Introduction: Malnutrition is found in children with cancer and is associated with negative clinical outcomes. Objective: To describe the prevalence of inadequate nutritional status of children and adolescents with malignant neoplasm at hospital admission in childhood cancer reference centers in Brazil. Method: Cross-sectional study nested in a multicenter, hospital-based cohort study. The probabilistic sample was carried out in two stages in each stratum by macro-region using the probability method proportional to the size with one year of collection in each institution. Clinical, anthropometric, body composition data and the Pediatric Subjective Global Nutritional Assessment (SGNA) questionnaire were collected from 13 reference institutions within 48 hours of hospital admission, from March 2018 to August 2019. Results: The study totaled 723 patients in the 5 regions of Brazil. The prevalence of moderate and severe malnutrition was 25.9% in the age group of 2 to 5 years, 40.1% in 5 to 10 years and 39.7% in 10 to 19 years, according to the SGNA. According to the Body Mass/Age Index (BMI/I), thinness and marked thinness totaled 13%, risk of overweight, overweight and obesity showed a prevalence of 26.7% from 2 to 5 years, 24.9% from 5 to 10 years and 25.7% from 10 to 19 years. Conclusion: There was a high prevalence of nutritional inadequacy by the SGNA, suggesting that malnutrition can be underdiagnosed when using only the BMI/I, strengthening the need to use complementary methods in the nutritional assessment of children with cancer. Introducción: La desnutrición se observa en niños con cáncer y se asocia con resultados clínicos negativos. Objetivo: Describir la prevalencia del estado nutricional inadecuado de niños y adolescentes con neoplasia maligna al ingreso hospitalario en centros de referencia de cáncer infantil en Brasil. Método: Estudio transversal anidado en un estudio de cohorte hospitalario multicéntrico. La muestra probabilística se realizó en dos etapas en cada estrato por macrorregión utilizando el método de probabilidad proporcional al tamaño con un ano de recolección en cada institución. Se recopilaron datos clínicos, antropométricos, de composición corporal y el cuestionario Global Pediatric Subjective Nutritional Assessment (ANSGP) de 13 instituciones de referencia dentro de las 48 horas posteriores al ingreso hospitalario, desde marzo de 2018 hasta agosto de 2019. Resultados: El estudio totalizo 723 pacientes en las 5 regiones de Brasil. La prevalencia de desnutrición moderada y severa fue de 25,9% en el grupo de edad de 2 a 5 años, 40,1% de 5 a 10 anos y 39,7% de 10 a 19 años, según la ANSGP. Según el Índice de Masa Corporal/Edad (IMC/I), la delgadez y la delgadez marcada totalizaron 13%, el riesgo de sobrepeso, sobrepeso y obesidad mostro una prevalencia de 26,7% de 2 a 5 años, 24,9% de 5 a 10 años y 25,7% de 10 a 19 años. Conclusión: Hubo una alta prevalencia de insuficiencia nutricional por parte de la ANSGP, lo que sugiere que la desnutrición puede ser infradiagnosticada cuando se utiliza solo el IMC/I, fortaleciendo la necesidad de utilizar métodos complementarios en la evaluación nutricional de los niños con cáncer. Introdução: A desnutrição e observada em crianças com câncer e está associada a desfechos clínicos negativos. Objetivo: Descrever a prevalência de inadequação do estado nutricional de crianças e adolescentes com neoplasia maligna na admissão hospitalar em Centros de Referência do câncer infantil no Brasil. Método: Estudo transversal aninhado a um estudo de coorte, multicêntrico, de base hospitalar. A amostra probabilística foi feita em dois estágios em cada estrato por Macrorregião pelo método de probabilidade proporcional ao tamanho com um ano de coleta em cada instituição. Foram coletados em 13 instituições de referência dados clínicos, antropométricos, de composição corporal e sobre o questionário de Avaliação Nutricional Subjetiva Global Pediátrica (ANSGP), em até 48 horas da admissão hospitalar, entre marco de 2018 e agosto de 2019. Resultados: O estudo totalizou 723 pacientes nas cinco regiões do Brasil. A prevalência de desnutrição moderada e grave foi de 25,9% na faixa etária de 2 a 5 anos, 40,1% de 5 a 10 anos e 39,7% de 10 a 19 anos, de acordo com ANSGP. Segundo o Índice de Massa Corporal/Idade (IMC/I), magreza e magreza acentuada totalizaram 13%, risco de sobrepeso, sobrepeso e obesidade apresentaram uma prevalência de 26,7% de 2 a 5 anos; 24,9% de 5 a 10 anos; e 25,7% de 10 a 19 anos. Conclusão: Evidenciou-se alta prevalência de inadequação nutricional pela ANSGP, sugerindo que a desnutrição pode ser subdiagnosticada quando utilizado somente o IMC/I, fortalecendo a necessidade de utilização de métodos complementares na avaliação nutricional de crianças com câncer.

