Abstract: The Carbon monOxide Mapping Array Project (COMAP) Pathfinder survey continues to demonstrate the feasibility of line-intensity mapping using high-redshift carbon monoxide (CO) line emission traced at cosmological scales. The latest COMAP Pathfinder power spectrum analysis is based on observations through the end of Season 2, covering the first three years of Pathfinder operations. We use our latest constraints on the CO(1-0) line-intensity power spectrum at $z\sim3$ to update corresponding constraints on the cosmological clustering of CO line emission and thus the cosmic molecular gas content at a key epoch of galaxy assembly. We first mirror the COMAP Early Science interpretation, considering how Season 2 results translate to limits on the shot noise power of CO fluctuations and the bias of CO emission as a tracer of the underlying dark matter distribution. The COMAP Season 2 results place the most stringent limits on the CO tracer bias to date, at $\langle{Tb}\rangle<4.8$ $\mu$K. These limits narrow the model space significantly compared to previous CO line-intensity mapping results while maintaining consistency with small-volume interferometric surveys of resolved line candidates. The results also express a weak preference for CO emission models used to guide fiducial forecasts from COMAP Early Science, including our data-driven priors. We also consider directly constraining a model of the halo-CO connection, and show qualitative hints of capturing the total contribution of faint CO emitters through the improved sensitivity of COMAP data. Abstract: We present updated constraints on the cosmological 3D power spectrum of carbon monoxide CO(1-0) emission in the redshift range $2.4$-$3.4$. The constraints are derived from the two first seasons of Carbon monOxide Mapping Array Project (COMAP) Pathfinder line-intensity mapping observations aiming to trace star-formation during the Epoch of Galaxy Assembly. These results improve on the previous Early Science (ES) results through both increased data volume and improved data processing methodology. On the methodological side, we now perform cross-correlations between groups of detectors (''feed-groups''), as opposed to cross-correlations between single feeds, and this new feed-group pseudo power spectrum (FGPXS) is constructed to be more robust against systematic effects. In terms of data volume, the effective mapping speed is significantly increased due to an improved observational strategy as well as better data selection methodology. The updated spherically- and field-averaged FGPXS, $\tilde{C}(k)$, is consistent with zero, at a probability-to-exceed of around $34\,\%$, with an excess of $2.7\,\sigma$ in the most sensitive bin. Our power spectrum estimate is about an order of magnitude more sensitive in our six deepest bins across ${0.09\,\mathrm{Mpc}^{-1} < k < 0.73\,\mathrm{Mpc}^{-1}}$, as compared to the feed-feed pseudo power spectrum (FPXS) of COMAP ES. Each of these bins individually constrains the CO power spectrum to ${kP_\mathrm{CO}(k)< 2400-4900\,\mathrm{\mu K^2 Mpc^{2}}}$ at $95\,\%$ confidence. To monitor potential contamination from residual systematic effects, we analyze a set of 312 difference-map null tests and find that these are consistent with the instrumental noise prediction. Abstract: The CO Mapping Array Project (COMAP) Pathfinder is performing line intensity mapping of CO emission to trace the distribution of unresolved galaxies at redshift $z \sim 3$. We present an improved version of the COMAP data processing pipeline and apply this to the first two seasons of observations. This analysis improves on the COMAP Early Science (ES) results in several key aspects. On the observational side, all second season scans were made in constant-elevation mode, after noting that the previous Lissajous scans were associated with increased systematic errors; those scans accounted for 50% of the total Season 1 data volume. Secondly, all new observations were restricted to an elevation range of 35-65 degrees, to minimize sidelobe ground pickup. On the data processing side, more effective data cleaning in both the time- and map-domain has allowed us to eliminate all data-driven power spectrum-based cuts. This increases the overall data retention and reduces the risk of signal subtraction bias. On the other hand, due to the increased sensitivity, two new pointing-correlated systematic errors have emerged, and we introduce a new map-domain PCA filter to suppress these. Subtracting