We present a study of the Effective Field Theory (EFT) generalization of stochastic inflation in a model-independent single-field framework and its impact on primordial black hole (PBH) formation. We show how the Langevin equations for the "soft" modes in quasi de Sitter background is described by the Infra-Red (IR) contributions of scalar perturbations, and the subsequent Fokker-Planck equation driving the probability distribution for the stochastic duration ${\cal N}$, significantly modify in the present EFT picture. An explicit perturbative analysis of the distribution function by implementing the stochastic-$\delta N$ formalism is performed up to the next-to-next-to-next-to-leading order (NNNLO) for both the classical-drift and quantum-diffusion dominated regimes. In the drift-dominated limit, we perturbatively analyse the local non-Gaussianity parameters $(f_{\rm NL}, g_{\rm NL}, \tau_{\rm NL})$ with the EFT-induced modifications. The Apatani community in Arunachal Pradesh, India has long practiced rice-fish coculture, integrating rice cultivation with fish farming to enhance food security and promote environmental sustainability. This traditional method utilizes the region’s wet rice terraces, allowing for the simultaneous production of rice and fish, contributing to efficient land and water resource utilization. Despite the organic nature of this practice, challenges such as suboptimal input distribution, inappropriate combinations of rice and fish species, and limited access to training have hindered productivity. The overall average yields of 1389 kg/ha for rice and 209 kg/ha for fish, indicating room for improvement. This study focused on optimizing resource utilization and fund allocation among Apatani farmers in Ziro Valley to address these issues. Data were collected from a random sample of 200 farmers across the valley. Using Cobb-Douglas production functions, researchers estimated rice and fish yields through regression models. Subsequently, a non-linear constrained optimization problem based on the Nash-bargaining framework was solved to assess the effects of optimization on fund allocation for various inputs. The findings revealed that rice and fish yields could potentially exceed 2000 kg/ha and 300 kg/ha, respectively, with proper resource management. Critical factors identified for enhancing productivity included using mixed manure and stocking fingerlings. Moreover, improving farmers’ technical knowledge and skills through institutional support and policy interventions was deemed vital for establishing rice-fish coculture as a sustainable livelihood in Ziro Valley. In conclusion, while the Apatani community’s traditional rice-fish coculture system offers a foundation for sustainable agriculture, targeted optimization of resource utilization, combined with enhanced training and institutional support, is essential to realize its full potential in improving productivity and ensuring long-term sustainability.

IntroductionAquatic foods, particularly fish, are essential for addressing malnutrition, especially in vulnerable populations like children and women. In India, traditional aquaculture practices centered around carp species often overlooked the production of nutrient-rich small fish. To address this, nutrition-sensitive aquaculture approaches advocate for integrating species like mola carplet (Amblypharyngodon mola) rich in micronutrients, into existing systems. In Odisha, India, where poverty and food insecurity are prevalent, the government initiated a program to empower women through aquaculture, focusing on nutrition-sensitive carp-mola polyculture in community ponds through Women Self-Help Groups (WSHGs).MethodsThis study evaluates the effectiveness of this government program in enhancing income, household nutrition, and women’s empowerment. Data from field surveys conducted across all 30 districts of Odisha were analyzed to assess participation, capacity building, adoption of better management practices (BMPs), productivity of carp-mola polyculture, household fish consumption, and profitability.Results and discussionThe study found widespread participation and adoption of BMPs among WSHGs, leading to increased productivity and income. Carp-mola polyculture systems showed higher productivity and consumption rates, contributing to improved nutrition among WSHGs and their communities. Despite challenges such as input costs and limited mola availability, WSHGs reported profitability from fish farming, with carp-mola polyculture systems yielding higher net income. Factors influencing productivity and profitability included water retention period, stocking density, feed application, and training. The program’s impact extended beyond economic benefits, encompassing environmental improvement, women’s empowerment, and enhanced nutrition outcomes.The study highlights the success of the government program in promoting sustainable aquaculture practices and improving nutrition outcomes in Odisha. Continued support, capacity building, and collaboration among stakeholders are essential for scaling up nutrition-sensitive aquaculture interventions and ensuring long-term sustainability. Strengthening dissemination processes, addressing challenges, and further research on small indigenous fish production techniques are crucial for maximizing the program’s impact on food security and rural development.

The eyes are the only sense organ required for vision. Diseases like glaucoma, cataract, diabetic retinopathy etc. affect the proper functioning of the eyes and sometimes lead to blindness. The treatment of eye disorders is very challenging because of the unique structure of this organ. The traditional treatment approaches are not effective in providing good ocular bioavailability. The Provesicular systems are new-generation delivery systems that can improve drug bioavailability and provide therapeutic responses in a controlled manner for desired time. Among all, liposomes are the first such delivery vehicle but due to the lack of stability and the high cost, niosomes were formulated. Niosomes are nanosized vesicles composed of non-ionic surfactants that can encapsulate both lipophilic and hydrophilic drugs. The drawbacks associated with niosomes, like fusion, aggregation, sedimentation, difficulty in sterilization, leaking, etc., gave birth to proniosomes. Proniosomes are more stable and bioavailable than niosomes and liposomes. Proniosomes are dry formulations of hydrophilic carrier particles coated with a water-soluble non-ionic surfactant that, when hydrated, instantly transforms into niosomes. Proniosomes can be used as stable, non-toxic carriers to improve the ocular residence and bioavailability of many drugs. 33. Clinical characteristics, therapeutics, and treatment outcomes of adult patients with anti-melanoma differentiation-associated gene 5 dermatomyositis: A single-center experience from South India