No. of bitstreams: 0 Previous issue date: 2020-12-01 Bill and Melinda Gates Foundation Fundação de Amparo à Pesquisa do Estado do Rio de Janeiro (FAPERJ) Pooled data analysis in the field of maternal and child nutrition rarely incorporates data from low- and middle-income countries and existing studies lack a description of the methods used to harmonize the data and to assess heterogeneity. We describe the creation of the Brazilian Maternal and Child Nutrition Consortium dataset, from multiple pooled longitudinal studies, having gestational weight gain (GWG) as an example. Investigators of the eligible studies published from 1990 to 2018 were invited to participate. We conducted consistency analysis, identified outliers, and assessed heterogeneity for GWG. Outliers identification considered the longitudinal nature of the data. Heterogeneity was performed adjusting multilevel models. We identified 68 studies and invited 59 for this initiative. Data from 29 studies were received, 21 were retained for analysis, resulting in a final sample of 17,344 women with 72,616 weight measurements. Fewer than 1% of all weight measurements were flagged as outliers. Women with pre-pregnancy obesity had lower values for GWG throughout pregnancy. GWG, birth length and weight were similar across the studies and remarkably similar to a Brazilian nationwide study. Pooled data analyses can increase the potential of addressing important questions regarding maternal and child health, especially in countries where research investment is limited. OBJETIVO: descrever o consumo de álcool e cigarro em gestantes adultas e identificar a associação desse consumo ao resultado obstétrico. MÉTODOS: trata-se de um estudo analítico do tipo transversal, no qual foram incluídas 433 puérperas adultas e seus conceptos atendidos em maternidade pública do Rio de Janeiro, no período de 1999 a 2006. As informações sobre as puérperas e os recém-nascidos foram coletadas no momento do parto e no puerpério, por meio de entrevista e consultas aos prontuários. Considerou-se "uso de álcool na gestação" e "uso de cigarro na gestação" quando esses foram detectados pelo profissional de Saúde na consulta de pré-natal em qualquer idade gestacional e registrados no prontuário. RESULTADOS: verificou-se que 5,5 e 7,4% das puérperas relataram uso de cigarro e álcool durante a gestação, respectivamente. As características maternas associadas ao fumo na gestação foram: situação marital (p=0,005); idade materna (p=0,01) e assistência nutricional pré-natal (p=0,003). O fumo durante a gestação foi fortemente associado ao uso do álcool, sendo que 31,3% das gestantes fizeram uso concomitante de cigarro e álcool (p0,05). CONCLUSÕES: os achados sugerem que o uso de cigarro e álcool na gestação deve ser investigado na assistência pré-natal dentre todas as mulheres, especialmente entre as que vivem sem o companheiro, com mais de 35 anos, com história de aborto, e que não planejaram a gestação. A assistência nutricional mostrou efeito protetor contra o tabagismo na gestação, de forma que as gestantes devem ser esclarecidas quanto aos efeitos deletérios de tais substâncias contribuindo dessa forma para melhores resultados obstétricos. PURPOSE: to describe alcohol and tobacco use in adult pregnant women and determine its association with the obstetric outcome. METHODS: analytical transversal study, in which 433 adult pregnant women and their newborns have been included, attended at a public maternity hospital in Rio de Janeiro, from 1999 to 2006. Information on the mother and the newborn was collected at the moment of delivery and during puerperium through an interview and inspection of the medical records. "Use of alcohol during gestation" and "use of cigarette during gestation" have been considered when detected at any gestational age and written down on the medical record. RESULTS: it was observed that 5.5 and 7.7% of the pregnant women reported cigarette and alcohol use during gestation, respectively. Maternal features related to tobacco use during pregnancy were marital status (p=0.005), age (p=0.01) and pre-natal nutritional guidance (p=0.003). Tobacco use during pregnancy has been strongly associated with alcohol use, 31.3% of the women reporting concomitant use of both substances (p0.05) has been detected. CONCLUSIONS: these results suggest that tobacco and alcohol use should be investigated during pre-natal care among all women, particularly single women, over 35 years old, with history of miscarriage, and with unwanted pregnancy. Nutritional guidance had a protective effect against tobacco use during gestation, and thus pregnant women should be informed as to the harmful effects of substance use to ensure better obstetric outcome.