only 5 out of 256 PCA modes, we find that the standard deviation of the cleaned maps decreases by 67% on large angular scales, and after applying this filter, the maps appear consistent with instrumental noise. Combining all these improvements, we find that each hour of raw Season 2 observations yields on average 3.2 times more cleaned data compared to ES analysis. Combining this with the increase in raw observational hours, the effective amount of data available for high-level analysis is a factor of 8 higher than in ES. The resulting maps have reached an uncertainty of $25$-$50\,\mu K$ per voxel, providing by far the strongest constraints on cosmological CO line emission published to date., Comment: 23 pages, 22 figures, for submission to Astronomy and Astrophysics

Abstract: The Square Kilometre Array (SKA) is a planned large radio interferometer designed to operate over a wide range of frequencies, and with an order of magnitude greater sensitivity and survey speed than any current radio telescope. The SKA will address many important topics in astronomy, ranging from planet formation to distant galaxies. However, in this work, we consider the perspective of the SKA as a facility for studying physics. We review four areas in which the SKA is expected to make major contributions to our understanding of fundamental physics: cosmic dawn and reionisation; gravity and gravitational radiation; cosmology and dark energy; and dark matter and astroparticle physics. Abstract: Square Kilometre Array Cosmology Science Working Group: et al., arXiv:1811.02743v1, We present a detailed overview of the cosmological surveys that we aim to carry out with Phase 1 of the Square Kilometre Array (SKA1) and the science that they will enable. We highlight three main surveys: a medium-deep continuum weak lensing and low-redshift spectroscopic HI galaxy survey over 5 000 deg2; a wide and deep continuum galaxy and HI intensity mapping (IM) survey over 20 000 deg2 from to 3; and a deep, high-redshift HI IM survey over 100 deg2 from to 6. Taken together, these surveys will achieve an array of important scientific goals: measuring the equation of state of dark energy out to with percent-level precision measurements of the cosmic expansion rate; constraining possible deviations from General Relativity on cosmological scales by measuring the growth rate of structure through multiple independent methods; mapping the structure of the Universe on the largest accessible scales, thus constraining fundamental properties such as isotropy, homogeneity, and non-Gaussianity; and measuring the HI density and bias out to z = 6. These surveys will also provide highly complementary clustering and weak lensing measurements that have independent systematic uncertainties to those of optical and near-infrared (NIR) surveys like Euclid, LSST, and WFIRST leading to a multitude of synergies that can improve constraints significantly beyond what optical or radio surveys can achieve on their own. 16. Constraints on the evolution of the relationship between H i mass and halo mass in the last 12 Gyr

Abstract: The neutral hydrogen (H I) content of dark matter haloes forms an intermediate state in the baryon cycle that connects the hot shock-heated gas and cold star-forming gas in haloes. Measurement of the relationship between H I mass and halo mass therefore puts important constraints on galaxy formation models. We combine radio observations of H I in emission at low redshift (z ∼ 0) with optical/UV observations of H I in absorption at high redshift (1 < z < 4) to derive constraints on the evolution of the H I-mass–halo-mass (HIHM) relation from redshift z = 4 to 0. We find that one can model the HIHM relation similar to the stellar-mass–halo-mass (SHM) relation at z ∼ 0. At z = 0, haloes with mass 1011.7 M⊙ have the highest H I mass fraction (∼1 per cent), which is about four times smaller than their stellar-mass fraction. We model the evolution of the HIHM relation in a manner similar to that of the SHM relation. Combining this parametrization with a redshift- and mass-dependent modified Navarro–Frenk–White profile for the H I density within a halo, we draw constraints on the evolution of the HIHM relation from the observed H I column density, incidence rate and clustering bias at high redshift. We compare these findings with results from hydrodynamical simulations and other approaches in the literature and find the models to be consistent with each other at the 68 per cent confidence level.

