- Abstract
Background: Fatigue with reduced tolerance to exercise is a common persistent long-lasting feature amongst COVID-19 survivors. The assessment of muscle function in this category of patients is often neglected.Aim.: To evaluate the potential impact of a daily supplementation based on amino acids, minerals, vi-tamins, and plant extracts (Apportal (R)) on muscle function, body composition, laboratory parameters and self-rated health in a small group of COVID-19 survivors affected by fatigue.Methods: Thirty participants were enrolled among patients affected by physical fatigue during or after acute COVID-19 and admitted to the post-COVID-19 outpatient service at Fondazione Policlinico Gemelli in Rome between 1st March 2021 and 30th April 2021. All participants were evaluated at first visit (t0) and at control visit (t1), after taking a daily sachet of Apportal (R) for 28 days. Muscle function was analyzed using hand grip strength test, exhaustion strength time and the number of repetitions at one -minute chair stand test. Body composition was assessed with bioelectrical impedance analysis (BIA). Laboratory parameters, including standard blood biochemistry and ferritin levels, were evaluated at the first visit and during the control visit. A quick evaluation of self-rated health, before COVID-19, at t0 and t1, was obtained through a visual analogue scale (VAS). Results: Participants aged 60 years and older were 13 (43%). Females represented the 70% of the study sample. Participants hospitalized for COVID-19 with low -flow oxygen supplementation represented the 43.3% of the study sample while 3.3% received noninvasive ventilation (NIV) or invasive ventilation. Hand grip strength improved from 26.3 Kg to 28.9 Kg (p < 0.05) at t1 as compared to t0. The mean time of strength exhaustion increased from 31.7 s (sec) at t0 to 47.5 s at t1 (p < 0.05). - Abstract
Restrictive measures aimed at containing COVID-19 pandemic had a significant impact on lifestyle habits, quality of life, psychosocial well-being, and mobility in individuals older than 80 years who have not contracted SARS-CoV-2 infection. A comprehensive multidimensional assessment should be routinely implemented to determine and manage the potential negative consequences of public health measures on overall health status and quality of life of older adults.

- Abstract
Aim: To determine the prevalence and associated factors of vitamin D deficiency in COVID-19 survivors and the relationship between vitamin D status and physical performance. Methods: Vitamin D status was assessed in a sample of patients who had recovered from COVID-19 and were admitted to a post-acute outpatient service at the Fondazione Policlinico Universitario Agostino Gemelli IRCCS (Rome, Italy). Participants were offered comprehensive medical assessment, including physical performance and muscle strength tests. Self-rated health was assessed. Vitamin D deficiency was defined as a serum concentration of 25-OH vitamin D < 20 ng/mL. Results: Mean age of 681 participants was 53.4 ± 15.2 years and 49% were women. Vitamin D deficiency was detected in 35.6% of the whole study population, and in 40.2% of those 65 and older. Vitamin D deficiency was associated with diabetes, higher body mass index, and COVID-19 severity, and showed a seasonal pattern with a peak in autumn/winter. Participants with vitamin D deficiency performed poorer on the six-minute walking test, with the lowest performance observed in those 65 and older. No significant associations with any other parameters were found. Conclusion: Our findings indicate that vitamin D deficiency is frequent in COVID-19 survivors, especially in older adults. Low vitamin D levels are associated with poor physical performance, in particular in old age.

