Abstract

BackgroundRare disease communities are spread around the globe and segmented by their condition. Little research has been performed on the majority of rare diseases. Most patients who are affected by a rare disease have no research on their condition because of a lack of knowledge due to absence of common groups in the research community. ObjectiveWe aimed to develop a safe and secure community of rare disease patients, without geographic or language barriers, to promote research. MethodsCocreation design methodology was applied to build Share4Rare, with consultation and input through workshops from a variety of stakeholders (patients, caregivers, clinicians, and researchers). ResultsThe workshops allowed us to develop a layered version of the platform based on educating patients and caregivers with publicly accessible information, a secure community for the patients and caregivers, and a research section with the purpose of collecting patient information for analysis, which was the core and final value of the platform. ConclusionsRare disease research requires global collaboration in which patients and caregivers have key roles. Collective intelligence methods implemented in digital platforms reduce geographic and language boundaries and involve patients in a unique and universal project. Their contributions are essential to increase the amount of scientific knowledge that experts have on rare diseases. Share4Rare has been designed as a global platform to facilitate the donation of clinical information to foster research that matters to patients with rare conditions. The codesign methods with patients have been essential to create a patient-centric design.

Abstract

Context. Abundances of s- and r-process elements in Sun-like stars constrain nucleosynthesis in extreme astrophysical events, such as compact binary mergers and explosions of highly magnetised rapidly rotating massive stars. Aims. We measure solar abundances of yttrium (Y) and europium (Eu) using 3D non-local thermal equilibrium (NLTE) models. We use the model to determine the abundance of Y, and also explore the model's ability to reproduce the solar centre-to-limb variation of its lines. In addition, we determine the Eu abundance using solar disc-centre and integrated flux spectra. Methods. We developed an NLTE model of Eu and updated our model of Y with collisional data from detailed quantum-mechanical calculations. We used the IAG spatially resolved high-resolution solar spectra to derive the solar abundances of Y across the solar disc and of Eu for integrated flux and at disc centre using a set of carefully selected lines and a 3D radiation-hydrodynamics model of the solar atmosphere. Results. We find 3D NLTE solar abundances of A(Y)$_{\textrm{3D NLTE}}$=$2.30 \pm 0.03_{\textrm{stat}} \pm 0.07_{\textrm{syst}}$ dex based on observations at all angles and A(Eu)$_{\textrm{3D NLTE}}$=$0.57 \pm 0.01_{\textrm{stat}} \pm 0.06_{\textrm{syst}}$ dex based on the integrated flux and disc-centre intensity. 3D NLTE modelling offers the most consistent abundances across the solar disc, and resolves the problem of severe systematic bias in Y and Eu abundances inherent to 1D LTE, 1D NLTE, and 3D LTE modelling., Comment: 8 pages, 4 figures, accepted by A&A

Abstract

We theoretically study the Totally Asymmetric Exclusion Process (TASEP) with quenched jumping rates disorder and finite lifetime chain. TASEP is widely used to model the translation of messenger RNAs by Ribosomes in protein synthesis. Since the exact solution of the TASEP model is analytically and computationally intractable for biologically relevant systems parameters, the canonical Mean-Field (MF) approaches of solving coupled non-linear differential equations is also computational expensive for the scale of relevant biological data analysis. In this article, we provide alternative approach to computing the MF steady state solution via a computationally efficient system of non-linear algebraic equations. We further outline a framework for including correlations progressively via the exact solution of small size TASEP system. Leading order approximation in the biologically relevant entry rate limited regime shows remarkable agreement with the full Monte-Carlo simulation result for a wide range of system parameter space. These results could be of importance to the kinetic rates inference in Ribo-Seq data analysis and other related problems., Comment: 7 pages, 10 figures

9. Emergence of certainty representations for guiding concept learning

Abstract

Previous research has shown that our subjective sense of certainty doesn't always accurately reflect the strength of the evidence that has been presented to us. We investigate several key factors that drive children's certainty using a Boolean concept learning task. We created an idealized learning model to predict children's accuracy and certainty during the experiment, given past evidence that they have seen in the task, and we compared its predictions with our behavioral results. Our results suggest that while predictors from the idealized learning model capture children's accuracy, behavioral predictors generated by the behavioral data can better predict children's certainty. We also show that younger children's certainty can be explained by the idealized learning model, while older children's certainty is primarily predicted by how well they observed themselves doing in the experiment.