Abstract: Anaemia is associated with a reduction in quality of life, and is common in patients with colorectal cancer . We recently reported the findings of the intravenous iron in colorectal cancer-associated anaemia (IVICA) trial comparing haemoglobin levels and transfusion requirements following intravenous or oral iron replacement in anaemic colorectal cancer patients undergoing elective surgery. In this follow-up study, we compared the efficacy of intravenous and oral iron at improving quality of life in this patient group. We conducted a multicentre, open-label randomised controlled trial. Anaemic colorectal cancer patients were randomly allocated at least two weeks pre-operatively, to receive either oral (ferrous sulphate) or intravenous (ferric carboxymaltose) iron. We assessed haemoglobin and quality of life scores at recruitment, immediately before surgery and at outpatient review approximately three months postoperatively, using the Short Form 36, EuroQoL 5-dimension 5-level and Functional Assessment of Cancer Therapy - Anaemia questionnaires. We recruited 116 anaemic patients across seven UK centres (oral iron n = 61 (53%), and intravenous iron n = 55 (47%)). Eleven quality of life components increased by a clinically significant margin in the intravenous iron group between recruitment and surgery compared with one component for oral iron. Median (IQR [range]) visual analogue scores were significantly higher with intravenous iron at a three month outpatient review (oral iron 70, (60-85 [20-95]); intravenous iron 90 (80-90 [50-100]), p = 0.001). The Functional Assessment of Cancer Therapy - Anaemia score comprises of subscales related to cancer, fatigue and non-fatigue items relevant to anaemia. Median outpatient scores were higher, and hence favourable, for intravenous iron on the Functional Assessment of Cancer Therapy - Anaemia subscale (oral iron 66 (55-72 [23-80]); intravenous iron 71 (66-77 [46-80]); p = 0.002), Functional Assessment of Cancer Therapy - Anaemia trial outcome index (oral iron 108 (90-123 [35-135]); intravenous iron 121 (113-124 [81-135]); p = 0.003) and Functional Assessment of Cancer Therapy - Anaemia total score (oral iron 151 (132-170 [69-183]); intravenous iron 168 (160-174 [125-186]); p = 0.005). Abstract: DESI (Dark Energy Spectropic Instrument) is a Stage IV ground-based dark energy experiment that will study baryon acoustic oscillations and the growth of structure through redshift-space distortions with a wide-area galaxy and quasar redshift survey. The DESI instrument is a robotically-actuated, fiber-fed spectrograph capable of taking up to 5,000 simultaneous spectra over a wavelength range from 360 nm to 980 nm. The fibers feed ten three-arm spectrographs with resolution $R= lambda/Deltalambda$ between 2000 and 5500, depending on wavelength. The DESI instrument will be used to conduct a five-year survey designed to cover 14,000 deg$^2$. This powerful instrument will be installed at prime focus on the 4-m Mayall telescope in Kitt Peak, Arizona, along with a new optical corrector, which will provide a three-degree diameter field of view. The DESI collaboration will also deliver a spectroscopic pipeline and data management system to reduce and archive all data for eventual public use.

Abstract: IntroductionPreoperative anaemia is a strong predictor of blood transfusion requirement. Patient Blood Management guidelines highlight the need for preoperative anaemia assessment and management in order to optimise haemoglobin prior to elective surgery. However, there is no randomised control trial elucidating the clinical effectiveness of the type of iron preparation that should be used preoperatively. The main objective of this study is to compare the efficacy of intravenous ferric carboxymaltose (Ferinject) therapy and oral iron on pre-operative haemoglobin levels in anaemic patients undergoing elective cardiac surgery.Method50 anaemic patients who were listed for elective cardiac surgery were recruited preoperatively and randomised to receive either oral iron (ferrous sulphate) or intravenous iron (ferric carboxymaltose). Change in haemoglobin level after an intervention was measured and data were collected on blood transfusion use and postoperative outcomes. Comparisons of continuous variables were performed using the t-test for normally distributed variables and the Mann-Whitney U test for skewed distributions. Comparisons of categorical variables were performed using chi-square test.ResultsThere were no differences in the baseline clinical, demographic characteristics and surgical procedures between the two groups. The median rise in