- Abstract
Background: Severe clinical pictures and sequelae of COVID-19 disease are immune mediated and characterized by a ‘cytokine storm’. Skeletal muscle has emerged as a potent regulator of immune system function. The aim of the present study is to define the prevalence of sarcopenia among COVID-19 survivors and the negative impact of sarcopenia on the post-acute COVID-19 syndrome and its related risk factors. Methods: A total of 541 subjects recovered from COVID-19 disease were enrolled in the Gemelli Against COVID-19 Post-Acute Care between April 2020 and February 2021. They underwent a multidisciplinary clinical evaluation and muscle strength and physical performance assessment. Results: Mean age was 53.1 years (SD 15.2, range from 18 to 86 years), and 274 (51%) were women. The prevalence of sarcopenia was 19.5%, and it was higher in patients with a longer hospital stay and lower in patients who were more physically active and had higher levels of serum albumin. Patients with sarcopenia had a higher number of persistent symptoms than non-sarcopenic patients (3.8 ± 2.9 vs. 3.2 ± 2.8, respectively; P = 0.06), in particular fatigue, dyspnoea, and joint pain. Conclusions: Sarcopenia identified according to the EWGSOP2 criteria is high in patients recovered from COVID-19 acute illness, particularly in those who had experienced the worst clinical picture reporting the persistence of fatigue and dyspnoea. Our data suggest that sarcopenia, through the persistence of inflammation, could be the biological substrate of long COVID-19 syndrome. Physical activity, especially if associated with adequate nutrition, seems to be an important protective factor.

- Abstract
Long COVID affects a large share of persons of all age groups but may be especially burdensome for vulnerable older adults. Long COVID has a complex pathophysiology encompassing inflammatory and autoimmune processes, perturbations in metabolic pathways, and alterations in endothelial function and redox homeostasis.The management of long COVID requires a multidimensional approach that should include a comprehensive nutritional assessment. Several food bioactive compounds, nutraceuticals, and supplements may target specific pathways involved in long COVID and may, therefore, be used as an adjunctive therapy to manage the condition.

- Abstract
BACKGROUND: Neurological manifestations of Sars-CoV-2 infection have been described since March 2020 and include both central and peripheral nervous system manifestations. Neurological symptoms, such as headache or persistent loss of smell and taste, have also been documented in COVID-19 long-haulers. Moreover, long lasting fatigue, mild cognitive impairment and sleep disorders appear to be frequent long term neurological manifestations after hospitalization due to COVID-19. Less is known in relation to peripheral nerve injury related to Sars-CoV-2 infection. CASE PRESENTATION: We report the case of a 47-year-old female presenting with a unilateral chest pain radiating to the left arm lasting for more than two months after recovery from Sars-CoV-2 infection. After referral to our post-acute outpatient service for COVID-19 long haulers, she was diagnosed with a unilateral, atypical, pure sensory brachial plexus neuritis potentially related to COVID-19, which occurred during the acute phase of a mild Sars-CoV-2 infection and persisted for months after resolution of the infection. CONCLUSIONS: We presented a case of atypical Parsonage-Turner syndrome potentially triggered by Sars-CoV-2 infection, with symptoms and repercussion lasting after viral clearance. A direct involvement of the virus remains uncertain, and the physiopathology is unclear. The treatment of COVID-19 and its long-term consequences represents a relatively new challenge for clinicians and health care providers. A multidisciplinary approach to following-up COVID-19 survivors is strongly advised.

