Even if apnea of prematurity is by definition a self-limiting condition, it can cause serious problems during the hospital stay and can potentially have long-term neurological and cognitive consequences depending on the severity and intensity of the episodes. The diagnosis of apnea of prematurity can be made only after excluding a number of diseases of the preterm infant in which apnea may be an epiphenomenon. Etiological diagnosis is essential for selection of appropriate treatment, which may be nonpharmacological or involve use of drugs.Keywords: apnea of prematurity, idiopathic and secondary apnea, caffeine

Abstract

Aquifer Thermal Energy Storage (ATES) system make use of the groundwater to exchange energy with the building: in winter, groundwater is pumped from the warm well to the buildings heat exchanger and the building extracts heat from the groundwater as energy source for the heat pumps, while the groundwater will be injected in the cold well at lower temperature; in summer, the direction will be reversed and groundwater will be pumped out of the cold well to the heat exchanger, where the building will gather cold from the groundwater. ATES combined with solar collectors at two sites in Belgium and the Netherlands proved to allow for additional efficiencies and energy savings. Pilot sites showed promising results for combining ATES with groundwater remediation by enhanced natural attenuation for chlorinated solvents. Pilot sites were also realized in two countries with a less mature ATES market, Spain and Italy. In Spain an innovative hybrid technology between a closed loop and an open loop system has been tested, allowing ATES to be used also in water scarcity conditions. In Italy, a small scale integration with a cold low temperature district heating system has been realized. Technical and economic performances of the pilots are briefly described. The lessons learnt in the pilots provide useful insights for the replication of these solutions.

Abstract

A transition to a low carbon energy system is needed to respond to global challenge of climate change mitigation. Aquifer Thermal Energy Storage (ATES) is a technology with worldwide potential to provide sustainable space heating and cooling by (seasonal) storage and recovery of heat in the subsurface. However, adoption of ATES varies strongly across Europe, because of both technical as well as organizational barriers, e.g. differences in climatic and subsurface conditions and legislation respectively. After identification of all these barriers in a Climate-KIC research project, six ATES pilot systems have been installed in five different EU-countries aiming to show how such barriers can be overcome. This paper presents the results of the barrier analysis and the results of the pilot plants. The barriers are categorized in general barriers, and barriers for mature and immature markets. Two pilots show how ATES can be successfully used to re-develop contaminated sites by combining ATES with soil remediation. Two other pilots show the added value of ATES because its storage capacity enables the utilization of solar heat in combination with solar power production. Finally, two pilots are realized in countries with legal barriers where ATES systems have not previously been applied at all.

Abstract

Heating and cooling using aquifer thermal energy storage (ATES) has hardly been applied outside the Netherlands, even though it could make a valuable contribution to the energy transition. The Climate-KIC project “Europe-wide Use of Energy from aquifers” – E-USE(aq) – aimed to pave the way for Europe-wide application of ATES, through the realization and monitoring of six ATES pilot plants across five different EU countries. In a preceding paper, based on preliminary results of E-USE(aq), conclusions were already drawn, demonstrating how the barriers for this form of shallow geothermal energy can be overcome, and sometimes even leveraged as opportunities. Based on final pilot project results, key economic and environmental outcomes are now presented. This paper starts with the analysis of specific technological barriers: unfamiliarity with the subsurface, presumed limited compatibility with existing energy provision systems (especially district heating), energy imbalances and groundwater contamination. The paper then shows how these barriers have been tackled, using improved site investigation and monitoring technologies to map heterogeneous subsoils. In this way ATES can cost-efficiently be included in smart grids and combined with other sources of renewable (especially solar) energy, while at the same time achieving groundwater remediation. A comparative assessment of economic and environmental impacts of the pilots is included, to demonstrate the sustainability of ATES system with different renewables and renewable-based technologies. Abstract

Background There are no Italian data regarding the strategies for preventing neonatal group B streptococcal (GBS) infection. We conducted a national survey in order to explore obstetrical, neonatal and microbiological practices for the GBS prevention. Methods Three distinct questionnaires were sent to obstetricians, neonatologists and microbiologists. Questionnaires included data on prenatal GBS screening, maternal risk factors, intrapartum antibiotic prophylaxis, microbiological information concerning specimen processing and GBS antimicrobial susceptibility. Results All respondent obstetrical units used the culture-based screening approach to identify women who should receive intrapartum antibiotic prophylaxis, and more than half of the microbiological laboratories (58%) reported using specimen processing consistent with CDC guidelines. Most neonatal units (89 out of 107, 82%) reported using protocols for preventing GBS early-onset sepsis consistent with CDC guidelines. Conclusions The screening-based strategy is largely prevalent in Italy, and most protocols for preventing GBS early-onset sepsis are consistent with CDC guidelines. However, we found discrepancies in practices among centers that may reflect the lack of Italian guidelines issued by public health organizations.