haemoglobin is higher in the oral iron group (3.5 g/L; p=0.72) compared to intravenous group (2 g/L) but was not statistically significant. The first 12 hour blood loss was high in the intravenous group (median 655 (IQR 162–1540) ml versus312.5 (150–1750) ml in oral iron group ml; p=0.007). There were no comparable differences observed in post operative outcomes but the mean transfusion was more in the intravenous group (2.6 vs 1.8, p=0.1). At surgery, intravenous group had higher ferritin levels than oral group (median 392 µg/L vs 49) µg/l; p<0.0001)ConclusionIntravenous iron was not effective in improving preoperative haemoglobin after elective cardiac surgery and did not reduce blood transfusion requirements. Moreover, intravenous iron appears to significantly increase the risks of intraoperative bleeding. Abstract: IntroductionAnaemia is associated with multiple symptoms including fatigue which in turn can contribute to impaired quality of life. It is a common finding in colorectal cancer as a result of chronic insidious haemorrhage and impaired iron haemostasis. Consequently, colorectal cancer patients are at risk of the symptomatology of anaemia.MethodPreoperative colorectal cancer patients (n=116) who were found to be anaemic (>1g/dL below the World Health Organisation definition) were randomised to receive either intravenous (IV) or oral iron. Quality of life (QOL) questionnaires were performed at; [i] recruitment, [ii] at least 14 days after iron therapy and [iii] post-operatively in the first outpatient follow-up. QOL assessments undertaken included the following; [a] Functional Assessment of Cancer Therapy-Anaemia (FACT-AN) [b] EuroQol EQ-5D-5L (EQ5D) and [c] modified Short-Form 36 (SF36) v1 questionnaires.ResultsBoth groups were comparable in patient demographics, starting haemoglobin, operative details, tumour histology and time from recruitment to postoperative review (oral 101 days [IQR 62-193]; IV 91 days [IQR 61-135, P=0.98]. Despite homogeneity in all initial QOL scores at recruitment significant differences were evident between groups at outpatient review in the all bar two of the SF36 components including: General Health(Oral 64.79u [58.8-70.8]; IV 73.63u [68.4-78.8]; P<0.01), Vitality(Oral 60.49u [53.7-67.3]; IV 74.83u [69.2-80.5]; P<0.001). EQ5D scores for the visual analogue scale were also significantly higher in the IV group at outpatient review (OI 70.9u [65.1-76.8]; IV 82.53u [77.8-87.2]; P<0.001), with parity in the remaining 4 components. Furthermore, significant differences were evident in FACT-AN Total scores(Oral 148.3u [139-158];IV 166.1u [160-172.3]; P<0.01), FACT-AN Anaemia subscale scores(Oral 59.6u [54.4-64.8]; IV 69.1u [65.7-72.5]; P<0.01), Functional Well Being(Oral 20.2u [17.9-22.5]; IV 24.1u [22.2-26];P<0.01) and Emotional Well Being(Oral 19.9u [18.5-21.2]; IV 21.5u [20-23]; P<0.05) at outpatient review. None of the QOL measures had higher scores in the oral group.ConclusionIntravenous iron is more efficacious than oral iron at improving the post-operative quality of life of anaemic colorectal cancer patients. Abstract: Square Kilometre Array: et al., arXiv:1810.02680v3, The Square Kilometre Array (SKA) is a planned large radio interferometer designed to operate over a wide range of frequencies, and with an order of magnitude greater sensitivity and survey speed than any current radio telescope. The SKA will address many important topics in astronomy, ranging from planet formation to distant galaxies. However, in this work, we consider the perspective of the SKA as a facility for studying physics. We review four areas in which the SKA is expected to make major contributions to our understanding of fundamental physics: cosmic dawn and reionisation; gravity and gravitational radiation; cosmology and dark energy; and dark matter and astroparticle physics. These discussions demonstrate that the SKA will be a spectacular physics machine, which will provide many new breakthroughs and novel insights on matter, energy, and spacetime., A. Abstract: Background: LCP1 encodes L-plastin, an actin-bundling protein primarily expressed in hematopoietic cells. In mouse and fish models, LCP1 deficiency has been shown to result in hematologic and immune defects., Objective: This study aimed to determine the nature of a human inborn error of immunity resulting from a novel genetic variant of LCP1., Methods: We performed genetic, protein, and cellular analysis of PBMCs from a kindred with apparent autosomal dominant immune deficiency. We identified a candidate causal mutation in LCP1, which we evaluated by engineering the orthologous mutation in mice and Jurkat