- Abstract
Background: Ibrexafungerp (SCY-078) is the newest oral and intravenous antifungal drug with broad activity, currently undergoing clinical trials for invasive candidiasis. Objective: The aim of this study was to assess the in vitro activity of ibrexafungerp and comparators against a collection of 434 European blood isolates of Candida. Methods: Ibrexafungerp, caspofungin, fluconazole, and micafungin minimum inhibitory concentrations (MICs) were collected from 12 European laboratories for 434 blood isolates, including 163 Candida albicans, 108 Candida parapsilosis, 60 Candida glabrata, 40 Candida tropicalis, 29 Candida krusei, 20 Candida orthopsilosis, 6 Candida guilliermondii, 2 Candida famata, 2 Candida lusitaniae, and 1 isolate each of Candida bracarensis, Candida catenulata, Candida dubliniensis, and Candida kefyr. MICs were determined by the EUCAST broth microdilution method, and isolates were classified according to recommended clinical breakpoints and epidemiological cutoffs. Additionally, 22 Candida auris from different clinical specimens were evaluated. Results: Ibrexafungerp MICs ranged from 0.016 to ≥8 mg/L. The lowest ibrexafungerp MICs were observed for C. albicans (geometric MIC 0.062 mg/L, MIC range 0.016–0.5 mg/L) and the highest ibrexafungerp MICs were observed for C. tropicalis (geometric MIC 0.517 mg/L, MIC range 0.06–≥8 mg/L). Modal MICs/MIC50s (mg/L) against Candida spp. were 0.125/0.06 for C. albicans, 0.5/0.5 for C. parapsilosis, 0.25/0.25 for C. glabrata, 0.5/0.5 for C. tropicalis, 1/1 for C. krusei, 4/2 for C. orthopsilosis, and 0.5/0.5 for C. auris. Ibrexafungerp showed activity against fluconazole- and echinocandin-resistant isolates. - Abstract
Objectives: Symptom persistence weeks after laboratory-confirmed severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) clearance is a relatively common long-term complication of Coronavirus disease 2019 (COVID-19). Little is known about this phenomenon in older adults. The present study aimed at determining the prevalence of persistent symptoms among older COVID-19 survivors and identifying symptom patterns. Design: Cross-sectional study. Setting and Participants: We analyzed data collected in people 65 years and older (n = 165) who were hospitalized for COVID-19 and then admitted to the Day Hospital Post-COVID 19 of the Fondazione Policlinico Universitario “Agostino Gemelli” IRCCS (Rome, Italy) between April and December 2020. All patients tested negative for SARS-CoV-2 and met the World Health Organization criteria for quarantine discontinuation. Measures: Patients were offered multidisciplinary individualized assessments. The persistence of symptoms was evaluated on admission using a standardized questionnaire. Results: The mean age was 73.1 ± 6.2 years (median 72, interquartile range 27), and 63 (38.4%) were women. The average time elapsed from hospital discharge was 76.8 ± 20.3 days (range 25−109 days). On admission, 137 (83%) patients reported at least 1 persistent symptom. Of these, more than one-third reported 1 or 2 symptoms and 46.3% had 3 or more symptoms. The rate of symptom persistence was not significantly different when patients were stratified according to median age. Compared with those with no persistent symptoms, patients with symptom persistence reported a greater number of symptoms during acute COVID-19 (5.3 ± 3.0 vs 3.3 ± 2.0; P < .001). The most common persistent symptoms were fatigue (53.1%), dyspnea (51.5%), joint pain (22.2%), and cough (16.7%). A patient with COVID-19-related severe respiratory failure, with insufficient response to an antiretroviral therapy, hydroxychloroquine and Interleukin-6 (IL-6) antagonist therapy, presented a prompt resolution of the respiratory function and improvement in the radiological picture after baricitinib at an oral dose of 4 mg per day for 2 weeks.

- Abstract
Mild cognitive impairment (MCI), also termed mild neurocognitive disorder, includes a heterogeneous group of conditions characterized by declines in one or more cognitive domains greater than that expected during "normal" aging but not severe enough to impair functional abilities. MCI has been associated with an increased risk of developing dementia and even considered an early stage of it. Therefore, noninvasively accessible biomarkers of MCI are highly sought after for early identification of the condition. Systemic inflammation, metabolic perturbations, and declining physical performance have been described in people with MCI. However, whether biological and functional parameters differ across MCI neuropsychological subtypes is presently debated. Likewise, the predictive value of existing biomarkers toward MCI conversion into dementia is unclear. The "develOpment of metabolic and functional markers of Dementia IN Older people" (ODINO) study was conceived as a multi-dimensional investigation in which multi-marker discovery will be coupled with innovative statistical approaches to characterize patterns of systemic inflammation, metabolic perturbations, and physical performance in older adults with MCI. The ultimate aim of ODINO is to identify potential biomarkers specific for MCI subtypes and predictive of MCI conversion into Alzheimer's disease or other forms of dementia over a three-year follow-up. Here, we describe the rationale, design, and methods of ODINO.

- Abstract
On February 20, 2020, a man living in the north of Italy was admitted to the emergency room with an atypical pneumonia that later proved to be COVID-19. This was the trigger of one of the most serious clusters of COVID-19 in the world, outside of China. Despite aggressive restraint and inhibition efforts, COVID-19 continues to increase, and the total number of infected patients in Italy is growing daily. After 6 weeks, the total number of patients reached 128,948 cases (April 5, 2020), with the higher case-fatality rate (15,887 deaths) dominated by old and very old patients. This sudden health emergency severely challenged the Italian Health System, in particular acute care hospitals and intensive care units. In 1 hospital, geriatric observation units were created, the experience of which can be extremely useful for European countries, the United States, and all countries that in the coming days will face a similar situation.

