2. Intervención mindfulness de rehabilitación de un deportista lesionado : caso en fútbol profesional

El gran número de lesiones en el deporte y concretamente en fútbol se está convirtiendo en un problema de interés prioritario para los profesionales que trabajan con deportistas. El estudio de la epidemiologia de la lesión y de las complejas variables que intervienen el proceso de recuperación del deportista lesionado nos obliga a incorporar nuevas técnicas de intervención. En este estudio se propone una intervención basada en mindfulness o atención plena como complemento al protocolo de rehabilitación de un futbolista profesional lesionado. La propuesta se desarrolla en ocho sesiones donde se trabajan aspectos teóricos y prácticos y se evalúan variables psicológicas y fisiológicas antes durante y después de la intervención. Aceptando las limitaciones que presenta un estudio aplicado de caso único, podemos sugerir que, con los resultados obtenidos, este tipo de intervenciones pueden ser de utilidad en la rehabilitación de deportistas profesionales lesionados; ya que se observa una mejora de la percepción subjetiva del estado de ánimo durante la intervención y aumento de coherencia cardiaca entre otros componentes. La evaluación de seguimiento mostró la importancia de la fase de incorporación a la competición por la carga emocional que pudimos registrar. Se deben mejorar y ampliar la investigación en este ámbito de aplicación. The big number of sport injuries, especially in soccer, is becoming an important problem for the professionals who work with sportists. The study of sport's injuries epidemiology and the complexity of the variables that are present in the sportist's recuperation process make us incorporate new intervention techniques. In this study a mindfulness-based intervention is proposed as a complement to rehabilitation of an injured soccer player. Through eight sessions theorical and practical aspects are worked and psychological and physiological variables are evaluated during and after the intervention. With the limitations of a study like this, we can suggest that this kind of interventions can be useful in rehabilitation of injured sportists. There is an improvement in the state's perception and in the cardiac coherence among other variables. The follow-up evaluation showed the importance of the back to competition phase because of the emotional load we were able to record; so it is recommended to increase the research in this field.

The Erice 2018 Charter was unanimously approved at the conclusion of the 53rd Residential Course of the International School of Epidemiology and Preventive Medicine “Adapted Physical Activity in Sport, Wellness and Fitness; the role of the Departments of Prevention and of the National Health Service in doping prevention and health promotion”, held on 15-19 May 2018 in Erice, Italy, at the “Ettore Majorana” Foundation and Centre for Scientific Culture, and promoted by the Study Group on “Movement Sciences for Health” of the Italian Society of Hygiene, Preventive Medicine and Public Health. The event was part of a larger project supported by the Ministry of Health aimed at preventing doping in the general population involved in sport and physical activities. After an intense discussion the participants focused on ten statements involving the following critical issues: responsibility, priority, message, alphabetization, networks and alliances, school promoting health, player and opportunities, competences, know-how, programming and acting. These statements provide hints to approach doping within a public health frame and summarize the role of the Departments of Prevention and NHS in promoting and coordinating preventive actions with other institutions and stakeholders. Doping represents a complex phenomenon related to cultural, social, economic and legal issues. In addition to regulatory or repressive actions, education to health and legality is proposed as the fundamental strategy to contrast doping by promoting healthy lifestyles, based on scientific knowledge and respect for legality.

Abstract Background Ethanol is the most widely used drug in the world and a human teratogen whose consumption among women of childbearing age has been steadily increasing. There are no Italian or Spanish statistics on ethanol consumption during pregnancy nor any information regarding prevalence of fetal alcohol syndrome (FAS) and fetal alcohol spectrum disorders (FASD). There is also a reasonable suspicion that these two diseases are underdiagnosed by professionals from the above-reported countries. The objectives of this study were: 1) to evaluate the experience, knowledge and confidence of Italian and Spanish neonatologists and paediatricians with respect to the diagnosis of FAS and FASD, and 2) to evaluate professionals awareness of maternal drinking patterns during pregnancy. Methods A multiple-choice anonymous questionnaire was e-mailed to Italian neonatologists registered in the mailing list of the corresponding Society and administered to Italian and Spanish paediatricians during their National Congress. Results The response rate was 16% (63/400) for the Italian neonatologists of the National Society while a total of 152 Spanish and 41 Italian paediatricians agreed to complete the questionnaire during National Congress. Over 90% of the surveyed physicians declared that FAS is an identifiable syndrome and over 60% of them identified at least one of the most important features of FAS. Although over 60% Italian responders and around 80% Spanish responders were aware that ethanol use in pregnancy is dangerous, approximately 50% Italian responders and 40% Spanish ones allowed women to drink sometimes a glass of wine or beer during pregnancy. Neonatologists and paediatricians rated confidence in the ability to diagnosis FAS and FASD as low, with over 50% responders feeling they needed more information regarding FAS and FASD identification in newborn and child. Conclusions Italian and Spanish neonatologists and paediatricians do not feel confident about diagnosing FAS and FASD. More training is needed in order to accurately diagnose ethanol use during pregnancy and correctly inform pregnant women on the consequences on the newborn.