Abstract

Abstract Background Neuropathic pain (NP) is a chronic and disabling condition, caused by a lesion or disease of the somatosensory nervous system, characterized by a systemic inflammatory state. Signs and associated symptoms are rarely recognized, and response to usual analgesic drugs is poor. Antidepressant drugs are first-line agents for the treatment of NP. This narrative review aims to summarize the role of antidepressant drugs in treating NP and their mechanism of action, focusing on the effects on inflammatory cytokines. Main text. Peripheral nerve injury leads to a local inflammatory response and to the disruption of the blood-medullary barrier, allowing the influx of peripheral immune cells into the central nervous system. Antidepressants have antinociceptive effects because they recruit long-term neuronal plasticity. Amitriptyline modulates the inflammatory response due to the reduction of the mRNA of pro-inflammatory cytokines acting as an adenosine agonist and leading to the activation of the A3AR receptor. Through toll-like receptors, local inflammation determines the release of pro-inflammatory cytokines such as tumor necrosis factor-alpha (TNF-α) that drives and stimulates the hyperflammation in NP. Nortriptyline has an important antiallodynic effect in NP as it determines the recruitment of norepinephrine in the dorsal root ganglia. By modulating the β2-adrenoreceptors expressed by non-neuronal satellite cells, it inhibits the local production of TNF-α and determines a reduction of NP symptoms. Following the administration of antidepressants, there is a reduction in the production of TNF-α in the brain which in turn transforms the function of the α2-adrenergic receptor from an inhibitor to an activator of the release of norepinephrine. This is important to prevent the development of chronic pain. Conclusion Inflammatory cytokines are the main players in a bidirectional communication between the central and peripheral nervous system and the immune system in NP. Antidepressants have an important role in NP. Further research should explore the interaction between neuroinflammation in NP, the effects of antidepressants and the clinical relevance of this interaction.

Abstract

In recent experiments, unprecedentedly large values for the conductivity of electrolytes through carbon nanotubes (CNTs) have been measured, possibly owing to flow slip and a high pore surface charge density whose origin is still unknown. By accounting for the coupling between the {quantum} CNT and the {classical} electrolyte-filled pore capacitances, we study the case where a gate voltage is applied to the CNT. The computed surface charge and conductivity dependence on reservoir salt concentration and gate voltage are intimately connected to the CNT electronic density of states. This approach provides key insight into why metallic CNTs have larger conductivities than semi-conducting ones.

Abstract

The cooperative binding of molecular agents onto a substrate is pervasive in living systems. To study whether a system shows cooperativity, one can rely on a fluctuation analysis of quantities such as the number of substrate-bound units and the residence time in an occupancy state. Since the relative standard deviation from the statistical mean monotonically decreases with the number of binding sites, these techniques are only suitable for small enough systems, such as those implicated in stochastic processes inside cells. Here, we present a general-purpose grand canonical Hamiltonian description of a small one-dimensional (1D) lattice gas with either nearest-neighbor or long-range interactions as prototypical examples of cooperativity-influenced adsorption processes. First, we elucidate how the strength and sign of the interaction potential between neighboring bound particles on the lattice determine the intensity of the fluctuations of the mean occupancy. We then employ this relationship to compare the theoretical predictions of our model to data from single molecule experiments on bacterial flagellar motors (BFM) of E. coli. In this way, we find evidence that cooperativity controls the mechano-sensitive dynamical assembly of the torque-generating units, the so-called stator units, onto the BFM. Furthermore, in an attempt to quantify fluctuations and the adaptability of the BFM, we estimate the stator-stator interaction potential. Abstract

Abstract The ALICE experiment was proposed in 1993, to study strongly-interacting matter at extreme energy densities and temperatures. This proposal entailed a comprehensive investigation of nuclear collisions at the LHC. Its physics programme initially focused on the determination of the properties of the quark–gluon plasma (QGP), a deconfined state of quarks and gluons, created in such collisions. The ALICE physics programme has been extended to cover a broader ensemble of observables related to Quantum Chromodynamics (QCD), the theory of strong interactions. The experiment has studied Pb–Pb, Xe–Xe, p–Pb and pp collisions in the multi-TeV centre of mass energy range, during the Run 1–2 data-taking periods at the LHC (2009–2018). The aim of this review is to summarise the key ALICE physics results in this endeavor, and to discuss their implications on the current understanding of the macroscopic and microscopic properties of strongly-interacting matter at the highest temperatures reached in the laboratory. It will review the latest findings on the properties of the QGP created by heavy-ion collisions at LHC energies, and describe the surprising QGP-like effects in pp and p–Pb collisions. Measurements of few-body QCD interactions, and their impact in unraveling the structure of hadrons and hadronic interactions, will be discussed. ALICE results relevant for physics topics outside the realm of QCD will also be touched upon. Finally, prospects for future measurements with the ALICE detector in the context of its planned upgrades will also be briefly described.