- Abstract
Background and purpose: The objective of this study was to assess the neurological manifestations in a series of consecutive severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2)-positive patients, comparing their frequency with a population hospitalized in the same period for flu/respiratory symptoms, finally not related to SARS-CoV-2. Methods: Patients with flu/respiratory symptoms admitted to Fondazione Policlinico Gemelli hospital from 14 March 2020 to 20 April 2020 were retrospectively enrolled. The frequency of neurological manifestations of patients with SARS-CoV-2 infection was compared with a control group. Results: In all, 213 patients were found to be positive for SARS-CoV-2, after reverse transcriptase polymerase chain reaction on nasal or throat swabs, whilst 218 patients were found to be negative and were used as a control group. Regarding central nervous system manifestations, in SARS-CoV-2-positive patients a higher frequency of headache, hyposmia and encephalopathy always related to systemic conditions (fever or hypoxia) was observed. Furthermore, muscular involvement was more frequent in SARS-CoV-2 infection. Conclusions: Patients with COVID-19 commonly have neurological manifestations but only hyposmia and muscle involvement seem more frequent compared with other flu diseases.

- Abstract
Invasive fungal infections, such as aspergillosis, candidiasis, and cryptococcosis, have significantly increased among immunocompromised people. To tackle these infections the first and most decisive step is the accurate identification of the causal pathogen. Routine identification of invasive fungal infections has progressed away from culture-dependent methods toward molecular techniques, including DNA barcoding, a highly efficient and widely used diagnostic technique. Fungal DNA barcoding previously relied on a single barcoding region, the internal transcribed spacer (ITS) region. However, this allowed only for 75% of all fungi to be correctly identified. As such, the translational elongation factor 1α (TEF1α) was recently introduced as the secondary barcode region to close the gap. Both loci together form the dual fungal DNA barcoding scheme. As a result, the ISHAM Barcoding Database has been expanded to include sequences for both barcoding regions to enable practical implementation of the dual barcoding scheme into clinical practice. The present study investigates the impact of the secondary barcode on the identification of clinically important fungal taxa, that have been demonstrated to cause severe invasive disease. Analysis of the barcoding regions was performed using barcoding gap analysis based on the genetic distances generated with the Kimura 2-parameter model. The secondary barcode demonstrated an improvement in identification for all taxa that were unidentifiable with the primary barcode, and when combined with the primary barcode ensured accurate identification for all taxa analyzed, making DNA barcoding an important, efficient and reliable addition to the diagnostic toolset of invasive fungal infections. Two strains of Torulaspora delbrueckii, IGC 5321 and IGC 5323 retain their fermentative ability even after 15 days of storage and 2.5 hours of prefermentation, despite a decrease in viable cells. The leavening ability of all the strains of Saccharomyces cerevisiae, except IGC 4072, decreases with an increase in storage period. Subjecting the dough to bulk fermentation before freezing also has a negative effect on the yeast leavening ability. Further, the yeast fermentative ability decreases with an increase in prefermentation time.