Identification of cocaine use based only on urinalysis may miss some cases because of the short elimination half-life of the drug. Moreover, many abusers know how to time their cocaine consumption in such a way that they can 'beat' the urinalysis, having a series of negative urine tests. We report on the use of hair testing to disclose sporadic cocaine consumption in seven subjects attending the Local Medical Commission to reobtain driver licence, with constant negative urinalysis. Even with one or two weekly negative urine screens along several months, all the subjects were positive using hair testing for cocaine and benzoylecgonine, above the internationally recommended limit of quantification: 0.5ng/mg and 0.05ng/mg for cocaine and benzoylecgonine, respectively (concentration range for cocaine: 0.51-2.23ng/mg hair; concentration range for benzoylecgonine: 0.08-1.70ng/mg hair). The obtained results support strongly the use of hair testing for cocaine in drug addicts and occasional abusers applying for regranting of driver licence in order to minimize social risk behaviours. Skeletal remodelling is a continuous process during life and is still active also in extreme senescence. In the elderly, bone resorption often prevails over bone formation, causing bone loss and fragility. Elderly subjects are exposed to the risk of fractures, and loss of self-sufficiency, if considering that the proximal femur is the most frequently involved site. Bone remodelling can maintain circulating calcium within physiological ranges, at the expense of a substantial loss of this ion from the skeleton, particularly during senescence. Calcium metabolism is regulated at cellular/molecular level by a network of cytokines, growth factors, systemic hormones that act on bone in paracrine/autocrine/systemic fashion. Among the molecules involved in bone metabolism, parathyroid hormone (PTH) and vitamin D present some peculiar aspects during senescence. The osteometabolic features in a consistent group of centenarians have been evaluated. It results that a severe hypovitaminosis D was present in 99 out of 104 centenarians (25-OH vitamin D below 5 nmol/L), and that it plays an important role as a factor inducing a vicious circle involving hypocalcemia, secondary hyperparathyroidism, together with biochemical features indicating a consistent bone loss. Serum C-terminal cross-linking telopeptide, a specific marker of bone resorption was elevated in 92% of these subjects. Moreover, it has been found that several femoral fractures had occurred after 90 years of age. These data offer a rational for the possible prevention of elevated bone turnover, bone loss and consequently the reduction of osteoporotic fractures and fractures-induced disability, in the oldest olds, through the simple supplementation with calcium and vitamin D.

11. Intervenciones basadas en Mindfulness (Atención Plena) en Psicología del Deporte

En este estudio se pretende analizar los trabajos aplicados de mindfulness (atención plena) en publicaciones de psicología del deporte. Para ello se realiza una revisión sistemática a partir de criterios de inclusión y exclusión de trabajos publicados. Se realiza el análisis de referencias a partir de bases de datos bibliográficos como Medline, Psychinfo, Web of Science, Cochrane, Ebscohost o Isi Web of Knowledge durante el periodo entre 1985 y diciembre 2014. Las palabras claves de búsqueda fueron: psicología del deporte, mindfulness, deporte, estrés y entrenamiento. Inicialmente se partió de 22 artículos, de los que se cribaron 11 por motivos varios (deficiencias metodológicas, conclusiones poco claras, tesis o artículos no publicados). Los 11 artículos analizados realizan intervenciones basadas en mindfulness en el ámbito de la psicología del deporte. Los resultados indican que, aunque se observan deficiencias metodológicas en los diseños de investigación, la mayoría de trabajos muestran mejoras en el rendimiento deportivo, y en variables como ansiedad, burn-out, niveles de flow y niveles de mindfulness. Se recomiendan más estudios y con una mejor metodología para confirmar esta tendencia y continuar profundizando en el conocimiento de la efectividad de la conciencia plena en psicología del deporte., The aim of this article is to identify work on the mindfulness concept in sports psychology research. Research into published works was undertaken using the Medline, Psychinfo, Web of Science, Cochrane, Ebscohost and Isi Web of Knowledge databases and references to articles retrieved from 1985 to December 2014, using inclusion and exclusion criteria. The main search terms were sports psychology, mindfulness, sport, stress and training. From the initial 22 articles we excluded 11 for various reasons (methodological problems, vague conclusions, theses or unpublished articles). The 11 final articles set out different mindfulness-based interventions in the field of sports psychology. Despite methodological limitations in the research design, the results show some improvements in sports performance and variables such as anxiety levels, burn out, flow and mindfulness levels. To confirm these preliminary results, and to continue to enhance knowledge about the effectiveness of mindfulness in sports psychology, further research, with improved methodology, is recommended., Neste estudo pretendeu-se analisar os trabalhos aplicados de mindfulness (atenção plena) em publicações de psicologia do desporto. Para tal, realizou-se uma revisão sistemática a partir de critérios de inclusão e exclusão de trabalhos publicados. Foi realizada uma análise de referências a partir de bases de dados bibliográficos como Medline, Psychinfo, Web of Science, Cochrane, Ebscohost ou Isi Web of Knowledge durante o período entre 1985 e Dezembro de 2014. Inicialmente partiuse de 22 artigos, dos quais foram eliminados 11 por motivos vários (deficiências metodológicas, conclusões pouco claras, teses ou artigos não publicados). Os 11 artigos analisados versam sobre intervenções realizadas com base no mindfulness no âmbito da psicologia do desporto. Os resultados indicam que, embora se observem deficiências metodológicas nos delineamentos de investigação, a maioria dos trabalhos revelam melhorias no rendimento desportivo, e em variáveis como a ansiedade, burnout, níveis de flow e níveis de mindfulness. São recomendados estudos adicionais e com uma melhor metodologia para confirmar esta tendência e continuar a aprofundar o conhecimento da efectividade da consciência plena em psicologia do desporto.