Abstract

Aquifer Thermal Energy Storage (ATES) system make use of the groundwater to exchange energy with the building: in winter, groundwater is pumped from the warm well to the buildings heat exchanger and the building extracts heat from the groundwater as energy source for the heat pumps, while the groundwater will be injected in the cold well at lower temperature; in summer, the direction will be reversed and groundwater will be pumped out of the cold well to the heat exchanger, where the building will gather cold from the groundwater. ATES combined with solar collectors at two sites in Belgium and the Netherlands proved to allow for additional efficiencies and energy savings. Pilot sites showed promising results for combining ATES with groundwater remediation by enhanced natural attenuation for chlorinated solvents. Pilot sites were also realized in two countries with a less mature ATES market, Spain and Italy. In Spain an innovative hybrid technology between a closed loop and an open loop system has been tested, allowing ATES to be used also in water scarcity conditions. In Italy, a small scale integration with a cold low temperature district heating system has been realized. Technical and economic performances of the pilots are briefly described. Abstract

A transition to a low carbon energy system is needed to respond to global challenge of climate change mitigation. Aquifer Thermal Energy Storage (ATES) is a technology with worldwide potential to provide sustainable space heating and cooling by (seasonal) storage and recovery of heat in the subsurface. However, adoption of ATES varies strongly across Europe, because of both technical as well as organizational barriers, e.g. differences in climatic and subsurface conditions and legislation respectively. After identification of all these barriers in a Climate-KIC research project, six ATES pilot systems have been installed in five different EU-countries aiming to show how such barriers can be overcome. This paper presents the results of the barrier analysis and of the pilot plants. The barriers are categorized in general barriers, and barriers for mature and immature markets. Two pilots show how ATES can be successfully used to re-develop contaminated sites by combining ATES with soil remediation. Two other pilots show the added value of ATES because its storage capacity enables the utilization of solar heat in combination with solar power production. Abstract

The late-preterm infants are a group of premature steadily increasing, different from term infants as immature in terms of respiratory, metabolic, neurological, and immunological features. They may present at birth and during the first week of life various diseases and brain lesions echographically evident. We analyzed the neonatal outcomes of 417 late-preterm infants, born in our Department of Neonatology in a period of two and a half years, evaluating respiratory problems (RDS, transient tachypnea, pneumonia, pneumothorax, and apnea), metabolic problems (hypoglycemia, hypomagnesemia, hypo-hypernatremia, dehydration, hypocalcemia, and hyperbilirubinemia), infections, neurological symptoms associated with electrolyte disturbances, the disease patterns observed by ultrasound examination of the brain, the kidney ultrasound images, genital malformations.

Abstract

Background Neonatal units’ volume of activity, and other quantitative and qualitative variables, such as staffing, workload, work environment, care organization and geographical location, may influence the outcome of high risk newborns. Data about the distribution of these variables and their relationships among Italian neonatal units are lacking. Methods Between March 2010-April 2011, 63 neonatal intensive care units adhering to the Italian Neonatal Network participated in the SONAR Nurse study. Their main features and work environment were investigated by questionnaires compiled by the chief and by physicians and nurses of each unit. Twelve cross-sectional monthly-repeated surveys on different shifts were performed, collecting data on number of nurses on duty and number and acuity of hospitalized infants. Results Six hundred forty five physicians and 1601 nurses compiled the questionnaires. In the cross-sectional surveys 702 reports were collected, with 11082 infant and 3226 nurse data points. A high variability was found for units’ size (4–50 total beds), daily number of patients (median 14.5, range 3.4-48.7), number of nurses per shift (median 4.2, range 0.7-10.8) and number of team meetings per month. Northern regions performed better than Central and Southern regions for frequency of training meetings, qualitative assessment of performance, motivation within the unit and nursing work environment; mean physicians’ and nurses’ age increased moving from North to South. After stratification by terciles of the mean daily number of patients, the median number of nurses per shift increased at increasing volume of activity, while the opposite was found for the nurse-to-patient ratio adjusted by patients’ acuity. On average, in units belonging to the lower tercile there was 1 nurse every 2.5 patients, while in those belonging to the higher tercile the ratio was 1 nurse every 5 patients. Conclusions In Italy, there is a high variability in organizational characteristics and work environment among neonatal units and an uneven distribution of human resources in relation to volume of activity, suggesting that the larger the unit the greater the workload for each nurse. Urgent modifications in planning and organization of services are needed in order to pursue more efficient, homogeneous and integrated regionalized neonatal care systems.

