This thesis studies two important aspects of labour market earnings dynamics: the post-displacement earning losses experienced by high-tenure workers, and the evolution of the gender wage gap within rms linked to export activity. The rst two essays aim at understanding and quantifying the forces behind the post-displacement earning losses observed in the data. I rst introduce the main empirical and theoretical works in the literature. Then, I propose a structural model of the labour market with on the job search, in which rms are heterogeneous in productivity and workers accumulate both general and speci c skills while employed. Jobs are destroyed at an endogenous rate due to idiosyncratic productivity shocks and workers' skills depreciate during unemployment. The model is estimated via simulated method of moments using matched employer-employee data on Germany. By matching moments related to workers' mobility and wage dynamics, the model reproduces the size and persistence of the earning losses observed in the data. The key driver of the post-displacement earning losses is the interaction between the loss in speci c (mostly) and general human capital and endogenous separation. Finally, the third essay studies the e ect of rms' export activity on the gender wage gap among its workers. Using matched employer-employee data from Germany for the period between 1993 and 2007, we show that an increase in a rm's export widens the wage gap between male and female blue collar workers, while it reduces it between male and female white collars. In particular, the former e ect is stronger for workers in routine manual tasks, while the latter is driven by employees performing interactive tasks. This evidence is consistent with the hypothesis that serving foreign markets relies more on interpersonal skills, which reinforces female comparative advantage and reduces (widens) the gender wage gap in white (blue) collar occupations.

In the shift toward sustainable energy production, offshore wind power has experienced notable expansion. Several projects to install floating offshore wind farms in European waters, ranging from a few to hundreds of turbines, are currently in the planning stage. The underwater operational sound generated by these floating turbines has the potential to affect marine ecosystems, although the extent of this impact remains underexplored. This study models the sound radiated by three planned floating wind farms in the Strait of Sicily (Italy), an area of significant interest for such developments. These wind farms vary in size (from 250 MW to 2800 MW) and environmental characteristics, including bathymetry and seabed substrates. Propagation losses were modeled in one-third-octave bands using JASCO Applied Sciences' Marine Operations Noise Model, which is based on the parabolic equation method, combined with the BELLHOP beam-tracing model. Two sound speed profiles, corresponding to winter and summer, were applied to simulate seasonal variations in sound propagation. Additionally, sound from an offshore supply ship was incorporated with one of these wind farms to simulate maintenance operations. Results indicate that sound from operating wind farms could reach a broadband sound pressure level (L
p ) of 100 dB re 1 µPa as far as 67 km from the wind farm. Nevertheless, this sound level is generally lower than the ambient sound in areas with intense shipping traffic. The findings are discussed in relation to local background sound levels and current guidelines and regulations. The implications for environmental management include the need for comprehensive monitoring and mitigation strategies to protect marine ecosystems from potential acoustic disturbances. Humpback whales songs have been widely investigated in the past few decades. This study proposes a new approach for the classification of the calls detected in the songs with the use of Hidden Markov Models (HMMs). HMMs have been used once before for such task but in an unsupervised algorithm with promising results. Here HMMs were trained and two models were employed to classify the calls into their component units and subunits. The results show that classification of humpback whale songs from one year to another is possible even with limited training. The classification is fully automated apart from the labelling of the training set and the input of the initial HMM prototype models. Two different models for the song structure are considered: one based on song units and one based on subunits. The latter model is shown to achieve better recognition results with a reduced need for updating when applied to a variety of recordings from different years and different geographic locations.

High-tenure workers losing their job experience a large and prolonged fall in wages and earnings. The aim of this paper is to understand and quantify the forces behind this empirical regularity. We propose a structural model of the labor market with (i) on-the-job search, (ii) general human capital, and (iii) firmspecific human capital. Jobs are destroyed at an endogenous rate due to idiosyncratic productivity shocks and the skills of workers depreciate during periods of non-employment. The model is estimated on German Social Security data. By jointly matching moments related to workers' mobility and wages, the model can replicate the size and persistence of the losses in earnings and wages observed in the data. We find that the loss of a job with a more productive employer is the primary driver of the cumulative wage losses following displacement (about 50 percent), followed by the loss of firm-specific human capital (about 30 percent).

This paper studies the effect of firms'export activity on the gender wage gap among its workers. Using matched employer-employee data from Germany for the period be- tween 1993 and 2007, we show that an increase in a firm's export widens the wage gap between male and female blue-collar workers, while it reduces it between male and female white collars. In particular, the former effect is stronger for workers in routine manual tasks, while the latter is driven by employees performing interactive tasks. This evidence is consistent with the hypothesis that serving foreign markets relies more on in- terpersonal skills, which reinforces female comparative advantage and reduces (widens) the gender wage gap in white-collar (blue-collar) occupations. Our results, identified out of the variation in wages within firm-worker pairs, are robust to controlling for a series of worker and firm characteristics, and a host of firm, sector, time and state fixed effects, and heterogeneous trends.