Abstract

This study critically examines the public’s perception of Florence Nightingale’s legacy through a critical discourse analysis (CDA) of digital media, specifically podcasts and YouTube. Nightingale, who is often remembered as “The Lady with the Lamp”, holds a complex identity within modern narratives that is celebrated for her pioneering contributions to nursing and public health, even if there are some disagreements about her, given the colonialist setting that may have shaped some of her opinions and decisions. This research employed CDA to analyze 25 podcasts and 18 YouTube videos, which were systematically included according to a priori inclusion criteria. The study synthesized how these media products portray Nightingale and, by extension, shape public discourse about the nursing profession. The findings reveal five thematic representations of Nightingale: as a legendary figure, a modern feminist, a dedicated statistician, a pioneer in public health, and a pivotal STEM contributor. These portrayals challenge traditional nursing stereotypes by emphasizing Nightingale’s role as a rigorous scientist and reformer, suggesting broader perceptions of nurses that encompass leadership, analytical skills, and strategic thinking. The study supports the hypothesis that digital narratives significantly influence the public’s understanding and appreciation of nursing, advocating for a more nuanced professional identity that integrates traditional caregiving roles with critical and analytical capabilities.

Abstract

Abstract Background Influenza represents a serious public health threat, especially for the management of severe cases and complications of the disease, requiring the implementation of control measures. We aimed to assess the acceptance and impact of qLAIV vaccination among a representative sample of family paediatricians (FPs) operating in Palermo Local Health Authority (LHA). To this end we evaluated vaccination coverage rates, comparing it with that observed in Sicilian context, while actively monitoring possible adverse reactions and their severity. Methods An observational descriptive non-controlled study was conducted in two phases, from September 2022 to June 2023. The first phase involved a formative and educational intervention with a pre-intervention questionnaire to assess the knowledge and attitudes of FPs on paediatric influenza vaccination. The second phase consisted of an active surveillance on qLAIV safety and acceptance among the paediatric population assisted by the participating FPs, from October 2022 to April 2023. Frequencies, chi-squared tests, and comparisons statistics were performed using Stata/MP 14.1. Results The overall coverage rate among the paediatric population involved in the intervention was 13.2%, with an I.M./qLAIV ratio of vaccine administered of 1/4.25. Abstract

Standardization of data items collected in paediatric clinical trials is an important but challenging issue. The Clinical Data Interchange Standards Consortium (CDISC) data standards are well understood by the pharmaceutical industry but lack the implementation of some paediatric specific concepts. When a paediatric concept is absent within CDISC standards, companies and research institutions take multiple approaches in the collection of paediatric data, leading to different implementations of standards and potentially limited utility for reuse. To overcome these challenges, the conect4children consortium has developed a cross-cutting paediatric data dictionary (CCPDD). The dictionary was built over three phases - scoping (including a survey sent out to ten industrial and 34 academic partners to gauge interest), creation of a longlist and consensus building for the final set of terms. The dictionary was finalized during a workshop with attendees from academia, hospitals, industry and CDISC. The attendees held detailed discussions on each data item and participated in the final vote on the inclusion of the item in the CCPDD. Nine industrial and 34 academic partners responded to the survey, which showed overall interest in the development of the CCPDD. Following the final vote on 27 data items, three were rejected, six were deferred to the next version and a final opinion was sought from CDISC. The first version of the CCPDD with 25 data items was released in August 2019. The continued use of the dictionary has the potential to ensure the collection of standardized data that is interoperable and can later be pooled and reused for other applications. The dictionary is already being used for case report form creation in three clinical trials. The CCPDD will also serve as one of the inputs to the Paediatric User Guide, which is being developed by CDISC.