En este trabajo se expone una revisión del estado actual de la lesión deportiva (LD) desde la perspectiva de la Psicología del Deporte. En un primer apartado se ubica el tema a partir de su definición, de las variables más relevantes que la diferencian (nivel, factores de causa, tipos), y de los modelos que explican la relación entre factores y nivel de vulnerabilidad a la lesión que se presentan de forma esquemática. En el segundo apartado, titulado evolución histórica en España, se deja constancia del desarrollo de los trabajos realizados por diferentes autores y equipos de investigación en el ámbito de la psicología de la LD en el estado español. Por último, en el tercer apartado, se presentan los trabajos que aparecen en este monográfico, sirviendo este artículo como presentación del mismo. In this paper we analyse the state of the art regarding sports injuries from the Sport Psychology perspective. In the first section we introduce this concept, its definition, the most important variables (levels, causes, types) and the four models that explain the relationship between factors and vulnerability level to injury that is presented in table 1. In the second section, called "historical evolution in Spain", we make a record of the main research lines of different authors and groups in the field of sport psychology in Spain. Finally, in the third section the papers included in this monographic are introduced, being our contribution the first of them. Neste trabalho é exposta uma revisão do estado actual da lesão desportiva (LD) a partir da perspectiva da Psicologia do Desporto. Na primeira secção, aborda-se o tema a partir da sua definição, das variáveis mais relevantes que a diferenciam (nível, factores causais, tipos), e dos quatro modelos que explicam a relação entre factores e o nível de vulnerabilidade à lesão que se apresentam esquematicamente na tabela 1. Na segunda secção, intitulada evolução histórica em Espanha, apresentam-se os trabalhos realizados por diferentes autores e equipas de investigação no âmbito da psicologia da LD no nosso país. Por último, na terceira secção, são apresentados os trabalhos que constam neste monográfico, servindo este artigo como apresentação do mesmo.

El nostre estudi té com a objecte analitzar la influència del treball per objectius en la disminució del nombre d’errors en atac en handbol, i per tant, sobre la seva eficàcia. L’eix vertebrador d’aquesta investigació el constitueixen les estratègies provinents de la psicologia esportiva aplicades a l’handbol, avaluades mitjançant la metodologia observacional. Proposem, sota una visió interdisciplinària, un sistema d’avaluació de l’eficàcia en atac en handbol basat en l’anàlisi per seqüències. Així mateix, desenvolupem la nostra proposta de model d’estat òptim de rendiment per explicar el marc teòric en el qual apliquem la tècnica d’establiment d’objectius, i finalment, utilitzem la metodologia observacional, basada en la observació permanent i en el registre de les variables objecte d’estudi. Arran de les aportacions d’un primer estudi pilot, iniciem la part empírica, en la qual desenvolupem el disseny de relació de variables i apliquem el treball per objectius a un equip d’handbol juvenil masculí durant les 14 jornades de la Lliga 2002-03 d’Aragó. Posteriorment, realitzem un nou treball (estudi de referència) en el qual contrastem els valors de l’estudi empíric amb els obtinguts en el treball d’observació del Campionat d’Espanya 2003 de la categoria i que serveix com a referència per valorar la millora relativa observada. Algunes de les conclusions més rellevants indiquen que: L’instrument d’observació creat ad hoc resulta vàlid i fiable, i respon als objectius plantejats tant per a l’estudi empíric com per a l’estudi de referència. L’anàlisi de l’Eficàcia en Atac, en termes relatius, proporciona una informació més rellevant que la basada únicament en l’Eficàcia en el Llançament i en dades absolutes. Els errors en atac presenten associacions estadísticament significatives en tots dos estudis pel que fa a les conseqüències negatives que produeixen, per la qual cosa queda plenament justificat l’objectiu d’aquesta investigació. Els errors en atac no es produeixen en funció del nivell de l’oponent. El programa d’intervenció no provoca estadísticament una millora en la disminució dels errors en atac en l’equip objecte d’estudi a causa que, probablement i malgrat el compromís adquirit, els objectius de la investigació pot ser que no siguin sempre prioritaris per a l’equip/entrenador/jugador-s. Les conclusions d’aquest treball confirmen la dificultat d’una intervenció eficaç que doni resultats de millora en els esports col·lectius, en els quals una sola variable es mostra insuficient per modificar/explicar el rendiment esportiu.

