Abstract

Healthcare-associated infections (HAIs) surveillance is an essential part of any infection prevention and control programme. Repeated point prevalence surveys (PPSs) according to European Centre for Disease Prevention and Control (ECDC) protocol have been implemented in all Friuli Venezia Giulia (FVG) region (Italy) acute hospitals to reduce and control HAIs.Using the repeated PPSs within a regional-healthcare system (RHS) to promote and evaluate infection prevention and control (IPC) programmes.The standard versions of the ECDC PPS protocols were used in all four surveys (2011, 2013, 2015, 2017). All RHS public and private accredited hospitals were involved within the 'safe care network' programme.The numbers of surveyed patients in the four PPSs were 3172, 3253, 2969 and 3036, respectively. Prevalence of HAIs and antimicrobial use (AU) decreased significantly from 2011: HAIs (P0.05) 7.1%, 6.3%, 5.5%, 5.8% and AU (P0.01) 40.4%, 39.2%, 36.0%, 37.2%, respectively. The appropriateness of duration of surgical prophylaxis increased significantly (24 h increased through surveys related to one in 2011: odds ratio (OR), 95% confidence interval (CI) 1.29, 0.92-1.81; 1.95, 1.31-2.91; 1.78, 1.20-2.64, respectively). The most frequently detected HAIs were: bloodstream, urinary tract, pneumonia and surgical site (more than the 70% of HAIs in each PPS).The FVG regional approach to HAIs and AU surveillance was able to contribute to reduce prevalence over a 7-year period. Furthermore, it was able to keep hospital attention on HAIs and AU through the years and to guarantee a standardized and comparable evaluation of HAIs and AU burden in all RHS hospitals, as well as impacting on HAIs and AU regional programmes.

Abstract

Carbonate lithologies host considerable quantities of the Earth's freshwater resources and partially supply a quarter of the global population with drinkable water. In addition, carbonates constitute substantial amounts of the global coastlines, yet it is not known if and how they can sustain freshened groundwater offshore. Here, we use controlled-source electromagnetic, seismic reflection, and core sample data to derive a lithological model for the eastern margin of the Maltese Islands and identify four distinct resistivity anomalies within the Upper Coralline and Globigerina Limestone formations. The anomalies hosted in the former are likely associated with low porosities, whereas the anomaly within the latter is indicative of pore fluid freshening. Hydrogeological modeling suggests that freshened pore fluids, emplaced during sea-level lowstands and preserved in low permeability units, are potentially still found within carbonate shelves. However, resource potential is low due to its relict nature and low permeability host environment.

The Maltese Islands are intersected by two major fault systems associated with two diverse rifting episodes affect the islands. The first and most widespread system is Early Miocene to mid-Pliocene in age, and consists of faults that are orientated ENE-WSW. The most distinct of these faults is the Great Fault (known also as the Victoria Lines Fault). The younger system of faults (Late Miocene-Early Pliocene) is still active and consists of faults striking NW to SE that often cross-cut the first generation of faults. For this reason the Brazilian Diabetes Society (SBD) and the Brazilian Federation of Gynecology and Obstetrics Societies (FEBRASGO), got together to introduce this proposal. We present here a joint consensus regarding the standardization of clinical management for pregnant women with any degree of Dysglycemia, on the basis of current information, to improve medical assistance and to avoid related complications of Dysglycemia in pregnancy to the mother and the fetus. Abstract

Background: Scarce information is available about the variation in the incidence of Type 1 diabetes in the Brazilian population in the last decades. Aim: The objective of this study was to assess the long-term trends (1986–2006) in the incidence of Type 1 diabetes in Bauru, Sao Paulo State, Brazil. Subjects and methods: The annual incidence of Type 1 diabetes (per 100,000 per yr) from 1986 to 2006 was determined in children ≤14 yr of age, using the capture and recapture method. Results: A total of 176 cases were diagnosed in the study population. The overall incidence was 10.4/100,000 with a range of 2.82/100,000 in 1987 to 18.49/100,000 in 2002 representing a 6.56-fold increase within the same population. The estimated incidence, using the capture and recapture method varied from 2.82/100,000 per yr in 1987 to 27.20/100,000 per yr in 2002, representing a 9.6-fold variation. The global pattern of incidence variation was categorized as high (10-19.99/100,000 per yr), and very high (≥20/100,000 per yr) in 71.43% of the study-years. Incidence was slightly higher among females, Caucasians, children in the 5–9 yr of age range and belonging to lower socio-economic classes. Most diagnoses were established during the colder months and/or with higher pluviometric indexes. Conclusions: The incidence of Type 1 diabetes in children is increasing in Bauru, Sao Paulo State, Brazil, and the global pattern of incidence was classified as high or very high, mainly in the last 10 yr. All Brazilian regions should be involved in the study.

Abstract

This research aims to investigate, with instructors of karate-do, the understanding about the relationship between corporal techniques developed in this martial art and the formation of character of the practitioner, a relation that according to the authors of the area is direct and unavoidable. As an instrument of data collection, interviews with all instructors working in this martial art in the Santarém-PA city. As a result, for example, 38% of professors demonstrated that they were directly and objectively trained in character and personality, without considering that the techniques themselves are instruments of formation of this character, another 54% could do so, while 8% of respondents have shown that they do so only with the children, and it is concluded that the instructors methodologically still do not demonstrate cohesion of work, and some of them still need to be more closely aligned with the authors of the area.

Abstract

Recent studies have demonstrated that offshore seismic reflection data can be used to detect boundaries in the water column. This novel approach, called "seismic oceanography", has produced clear pictures of water mass interfaces (i.e., thermoclines, internal waves, fronts, eddies) in different oceanic environments. As the study of ocean circulation requires an extensive spatial and temporal coverage of observations, the possibility of using seismic data available from industrial or academic investigations to support and enhance oceanographic surveys seems very challenging. Since the Southern Ocean represents a key study area for climate due to the deep water formation process which triggers the Global Thermohaline Circulation, we began to reprocess seismic lines recently acquired off Wilkes Land (east Antarctica) in the framework of the WEGA project, to investigate the presence of reflections in the seawater column. A first evaluation of the possibility of recognizing fine structures in the water column is positive: seismic lines acquired along the Wilkes Land continental margin show signals correlated with boundaries inferred by conventional oceanographic measurements. This article summarizes the state of the art in research on cardiovascular diseases (CVD) and occupational exposure. The major chemical and physical agents are considered and the methodological difficulties of epidemiological surveys are discussed. Work-related CVD, like all occupational disorders, are changing continuously as a consequence of modifications in industrial processes, improvements in preventative measures, and the tertiarization of the economy. The cornerstones of a preventative strategy of CVD are still based on environmental monitoring and medical surveillance, but at the same time on multidisciplinary and multicentred scientific research. Where possible, health promotion will include more general programmes directed at changing personal risk factors. [ABSTRACT FROM PUBLISHER]
