The MILAGRO field campaign was a multi-agency international collaborative project to evaluate the regional impacts of the Mexico City air pollution plume as a means of understanding urban impacts on the global climate. Mexico City lies on an elevated plateau with mountains on three sides and has complex mountain and surface-driven wind flows. This paper asks what the wind transport was in the basin during the field campaign and how representative it was of the climatology. Surface meteorology and air quality data, radiosoundings and radar wind profiler data were collected at sites in the basin and its vicinity. Cluster analysis is used to identify the dominant wind patterns both during the campaign and within the past 10 years of operational data from the warm dry season. Our analysis shows that March 2006 was representative of typical flow patterns experienced in the basin. Six episode types were identified for the basin scale circulation providing a way of interpreting atmospheric chemistry and particulate data collected during the campaign. Decoupling between surface winds and those aloft had a strong influence in leading to convection and poor air quality episodes. Hourly characterisation of wind circulation during the MILAGRO, MCMA2003 and IMADA field campaigns will enable the comparisons of similar air pollution episodes and the evaluation of the impact of wind transport on measurements of the atmospheric chemistry taking place in the basin. Carbohydrate-binding modules (CBM) have emerged as useful tools for a wide range of tasks, including the use as purification tags or for cellulose fiber modification. For this purpose, the CBM needs to be attached to a target protein leading to large constructs. We investigated if short peptides from the carbohydrate binding site of CBMs can bind in a similar way as native, full-length CBMs to nanocrystalline cellulose (NCC) or cotton linter paper. We designed our cellulose-binding peptides to be less hydrophobic and shorter than those previously reported. Starting from the binding site of Cel7A-CBM1, we incorporated the essential amino acids involved in cellulose binding into our peptides. These peptides, as well as control peptides with scrambled sequences or a lack of essential amino acids, bound to cellulose with similar affinity as CBM regardless of their secondary structure, sequence, or hydrophobicity. This unspecific mode of cellulose binding displayed by the presented peptides may be exploited to functionalize cellulose-based biomaterials by means of peptide-conjugates.

When the K + channel-like protein Kesv from Ectocarpus siliculosus virus 1 is heterologously expressed in mammalian cells, it is sorted to the mitochondria. This targeting can be redirected to the endoplasmic reticulum (ER) by altering the codon usage in distinct regions of the gene or by inserting a triplet of hydrophobic amino acids (AAs) into the protein's C-terminal transmembrane domain (ct-TMD). Systematic variations in the flavor of the inserted AAs and/or its codon usage show that a positive charge in the inserted AA triplet alone serves as strong signal for mitochondria sorting. In cases of neutral AA triplets, mitochondria sorting are favored by a combination of hydrophilic AAs and rarely used codons; sorting to the ER exhibits the inverse dependency. This propensity for ER sorting is particularly high when a common codon follows a rarer one in the AA triplet; mitochondria sorting in contrast is supported by codon uniformity. Since parameters like positive charge, hydrophobic AAs, and common codons are known to facilitate elongation of nascent proteins in the ribosome the data suggest a mechanism in which local changes in elongation velocity and co-translational folding in the ct-TMD influence intracellular protein sorting., (© 2023 The Authors. Background: Data on the treatment of hip fractures in acute care settings have been collected in a report card system for quality assurance in Germany since the beginning of the 1990s. However, there are no data on the long-term outcome and long-term quality of care., Material and Method: In a retrospective study, data on 1393 patients from 1999 were collected from different sources: from the department of quality assurance at the medical association of Westfalia-Lippe, the Statutory Health Insurance Funds (AOK), and the Medical Review Board of the Statutory Health Insurance Funds (Medizinischer Dienst der Krankenkasse, MDK). Statistical analyses were performed by the Center for Clinical Studies of the University of Düsseldorf., Results: Uni- and multivariate analyses reveal the following prognostic parameters for survival after hip fracture: sex, age, nursing care dependency, living in a nursing home, risk stratification according to ASA, and postoperative complications. Timing of the operation had no affect on survival., Conclusions: Prognostic factors for the outcome after hip fracture can only be obtained by analyzing data from the hospital stay and the post-hospital setting as well. Chances of survival can be significantly improved by rehabilitative care.

Aim of the project was a comprehensive assessment of short- and middle-term outcome of femoral neck fracture by linkage and analysis of available data from routine care. For this purpose, a generic model of data linkage was developed, agreed with the data security officer, and practically applied. Included were all patients of the AOK Westphalia-Lippe, who were treated in a general or trauma surgery hospital in 1995/1999 for a femoral neck fracture (ICD-9: 820). For these patients, the linkage was based on the following sources: the data regarding the initial hospital stay were provided by the office of quality assurance of the chamber of physicians of Westphalia-Lippe; the administrative data were provided by the AOKWestphalia-Lippe; and the data evaluating the nursing needs were obtained from the Medical Services of the Health Insurance of Westphalia-Lippe (MDK). This paper presents the model of data linkage and describes its practical implementation; it also presents medical data demonstrating that femoral neck fractures are associated with high mortality and increase of nursing needs in the course of disease. The benefit of the new model is manifold and can be easily extended to other clinical questions.