Abstract

Even though the electromagnetic counterpart AT2017gfo to the binary neutron star merger GW170817 is powered by the radioactive decay of r-process nuclei, only few tentative identifications of light r-process elements have been made so far. One of the major limitations for the identification of heavy nuclei is incomplete or missing atomic data. While substantial progress has been made on lanthanide atomic data over the last few years, for actinides there has been less emphasis, with the first complete set of opacity data only recently published. We perform atomic structure calculations of neodymium $(Z=60)$ as well as the corresponding actinide uranium $(Z=92)$. Using two different codes (FAC and HFR) for the calculation of the atomic data, we investigate the accuracy of the calculated data (energy levels and electric dipole transitions) and their effect on kilonova opacities. For the FAC calculations, we optimise the local central potential and the number of included configurations and use a dedicated calibration technique to improve the agreement between theoretical and available experimental atomic energy levels (AELs). For ions with vast amounts of experimental data available, the presented opacities agree quite well with previous estimations. On the other hand, the optimisation and calibration method cannot be used for ions with only few available AELs. For these cases, where no experimental nor benchmarked calculations are available, a large spread in the opacities estimated from the atomic data obtained with the various atomic structure codes is observed.We find that the opacity of uranium is almost double the neodymium opacity., Comment: 20 pages, 13 figures. Abstract Although co-stimulation of T cells with agonist antibodies targeting 4-1BB (CD137) improves antitumor immune responses in preclinical studies, clinical success has been limited by on-target, off-tumor activity. Here, we report the development of a tumor-anchored ɑ4-1BB agonist (ɑ4-1BB-LAIR), which consists of a ɑ4-1BB antibody fused to the collagen-binding protein LAIR. While combination treatment with an antitumor antibody (TA99) shows only modest efficacy, simultaneous depletion of CD4+ T cells boosts cure rates to over 90% of mice. Mechanistically, this synergy depends on ɑCD4 eliminating tumor draining lymph node regulatory T cells, resulting in priming and activation of CD8+ T cells which then infiltrate the tumor microenvironment. The cytotoxic program of these newly primed CD8+ T cells is then supported by the combined effect of TA99 and ɑ4-1BB-LAIR. The combination of TA99 and ɑ4-1BB-LAIR with a clinically approved ɑCTLA-4 antibody known for enhancing T cell priming results in equivalent cure rates, which validates the mechanistic principle, while the addition of ɑCTLA-4 also generates robust immunological memory against secondary tumor rechallenge. Thus, our study establishes the proof of principle for a clinically translatable cancer immunotherapy.

Abstract

Gene expression consists in the synthesis of proteins from the information encoded on DNA. One of the two main steps of gene expression is the translation of messenger RNA (mRNA) into polypeptide sequences of amino acids. Here, by taking into account mRNA degradation, we model the motion of ribosomes along mRNA with a ballistic model where particles advance along a filament without excluded volume interactions. Unidirectional models of transport have previously been used to fit the average density of ribosomes obtained by the experimental ribo-sequencing (Ribo-seq) technique. In this case an inverse fit gives access to the kinetic rates: the position-dependent speeds and the entry rate of ribosomes onto mRNA. The degradation rate is not, however, accounted for and experimental data from different experiments are needed to have enough parameters for the fit. Here, we propose an entirely novel experimental setup and theoretical framework consisting in splitting the mRNAs into categories depending on the number of ribosomes from one to four. We solve analytically the ballistic model for a fixed number of ribosomes per mRNA, study the different regimes of degradation, and propose a criteria for the quality of the inverse fit. The proposed method provides a high sensitivity to the mRNA degradation rate. The additional equations coming from using the monosome (single ribosome) and polysome (arbitrary number) ribo-seq profiles enable us to determine all the kinetic rates in terms of the experimentally accessible mRNA degradation rate., Comment: 10 pages, 7 figures, 2 tables

The Phase Retrieval problem is dealt with for the challenging case where just a single set of (phaseless) radiated field data is available. In particular, even still emulating the solution of crosswords puzzles, we provide decisive improvements over our recent approaches. In fact, by exploiting bandlimitedness and a suitable set of intersecting curves, we definitively lower the computational complexity (thus eliminating drawbacks) of our previous techniques. Numerical examples, concerning applications of actual interest, support the given theory and confirm the effectiveness of the developed procedure.

Abstract

Our recent re-analysis of the solar photospheric spectra with non-local thermodynamic equilibrium (non-LTE) models resulted in higher metal abundances compared to previous works. When applying the new chemical abundances to Standard Solar Model calculations, the new composition resolves the long-standing discrepancies with independent constraints on the solar structure from helioseismology. Critical to the determination of chemical abundances is the accuracy of the atomic data, specially the $f$-values, used in the radiative transfer models. Here we describe in detail the calculations of $f$-values for neutral oxygen and nitrogen used in our non-LTE models. Our calculations of $f$-values are based on a multi-method, multi-code approach and are the most detailed and extensive of its kind for the spectral lines of interest. We also report in this paper the details of extensive R-matrix calculation of photo-ionization cross sections for oxygen. Our calculation resulted in reliable $f$-values with well constrained uncertainties. We compare our results with previous theoretical and experimental determinations {of atomic data. We also quantify the influence of adopted photo-ionisation cross-sections on the spectroscopic estimate of the solar O abundance, using the data from different sources. We confirm that our 3D non-LTE value is robust and unaffected by the choice of photo-ionisation data, contrary to the recent claim made by Nahar., Comment: 16 pages, 6 tables, 5 figures. Abstract