Introduction: To study the clinical characteristics and treatment outcomes of patients with anti-melanoma differentiation-associated gene 5 (MDA5) dermatomyositis (DM). Methods: This was a retrospective study done between 2019 and 2021 in a tertiary care center in South India. All consecutive patients, presenting to the adult rheumatology department, classified as idiopathic inflammatory myositis (IIM), and positive for anti-MDA5 antibodies, were included in the study. Baseline characteristics of anti-MDA5 patients were compared with the data of non-MDA5 patients over the last 10 years. Clinical, biochemical, and treatment responses were assessed on follow-up. Complete and partial responders were identified using predefined criteria. Factors predicting mortality were determined by logistic regression analysis. Data were analyzed using the Statistical Package for the Social Sciences (SPSS) version 25. Results: A total of 29 adult patients of IIM were positive for anti-MDA5 antibody during the study period. The mean (±standard deviation) age of the patients was 40.3 (±13.02) years with a female:male ratio of 1.4:1. Panniculitis, calcinosis, palmar papules, and ulcerated Gottron's were the specific cutaneous manifestations, seen in 3 (10%), 7 (24%), 4 (13.4%), and 7 (24%) patients, respectively. A total of 14 patients (48.1%) had clinically amyopathic DM and 17 patients (60.7%) had interstitial lung disease (ILD) diagnosed by high-resolution computerized tomography scan, of which organizing pneumonia was the predominant pattern. Complete response was seen in 10 patients (43.4%), while a partial response was seen in 8 patients (34.7%). Five patients died on follow-up, accounting for mortality of 21%. Age >50 years was significantly associated with mortality (P = 0.025). Gottron's sign (P < 0.001), panniculitis (P < 0.001), calcinosis (P < 0.001), cutaneous ulcerations (P < 0.001), inflammatory arthritis (P < 0.001), and ILD (P < 0.02) were present more commonly in the anti-MDA5 IIM group, whereas myopathy (P < 0.001), elevated creatine phosphokinase (P < 0.001), and lactate dehydrogenase (P < 0.001) were more frequently present in the non-anti-MDA5 IIM group (n = 421). Conclusions: Anti-MDA5 DM represents a distinct and unique subset of IIM with characteristic clinical manifestations. Elderly age is a poor prognostic factor of mortality.

Introduction: Safety of rituximab (RTX) in autoimmune rheumatic diseases (AIRDs) has been a concern during this ongoing coronavirus (COVID-19) pandemic. Delayed worsening of COVID symptoms, increased hospital stays, and mortality has been reported among those infected post-RTX. This study describes the occurrence and course of COVID-19 infection among AIRD Indian patients who received RTX during this pandemic. Patients and Methods: Adult patients (≥18 years) with any AIRD, who received RTX between October 2019 and May 2021, were enrolled in this study. Patients satisfying the inclusion criteria were telephonically enquired about contracting confirmed COVID-19, its course, treatment, and outcome. Baseline parameters, immunoglobulin G level, vaccination, and comorbidity status before RTX were compared between the COVID-19 infected and noninfected patients to determine factors affecting the outcome. Results: Out of 1081 patients admitted during the study period, 218 patients received RTX. The mean age of these patients was 40.1 ± 14.2 years and comprised mostly of women (81.7%). Rheumatoid arthritis followed by lupus and anti-neutrophilic cytoplasmic antibodies-associated vasculitis was the predominant AIRD. Among the patients contacted (207/218 [94.9%]), 11 (5.3%) patients reported confirmed COVID-19 infections. Out of these, three (27.3%) had severe COVID-19 and one patient succumbed to it. Others became symptom-free after a mean duration of 14.4 ± 4.7 days of onset of symptoms. No significant difference among the baseline parameters observed predicted COVID-19 susceptibility. Conclusion: Among our AIRD patients treated with RTX, occurrence, and mortality of COVID-19 infection was low. Younger age of our patient cohort and female predominance might have contributed in reducing the severity of SARS-CoV-2 infection.