cells., Results: A splice-site variant in LCP1 segregated with lymphopenia, neutropenia, and thrombocytopenia. The splicing defect resulted in at least 2 aberrant transcripts, producing an in-frame deletion of 24 nucleotides, and a frameshift deletion of exon 8. Cellular analysis of the kindred revealed a proportionate reduction of T and B cells and a mild expansion of transitional B cells. Similarly, mice carrying the orthologous genetic variant exhibited the same in-frame aberrant transcript, reduced expression Lcp1 and gene dose-dependent leukopenia, mild thrombocytopenia, and lymphopenia, with a significant reduction of T-cell populations. Functional analysis revealed that LCP1 c740 - 1G>A confers a defect in platelet development and function with aberrant spreading on collagen. Immunologic analysis revealed defective actin organization in T cells, reduced migration of PBMCs from patients, splenocytes from mutant mice, and a mutant Jurkat cell line in response to CXCL12; impaired germinal center B-cell expansion after immunization; and reduced cytokinesis during T cell proliferation., Conclusions: We describe a unique human hematopoietic defect affecting neutrophils, lymphocytes, and platelets arising from partial LCP1 deficiency., (Copyright © 2024 The Authors. Abstract: Cascade testing for families with BRCA pathogenic variants is important to identify relatives who are carriers. These relatives can benefit from appropriate risk management and preventative strategies arising from an inherited increased risk of breast, ovarian, prostate, melanoma, and pancreatic cancers. Cascade testing has the potential to enable cost-effective cancer control even in low- and middle-income settings, but few studies have hitherto evaluated the psychosocial impact of cascade testing in an Asian population, where the cultural and religious beliefs around inheritance and destiny have previously been shown to influence perception and attitudes toward screening. In this study, we evaluated the short- and long-term psychosocial impact of genetic testing among unaffected relatives of probands identified through the Malaysian Breast Cancer Genetics Study and the Malaysian Ovarian Cancer Study, using validated questionnaires (Hospital Anxiety and Depression Scale and Cancer Worry Scale) administered at baseline, and 1-month and 2-year post-disclosure of results. Of the 305 unaffected relatives from 98 independent families who were offered cascade testing, 256 (84%) completed predictive testing and family history of cancers was the only factor significantly associated with uptake of predictive testing. We found that the levels of anxiety, depression, and cancer worry among unaffected relatives decreased significantly after result disclosure and remained low 2-year post-result disclosure. Younger relatives and relatives of Malay descent had higher cancer worry at both baseline and after result disclosure compared to those of Chinese and Indian descent, whereas relatives of Indian descent and those with family history of cancers had higher anxiety and depression levels post-result disclosure. 44. Large Exchange Coupling Between Localized Spins and Topological Bands in MnBi 2 Te 4 .

Abstract: Magnetism in topological materials creates phases exhibiting quantized transport phenomena with potential technological applications. The emergence of such phases relies on strong interaction between localized spins and the topological bands, and the consequent formation of an exchange gap. However, this remains experimentally unquantified in intrinsic magnetic topological materials. Here, this interaction is quantified in MnBi 2 Te 4 , a topological insulator with intrinsic antiferromagnetism. This is achieved by optically exciting Bi-Te p states comprising the bulk topological bands and interrogating the consequent Mn 3d spin dynamics, using a multimodal ultrafast approach. Ultrafast electron scattering and magneto-optic measurements show that the p states demagnetize via electron-phonon scattering at picosecond timescales. Despite being energetically decoupled from the optical excitation, the Mn 3d spins, probed by resonant X-ray scattering, are observed to disorder concurrently with the p spins. Together with atomistic simulations, this reveals that the exchange coupling between localized spins and the topological bands is at least 100 times larger than the superexchange interaction, implying an optimal exchange gap of at least 25 meV in the surface states. 45. Attitudes and training needs of oncologists and surgeons in mainstreaming breast cancer genetic counseling in a low-to-middle income Asian country.