BACKGROUND: Proper risk stratification of patients for early mortality after cancer-associated thrombosis may lead to personalized anticoagulation protocols. Therefore, we aimed to derive and validate a scoring system to predict early mortality in this population. To this end, we selected patients with active cancer and thrombosis from the Computerized Registry of Patients with Venous Thromboembolism database. METHODS: The main outcome was all cause mortality within the month following a thrombotic event. We used a simple random selection to split are data in a derivation and a validation cohort. In the derivation cohort, we used recursive partitioning and binary logistic regression to identify groups at risk and to determine the likelihood of the primary outcome. The risk score was developed based on odds ratios from the final multivariate model, and then tested in the validation cohort. RESULTS: In 10,025 eligible patients, we identified 6 predictors of 30-day mortality: leukocytosis =11.5x109/L; platelet count =160x109/L, metastasis, recent immobility, initial presentation as pulmonary embolism and Body Mass Index

Patients with autoimmune disorders are at an increased risk of venous thromboembolism (VTE), but this association has not been consistently evaluated. We used the RIETE (Registro Informatizado Enfermedad Trombo Embólica) database to compare the rates of VTE recurrences, major bleeding, and death during the course of anticoagulation, according to the presence or absence of autoimmune disorders. Of 71 625 patients with VTE recruited in February 2018, 1800 (2.5%) had autoimmune disorders. Median duration of anticoagulant therapy was slightly longer in patients with autoimmune disorders (median, 190 vs 182 days; P =.001). On multivariable analysis, patients with autoimmune disorders had a similar risk of VTE recurrences (hazard ratio [HR]: 0.93; 95% confidence interval [CI]: 0.68-1.27) or major bleeding (HR: 1.07; 95% CI: 0.82-1.40) and a lower risk to die (HR: 0.66; 95% CI: 0.54-0.81) than those without autoimmune disorders. Patients with giant cell arteritis had the highest rates of major bleeding (8.6 events per 100 patient-years) and the lowest rate of recurrences (zero). In other subgroups, the rates of both events were more balanced. During anticoagulation, patients with or without autoimmune disorders had similar rates of VTE recurrences or major bleeding. However, there were some differences between subgroups of patients with autoimmune disorders. [ABSTRACT FROM AUTHOR]

International audience; This study proposes a new approach for the classification of the calls detected in the songs with the use of Hidden Markov Models (HMMs) based on the concept of subunits as building blocks. HMMs have been used once before for such task but in an unsupervised algorithm with promising results, and they are used extensively in speech recognition and in few bioacoustics studies. Their flexibility suggests that they may be suitable for the analysis of the varied repertoire of humpback whale (Megaptera novaeangliae) calls because they cope well with variations in the call durations, which is a common feature in humpback whale vocalizations. Another attractive characteristic of HMMs is that highly developed tool-set is widely available as a consequence of the widespread use of their employment for human speech analysis. We describe the HMM classification method and show that a high level of performance can be achieved with modest requirements both in terms of computational load and storage. Training stage requires minimal manual input and once trained the recognition process is fully automated. We will present how the classification performance is affected by different amount of training.

- Abstract

Although spontaneous cervical artery dissection (CAD) is an uncommon cause of stroke in the general population, it accounts for 10–25% of cerebrovascular events in young to middle-aged patients. Two or more vessels are involved in fewer than 15% of dissections, but multiple spontaneous CADs are likely to be underestimated owing to frequent spontaneous recanalization and the possible oligo-symptomatic presentation. An extensive review of the literature shows that in the last 30 years only 28 cases of multiple CADs have been reported, and that in half of these patients symptoms were minor and transient. We describe two cases of multiple spontaneous CADs presenting as transient ischemic attack (TIA), in which only a specific diagnostic flow-chart allowed us to recognize multiple vessel involvement and start the appropriate therapy. - Abstract

- Abstract

- Abstract
A 65-year-old male patient with primary idiopathic dermatomyositis and on regular immunosuppressive therapy was admitted to our Department for fever and dyspnea. A diagnosis of hemorrhagic Salmonella enteritidis pericarditis was made. Treatment necessitated aggressive medical/surgical supervision. A review of the pertinent literature revealed that during the last decade Salmonella enteritidis has been identified as the most frequent agent of pericarditis caused by Salmonella species apart from Salmonella typhi. This, possibly as a consequence of the widespread contamination of poultry foods by this Salmonella species and/or owing to a possible peculiar affinity of Salmonella enteritidis to the pericardium.