IntroductionSmall indigenous fish species (SIS) can be particularly rich in micronutrients and make a crucial contribution toward improving human nutrition. The introduction of mola (Amblypharyngodon mola), an SIS, which is particularly rich in vitamin A, into smallholder carp aquaculture systems has been widely promoted over the past decade as a promising nutrition-sensitive innovation. However, standardized techniques for the hatchery mass production of mola do not yet exist. We hypothesized that the lack of commercial hatchery mass-production techniques for mola seed is the key barrier limiting potential for widespread adoption of carp–SIS polyculture.MethodsTo address this gap, we conducted breeding trials at a private hatchery in Odisha, India, from July to September 2022, to identify standardized methods for the hatchery-based mass production of mola seed. Breeding was induced using a synthetic gonadotropin-releasing hormone analog (SGnRHa) at 0.5 mL and 0.25 mL per kg of body weight of female fish and male fish, respectively. Fish spawned in double hapas in breeding tanks.ResultsThe average fertilization, spawning, and hatching rates over 10 breeding cycles were 81%, 82%, and 85%, respectively. A total of 8.5 million fertilized eggs and 6.4 million hatchlings were produced. The survival of fry during larval rearing trials at a stocking rate of 500 hatchlings/m2 was 58% after 22 days. The mola hatchlings and fry were sold to 29 farmers at prices comparable to those of Indian major carp.DiscussionThis article makes a unique contribution to the literature by documenting the entire process of hatchery-based mass mola seed production, including broodfish collection and maintenance, hormone dose optimization, breeding arrangements, breeder characteristics, breeding behavior and performance fecundity, larval rearing, and seed sales to farmers. This information is intended to serve as a protocol to be followed by any individual or institution with an interest in mola breeding and represents an important contribution to the development of nutrition-sensitive aquaculture.

Immunoglobulin G4-related disease (IgG4-RD) is a systemic autoimmune disorder with a variety of manifestations. Association with other systemic autoimmune diseases is uncommonly reported in the literature. Hence, we intended to understand better the uniqueness of the overlapping associations of IgG4RD with other systemic autoimmune diseases. Retrospective electronic medical records-based review of IgG4RD occurring as an overlap with other systemic autoimmune diseases seen in the rheumatology department was done. We present two cases of IgG4 disease overlapping with systemic rheumatic diseases with a brief review of the literature. The first case was an overlap IgG4RD with lupus and the second interesting case had IgG4RD overlapping with 2 autoimmune diseases (lupus and rheumatoid arthritis) in the same patient. Both patients were treated with high-dose steroids and mycophenolate mofetil with good response, however, the second patient needed to be additionally treated with rituximab for better control of arthritis. Although uncommon, here, we report the first case series of 2 patients with IgG4 disease overlapping with other systemic autoimmune diseases. In our case series, the presence of the overlap did not impact the immediate clinical outcome or treatment response of IgG4RD or the co-existent systemic autoimmune disease.

44. An unusual case of vomiting and arthritis: A diagnostic enigma

Sarcoidosis is a multisystem inflammatory disorder, which can have a myriad of presentations, with atypical presentation not being uncommon. Unless the clinician has a strong suspicion of sarcoidosis, it can be easily missed out. Here, we present an interesting case of a 44-year-old gentleman who had presented with inflammatory arthritis and vomiting. His serum cortisol levels were low. On further evaluation, he was found to have panhypopituitarism. Magnetic resonance imaging brain showed infiltrative disease involving the pituitary stalk. Imaging of the chest revealed mediastinal nodes which showed nonnecrotizing granulomatous inflammation on histopathology. After ruling out infectious causes a diagnosis of sarcoidosis was made, and the patient had a full recovery with steroids and methotrexate. Sarcoidosis can be a great mimicker presenting with a myriad of complications. Clinicians must be aware of the atypical presentations to diagnose the disease earlier.

46. An EPQ model with different demand and deterioration rate for two warehouses under shortage, learning and imperfect production

- Abstract
This paper presents a mathematical framework to obtain a production model for deteriorating items with learning effect in production cost. The study considers different demand rates and different deterioration rate. In this model, one is own warehouse (OW) and other one is rented warehouse (RW) with different demand rate is considered. Every producer wants to get maximum benefit in his business, and he want to vacate the RW very soon, due to which he has to pay the least rent, so demand rate for RW is strictly increasing function of time. On the contrary, he can use his OW in such a way that the producer gets benefit and can build the selling price of inventory according to his profit, so demand rate for OW is selling price dependent. These assumptions effects on demand therefore production rate is taken as demand dependent. Shortage is also considered. Numerical example and sensitivity analysis of some parameters provided to examine the impact on the optimal total cost of the system. [Received: 28 January 2022; Accepted: 23 August 2023]

47. Development of trust among customer and producer inventory model, when demand is dependent on selling price and with shortage under the environment of uncertainty

- Abstract
Mostly, it is observed that when the individual inventory model is given importance, then demand is either constant or time dependent, but at the same time, the selling value of the inventory cannot be ignored. Generally, for many consumer products, it is observed that the selling price of an item greatly influences the rate of consumption. Due to which the demand of the product may vary with the selling price, also it is necessary to check the reliability of the inventory maintained during production. And when the demand of the product depends on the selling price and is favourable to the customer, then the possibility of shortage is also high. Therefore, it is necessary to give importance to the impact of selling price and shortage while developing the inventory model. Along with this, many types of uncertainties arise during the production, and to overcome these uncertainties, the model is developed by making a fuzzy model. Numerical examples and sensitivity analysis of key parameters are provided in order to investigate the effect on the optimal total cost of the system.
