Abstract

Climate change is with us. As professionals who place value on evidence-based practice, climate change is something we cannot ignore. The current pandemic of the novel coronavirus, SARS-CoV-2, has demonstrated how global crises can arise suddenly and have a significant impact on public health. Global warming, a chronic process punctuated by acute episodes of extreme weather events, is an insidious global health crisis needing at least as much attention. Many neurological diseases are complex chronic conditions influenced at many levels by changes in the environment. This review aimed to collate and evaluate reports from clinical and basic science about the relationship between climate change and epilepsy. The keywords climate change, seasonal variation, temperature, humidity, thermoregulation, biorhythm, gene, circadian rhythm, heat, and weather were used to search the published evidence. A number of climatic variables are associated with increased seizure frequency in people with epilepsy. Climate change-induced increase in seizure precipitants such as fevers, stress, and sleep deprivation (e.g. as a result of more frequent extreme weather events) or vector-borne infections may trigger or exacerbate seizures, lead to deterioration of seizure control, and affect neurological, cerebrovascular, or cardiovascular comorbidities and risk of sudden unexpected death in epilepsy. Risks are likely to be modified by many factors, ranging from individual genetic variation and temperature-dependent channel function, to housing quality and global supply chains. According to the results of the limited number of experimental studies with animal models of seizures or epilepsy, different seizure types appear to have distinct susceptibility to seasonal influences. Increased body temperature, whether in the context of fever or not, has a critical role in seizure threshold and seizure-related brain damage. Links between climate change and epilepsy are likely to be multifactorial, complex, and often indirect, which makes predictions difficult. We need more data on possible climate-driven altered risks for seizures, epilepsy, and epileptogenesis, to identify underlying mechanisms at systems, cellular, and molecular levels for better understanding of the impact of climate change on epilepsy. Further focussed data would help us to develop evidence for mitigation methods to do more to protect people with epilepsy from the effects of climate change.

11. An Unusual case of adult-onset Acute Disseminated Encephalomyelitis.

Abstract

A 57 year old gentleman presented to our emergency department with a ten day history of progressive loss of balance, left-sided weakness and gait disturbance. CT brain showed bilateral subcortical hypodensities in the parietal lobes with right sided cortical involvement. Subsequent MRI of the neuro-axis showed symmetrical high FLAIR signal in the parietal lobes bilaterally, suggestive of Acute Disseminated EncephaloMyelitis (ADEM), while excluding cord lesions. CSF and serum analysis excluded alternative diagnoses. He was treated with high dose IV methylprednisolone followed by an oral steroid taper, with rapid clinical response aided by physical and occupational therapy. [ABSTRACT FROM AUTHOR]

13. SOD1 D91A variant in the southernmost tip of Europe: a heterozygous ALS patient resident on the island of Gozo

Abstract

Amyotrophic lateral sclerosis (ALS) is frequently caused by mutations in the SOD1gene. Here, we report the first SOD1variant in Malta, an archipelago of three inhabited islands in southern Europe. We describe a patient with a sporadic form of ALS living on the island of Gozo in which the heterozygous SOD1c.272A>C; p.(Asp91Ala) variant was detected. The patient had a late onset (79 years), sensory impairments and rapid disease progression culminating in respiratory failure. ALS has not yet developed in any of the three additional family members in which the D91A variant was identified. None of the healthy controls from the Maltese population were found to carry this variant. This report underscores the high prevalence of the D91A variant in Europe, despite the presence of a North-South gradient in its frequency, and confirms that this variant can be associated with dominant cases in Mediterranean countries.

Abstract

Climate change is with us. As professionals who place value on evidence-based practice, climate change is something we cannot ignore. The current pandemic of the novel coronavirus, SARS-CoV-2, has demonstrated how global crises can arise suddenly and have a significant impact on public health. Global warming, a chronic process punctuated by acute episodes of extreme weather events, is an insidious global health crisis needing at least as much attention. Many neurological diseases are complex chronic conditions influenced at many levels by changes in the environment. This review aimed to collate and evaluate reports from clinical and basic science about the relationship between climate change and epilepsy. The keywords climate change, seasonal variation, temperature, humidity, thermoregulation, biorhythm, gene, circadian rhythm, heat, and weather were used to search the published evidence. A number of climatic variables are associated with increased seizure frequency in people with epilepsy. Climate change-induced increase in seizure precipitants such as fevers, stress, and sleep deprivation (e.g. as a result of more frequent extreme weather events) or vector-borne infections may trigger or exacerbate seizures, lead to deterioration of seizure control, and affect neurological, cerebrovascular, or cardiovascular comorbidities and risk of sudden unexpected death in epilepsy. Risks are likely to be modified by many factors, ranging from individual genetic variation and temperature-dependent channel function, to housing quality and global supply chains. According to the results of the limited number of experimental studies with animal models of seizures or epilepsy, different seizure types appear to have distinct susceptibility to seasonal influences. Increased body temperature, whether in the context of fever or not, has a critical role in seizure threshold and seizure-related brain damage. A 70-year-old lady presented three weeks after a mild SARSCoV- 2 infection with a nine-day history of worsening back pain, progressive lower limb weakness and paraesthesia, dysphagia, constipation and difficulty in completing full sentences. Outcome of her investigations were in keeping with a diagnosis of acute inflammatory demyelinating polyneuropathy. Abstract

