%) underwent auto‐HSCT. On D + 8, OM grades III and IV were observed in 8 of 31 (25.8%) patients. In the first salivary collection, EBV was found in 24 of 31 (77.4%), followed by HHV‐6 (7/31, 22.6%) and HHV‐7 (8/31 25.8%). In the second collection, EBV was found in 24 of 27(89%), followed by HSV‐1 (8/27, 30%) and CMV, HHV‐6, and HHV‐7 (5/27, 18.5%, each one). On D + 8, OM grades II and IV were associated with the presence of HSV‐1. HSV‐1 was also associated with worsening degrees of OM on D + 15. Conclusion: The presence of HSV‐1 and CMV in oral samples was more frequent on day D + 8 after HSCT. HSV‐1 detection was associated with severity and worsening of OM. Abstract

The treatment for patients with metastatic colorectal cancer has progressively improved over the past few decades with the development of more effective anti-cancer drugs and multi-disciplinary management approaches that combine sequential lines of non-cross-resistant drugs and increased use of potentially curative surgery for metastases of the liver and lung. In this setting, the introduction of monoclonal antibody therapies that target the epidermal growth factor receptor (EGFR) (cetuximab and panitumumab) has made an important contribution to improved patient outcomes. However, the efficacy of therapies is generally limited to a small proportion of patients and associated with toxicity and high cost. There is an urgent clinical need for robust predictive biomarkers to guide the effective use of therapy options. In this chapter we review clinical and molecular predictive markers of primary therapy response for metastatic colorectal cancer, focusing on anti-EGFR antibody therapies, discussing both currently approved and emerging biomarkers.