IntroductionThe conect4children (c4c) project aims to facilitate efficient planning and delivery of paediatric clinical trials. One objective of c4c is data standardization and reuse. Interoperability and reusability of paediatric clinical trial data is challenging due to a lack of standardization. The Clinical Data Interchange Standards Consortium (CDISC) standards that are required or recommended for regulatory submissions in several countries lack paediatric specificity with limited awareness within academic institutions. To address this, c4c and CDISC collaborated to develop the Pediatrics User Guide (PUG) consisting of cross-cutting data items that are routinely collected in paediatric clinical trials, factoring in all paediatric age ranges.Methods and ResultsThe development of the PUG consisted of six stages. During the scoping phase, subtopics (each containing several clinically relevant concepts) were suggested and debated for inclusion in the PUG. Ninety concepts were selected for the modelling phase. Concept maps describing the Research Topic and representation procedure were developed for the 19 concepts that had no (or partial) previous modelling in CDISC. Next, metadata and implementation examples were developed for concepts. This was followed by a CDISC internal review and a public review. For both these review stages, the feedback comments were either implemented or rejected based on budget, timelines, expert review, and scope. The PUG was published on the CDISC website on February 23, 2023.DiscussionThe PUG is a first step in bridging the lack of child specific CDISC standards, particularly within academia. Several academic and industrial partners were involved in the development of the PUG, and c4c has undertaken multiple steps to publicize the PUG within its academic partner organizations – in particular, the European Reference Networks (ERNs) that are developing registries and dictionaries in 24 disease areas. In the long term, continued use of the PUG in paediatric clinical trials will enable the pooling of data from multiple trials, which is particularly important for medical domains with small populations.

Abstract

In ultrasound shear wave elasticity (SWE) imaging, a number of algorithms exist for estimating the shear wave speed (SWS) from spatiotemporal displacement data. However, no method provides a well-calibrated and practical uncertainty metric, hindering SWE's clinical adoption and utility in downstream decision-making. Here, we designed a deep learning SWS estimator that simultaneously outputs a quantitative and well-calibrated uncertainty value for each estimate. Our deep neural network (DNN) takes as input a single 2D spatiotemporal plane of tracked displacement data and outputs the two parameters $m$ and $\sigma$ of a log-normal probability distribution. For training and testing, we used in vivo 2D-SWE data of the cervix collected from 30 pregnant subjects, totaling 551 acquisitions and >2 million space-time plots. Points were grouped by uncertainty into bins to assess uncertainty calibration: the predicted uncertainty closely matched the root-mean-square estimation error, with an average absolute percent deviation of 3.84%. We created a leave-one-out ensemble model that estimated uncertainty with better calibration (1.45%) than any individual ensemble member on a held-out patient's data. Lastly, we applied the DNN to an external dataset to evaluate its generalizability. Abstract

The chemical composition of the Sun is requested in the context of various studies in astrophysics, among them in the calculation of the standard solar models (SSMs), which describe the evolution of the Sun from the pre-main-sequence to its present age. In this work, we provide a critical re-analysis of the solar chemical abundances and corresponding SSMs. For the photospheric values, we employ new high-quality solar observational data collected with the IAG facility, state-of-the art non-equilibrium modelling, new oscillator strengths, and different atmospheric models, including the MARCS model, but also averages based on Stagger and CO5BOLD 3D radiation-hydrodynamics simulations of stellar convection. We perform new calculations of oscillator strengths for transitions in O I and N I. For O I - the critical element for the interior models - calculations are carried out using several independent methods. We find unprecedented agreement between the new estimates of transition probabilities, thus supporting our revised solar oxygen abundance. We also provide new estimates of the noble gas Ne abundance. We investigate our results in comparison with the previous estimates. We discuss the consistency of our photospheric measurements with meteoritic values taking into account systematic and correlated errors. Finally, we provide revised chemical abundances, leading to a new value of the solar photospheric present-day metallicity $Z/X = 0.0225$, and employ them in the calculations of the SSM. We find that the puzzling mismatch between the helioseismic constraints on the solar interior structure and the model is resolved with the new chemical composition., Comment: accepted for publication in A&A