El presente estudio tiene por objeto analizar la influencia del trabajo por objetivos en la disminución del número de errores en ataque en balonmano, y por lo tanto, sobre su eficacia. El eje vertebrador de esta investigación lo constituyen las estrategias provenientes de la psicología deportiva aplicadas al balonmano, evaluadas mediante la metodología observacional. Proponemos, bajo una visión interdisciplinar, un sistema de evaluación de la eficacia en ataque en balonmano basado en el análisis por secuencias. Asimismo, desarrollamos nuestra propuesta de modelo de estado óptimo de rendimiento para explicar el marco teórico en el que aplicamos la técnica de establecimiento de objetivos, y por último, utilizamos la metodología observacional, basada en la permanente observación y registro de las variables objeto de estudio. A raíz de las aportaciones de un primer estudio piloto, iniciamos la parte empírica, en la que desarrollamos el diseño de relación de variables y aplicamos el trabajo por objetivos a un equipo de balonmano juvenil masculino durante las 14 jornadas de la Liga 2002-03 de Aragón. Posteriormente, realizamos un nuevo trabajo (estudio de referencia) en el que contrastamos los valores del estudio empírico con los obtenidos en el trabajo de observación del Campeonato de España 2003 de la categoría y que sirve como referencia para valorar la mejora relativa observada. Algunas de las conclusiones más relevantes indican que: El instrumento de observación creado ad hoc resulta válido y fiable, respondiendo a los objetivos planteados tanto para el estudio empírico como para el estudio de referencia. El análisis de la Eficacia en Ataque, en términos relativos, proporciona una información más relevante que aquella basada únicamente en la Eficacia en el Lanzamiento y en datos absolutos. Los errores en ataque presentan asociaciones estadísticamente significativas en ambos estudios respecto a las consecuencias negativas que producen, por lo que queda plenamente justificado el objetivo de esta investigación. Los errores en ataque no están en función del nivel del oponente. El programa de intervención no provoca estadísticamente una mejora en la disminución de los errores en ataque en el equipo objeto de estudio debido a que, probablemente y a pesar del compromiso adquirido, los objetivos de la investigación puede que no sean siempre prioritarios para el equipo/entrenador/jugador-es. Las conclusiones de este trabajo confirman la dificultad de intervención eficaz que dé resultados de mejora en los deportes colectivos, en los que una sola variable se muestra insuficiente para modificar/explicar el rendimiento deportivo.