- Abstract
Previous studies have correlated the severity of recurrent vulvovaginal Candida infections (VVC) and balanitis in patients from China with the presence of some dominant genotypes at the ORF RLM1. Here we tested VVC vs non-VVC isolates from Portugal, Brazil and Greece and, although the same genotypes were identified in VVC isolates, they were present in only five out of 150 strains. However, this analysis showed that VVC isolates presented a higher percentage of genotypes with similar high molecular weight alleles, in comparison with strains isolated from other biological sources. © The Author 2017. Examinamos o impacto da dimensão da firma de auditoria, da adoção do Sistema de Normalização Contabilística (SNC) e de ter uma grande firma de auditoria após essa adoção, no custo da dívida, das maiores empresas portuguesas não cotadas durante o período de 2007 a 2012. O custo da dívida é mensurado pelo quociente entre o gasto dos juros e a média da dívida e a dimensão da firma de auditoria numa base da dicotomia Big 4 e não Big 4. Os resultados confirmam que o tipo de auditor não influencia o custo da dívida nem antes nem depois da adoção do SNC e que a adoção do SNC reduz o custo da dívida das empresas portuguesas não cotadas, confirmando que sendo baseado nas normas do International Accounting Standards (IASB) é entendido como de melhor qualidade. So far, the operational leases are accounted as an expense and not as an asset but there is a proposal by the International Accounting Standards Board (IASB) to accounting them as finance lease because IASB believes that that improves the quality and comparability. So, I test whether the accounting of an operational lease as a finance lease is more value relevant to investors, whether this has changed over time and whether the operating lease liabilities is more value relevant for the investor. My results suggest for the largest European firms, that the investor incorporates the information of operating leases on the share price but I do not conclude that there was a change of this over time neither the liability is more value relevant than the asset. O objetivo do estudo é conhecer o efeito da crise financeira de 2008 na determinação da materialidade e na análise do risco em auditoria, permitindo conhecer os indicadores e os fatores mais utilizados. A materialidade é analisada em termos quantitativos e qualitativos, ao nível do planeamento e da execução de auditoria. As alterações de escolha por parte dos auditores referente ao risco inerente, de controlo e aceitável também foram analisados. Confirma-se que na determinação de materialidade são utilizadas medidas quantitativas e qualitativas, e que os auditores usam fatores de risco inerente e controlo nas suas análises. Existe evidência que a materialidade e o risco sofreram alterações em consequência da crise financeira de 2008, mais especificamente no aumento de fatores quanto à materialidade qualitativa, no risco inerente e de controlo. - Abstract
Human and animal fungal pathogens are a growing threat worldwide leading to emerging infections and creating new risks for established ones. There is a growing need for a rapid and accurate identification of pathogens to enable early diagnosis and targeted antifungal therapy. Morphological and biochemical identification methods are time-consuming and require trained experts. Alternatively, molecular methods, such as DNA barcoding, a powerful and easy tool for rapid monophasic identification, offer a practical approach for species identification and less demanding in terms of taxonomical expertise. However, its wide-spread use is still limited by a lack of quality-controlled reference databases and the evolving recognition and definition of new fungal species/complexes. An international consortium of medical mycology laboratories was formed aiming to establish a quality controlled ITS database under the umbrella of the ISHAM working group on "DNA barcoding of human and animal pathogenic fungi." A new database, containing 2800 ITS sequences representing 421 fungal species, providing the medical community with a freely accessible tool at http://www.isham.org/ and http://its.mycologylab.org/ to rapidly and reliably identify most agents of mycoses, was established. The generated sequences included in the new database were used to evaluate the variation and overall utility of the ITS region for the identification of pathogenic fungi at intra-and interspecies level. The average intraspecies variation ranged from 0 to 2.25%. This highlighted selected pathogenic fungal species, such as the dermatophytes and emerging yeast, for which additional molecular methods/genetic markers are required for their reliable identification from clinical and veterinary specimens. © The Author 2015. Fungal infections constitute a relevant problem in hospitals from all over the world. Their easy transmission through the air, water, or by direct or indirect contact with contaminated persons, surfaces or objects lead to an increasing preoccupation with those infections, particularly in immunocompromised patients. Regarding the enormous importance of Candida parapsilosis as agent of bloodstream infections and hospital outbreaks, the presented work aimed to contribute to the development of new molecular tools able to answer to epidemiological questions related with infections by this species and their application in favor of patient safety As infecções fúngicas constituem um problema hospitalar relevante em todo o mundo. A fácil transmissão dos fungos pelo ar, água ou pelo contato direto ou indireto com pessoas, superfícies ou objetos contaminados conduz a uma crescente preocupação com este tipo de infecções, principalmente em pacientes imunocomprometidos. Considerando a grande relevância da levedura Candida parapsilosis em Portugal como agente etiológico de infecções sistémicas e de surtos hospitalares, o trabalho apresentado teve como objetivo contribuir para o desenvolvimento de novas ferramentas moleculares que permitissem responder a questões epidemiológicas relacionadas com esta espécie e posterior aplicação desta ferramenta em prol da segurança do paciente.

