Servikal nekrotizan fasiit, daha çok yaşlılarda ve immün yetmezliği bulunan hastalarda görülen, polimikrobiyal kökenli, hızlı ve yıkıcı seyreden ve bazen ölümcül olabilen bir derin boyun enfeksiyonudur. Nadiren enfeksiyon derin fasiyal planlar aracılığı ile mediastene yayılır ve desendan nekrotizan mediastenite neden olur. Desendan nekrotizan mediastinit yüksek mortaliteye sahip, acilen tanı konulup tedavi edilmesi gereken ciddi bir tablodur. Bu derlemede biri tip 1, diğeri de tip 2B desendan nekrotizan mediastinit tanısı ile servikal ve mediastinal drenaj ile tedavi edilmiş iki olgu sunuldu ve konu ile ilgili güncel literatür tartışıldı, Cervical necrotizing fasciitis is a polymicrobial, rapidly progressive, destructive, and sometimes fatal deep neck infection, that is most commonly seen in the elderly and immunocompromised patients. The infection rarely spreads to mediastina via deep fascial planes and causes descending necrotizing mediastinitis. Descending necrotizing mediastinitis is a serious clinical entity with a high mortality rate that should be promptly diagnosed and treated. In this review, we present two cases with type 1 and type 2B descending necrotizing mediastinitis treated with servical and mediastinal drainage, and also discuss relevant contemporary literature.

Amaç: Bu çalışmada paratrakeal lenf nodu PTLN metastazının ileri evre larengeal ve hipofarenks kanseri hastalarında sağkalıma etkisi değerlendirildi.Hastalar ve Yöntemler: Mayıs 1995 - Haziran 2008 tarihleri arasında ameliyat olan ileri evre larengeal ve hipofarengeal kanserli hastaların tıbbi kayıtları değerlendirildi ve PTLN metastazı olan hastaların 78’i 63 erkek, 15 kadın; ort. yaş 55±11.3 yıl; dağılım 25-76 yıl çalışmaya dahil edildi. Ortalama izlem süresi 23 aydı. Elli üç hasta primer larenks kanseri ve kalan 25 hasta primer hipofarengeal kanseri idi.Bulgular: Paratrakeal lenf nodu metastazı 53 hastanın sekizinde %15 larenks kanseri ile, 14 hastanın altısında %42 postkrikoid kanseri ile, ve yedi hastanın birinde %14 ise yutak arka duvar kanseri ile tespit edildi. Paratrakeal lenf nodu metastazı, hipofarenksin piriform sinüsünden gelişen kanserli hastalarda tespit edilmedi n=4 . Paratrakeal lenf nodu metastazı 11’inde ekstranodal yayılması ENS olan toplam 15 hastada saptandı. Çoklu analizler ENS varlığının genel sağkalım üzerinde en etkili prognostik faktör olduğunu gösterdi p, Objectives: This study aims to evaluate the effect of paratracheal lymph node PTLN metastasis on survival in patients with advanced laryngeal and hypopharyngeal cancer. Patients and Methods: Medical records of advanced laryngeal and hypopharyngeal cancers who underwent surgery between May 1995 and June 2008 were assessed and 78 of the patients 63 males, 15 females; mean age 55±11.3 years; range 25 to 76 years who has PTLN metastasis were included in this study. The mean follow-up period was 23 months. Fifty-three patients had primary laryngeal cancer, and the remaining 25 patients had primary hypopharyngeal cancer. Results: Paratracheal lymph node metastasis was detected in eight 15% of 53 patients with laryngeal carcinoma, six 42% of 14 patients with postcricoid carcinoma, and one 14% of seven patients with posterior pharyngeal wall carcinoma. Paratracheal lymph node metastasis was not detected in patients with pyriform sinus carcinoma n=4 . Paratracheal lymph node metastasis was detected in a total of 15 patients, of whom 11 had extranodal spread ENS . Adenolipom nadir görülen, glandular elementler ve matür yağ dokusu içeren selim bir tümördür. Bu yazıda, burun tıkanıklığı ve deformitesi olan 19 yaşında bir kadın olgu sunuldu. Fizik muayenesin- de sağ nazal kavite kartilajenöz çatıda yuvarlak, yumuşak, pulsasyon vermeyen submukozal kitle saptandı. Ayrıca sağ tarafta alar rim düzensizliği, nostril asimetrisi ve karşı tarafa septum deviyasyonu vardı. Bilgisayarlı tomografi incelemesinde sağ sub- mukoperikondrial seviyede yerleşimli nasal pasajı kapatan düzgün sınırlı, yoğun içerikli kitle saptandı. Kitleyi çıkarmak ve neden olduğu deformiteyi de düzeltmek için açık septorinoplasti yaklaşımı tercih edildi. Histopatolojik inceleme sonucu adenolipom olarak bildirildi. Ameliyat sonrası iki yıllık takip süre- since nüks görülmedi. Bildiğimiz kadarıyla, bu olgu İngilizce literatürde bildirilen ilk burunda adenolipom olgusudur, Adenolipoma is a rare benign neoplasm composed of both mature adipose tissue and glandular elements. In this article we report a case of 19-year-old woman, who had nasal blockage and deformity. On examination, a round, soft, nonpulsatile submucosal mass in the cartilagenous vault of the right nasal cavity was found. There was also alar rim notching, nostril asymmetry on the right side and septal deviation towards the opposite side. Computed tomography revealed a clearly demarcated dense mass located at the right submucopericondrial level obstructing the nasal passage. An open septorhinoplasty approach was preferred to remove the mass and also to correct the deformities caused by the mass. The histopathologic examination was reported as an adenolipoma. No recurrence was observed during postoperative two year follow-up. To our knowledge, this is the first case report of an adenolipoma of the nose in the English literature.