Anabolic Androgenic Steroids (AAS) abuse and misuse is nowadays a harmful habit involving both professional or recreational athletes, as well as general population. AAS are also frequently present in over-the-counter dietary supplements without being declared in the list of ingredients, leaving consumers unaware of the risks of adverse effects. Indeed, health risks of AAS consumption in pharmaceutical preparations or dietary complements seem still underestimated and under-reported. The variety of complications due to AAS misuse involves cardiovascular, central nervous, musculoskeletal and genitourinary systems of both males and females; psychiatric and behavioral effects, damages to metabolic system, skin and mainly liver. For instance, relevant concern has been raised by the AAS hepatotoxicity including adenoma, hepatocellular carcinoma, cholestasis, and peliosis hepatis. Synthetic cannabinoids (SCs) are psychotropic compounds, chemically created in laboratory to mimic cannabinergic brain activity of delta-9 tetrahydrocannabinol. The consumption of these compounds for recreational purposes can lead to a variety of adverse effects on health including overdose and deaths. Increasingly popular as substances of abuse since the 2000s, SCs were produced initially to bind and study cannabinoid receptors (they also can be called synthetic cannabimimetics) failing in eliminating the psychoactive effects. Currently, SCs are misused by students and young adults as "natural products" because of their herbal aspect. Actually, these apparently innocuous recreational substances hide toxic effects to health. Reported side effects are cardiovascular, gastrointestinal, neurological, renal, metabolic, ophthalmologic, pulmonary and psychoactive including dependence and withdrawal. A few cases of SCs ingestion have also been associated with liver failure. We herein review the recent literature on the SCs toxicity with particular attention to liver damage aspects. El objetivo del presente artículo es el de exponer el programa de entrenamiento psicológico utilizado con la selección olímpica de Hoquey sobre patines cara a su participación en los XXV J uegos Olímpicos en Barcelona. En este trabajo se plantean la duración y tipo de contrato realizado, así como las características del programa de intervención, regido bajo una perspectiva de modelo de campo, en el que se destacaban los siguientes puntos definidores: Entrenador de habilidades psicológicas, asesor del entrenador, visión interdisciplinar, mínima interferencia, i evaluacion objetiva. Para acabar se repasan algunos de los problemas con los que se encontró a lo largo del periodo de entrenamiento (la novedad del trabajo psicológico, la variedad de niveles de entrenameinto psicológico recibido por los jugadores,...). Finalmente se esbozan unas conclusiones para futuras intervenciones The main goal of this article is to expound the psychological training programme used in the Olympic rolling hockey team because of its entry in the XXV Olimpic Games in Barcelona. In this work it's expounded the lenght and the type of contract made, as well as the intervention programme characteristics, guided by a model field perspective, where you could point out the following definite subjects: the psychological abilities coach, the trainer adviser, the interdiscipline view, the minimum interference, and objective evaluation. In order to finish they are revised some problems which were found during the long training period (the newness of the psychological work, the variery of psychological training levels used by the players, ...). Finally they're outlined some conclusions for future interventions

This document presents a comprehensive overview of biochemistry and physiology of transferrin (Tf) and carbohydrate-deficient transferrin (CDT) and on the role of CDT as alcohol abuse marker. It discusses the characteristics of the available analytical methodologies for CDT determination and finally presents a set of recommendations. CDT measurements should be performed only in accredited laboratories, having a quality system in place and regularly performing IQC and participating in EQAS. Unequivocal patient identification and a documented chain of custody are needed. Non hemolysed serum is the preferred sample and, if it has to be stored for more than 8 h, freezing at -20°C is recommended. Immunological methods are acceptable only as first level testing. A confirmation of positive findings with analytical methods based on different physicochemical principles is recommended. The result should be expressed as percentage of disialo-Tf on total Tf. Each laboratory should establish its own decision limit based on the characteristics of the analytical method used and on the scope for which the measurement is performed. The employed analytical method should be indicated in the final report.

Synthetic cannabinoids (SCs) are psychotropic compounds, chemically created in laboratory to mimic cannabinergic brain activity of delta-9 tetrahydrocannabinol. The consumption of these compounds for recreational purposes can lead to a variety of adverse effects on health including overdose and deaths. Increasingly popular as substances of abuse since the 2000s, SCs were produced initially to bind and study cannabinoid receptors (they also can be called synthetic cannabimimetics) failing in eliminating the psychoactive effects. Currently, SCs are misused by students and young adults as "natural products" because of their herbal aspect. Actually, these apparently innocuous recreational substances hide toxic effects to health. Reported side effects are cardiovascular, gastrointestinal, neurological, renal, metabolic, ophthalmologic, pulmonary and psychoactive including dependence and withdrawal. A few cases of SCs ingestion have also been associated with liver failure. We herein review the recent literature on the SCs toxicity with particular attention to liver damage aspects.

Introduction: Tobacco use is one of the world's leading preventable causes of death and is a major preventable risk factor of non-communicable diseases. Although smokers are aware of the health risks, their attempts to quit often fail, primarily due to the strong nicotine and/or tobacco dependence. Antismoking helplines have become an integral part of tobacco control efforts in many countries. In Italy, the ISS Antismoking Helpine is active since 2000., Material and Methods: The professional staff of the ISS Antismoking Helpline have gathered socio-demographic and smoking-related data via an electronic form, related to the received calls. The collected data have been processed in a dedicated database and analyzed to monitor the use and the quality of the service. In this study, a descriptive statistical analysis was conducted to inform about the activity of the helpline over the years., Results: From May 2003 to June 2023 the helpline received 99,423 calls. Most smokers called to receive "support to quit" (82.6%). Counselling was provided in 11.4% of cases, and in the last two years has been strongly increased (40.0% of cases). The percentage of users requesting information on emerging tobacco and nicotine products is 1.2%, even if in 2023 this percentage has risen significantly (6.0%). Two legislative measures (in 2012 and in 2016) required to add the helpline number to all packets of tobacco cigarettes. Accordingly, the offer of counselling increased from 2.6% to 12.2%., Conclusions: The available resources in tobacco control, including the helpline, are still not sufficient to meet all the users needs. Adequate policies and stable funding to fight tobacco and nicotine dependence need increased commitment from government institutions to ensure equal access to treatments for all Italian citizens.

