Large congenital lid colobomas are traditionally repaired using 1- or 2-step vascularized flap-graft combinations. However, visual axis occlusion for weeks is a severe problem in small children and recent reports suggest that the flap pedicle does not contribute to blood perfusion. A "one-step" substitute for large lid defects has recently been reported in animals and humans, demonstrating the viability of a bilamellar autograft alone. We present an alternative "one-step" reconstructive approach in a 6-month-old infant who had a centrally-located large upper eyelid defect resulting from a congenital coloboma. The free full-thickness bilamellar autograft was harvested from the contralateral upper eyelid. The follow-up time was 48 months. Cosmetic and functional results were good, the bilamellar graft survived, and there was no graft ischemia, necrosis, or rejection. The boy developed madarosis, lid notching, and mild contour irregularity but needed no reoperation since the parent was satisfied with the surgical result. A free bilamellar eyelid autograft seems to be an outstanding alternative to both "conventional 2-step" and "modern 1-step" options for the reconstruction of large colobomatous eyelid openings, especially in young infants who cannot tolerate visual axis blockage. Purpose: The aim of this study is to detect changes in retinal layers in Retinitis pigmentosa. In this way, it was thought that it could guide researchers in the process of RP treatment development. Materials and Methods: Twenty-seven RP subjects with preserved foveal anatomy without cystoid macular edema and the same number of healthy people, equal in age and sex, were comprised as the study controls. Macula and retinal layer segmentation was performed by OCT imaging with a Spectralis OCT machine (Heidelberg Engineering, Germany). Results: In subjects with RP, total retinal thickness was not affected in the 1 mm central area, but it was statistically significantly thinner in all four parafoveal quadrants. In subjects with Retinitis Pigmentosa, ONL, IPL, GCL and RPE were statistically significantly thinner, and OPL and INL were considerably thicker (p < 0.05). Conclusions: In subjects with RP with foveal contour is preserved, there are non-homogeneous changes in the retinal layers and the total retinal thickness decreases in the parafoveal region. Purpose: Social media plays a growing role in the health system. Due to the presence of many videos with surgical content, the use of YouTube to obtain information is becoming more and more common among surgeons. This study aimed to evaluate the sufficiency and quality of YouTube videos on posterior capsular rupture (PC) management during phacoemulsification surgery according to a novel scoring system suggested by the authors. Materials and Methods: A YouTube search was performed phrases "posterior capsule rupture", "posterior capsule tear" and "posterior capsule rent". Ninety-six videos were included in this study and evaluated. The contents of the videos were scored by the authors according to their novel scoring criteria. Results: The mean score of the videos according to the novel scoring criteria was 8.67±2.43, with a range of 1-14. There was a moderate correlation between the total score and views, and the total score and likes (p<0.05). In 77 (80%) of the videos, the surgeons did what was initially expected according to scoring criteria when the PC rupture occurred. When the videos were filtered according to the minimum surgical requirements defined by the authors' scoring system, 10 videos (10.4%) were found to meet the requirements. Conclusions: PC rupture management videos are not fully sufficient to be used in surgical training. Some of these videos can be used in surgical training, but they should be used after necessary evaluations are performed. [ABSTRACT FROM AUTHOR]

Purpose: To evaluate the short-term effects of half-fluence photodynamic therapy (PDT) on retinal and choroidal vascular parameters obtained by optical coherence tomography angiography. Materials and Methods: Fifteen patients who underwent half-fluence PDT for chronic central serous chorioretinopathy between 20182020 were included in the study retrospectively. Patients underwent optic coherence tomography angiography imaging before and after PDT. Foveal avascular zone (FAZ) area, FAZ perimeter, acircularity index, foveal vascular density 300 microns around FAZ, outer retina flow, choriocapillaris flow and the whole area, foveal, parafoveal and perifoveal vessel densities of superficial and deep capillary networks were compared before and after treatment. Results: The vessel density of the superficial foveal capillary plexus was significantly decreased after PDT (p=0.044). However whole area, parafoveal and perifoveal vessel densities of superficial capillary plexus were not significantly different. The vessel density of the deep parafoveal capillary plexus was significantly increased after PDT (p=0.027). The whole area, foveal and perifoveal vessel densities of deep capillary plexus were not significantly different before and after PDT (p>0.05). Foveal avascular zone (FAZ) area, FAZ perimeter, acircularity index, outer retina flow and choriocapillaris flow did not change after photodynamic therapy in short term. Conclusion: This study showed that vessel density of superficial foveal capillary plexus was decreased and vessel density of deep parafoveal capillary plexus was increased in short term after half fluence PDT. However, the foveal avascular zone area, ourter retinal and choroidal flow, choroidal signal voids and vascular densities of other remaining macular vascular areas were not changed.