Objectives: This study aims to investigate the effects of cochlear implantation on middle ear aeration, tympanometric values, and otitis media with effusion OME occurrence. Patients and Methods: This prospective study conducted between February 2010 and May 2013 included 100 ears of 100 patients 57 males, 43 females; mean age 6.4±2.8 years; range, 1 to 18 years who underwent cochlear implantation and had no prior history of ear surgery. Patients who had middle and/or external ear anomalies were excluded. During the preoperative evaluation, otoscopic, rhinoscopic and nasopharynx examinations, along with tympanometric investigations were performed. Postoperative tympanometric evaluations over 12-36 months’ follow-up were performed and the values were compared, respectively. Results: In the postoperative otoscopic examinations, tympanic membrane retraction was observed in 14 ears. The mean values of compliance and gradient were significantly lower in the postoperative evaluations. The peak pressure and external ear canal volume measurements were not significantly different after cochlear implantation surgery. The mean preoperative value of peak pressure of the male patients was significantly lower than that in the female patients. Amaç: Bu çalışmada sosyal ve demografik özelliklerin prelingual pediatrik koklea implantı yapılan hastaların dil gelişimi üzerindeki etkileri gözlendi.Hastalar ve Yöntemler: Nisan 2006 - Nisan 2010 tarihleri arasında prelingual olarak implant takılan ve en az 36 ay implant deneyimi olan 44 çocuk 26 erkek, 18 kız; ort. yaş 81.1±16.9 ay; dağılım 54-115 ay retrospektif olarak incelendi. Yalnızca mental-motor retardasyonu, koklear anomalisi olmayan ve revizyon cerrahisi yapılmayan ve eğitimini ara vermeden kesintisiz sürdüren hastalar seçildi. Bu hastalara reseptif ve ekspresif kelime testleri yapıldı. Cinsiyet, implant yaşı, ebeveynlerin eğitim durumu ve yıllık gelirleri gibi sosyal ve demografik özellikleri kaydedildi. Dil gelişimi ve sosyo-demografik faktörler arasındaki ilişkiler incelendi.Bulgular: Otuz altı aydan önce implant takılan hastaların reseptif ve ekspresif dil düzeyleri daha iyiydi. Maternal eğitim düzeyi yüksek olan çocukların ekspresif ve reseptif eş değeri dil yaşları anlamlı düzeyde daha iyiydi. Ailelerin yıllık gelirlerinin 36 aydan önce implant takılan çocukların dil gelişimine anlamlı düzeyde olumlu etkileri vardı.Sonuç: Otuz altı aydan sonra takılan implantlarda hem ekspresif hem de reseptif dil becerileri, implant yaşı ve ebeveynlerin sosyo-ekonomik durumları ile anlamlı düzeyde ilişkilidir, Objectives: This study aims to observe the effects of social and demographic factors on the language development of prelingual pediatric cochlear implant patients. Patients and Methods: Between April 2006 and April 2010, 44 children 26 boys, 18 girls; mean age 81.1±16.9 months; range 54 to 115 months who were prelingually implanted and who had an implant experience of at least 36 months were retrospectively analyzed. Only the patients without mental-motor retardation, cochlear anomaly and revision surgery and who continued their education without any interruption were selected. Receptive and expressive vocabulary tests were performed on these patients. Social and demographic features including gender, implant age, parents’ education status and annual income were recorded. The relationship between language development and socio-demographic factors were investigated. Results: Patients implanted before the age of 36 months showed better levels of receptive and expressive language. Children with higher maternal education levels showed significantly better expressive and receptive equivalent language ages. Annual income of the families had significant positive impacts on the language development of the children who were implanted before the age of 36 months. Conclusion: Both expressive and receptive language skills over 36 months of implant experience are significantly associated with age at the time of the implant and socio-economic status of the parents.