The practice of drug of abuse adulteration is changing. Currently, the risk of new adulteration practices involves New Psychoactive Substances (NPS), which can also be used as adulterants. In particular, the phenomenon of adulteration concerns fentanyl and its analogs, substances that can be toxic even if taken in very small quantities. The adulteration that involves NPS is creating a serious threat to the health of drug users, not only because of the pharmacological action but because of the increased toxicity of these new cutting agents.

Performance enhancing drug use in sport arguably constitutes a crisis, and as such, targeted measures are needed in order to stem it. Substance abuse in professional sports and competitions has besmirched many world-class athletes' reputations and standing, in addition to jeopardizing their health. Furthermore, there are many instances of amateur athletes and school-aged competitors who have taken to using such substances as well, significantly exacerbating the overall picture. The widespread acknowledgement of the potentially life-threatening consequences of performance-enhancing drug use has prodded sports organizations and governments into cooperating on many different levels to preserve the ethical grounding and soundness of sport competitions; unlawful substance abuse is in fact liable to undermine the very core of fairness in competition. Doping, along with various forms of cheating, has been recorded throughout the history of sport: prohibition in itself is all but ineffective without reliable and systematic detection strategies and enforcement of sanctions.

This study presents the case of a 4-year-old healthy child admitted to the paediatric ward for suspected accidental intoxication due to ingestion of narcoleptic drugs (methylphenidate, sertraline and quetiapine), taken on a regular basis by his 8-year-old brother affected by Asperger syndrome.Intoxication can be objectively assessed by measurements of drugs and metabolites in biological matrices with short-term (blood and urine) or long-term (hair) detection windows. At the hospital, the child's blood and urine were analysed by immunoassay (confirmed by liquid chromatography-mass spectrometry), and sertraline and quetiapine and their metabolites were identified. The suspicion that the mother administered drugs chronically prompted the analysis of six, consecutive 2-cm segments of the child's hair, using ultra-high performance liquid chromatography-tandem mass spectrometry, thereby accounting for ingestion over the previous 12 months. Quetiapine was found in the first four segments with a mean concentration of 1.00 ng/mg ± 0.94 ng/mg hair while sertraline and its metabolite, desmethyl-sertraline, were found in all segments with a mean concentration of 2.65 ± 0.94 ng/mg and 1.50 ± 0.94 ng/mg hair, respectively. Hair analyses were negative for methylphenidate and its metabolite (ritalinic acid). Biological matrices testing for psychoactive drugs disclosed both acute and chronic intoxication with quetiapine and sertraline administered by the mother.

Introduction: Gender disparity in different fields of addiction such as tobacco smoking, alcohol use, drugs of abuse consumption and doping practice has been investigated in Italian population., Methods: We used the surveys and studies carried out for the above reported issues in recent years as revised by the "National Observatory on Tobacco smoke, Drugs of abuse, Alcohol and Doping" at Istituto Superiore di Sanità., Results: Concerning tobacco habit, the trend of smoking women has been in constant decrease from a 19.7% in 2010 to a 16.9% in 2015, differently from men who passed from a 23.9% in 2010 to a 25.1% in 2015 with a slight increase in the habit. With respect to alcohol, in the last five years an increasing trend of consumption has been observed in 18-24 years old women, with 53% drinking women in the age range of 18-19 years overcoming the 50.4% general female population. Generally speaking, a one to four ratio can be underlined in the percentage of elderly women with a risky alcohol consumption with respect to men, while in case of adolescents and young adults gender disparity is not so pronounced. Drug abuse still remains a prevalent male phenomenon. However, an increase in cannabis users for both genders has been reported with a prevalence of "once in the life" around 20%, although more pronounced in females (+2.66 percentage points for females vs +0.93 percentage points in male). With respect to cocaine, the second most consumed drug, a reduction in consumption has been recently observed mainly in female population (-42.1%) than in men one (-27.5%). Finally, there are significant gender differences in doping attitude and/or in doping profiling. First of all, males seem to be more exposed to doping than females The prohibited substances most frequently used by females athletes are "Diuretics and Masking Agents" (38.3% positive female vs 14% males) compared to males athletes who use mostly anabolic agents (20.1% males vs 11.2% females)., Conclusions: Results presented for the different fields of addiction show that a gender disparity is apparent and that females are less prone in having an addiction behaviour, although the young generation seems to increase that tendency.