Abstract: With the advent of poly-ADP-ribose polymerase inhibitor (PARPi) therapies, the focus of genetic testing for breast, ovarian, and other cancers has shifted from risk management to treatment decision-making in high-resource settings. Due to the shortage of genetic counselors worldwide, alternative ways of delivering genetic counseling have been explored, including training nongenetics healthcare professionals (NGHPs) to provide genetic counseling. However, little is known about the feasibility of adopting such models in healthcare settings with insufficient specialists, where population health literacy is low and where access to new therapies may be limited. In this study, we evaluated the attitudes, considerations, and self-efficacy of oncologists, breast surgeons, and general surgeons in mainstreaming breast cancer genetic counseling in Malaysia, a middle-income Asian country with a universal healthcare system. We developed a 32-item survey via a modified Delphi method, which was then distributed via a purposive and network sampling approach. While 77% of respondents expressed interest in providing breast cancer genetic counseling, 85% preferred to refer patients directly to genetic services for genetic counseling and testing. The main considerations for mainstreaming were the cost of genetic testing and PARPi therapy, as well as the availability of support from genetics professionals. Respondents reported a lack of confidence in communicating genetic risk, particularly to patients with poor health literacy, and in the clinical management of patients with variants of uncertain significance. Our results highlight the urgent need to train more NGHPs in providing genetic counseling and testing in low-to-middle income countries, and suggest that the mainstay for genetic counseling in this setting may be for risk management rather than access to PARPi therapy., (© 2022 National Society of Genetic Counselors.)

Abstract: Purpose: This study aims to evaluate the effect of rehabilitation with complete dentures versus implant-retained overdenture on activity in various parts of the brain cognition in a geriatric edentulous population via Functional MRI (fMRI) studies and the Mini-Mental State Examination (MMSE)., Methods: Ten completely edentulous patients were rehabilitated with both complete dentures and two-implant retained overdentures for three months each. fMRI studies were performed for each modality during chewing and recall tasks at three time periods: T0: Completely Edentulous (CE) T1: after three months of wearing Conventional Complete Dentures (CD) and T2: after three months of wearing Implant-retained Overdentures (IOD). The Z scores obtained from the fMRI at these phases of examination were tabulated and correlated with MMSE scores obtained at the corresponding time periods., Results: Z scores obtained during the memory recall tasks at T2 were the greatest (Prefrontal Cortex (p=0.059) and Hippocampus (p=0.036). The MMSE scores obtained were significantly higher for the IODs when compared to the CDs and Baseline values (p < 0.05)., Conclusion: IODs may potentially result in superior sensory feedback in edentulous patients and lead to improved cognitive performance when compared to conventional complete dentures.