Amaç: Bu çalışmada jüvenil nazofarengeal anjiofibrom nedeniyle ameliyat edilen hastaların demografik özellikleri, tümör evresi, cerrahi yöntem ve nüks oranları değerlendirildi.Hastalar ve Yöntemler: Bu retrospektif çalışmaya Mart 2006-Temmuz 2015 tarihleri arasında İstanbul Üniversitesi, İstanbul Tıp Fakültesi Kulak Burun Boğaz Hastalıkları kliniğinde ameliyat edilen 45 hasta 44 erkek, 1 kadın; ort. yaş 21 yıl; dağılım 9-55 yıl dahil edildi. Hastalar yaş, cinsiyet, başvuru semptomu, tümörün evresi, uygulanan cerrahi yöntem, ameliyat öncesi embolizasyon, ameliyat sırası kan transfüzyonu, komplikasyonlar ve nüks varlığına göre sınıflandırıldı.Bulgular: En sık başvuru semptomu burun kanaması %78 ve burun tıkanıklığı %73 idi. Hastaların tamamına ameliyat öncesi anjiyografi yapıldı ve uygun olan hastalara %69 embolizasyon uygulandı. Otuz bir hastada transnazal endoskopik yaklaşım, altı hastada midfasiyal degloving ve üç hastada lateral rinotomi yaklaşımı uygulandı. Genel nüks oranı %31 n=14 idi.Sonuç: Ameliyat sonrası nüks riskini belirleyen en önemli faktör, tümörün ameliyat öncesi evresidir. Ameliyat öncesi embolizasyon, ameliyat sırası kanama miktarını azaltmaktadır. Endoskopik transnazal yaklaşım komplikasyon oranını ve hastanede yatış süresini kısaltmaktadır, Objectives: This study aims to evaluate the demographic characteristics, tumor stage, surgical treatment and recurrence rate among patients operated with a juvenile nasopharyngeal angiofibroma. Patients and Methods: This retrospective study included 45 patients 44 males, 1 female; mean age 21 years, range 9 to 55 years who underwent surgery at Istanbul University, Istanbul Medical Faculty, Department of Otorhinolaryngology clinic between March 2006 and July 2015. The patients were classified according to age, sex, presenting symptom, tumor stage, surgical procedure applied, preoperative embolization, perioperative blood transfusion, complications, and the presence of recurrence. Results: The most common presenting symptoms were epistaxis 78% and nasal obstruction 73% . Preoperative angiography was performed on all patients and embolization was applied in eligible patients 69% . Transnasal endoscopic approach in 31 patients, midfacial degloving in six patients, and lateral rhinotomy approach in three patients were applied. The overall recurrence rate was 31% n=14 . Conclusion: The most important factor in determining the risk of postoperative recurrence is the preoperative tumor stage. Preoperative embolization reduces the amount of perioperative bleeding. Endoscopic transnasal approach decreases the rate of complications and length of hospitalization.

Aggressive papillary tumors of the middle ear constitute a small percent of adenomatous middle ear tumors. Patients usually present with long-lasting tinnitus, hearing loss, and vertigo and occasionally facial paralysis. Computed tomography findings predominantly suggest bone invasion. Thus, it is commonly misdiagnosed with chronic otitis media with a cholesteatoma. A 72-year-old man presented with chronic ear pain and ear drainage. The patient had a 63 decibels dB mixed-type hearing loss and a mass lesion at the temporal bone high resolution computed tomography. Explorative tympanotomy revealed a tumor with thick mucoid secretion and no bone erosion, for which an incisional biopsy revealed an adenoma. Radical modified mastoidectomy was performed and the definite pathology result was compatible with an aggressive papillary tumor. The patient was free of disease at six months of follow-up.

Parotis bezi kökenli malign periferik sinir kılı- fı tümörleri oldukça nadirdir. Olgular sporadik veya nörofibramotis tip 1 zemininde gelişebilir. Parotis bezinden köken alan neoplaziler genel olarak kolay palpe edilebilir ve masif karakterli- dir. Tümör habis karakterde dahi olsa, tanı esna- sında fasiyal paralizi görülme olasılığı çeşitli çalışmalarda %15 civarındadır. Halbuki sinirin kendisinden köken alan bir habis tümör, erken dönemde fasiyal paralizi yapmasına rağmen uzun bir süre fizik muayenede fark edilmeyebilir ve fasiyal paralizi etyolojisinde rol alan diğer neoplazi dışı hastalıklarla karışarak tanı ve tedavinin gecikmesine yol açabilir. Özellikle tip 1 nörofibromatozis olguları habis periferik sinir kılıfı tümörü gelişimi açısından yüksek riskli olgulardır. Bu yazıda nörofibromatozis tip 1 zemininde gelişen bir habis periferik sinir kılıfı tümörü olgusu bildirilmiştir, Malignant peripheral nerve sheath tumors arising from the parotid gland are very rare. They can develop as sporadic cases, or on the basis neurofibromatosis type 1. Tumors originating from the parotid gland are generally easy to palpate and have a solid characteristic. Even if the tumor is malignant in character, the incidence of facial paralysis at the time of diagnosis is around 15% in various studies. However, a malignant tumor originating from the nerve itself may not be noticed during the physical examination for a long period time although it cases facial paralysis and may be mistaken with other non-neoplastic diseases involved in the etiology of facial paralysis leading to a delay in the diagnosis and treatment. Especially patients with type 1 neurofibromatosis have a great tendency to develop malignant peripheral nerve sheath tumors. In this article a case of malignant peripheral nerve sheath tumor developed on the basis of neurofibromatosis type 1 was reported.