Introduction: We investigated emerging trends in consuming behaviours for non-controlled substances in a cross sectional study on urban Italian adolescents and young adults, the reasons for consumption and risk perception as function of age, the relation with lifestyles and finally risk factors associated., Methods: The survey methodology involved the administration of an anonymous questionnaire. It consisted of 68 questions, divided into five sections: personal details, socioeconomic characteristics, family and peer group, free time and lifestyles, and substances use., Results: A total of 2621 adolescents and young adults (14-35 years old) from seven different Italian cities answered the questionnaire. The substances examined were mainly used recreationally (alcohol, energy drinks and smart-drugs) or to improve physical and sexual performance (physical performance-enhancing drugs, anabolic steroids and male sexual enhancement). The knowledge of the health related harm arising from the use of these products was very high for alcohol (> 90%), high for smart-drugs (> 70%), but significantly lower for anabolic steroids, drugs for sexual enhancement (~ 60%), physical performance-enhancing drugs or energy drinks (~ 55%). The principal risks factors for consumption were: the influence of friends (OR: 8.8), attending recreational places (OR: 5.4) aged between 25-35 years (OR: 3.0), be male (OR: 2.5) and having a bad relationship with the mother (OR: 2.1)., Conclusions: These results have implication for prevention and early intervention programs of "physically and psychologically enhancing" non controlled substances use, which similarly to what is frequently advised for classical illicit drugs should focus on information campaigns and awareness initiatives especially addressed to young male adults who go clubbing, live outside the family and showed closed links with peers.

There is no clear regulation on electronic cigarettes (e-cig); their health effects are not yet fully investigated and there is insufficient standardisation and quality control of the product. Moreover, the e-cig could be a gateway for young people to nicotine addiction and traditional cigarette smoking. In Italy, the Ministry of Health banned the sale of e-cig with nicotine firstly to adolescents aged <16 years, then to people <18 years. Until further scientific evidence is available, it is mandatory to regulate the production and marketing of e-cigs, to make them less attractive, to forbid their use in enclosed areas, and prevent them from being promoted. E-cigs, however, seem to be much less dangerous than traditional cigarettes, although the few studies conducted are not sufficient to demonstrate either a clear therapeutic efficacy of e-cig or their total harmlessness. If e-cig had a known content, were made according to clear rules and in certified laboratories, without toxic substances, it could be used to help heavy smokers to quit, or at least to reduce smoking habits. There is a large proportion of smokers who are unable to quit. The revision of the European Directive (the proposal is being evaluated and we are waiting for its final approval) on tobacco recommends free sale for a minority of e-cigs only, those with a nicotine content <4 mg/ml. This will be difficult, considering that the business is just the free sale of e-cig and the much more dangerous tobacco cigarettes are still sold without any restriction.

- Abstract
The Nasorosso project of the Italian Youth Department and the National Institute of Health, aimed to raise awareness about drinking and driving under the influence of alcohol among club goers with a series of initiatives. Within the framework of the project, blood alcohol concentration (BAC) was measured on 106,406 individuals before and after clubbing in 66 different recreational sites from 11 Italian provinces, over 16 months. Participating individuals were interviewed regarding sociodemographic and environmental characteristics and alcohol intoxicated people were offered to be taken home. The BAC median at the club entry was 0.26 g/L with 65.3% subjects showing a BAC value under the driving legal limit of 0.5 g/L. At the exit from clubs, BAC median value rose to 0.44 g/L and subjects with BAC value under the legal limit decreased to 54.9%. Being male, aged between 18 and 34 years with a diploma, being a drinker and entering the disco with a BAC already beyond the legal limit predicted a BAC value beyond 0.5 g/L at exit from the recreational place. Conversely, being a driver, being a student and exiting from the disco before 4 a.m. reduced the probability of having a BAC higher than 0.5 g/L at the end of the night. Health policies to prevent harmful use of alcohol in young people should continue to offer targeted information/ prevention; in order to steadily increase the awareness of the dangers and the damages of excessive use of alcohol.

