Abstract

Lung transplantation offers a lifesaving option for patients with end-stage lung disease, but it is marred by a high risk of post-transplant infections, particularly involving multidrug-resistant bacteria, Cytomegalovirus, and fungal pathogens. This elevated infection rate, the highest among solid organ transplants, poses a significant challenge for clinicians, particularly within the first year posttransplantation, where infections are the leading cause of mortality. The direct exposure of lung allografts to the external environment exacerbates this vulnerability leading to constant immune stimulation and consequently to an elevated risk of triggering alloimmune responses to the lung allograft. The necessity of prolonged immunosuppression to prevent allograft rejection further complicates patient management by increasing susceptibility to infections and neoplasms, and complicating the differentiation between rejection and infection, which require diametrically opposed management strategies. This review explores the intricate balance between preventing allograft rejection and managing the heightened infection risk in lung transplant recipients. [ABSTRACT FROM AUTHOR]

Abstract

Mutual interactions between the diaphragm and lung transplantation (LTx) are known to exist. Before LTx, many factors can exert notable impact on the diaphragmatic function, such as the underlying respiratory disease, the comorbidities, and the chronic treatments of the patient. In the post-LTx setting, even the surgical procedure itself can cause a stressful trauma to the diaphragm, potentially leading to morphological and functional alterations. Conversely, the diaphragm can significantly influence various aspects of the LTx process, ranging from graft-to-chest cavity size matching to the long-term postoperative respiratory performance of the recipient. Despite this, there are still no standard criteria for evaluating, defining, and managing diaphragmatic dysfunction in the context of LTx to date. This deficiency hampers the accurate assessment of those factors which affect the diaphragm and its reciprocal influence on LTx outcomes. The objective of this narrative review is to delve into the complex role the diaphragm plays in the different stages of LTx and into the modifications of this muscle following surgery. Abstract

Donation after cardiac death (DCD) donors are still subject of studies. In this prospective cohort trial, we compared outcomes after lung transplantation (LT) of subjects receiving lungs from DCD donors with those of subjects receiving lungs from donation after brain death (DBD) donors (ClinicalTrial.gov: NCT02061462). Lungs from DCD donors were preserved in-vivo through normothermic ventilation, as per our protocol. We enrolled candidates for bilateral LT ≥14 years. Candidates for multi-organ or re-LT, donors aged ≥65 years, DCD category I or IV donors were excluded. We recorded clinical data on donors and recipients. Primary endpoint was 30-day mortality. Secondary endpoints were: duration of mechanical ventilation (MV), intensive care unit (ICU) length of stay, severe primary graft dysfunction (PGD3) and chronic lung allograft dysfunction (CLAD). 121 patients (110 DBD Group, 11 DCD Group) were enrolled. 30-day mortality and CLAD prevalence were nil in the DCD Group. DCD Group patients required longer MV (DCD Group: 2 days, DBD Group: 1 day, p = 0.011). ICU length of stay and PGD3 rate were higher in DCD Group but did not significantly differ. LT with DCD grafts procured with our protocols appears safe, despite prolonged ischemia times.

Abstract

The machine tending in a productive plant typically requires the transport of material from a storage area to a productive area. The plant logistics phase is a part of the production process that is often performed manually, due to the technological challenges related to the manipulation of objects in constrained environments, such as the shelf of a warehouse. However, an effort for its automation is justified by the fact that for an human operator this activity is fatiguing, not ergonomic and with low added value. This paper proposes a control framework for the automation of plant logistics for an industrial case study, integrating navigation and objects manipulation.

30. Grasp It Like a Pro 2.0: A Data-Driven Approach Exploiting Basic Shape Decomposition and Human Data for Grasping Unknown Objects

Abstract

With the improvements in their computational and physical intelligence, robots are now capable of operating in real-world environments. However, manipulation and grasping capabilities are still areas that require significant improvements. To address this, we introduce a new data-driven grasp planning algorithm called Grasp it Like a Pro 2.0. This algorithm utilizes a small number of human demonstrations to teach a robot how to grasp arbitrary objects. By decomposing objects into basic shapes, our algorithm generates candidate grasps that can generalize to different object's geometry. The algorithm selects the grasp to execute based on a selection policy that maximizes a novel grasp quality metric introduced in this article. This metric considers the complex interdependencies between the predicted grasp, the local approximation produced by the basic shape decomposition, and the gripper used. We evaluate our approach against multiple baselines using different grippers and objects. The results demonstrate the effectiveness of our method in generating and selecting high-quality and reliable grasps. With a soft underactuated robotic hand, our algorithm achieves a 94.0% success rate in 150 grasps across 30 different objects. Similarly, with a rigid gripper, it achieves an 85.0% success rate in 80 grasps across 16 different objects.

Abstract

The rise of collaborative robotics has offered new opportunities for integrating automation into the factories, al- lowing robots and humans to work side-by-side. However, this close physical coexistence inevitably brings new constraints for ensuring safe human-robot cooperation. During our research, we developed a trajectory planning algorithm to produce minimum- time yet safe motion plans along specified paths in shared workspaces with humans. To this end, a safety module was used to evaluate the safety of a time-optimal trajectory iteratively. A safe replanning module was developed to optimally adapt the generated trajectory online whenever the optimal plan violates dynamically provided safety limits.

Abstract

Control of two-wheeled humanoid robots poses several challenges due to the unstable dynamics of their mobile base and the coupling between upper and lower body dynamics. In this work, we present the latest results of our research on control of two-wheeled unstable humanoid robots. In particular, we present a whole-body dynamic control approach to stabilize this kind of robots around the upward position. We consider the nominal constrained dynamics of the robot in the quasi- velocities and applied to it a computed torque control approach. To validate this control method, we report the experimental results obtained on AlterEgo.

Abstract

Background: Malignant pleural mesothelioma (MPM) is a rare, highly aggressive tumor strongly associated with asbestos exposure and characterized by poor prognosis. Currently, diagnosis is based on invasive techniques; thus, there is a need to identify non-invasive biomarkers to detect the disease. In the present study, we measured the plasmatic concentrations of Mesothelin, Fibulin-3, and HMGB1 protein biomarkers and of hsa-miR-30e-3p and hsa-miR-103a-3p Extracellular-Vesicles- embedded micro RNAs (EV-miRNAs). We tested the ability of these biomarkers to discriminate between MPM and PAE subjects alone and in combination. Methods: The study was conducted on a population of 26 patients with MPM and 54 healthy subjects with previous asbestos exposure (PAE). Mesothelin, Fibulin-3, and HMGB1 protein biomarkers were measured by the enzyme-linked immunosorbent assay (ELISA) technique; the levels of hsa-miR-30e-3p and hsa-miR-103a-3p EV-miRNAs was assessed by real-time quantitative PCR (qPCR). Results: The most discriminating single biomarker resulted to be Fibulin-3 (AUC 0.94 CI 95% 0.88-1.0; Sensitivity 88%; Specificity 87%). After investigating the possible combinations, the best performance was obtained by the three protein biomarkers Mesothelin, Fibulin-3, and HMGB1 (AUC 0.99 CI 95% 0.97-1.0; Sensitivity 96%; Specificity 93%). Conclusions: The results obtained contribute to identifying new potential non-invasive biomarkers for diagnosing MPM.