Tremor is defined as an involuntary, oscillatory, rhythmic movement of a body part produced by the alternating contractions of antagonistic pairs or muscles. It is the most common movement abnormality encountered and investigated in both primary care and neurology practice. Tremor is not a diagnosis in itself; rather it is symptomatic of an underlying disorder that should be identified and when possible treated. The diagnostic process ought to include a detailed medical history, a careful assessment of the characteristics of the tremor and a search for other neurological or physical signs. This process may present challenges, particularly in describing tremor phenomenology and linking it to the likely diagnosis. This article outlines briefly the main points to explore when evaluating a patient’s tremor history, the key characteristics of tremor that should be examined for and the correct nomenclature for their description, followed by an overview of the common types of tremor encountered in clinical settings and pointers to their diagnosis., peer-reviewed

Abstract

Background: A quarter of people with intellectual disability (ID) have epilepsy, compared to approximately one in a hundred across the general population. Evidence for the safe and effective prescribing of antiepileptic drugs (AEDs) for those with ID is, however, limited. Aims of Study: This study seeks to strengthen the research evidence around Eslicarbazepine Acetate (ESL), a new AED, by comparing response of individuals with ID to those from the general population who do not have ID. Methods: A single data set was created through retrospective data collection from English and Welsh NHS Trusts. The UK‐based Epilepsy Database Research Register (Ep‐ID) data collection and analysis method were used. Results: Data were collected for 93 people (36 ID and 57 'no ID'). Seizure improvement of '>50%' was higher at 12 months for 'no ID' participants (56%), compared to ID participants (35%). Retention rates were slightly higher for those with ID (56% compared to 53%). Neither difference was significant. Conclusions: Tolerance and Efficacy for ID and 'no ID' people in our data set were similar. Seizure improvement and retention rates were slightly lower than that found in other European data sets, but findings strengthen the evidence for the use of ESL in the ID population. Abstract

Amantadine is an N-methyl-d-aspartate receptor agonist with secondary dopaminergic activity that is used to treat Parkinson's disease-related dyskinesia and to treat fatigue in multiple sclerosis. It is primarily renally excreted and so impaired kidney function prolongs its half-life and may lead to toxicity. We describe a woman with multiple sclerosis taking amantadine who developed acute renal impairment, which triggered florid visual hallucinations that resolved on stopping the medication. Abstract

Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a subtype of stiff-person syndrome (formerly stiff-man syndrome). It is rare and disabling, and characterised by brainstem symptoms, muscle stiffness, breathing issues and autonomic dysfunction. We describe a 65-year-old man who presented with odynophagia together with tongue and neck swelling, followed by multiple cranial nerve palsies culminating in bilateral vocal cord paralysis with acute stridor. He subsequently developed progressive generalised hypertonia and painful limb spasms. Serum antiglycine receptor antibody was strongly positive, but antiglutamic acid decarboxylase and other antibodies relating to stiff-person syndrome were negative. We diagnosed PERM and gave intravenous corticosteroids and immunoglobulins without benefit; however, following plasma exchange he has made a sustained improvement. [ABSTRACT FROM AUTHOR]

49. Quantifying Collaboration in Synchronous Document Editing

Abstract

Collaborative synchronous writing tools like Google Docs and Etherpad let multiple users edit the same document and see each others edits in near real-time to simplify collaboration and avoid merge-conflicts. These tools are used extensively across many domains, including education, in both research and industry. The very nature of needing to constantly synchronize state between multiple users means that very granular editing data is automatically captured and stored. In theory, this data could provide important insights into the editing process, the contributions of the different users, how the text developed over time, and other questions relevant to researchers studying writing from different theoretical and methodological angles. However, this extreme granularity of the data (down to individual key presses), makes analysis very complex. Most of the research focused on automatic analysis of collaborative writing to date has focused on asynchronous writing, and looked at the "diffs" between one editing session and the next. In this paper, we present a method and a tool to construct informative operations from text data, as well as preliminary metrics for measuring the collaborative writing process. Additionally, our method adds to previous work in that it can be used to assess the writing during the writing process rather than just being applied to an end product.

The article presents a case study of a 46-year-old woman with history of episodic headache induced by laugher. The patient's neurological and magnetic resonance imaging examinations show normal results. The patient was treated with nortriptyline after suffering from occupational head injury and depression. Also mentioned is the occurrence of laughter-triggered headache as a secondary headache form with Chiari type 1 malformation patients.