Abstract

This case study deals with a severe groundwater contamination by chlorinated ethenes and ethanes beneath the urbanized area of via Caretti, immediately to the east of the ancient historical center of the city of Ferrara (Emilia-Romagna region, northern Italy). Based on the concentrations as measured in the groundwater and conventional risk modeling, the contamination potentially poses a serious threat to on-site receptors (inhabitants) by indoor and outdoor vapor inhalation. The sources of contamination are two unlined waste dumps located inside abandoned clay quarries and filled, during the 1960’s-70’s, by a mixture of urban solid wastes and chlorinated pitches originated from the chloromethanes distillation process of nearby petrochemical plants. The area is located in the eastern and lowest sector of the Padana plain, a few kilometers southward of the Po river. The underground stratigraphy consists of alternating alluvial sand and silty-clayey layers that form a multi-aquifer system with alternance of three sandy permeable layers, down to about 50 m depth below ground level (b.g.l.), with low permeability aquitards. Contaminants occurrence in the aquifers, soil gas and indoor air were determined by a multi-step investigation approach based upon state of the art techniques. Contaminant plumes of mainly vinyl chloride (VC) occur in the two shallowest aquifers and have migrated up to a maximum distance of 500 m from the source below the houses of via Caretti. VC concentrations in the first aquifer attain a peak of about 1x103 μg/L (with a maximum value at the source of more than 2x104 μg/L); concentrations of about one order of magnitude higher were detected in the upper confined aquifer with an absolute peak of more than 1x105 μg/L. In addition high concentrations of perchloroethene (PCE), trichloroethene (TCE), dichloroethene DCE (1,1 and cis-1,2), 1,1,2,2-TeCA (tetrachloroethane) and chloromethanes were detected in both aquifers. Indoor air measurements were performed by means of sorption tubes by combining active and passive techniques, in two different sampling campaigns in the ground floor and the first floor of the buildings. Seasonal measurements of groundwater concentrations were also performed at four piezometers located in close proximity to the houses. Two different analytical models for risk assessment (Tier II) were applied with available site-specific data to predict indoor air concentrations and estimate risks for adult and children as receptors. In contrast to such severe concentrations in groundwater and to results of Tier II human health risk assessment based on analytical models, low and spotty evidence of contaminated vapors was found either in soil or in indoor gas and no direct relationship with subsoil conditions resulted. The occurrence of fine grained sediments (clays and silts) over the shallowest aquifer appears to block vapor intrusion and protect the residents from direct exposure by inhalation. Alternatively, biodegradation processes occurring at the interface between the saturated and the (oxygen rich) unsaturated zone may be capable of reducing the fluxes of contaminant that could potentially reach receptors. Detailed process and modeling studies are needed to elucidate these phenomena and improve site investigation tools and risk models.

Abstract

The aquifers below the city of Ferrara are characterized by a peculiar contamination by chlorinated ethenes with vinyl chloride concentrations up to more than 100000 ppb. These phenomena are observed in fully urbanized areas, with houses and also kindergartens, and are connected to the leaching of industrial wastes disposed of inside old clay pits or sugar factories lagoons. The paper presents the main results of different innovative technologies and tools for site characterization, source identification and risk assessment: direct-push drilling, isotopic fingerprinting, indoor gas monitoring.