47. Interlayer magnetophononic coupling in MnBi 2 Te 4 .

Abstract: The emergence of magnetism in quantum materials creates a platform to realize spin-based applications in spintronics, magnetic memory, and quantum information science. A key to unlocking new functionalities in these materials is the discovery of tunable coupling between spins and other microscopic degrees of freedom. We present evidence for interlayer magnetophononic coupling in the layered magnetic topological insulator MnBi 2 Te 4 . Employing magneto-Raman spectroscopy, we observe anomalies in phonon scattering intensities across magnetic field-driven phase transitions, despite the absence of discernible static structural changes. This behavior is a consequence of a magnetophononic wave-mixing process that allows for the excitation of zone-boundary phonons that are otherwise 'forbidden' by momentum conservation. Our microscopic model based on density functional theory calculations reveals that this phenomenon can be attributed to phonons modulating the interlayer exchange coupling. Moreover, signatures of magnetophononic coupling are also observed in the time domain through the ultrafast excitation and detection of coherent phonons across magnetic transitions. In light of the intimate connection between magnetism and topology in MnBi 2 Te 4 , the magnetophononic coupling represents an important step towards coherent on-demand manipulation of magnetic topological phases., (© 2022. Abstract: Background: Identifying patients with BRCA mutations is clinically important to inform on the potential response to treatment and for risk management of patients and their relatives. However, traditional referral routes may not meet clinical needs, and therefore, mainstreaming cancer genetics has been shown to be effective in some high-income and high health-literacy settings. To date, no study has reported on the feasibility of mainstreaming in low-income and middle-income settings, where the service considerations and health literacy could detrimentally affect the feasibility of mainstreaming., Methods: The Mainstreaming Genetic Counselling for Ovarian Cancer Patients (MaGiC) study is a prospective, two-arm observational study comparing oncologist-led and genetics-led counselling. This study included 790 multiethnic patients with ovarian cancer from 23 sites in Malaysia. We compared the impact of different method of delivery of genetic counselling on the uptake of genetic testing and assessed the feasibility, knowledge and satisfaction of patients with ovarian cancer., Results: Oncologists were satisfied with the mainstreaming experience, with 95% indicating a desire to incorporate testing into their clinical practice. The uptake of genetic testing was similar in the mainstreaming and genetics arm (80% and 79%, respectively). Patient satisfaction was high, whereas decision conflict and psychological impact were low in both arms of the study. Notably, decisional conflict, although lower than threshold, was higher for the mainstreaming group compared with the genetics arm. Overall, 13.5% of patients had a pathogenic variant in BRCA1 or BRCA2, and there was no difference between psychosocial measures for carriers in both arms., Conclusion: The MaGiC study demonstrates that mainstreaming cancer genetics is feasible in low-resource and middle-resource Asian setting and increased coverage for genetic testing., Competing Interests: Competing interests: None declared., (© Author(s) (or their employer(s)) 2022. Abstract: There is an increasing number of cancer patients undertaking treatment-focused genetic testing despite not having a strong family history or high a priori risk of being carriers because of the decreasing cost of genetic testing and development of new therapies. There are limited studies on the psychosocial outcome of a positive result among breast cancer patients who are at low a priori risk, particularly in women of Asian descent. Breast cancer patients enrolled under the Malaysian Breast Cancer Genetic Study between October 2002 and February 2018 were tested for BRCA1, BRCA2 and PALB2 genes. All 104 carriers identified were invited by a research genetic counsellor for result disclosure. Of the 104 carriers, 64% (N = 66) had low a priori risk as determined by PENN II scores. Psychosocial, risk perception and health behaviour measures survey were conducted at baseline (pre-result disclosure), and at two to six weeks after result disclosure. At baseline, younger carriers with high a priori risk had higher Cancer Worry Scale scores than those with low a priori risk but all scores were within acceptable range. Around 75% and 55% of high a priori risk carriers as well as 80% and 67% of low a priori risk carriers had problems in the "living with cancer" and "children" psychosocial domains respectively. All carriers regardless of their a priori risk demonstrated an improved risk perception that also positively influenced their intent to undergo risk management procedures. This study has shown that with sufficient counselling and support, low a priori risk carriers are able to cope psychologically, have improved perceived risk and increased intent for positive health behaviour despite having less anticipation from a family history prior to knowing their germline carrier status., Competing Interests: The authors have declared that no competing interests exist.

Abstract: The coexistence of ferroelectricity and metallicity seems paradoxical, since the itinerant electrons in metals should screen the long-range dipole interactions necessary for dipole ordering. The recent discovery of the polar metal LiOsO 3 was therefore surprising [as discussed earlier in Y. Shi et al., Nat. Mater . 2013 , 12 , 1024]. It is thought that the coordination preferences of the Li play a key role in stabilizing the LiOsO 3 polar metal phase, but an investigation from the combined viewpoints of core-state specificity and symmetry has yet to be done. Here, we apply the novel technique of extreme ultraviolet second harmonic generation (XUV-SHG) and find a sensitivity to the broken inversion symmetry in the polar metal phase of LiOsO 3 with an enhanced feature above the Li K-edge that reflects the degree of Li atom displacement as corroborated by density functional theory calculations. These results pave the way for time-resolved probing of symmetry-breaking structural phase transitions on femtosecond time scales with element specificity.