A importância da recolha sistematizada de informação para efeitos de monitorização e avaliação em planeamento é hoje consensual. A acelerada transformação física dos territórios urbanos ocorrida nas últimas décadas torna este conhecimento ainda mais relevante. Diversos países têm já sistemas consolidados neste domínio; em Portugal só recentemente começaram a ser desenvolvidas iniciativas para estabelecer rotinas de registo e análise permanente da evolução da ocupação física do território. Neste contexto, pretende-se apresentar investigação em curso sobre monitorização da urbanização e da edificação, na qual se perspetiva um modelo que, para além da recolha sistemática da informação habitual, seja capaz de gerar três tipos de diagnósticos de apoio ao ordenamento do território: diagnóstico sobre o tipo de ocupação edificada existente, distinguindo ocupação concentrada da dispersa, o que permitirá uma melhor delimitação de perímetros urbanos e classificações mais rigorosas do solo, com vista ao seu ordenamento; diagnóstico sobre a capacidade edificatória disponível face à infraestrutura e à edificação já instalada no território, o que permitirá regulamentar a edificabilidade com base em critérios racionais de aproveitamento de recursos existentes; diagnóstico sobre diferentes “processos de execução urbanística”, articulando-os com custos da execução (e respetiva distribuição) e com forma urbana resultante, o que permitirá produzir orientações neste domínio.A presente comunicação debruça-se sobretudo sobre esta última potencialidade do modelo, nomeadamente sobre uma tipologia de “processos de execução urbanística”, conceito que relaciona sequência de operações urbanísticas, agentes urbanísticos e afetação de custos e benefícios da urbanização, sendo relacionável com a forma urbana.

The study of urban dispersion on a local scale requires the adoption of operational concepts. This research uses the notion of "Base Land Unit" (BLU), which represents a functional and experiential reality. The current paper presents an explanation of this concept and a methodology to distinguish BLU of dispersed occupation from other types of Units (concentrated, unbuilt…). The developed methodology comprises three different methods: - Identification of Building Ensembles by Digital Method; - Calculation of the Disaggregation Index of Continuous Building Ensembles; - Criterion for Identifying BLU of Dispersed Occupation. A detailed description of each phase is presented, as well as the results of its application in the case studies of the Research Project (Évora and Aveiro-Ílhavo).

This work is part of the research project “Costs and Benefits of Urban Dispersion on a local scale”, developed by the universities of Aveiro and Évora, in partnership with DGOTDU, and explains a (experimental) method adopted for the identification of travel costs associated to local scale public amenities. The article also presents the results of the method application to several types of urban settlement (in order to confront concentrated and dispersed occupation).

The catches and discards of trawlers, seiners and trammel netters were studied in the Algarve (southern Portugal) using observers onboard commercial fishing vessels. Species diversity was high, with 236 species recorded as occasionally, frequently or regularly discarded. Mean discard rates per trip were 0.13, 0.20, 0.27, 0.62 and 0.70, respectively, for trammel nets, demersal purse seines, pelagic purse seines, fish trawls and crustacean trawls, with high variability in terms of discard volume and discard rate, This was especially so for se ncrs where from 0 to 100% of the total catch might be discarded. No significant relationships were found between discard quantity or discard rate and characteristics of the fishing vessels sampled within each metier (total length, TAB, hp, kW). Based on the above discard rates and the official landings, it was estimated that in 1996, Algarve trawlers discarded 9000-13000 tonnes (t) while seiners discarded 5500-8200 t. Discarding practices in these fisheries are reviewed and the reasons for discarding are presented by species and metier basis. The results support the need for more studies on the factors contributing to discarding, variability in discard quantities and rates, the fate of discards and their importance to the marine ecosystem, and on ways of reducing by-catch and discarding in these fisheries.