Abstract

Vapor intrusion occurs when volatile subsurface contaminants, migrating from the saturated zone through the unsaturated zone, accumulate in buildings. It is often the most relevant pathway for human health risks at contaminated sites, especially in urban areas; yet its assessment is controversial. Field assessment of vapor intrusion risk is complicated by two interrelated main factors that are controlled by the contaminant’s properties: transport processes in the unsaturated zone and biodegradation in the unsaturated zone. Commonly available vapor intrusion models either overlook significant properties at the field scale or, conversely, are too complex to be applicable at this scale. Specifically, moisture variation, liquid diffusion, dynamic processes such as water table variations, and biodegradation are not adequately accounted for. As a result, the soil gas and indoor air concentrations predicted by existing models frequently overestimate measured concentrations by several orders of magnitude. This thesis addressed transport and biodegradation processes of volatile organic compounds, focusing on aerobic unsaturated zones. The main aims were to i) characterize significant transport processes influencing vapor intrusion and ii) quantify and mechanistically describe biodegradation in unsaturated soils. Field experience, numerical modeling and laboratory experiments with toluene and vinyl chloride as reference compounds were combined to separate out the relevant processes influencing vapor intrusion. The main conclusions from this thesis indicate that soil moisture variations and aerobic biodegradation are crucial aspects to be jointly considered for the assessment of vapor intrusion. These may contribute to a significant reduction in the risk associated with dissolved volatile organic contaminants. Specific and relevant implications for modeling and monitoring vapor intrusion can be derived. With respect to vapor intrusion modeling, when including unsaturated zone biodegradation, the use of liquid phase biodegradation rates as derived from liquid mixed batches may underestimate by several orders of magnitude the liquid degradation rates in the unsaturated system. Therefore, biodegradation rates derived from unsaturated system appear more appropriate. With respect to monitoring, vertical soil moisture variations and contaminant/oxygen concentration profiles need to be measured in the field, in order to account for the above processes.

26. A coastal lagoon and barrier-lagoon system in NW Adriatic Sea (Italy) dating back 10.000 years ago

Abstract

A greater comprehension of the recent geological past, and particularly of the paleogeographic evolution, is crucial in coastal areas affected by sea level rise phenomena, in order to evaluate future scenarios and elaborate intervention strategies. To this aim, sedimentological, geochemical and micropaleontological data from sediment cores in NW Adriatic Sea were obtained to reconstruct stratigraphic framework and paleogeographic setting during the last post-glacial sea-level rise (14,000 - 6,000 yr BP). Within this record, two marine-transitional lithostratigraphic units were identified: A) a coastal lagoon system and B) a barrier-lagoon system, which is dated back around 10,000 yr BP. 48. Transport and biodegradation of volatile organic compounds : influence on vapor intrusion into buildings

Abstract

Vapor intrusion occurs when volatile subsurface contaminants, migrating from the saturated zone through the unsaturated zone, accumulate in buildings. It is often the most relevant pathway for human health risks at contaminated sites, especially in urban areas; yet its assessment is controversial. Field assessment of vapor intrusion risk is complicated by two interrelated main factors that are controlled by the contaminant’s properties: transport processes in the unsaturated zone and biodegradation in the unsaturated zone. Commonly available vapor intrusion models either overlook significant properties at the field scale or, conversely, are too complex to be applicable at this scale. Specifically, moisture variation, liquid diffusion, dynamic processes such as water table variations, and biodegradation are not adequately accounted for. As a result, the soil gas and indoor air concentrations predicted by existing models frequently overestimate measured concentrations by several orders of magnitude. This thesis addressed transport and biodegradation processes of volatile organic compounds, focusing on aerobic unsaturated zones. The main aims were to i) characterize significant transport processes influencing vapor intrusion and ii) quantify and mechanistically describe biodegradation in unsaturated soils. Field experience, numerical modeling and laboratory experiments with toluene and vinyl chloride as reference compounds were combined to separate out the relevant processes influencing vapor intrusion. The main conclusions from this thesis indicate that soil moisture variations and aerobic biodegradation are crucial aspects to be jointly considered for the assessment of vapor intrusion. These may contribute to a significant reduction in the risk associated with dissolved volatile organic contaminants. Abstract

Early discharge of healthy late preterm and full term newborn infants has become common practice because of the current social and economic necessities. Severe jaundice, and even kernicterus, has developed in some term infants discharged early. This study was designed to elaborate a percentile-based hour specific total serum bilirubin (TSB) nomogram and to assess its ability to predict the absence of risk for subsequent non physiologic severe hyperbilirubinaemia before discharge.