Introduction: Trochanteric femur fractures are a typical injury of elderly people and are of significant relevance for their quality of life., Material and Method: Data on 7252 proximal femur fractures from the department of external quality assurance of the chamber of physicians of Westphalia-Lippe/Germany were analysed. Topics of interest were: patient profile, type of femur fracture, method of treatment, intra- and postoperative complications, mobility before and after fracture., Results: 2883 of 7252 proximal femur fractures were trochanteric fractures, 89.9% of which were instable. Trochanteric fractures were either stabilised by extra-medullary implants, e.g. dynamic hip screw (DHS; 57.5%), or by intra-medullary implants, e.g. proximal femur nail (PFN; 38.3%). The following intra-operative complications were reported: bone injury, vessel injury, nerve injury, others. Intra-medullary implants caused nearly twice as many complications as DHS (1.8% vs. 1.1%; chi-square test, p < 0.01). Specialised orthopaedic trauma departments (1.7%) had fewer complications than non-specialised units (2.2%) (chi-square test, p < 0.001). Mobility after intra- and extra-medullary fixation was comparable. At the time of discharge, 58% of patients were able to walk with crutches. High-volume hospitals (151 - 175 patients) performed significantly better than low-volume hospitals (1 - 25 patients) concerning mortality and intra- and postoperative complications. However, it is not possible to infer good quality from high volume alone, because some low-volume hospitals showed excellent results., Conclusion: Treatment of trochanteric femur fractures is relatively safe. Extra-medullary implants can be used for instable trochanteric fractures with good results.

As part of a program to discover improved glycoside hydrolase family 12 (GH 12) endoglucanases, we have extended our previous work on the structural and biochemical diversity of GH 12 homologs to include the most stable fungal GH 12 found, Humicola grisea Cel12A. The H. grisea enzyme was much more stable to irreversible thermal denaturation than the Trichoderma reesei enzyme. It had an apparent denaturation midpoint (T(m)) of 68.7 degrees C, 14.3 degrees C higher than the T. reesei enzyme. There are an additional three cysteines found in the H. grisea Cel12A enzyme. To determine their importance for thermal stability, we constructed three H. grisea Cel12A single mutants in which these cysteines were exchanged with the corresponding residues in the T. reesei enzyme. We also introduced these cysteine residues into the T. reesei enzyme. The thermal stability of these variants was determined. Substitutions at any of the three positions affected stability, with the largest effect seen in H. grisea C206P, which has a T(m) 9.1 degrees C lower than that of the wild type. The T. reesei cysteine variant that gave the largest increase in stability, with a T(m) 3.9 degrees C higher than wild type, was the P201C mutation, the converse of the destabilizing C206P mutation in H. grisea. To help rationalize the results, we have determined the crystal structure of the H. grisea enzyme and of the most stable T. reesei cysteine variant, P201C. The three cysteines in H. grisea Cel12A play an important role in the thermal stability of this protein, although they are not involved in a disulfide bond.

Unlabelled: Data on 32 007 patients suffering from a medial fracture of the femoral neck have been collected between 1993 and 1999 in a database for external quality assurance organized by the chamber of physicians in Westfalia-Lippe. A statistical analysis (ANOVA, chi-square-test) has been performed to find out whether factors like specialization, annual volume or level of the hospital (primary, secondary or tertiary hospital) influence the outcome., Results: Patients with higher preoperative risk-factors are treated more often in primary hospitals. These clinics perform conservative treatment significantly more often than tertiary hospitals (6.5 % vs. 3.8 %). Osteosyntheses are performed more often in departments specialized in traumatology (13 %) or tertiary hospitals (16.8 %). Preoperative length of stay was 0.5-0.7 days shorter in these hospitals. There is no significant difference in postoperative complications all together (23.2-25.6 %), but a significantly lower rate in postoperative complications after osteosynthesis performed by departments specialized in traumatology (11.3 % vs. 18.8 %). A volume load of more than 50 cases per year correlates with a significant decline in postoperative complications (22.5 % vs. 28.2 %). Risk adjusted mortality does not show significant differences among the different levels of hospitals., Conclusions: There are distinct differences regarding the way of treatment and procedural quality, but not concerning the short-term outcome between hospitals of different levels.

Between Jan. 1st 1993 and Dec. 31st 1999 data was collected on 31.917 patients with fractures of the femoral neck in a statewide report card system in Westphalia-Lippe. Only 6.3% of patients were younger than 60 years. Ten percent of the population of Germany live in Westphalia-Lippe so that our data can be considered representative of Germany. A significant change was seen in the following indicators of quality: increase of operative treatment 1993 93.2%--1999 96.0% (p < 0.01 Chi Quadrat Test), decrease of mortality 1993 6.9%--1999 5.7% (p < 0.01 Chi Quadrat Test), decrease of cardiopulmonary complications 1993 11.2%--1999 8.4% (p < 0.01 Chi Quadrat Test), decrease of mean length of stay 1993 33.5 days--1999 22.2 days (p < 0.01 one-way-ANOVA), decrease of pre-operative length of stay 1993 2.57 days--1999 1.86 days (p < 0.01 one-way-ANOVA), increase of operative treatment on weekends (p < 0.01 Chi Quadrat Test). 25% of the patients had post-operative complications. Intra-operative complications were rare (1.4%). Only 48.6% of internal fixations were performed on the day of admission. The most frequent operation is hemiarthroplasty and its frequency is still increasing. Total hip replacement was done 34.1% of all cases. More patients are transferred from primary hospitals to rehabilitation facilities following operative procedures: 1993 8.3%; 1999 27.2%.
