Rheumatoid arthritis (RA) is the most common society chronic, erosive, autoimmune and inflammatory polyarthritis. Primarily as a joint diseases, but a variety of extra-articular involvement is a systemic disease. Saphenous Vein Graft Aneurysm (SVGA) are rare complications after coronery artery bypass grafting (CABG) and occur predominantly 10 to 20 years after the procedure. We report a case in which a 64-year old man with a history of coronary artery bypass grafting (CABG) 4 years ago and with a diagnosis of RA in remission who is taking methotrexate 10 mg once a week was admitted to hospital because of dyspnea and chest pain that was worsened in the preceding 4 months. The actual development of SVG aneurysm is unclear but known as risk factors include hypertension, saphenous vein graft trauma, hyperlipidemia and smoking. Although our patient was having none of these risk factors, multiple SVG aneurysms reaching large dimensions that have developed in the early period after CABG. We think that degenerative and inflammatory processes due to RA may have accelerated the development of giant SVG aneurysms

Elastofibroma dorsi is a rare soft tissue tumor. Friction of scapula to chest wall excessively in manuel laborers or genetic factors may be responsible for consist of elastofibroma dorsi. Thorax computerized tomography and magnetic resonance imaging are most frequent used imaging methods in diagnosis of elastofibroma dorsi. Surgical excision is a good treatment opsion for cases with symptomatic., Elastofibroma dorsi nadir görülen bir yumuşak doku tümörüdür. Oluşumunda kol gücüyle çalışan insanlarda skapulanın toraks duvarına aşırı sürtünmesi veya genetik faktörler sorumlu tutulur. Tanısında toraks bilgisayarlı tomografi ve manyetik rezonans görüntüleme en sık kullanılan görüntüleme yöntemleridir. Semptomatik olgularda cerrahi eksizyon iyi bir tedavi seçeneğidir.

Ill ÖZET DERİ LEZ YONLARINDA RENKLİ - POWER DOPPLER ULTRASONOGRAFİNİN YERİ Deri vücudun en büyük organı olup, burada birçok hastalık izlenebilmektedir. Tıbbın bir çok alanında olduğu gibi dermatolojide de ultrasonografı (US) kullanım alanına girmiştir. Günümüzde gri-skala US, renkli Doppler US (RDUS) ve power Doppler US (PDUS) dermatolojik hastalıkların tanı ve tedavisinde gittikçe daha önemli bir konuma geçmektedir. Çalışma Eylül 2002-Nisan 2003 tarihleri arasında deri lezyonları olup da kliniğimize başvuran 80 olgu üzerinde yapıldı. Lezyonların gri-skala US ile boyut, kontur ve ekojenitelerine, RDUS ve PDUS ile de kan akım varlığı, tipi ve spektral analizine bakıldı. Çalışmaya alınan 80 olgu tanı gruplarına göre klinik ve histopatolojik olarak benign, malign ve diğer deri hastalıkları diye üç gruba ayrıldığında, malign gruptaki lezyonların gri-skala ultrasonografik incelemede ; kontur irregülaritesi, pagoda eko bulgusu ve hipoekoik görünüm özellikleriyle ön plana çıktığı izlendi. Yapılan RDUS ve PDUS incelemesinde de malign gruptaki lezyonlarda kan akımının daha fazla olduğu görüldü. Spektral analizde malign lezyonlarda diğer gruplara göre daha yüksek hızlar ve daha düşük rezidif indeks (Rİ) ve pulsatil indeks (Pİ) izlendi. Bu nedenle RDUS ve PDUS'de kan akımı tespit edilen dermatolojik hastalara daha dikkatli yaklaşmak gerektiği ortaya çıkmıştır. Gri-skala US, RDUS ve PDUS kullanımı ile lezyonlar daha kolay, ucuz ve non-invaziv olarak tespit edilebilmektedir. Bizim çalışmamızda gerek gri-skala US ile gerekse RDUS ve PDUS ile lezyonların değerlendirildiğinde benign ve malign lezyonları tanı, takip ve tedavisinin daha kolaylaştığı izlenmiştir. Dolayısıyla tanısal amaçlı US, inspeksiyon ile histoloji arasındaki boşluğu doldurmaktadır. Bulunan bu değerlerin derinin benign ve malign lezyonlarının değerlendirilmesinde ilerde yapılacak çalışmalara bir yol gösterici olacağı düşüncesindeyiz. Anahtar kelimeler: Deri, Renkli Doppler US, Power Doppler US IV SUMMARY USAGE OF COLOR- POWER DOPPLER ULTRASONOGRAPHY IN THE EVALUATION OF SKIN LESIONS Skin is the largest organ of the body and many diseas can be seen in this organ. Ultrasonography (US), as in many branches of medicine, has taken its place in dermatology. Today, gray-scale US, color Doppler US (CDUS) and power Doppler US (PDUS) are gaining importance in the diagnosis and treatment of dermatologic disorders. The study included 80 cases who admitted to our clinics between September, 2002 - April, 2003. The dimensions, contours and echogenicities of the lesions were defined using gray-scale US. The existance of blood flow within the lesions the type of flow was examined and the spectral analysis of flow was done by CDUS and PDUS. 80 cases involved in this study were divided into theree diagnostic groups; malignant, benign and others according to their clinical and histological findings. The lesions included in the malignant group were evident in gray-scale ultrasonographic examination because of their irregular contours, findings of pagoda echo and hypoechogenic appearance. By the CDUS and PDUS examination it was seen that the blood flow was more prominent in the malignant group as compared to the other groups. The peak systolic velocity (Vp) and peak diastolic velocity (Vd) are relatively high and resistance index (RI) and pulsatility (PI) are relatively low for the malignant lesions as compared with the other diagnostic groups. Therefore the patients having blood flow demonstrated by CDUS and PDUS should be more carefully approached. The lesions can be more easily, cheaper and non-invasively detected by using gray scala US, CDUS & PDUS. It is also true for the follow-up of medical and surgical treatment of this lesions. In our study the evaluation of lesions by gray-scale US, CDUS & PDUS facilitated the diagnosis, follow-up and treatment of benign and malignant lesions. Diagnostic US fills the space between inspection and histological findings. We suggest that the data obtained from this study will guide the future studies for the evaluation benign and malignant lesions of the skin. Key words: Skin, Color Doppler US, Power Doppler US. 56