Indirect biomarkers of recombinant human growth hormone (rhGH), insulin-like growth factor-I (IGF-I), insulin-like growth factor-II (IGF-II), insulin-like growth factor binding proteins (IGFBP-2 and IGFBP-3) and insulin (C-peptide) were measured together with urinary parameters of renal damage (beta(2)-microglobulin and proteinuria) by immunoassays, in house validated for the purpose, in 61 subjects (36 elite athletes, 18 recreational athletes and 7 sedentary individuals) with different levels of physical fitness and endurance exercise. Validation parameters were good for the evaluated assays, excluding a high inter-assay imprecision and inaccuracy of 24 and 26% obtained for GH assay. The range of concentrations found in urine samples under investigation was generally covered by the calibration curves of the studied immunoassays. However, for the samples below or above the calibration curve, opportune dilution or concentration were performed. Particularly, C-peptide samples had to be diluted 1:5 and beta(2)-microglobulin ones assayed using a triple sample volume, to fall within the calibration range. Urinary C-peptide was the only biomarker statistically higher in samples of elite athletes when compared to recreational athletes and sedentary individuals. Among elite athletes, tae-kwon-do athletes showed the highest IGF-II basal values while weightlifting athletes showed the lower IGF-I and IGFBP-3 basal values. The trend observed in weightlifters' basal samples was confirmed in their training samples: IGF-I, IGF-II, IGFBP-3 and beta(2)-microglobulin were lower in with respect to those from synchronised swimming. Over the training season, within athlete variability was observed for IGFBP-3 for weightlifting athletes. In the studied subjects, no direct associations were found between biomarkers of GH or insulin misuse and urinary parameters of renal damage, eventually due to high-workload endurance training. The variations observed in different biomarkers should be taken in consideration in the hypothesis of setting reference concentration ranges for doping detection., (Copyright (c) 2010 Elsevier B.V. Background: In recent years, fatty acid ethyl esters (FAEEs) in meconium emerged as reliable, direct biological markers for establishing gestational ethanol exposure. Among the minor nonoxidative products of ethanol metabolism, there are ethyl glucuronide (EtG) and ethyl sulfate (EtS)., Objectives: The aim of the study was to analyse meconium specimens from two different Mediterranean cohorts to check for the presence of EtG and EtS, and to investigate the eventual correlation between meconium FAEEs and these two metabolites and their possible application as direct biomarkers of gestational ethanol exposure., Methods: FAEEs, EtG and EtS were quantified by liquid chromatography tandem mass spectrometry in meconium samples obtained from the Neonatal Intensive Care Unit of Arcispedale Santa Maria Nuova, Reggio Emilia, Italy (N= 96) and from the Pediatric Service of the Hospital del Mar in Barcelona, Spain (N=81)., Results: EtG was present in more than 80% meconium samples while EtS only in 50% specimens Although the samples from Spain and Italy originated from similar socio-demographic cohort, EtG values in the Barcelona samples (median value: 101.5 ng/g) were statistically higher than those from Reggio Emilia ones (median value: 15.6 ng/g). In the Barcelona cohort, EtG values could differentiate between samples with FAEEs below and those equal or above 2 nmol/g - the cut-off used to differentiate heavy maternal ethanol consumption during pregnancy from occasional or no use., Conclusion: For the first time the presence of EtG and EtS in meconium has been proven, with EtG concentration likely to discriminate heavy maternal ethanol consumption during pregnancy disclosed by FAEEs concentration in this matrix. Further investigations are needed to verify the use of these two ethanol metabolites as alternative biomarkers of chronic in utero exposure to ethanol.

- Abstract
Insulin and C-peptide have been proposed as possible biomarkers of human insulin hormone misuse in sport. An extended intra- and inter-laboratory validation of commercially available immunoassays was performed. Enzyme Amplified Sensitivity Immunoassay (EASIA) assays (Human Insulin-EASIA and C-peptide EASIA kits from BioSource) were evaluated for insulin and C-peptide in serum. The intra- and inter-laboratory precision and accuracy values were good for the evaluated assays with maximum imprecision and inaccuracy of 16% and 23%, respectively, obtained just for one day C-peptide assay evaluation. The range of concentrations found in serum samples under investigation was always covered by the calibration curves of the studied immunoassays. However, a 19.7% of the samples felt below the estimated insulin limit of quantification. High concordance between laboratory results was obtained for insulin assay (intraclass correlation coefficient -ICC=0.857), whereas that for C-peptide was lower (ICC=0.539). Evaluated immunoassays were used to measure serum concentrations of insulin and C-peptide in elite athletes of various sport disciplines at different moment of training season, in recreational athletes at baseline conditions and finally in sedentary individuals. Serum insulin was statistically lower both in recreational and elite athletes when compared to sedentary individuals. Among elite athletes, the specific sport affected serum insulin (e.g., weightlifting) and C-peptide (e.g., triathlon) concentrations. Over the training season, a within athletes variability was observed for taekwondo, swimming and weightlifting athletes. Variations due to those aspects should be taken in careful consideration in the hypothesis of setting reference concentration ranges for doping detection.