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPES A deficiência de vitamina E (DVE) está associada à diversos achados clínicos em recém-nascidos, como broncodisplasia pulmonar, retinopatia da prematuridade e hemorragia intracraniana. Essas intercorrências são estudadas em recém-nascidos pré-termos (RNPT) (< 37 semanas), mas em recém-nascidos termos (RNT) também foram encontrados baixos níveis circulantes da vitamina. Como uma inadequação de vitamina E na fase neonatal também pode comprometer a saúde cognitiva na infância, são necessários estudos que avaliem se há diferença no estado nutricional de vitamina E dos recém-nascidos segundo a idade gestacional do parto, para fornecer subsídios para os protocolos de monitoramento da saúde neonatal. Assim, o objetivo desse estudo foi comparar o estado nutricional de vitamina E entre recém-nascidos pré-termo e termo. Trata-se de uma revisão sistemática e metanálise baseada no Preferred Reporting Items for Systematic Review and Metaanalysis, que incluiu estudos originais do tipo transversal, caso-controle e coorte em todos os idiomas. Adotaram-se como critérios de elegibilidade a análise da concentração de alfatocoferol sérico / plasmático em recém-nascidos, incluindo RNPT e/ou RNT e o tipo de estudo observacional. As bases de dados utilizadas para a pesquisa foram PubMed, Medline, EMBASE, Web of Science, SciELO e Períodicos Capes para tese e dissertação. Todos os dados foram analisados e extraídos por dois revisores independentes. A avaliação da qualidade metodológica foi realizada por meio da ferramenta Newcastle-Otawa Scale. As diferenças medias e intervalos de confiança (IC95%) foram calculados no software R. A revisão foi registrada no PROSPERO sob registro CRD42021165152. Na revisão sistemática foram encontrados 1809 artigos, sendo ao final da busca 9 elegíveis para a revisão, com população de recém-nascidos entre 14 e 235 avaliados, 04 foram estudos de coortes, 04 estudos transversal e 01 caso-controle. Apenas 03 estudos foram incluídos na metanálise por utilizarem método padronizado para determinação analítica do alfa-tocoferol, por conterem a informação de média e desvio do alfa-tocoferol e por terem feito a análise tanto no RNPT quanto RNT. Os estudos apresentaram alta heterogeneidade, explicada pelo tamanho da amostra, pelos níveis séricos médios da vitamina e pelo risco de viés. Mais de 90% dos casos foram classificados com DVE em ambos os grupos. No modelo final foram incluídos 166 RNPT e 194 RNT, não sendo encontradas diferenças significativas nas médias da concentração de alfa-tocoferol entre os grupos (-0,15 [IC 95% -0,72; 0,42], p = 0,61). Apesar da necessidade de serem realizados mais estudos com o tema, tais resultados demonstram que independentemente da idade gestacional do nascimento, ambos recém-nascidos podem apresentar status semelhante de vitamina E, sendo necessário avaliar se esses baixos níveis se mantem durante a fase pós-natal ou são transitórios devido a maior utilização da vitamina E durante o parto. Vitamin E deficiency (VED) is associated with several clinical findings in newborns, such as pulmonary bronchodysplasia, retinopathy of prematurity and intracranial hemorrhage. These complications are studied in preterm newborns (PTNB) (< 37 weeks), but low circulating levels of the vitamin were also found in term newborns (TNB). As an inadequacy of vitamin E in the neonatal phase can also compromise cognitive health in childhood, studies are needed to assess whether there is a difference in the nutritional status of vitamin E of newborns according to the gestational age of delivery, to provide support for the protocols of monitoring of neonatal health. Thus, the aim of this study was to compare the vitamin E nutritional status between preterm and full-term newborns. This is a systematic review and metaanalysis based on the Preferred Reporting Items for Systematic Review and Metaanalysis, which included original cross-sectional, case-control and cohort studies in all languages. As eligibility criteria, the analysis of serum/plasma alpha-tocopherol concentration in newborns was adopted, including PTNB and/or TNB and type of observational study. The databases used for the research were PubMed, Medline, EMBASE, Web of Science, SciELO and Capes Periodicals for thesis and dissertation. All data were analyzed and extracted by two independent reviewers. The assessment of methodological quality was performed using the Newcastle-Otawa Scale tool. Mean differences and confidence intervals (95%CI) were calculated using the R software. The revision was registered in PROSPERO under registration CRD42021165152. In the systematic review, 1809 articles were found, and at the end of the search 9 were eligible for the review, with a population of newborns between 14 and 235 evaluated, 04 cohort studies, 04 cross-sectional studies and 01 case-control. Only 03 studies were included in the meta-analysis for using a standardized method for the analytical determination of alpha-tocopherol, for containing information on the mean and standard deviation of alpha-tocopherol and for having performed the analysis in both the PTNB and the TNB. The studies showed high heterogeneity, explained by the sample size, mean serum vitamin E levels and risk of bias. More than 90% of cases were classified with VED in both groups. In the final model, 166 PTNB and 194 TNB were included, with no significant differences found in the means of alpha-tocopherol concentration between the groups (-0.15 [CI 95% -0.72; 0.42], p = 0.61). Despite the need for further studies on the topic, these results demonstrate that regardless of the gestational age at birth, both newborns may have similar vitamin E status, and it is necessary to assess whether these low levels are maintained during the postnatal phase or are transient due to increased use of vitamin E during the birth.
