INTRODUCTION: Spondylodiscitis (SD) is an infection of the intervertebral disc with involvement of the adjacent vertebral bodies. Diagnostic tests with CT-guided biopsy only provide a positive yield in 14%-48% of cases. Percutaneous endoscopic debridement and drainage (PEDD) has recently shown promise in the treatment of spondylodiscitis. RESEARCH QUESTION: The purpose of this study is to determine differences in pathogen identification and clinical outcomes for PEDD versus CT-guided needle biopsy in SD patients. MATERIALS AND METHODS: We conducted a systematic review of the literature using PRISMA guidelines to determine differences in positive microbiology results, perioperative complications, pain control, and long-term clinical outcomes for PEDD vs. CT-guided needle biopsy in SD patients. RESULTS: 1078 studies were evaluated, 87 of which underwent full review. 15 studies met the inclusion and exclusion criteria, including 7 PEDD, 7 CT-guided biopsy, and 1 CT-guided biopsy vs. PEDD article, for a total of 192 PEDD patients and 604 CT-guided biopsy patients. We found 36.59% of CT-guided biopsy patients had positive microbiology results, compared to 84.38% of PEDD patients. No major perioperative complications occurred as a result of the PEDD procedure. Of the five PEDD studies that reported pain outcomes, greater than 80% of patients experienced relief after intervention. DISCUSSION AND CONCLUSION: These results suggest that PEDD may improve pathogen identification while simultaneously reducing pain compared to CT-guided needle biopsy in SD. Although current treatment guidelines recommend CT-guided biopsy, in patients with severe back pain and suspected SD, PEDD can be considered an alternative intervention.

The interest in crystal effects and more precisely for channeling radiation in axially oriented crystals is underlined. An association of a photon radiator using a crystal followed by an amorphous converter may have advantages concerning heating considerations. Such a set of targets, known as a hybrid source has been already investigated and experimental results gathered at KEK are presented. As the heating load with the accompanying thermal stresses have to be studied, variants for the amorphous converter have been considered and also realized: these variants are using small spheres of high Z material, as tungsten: they are providing a better heat dissipation and lower thermo-mechanical effects, leading to better lifespan. These converters, granular, are described and compared to the bulk converters generally used. Their characteristics concerning the yield and the thermal load are analyzed. Experiments carried out at KEK with such converters provided results which are also related. Such granular targets can also be used in conventional sources. [ABSTRACT FROM AUTHOR]

This work explores the effects of embedded software-driven measurements on a sensory target when using a LED as a photodetector. Water turbidity is used as the sensory target in this study to explore these effects using a practical and important water quality parameter. Impacts on turbidity measurements are examined by adopting the Paired Emitter Detector Diode (PEDD) capacitive discharge technique and comparing common embedded software/firmware implementations. The findings show that the chosen software method can (a) affect the detection performance by up to 67%, (b) result in a variable sampling frequency/period, and (c) lead to an disagreement of the photo capacitance by up to 23%. Optimized code is offered to correct for these issues and its effectiveness is shown through comparative analyses, with the disagreement reduced significantly from 23% to 0.18%. Overall, this work demonstrates that the embedded software is a key and critical factor for PEDD capacitive discharge measurements and must be considered carefully for future measurements in sensor related studies.

Turbidity is one of the primary metrics to determine water quality in terms of health and environmental concerns, however analysis typically takes place in centralized facilities, with samples periodically collected and transported there. Large scale autonomous deployments (WSNs) are impeded by both initial and per measurement costs. In this study we employ a Paired Emitter-Detector Diode (PEDD) technique to quantitatively measure turbidity using analytical grade calibration standards. Our PEDD approach compares favorably against more conventional photodiode-LED arrangements in terms of spectral sensitivity, cost, power use, sensitivity, limit of detection, and physical arrangement as per the ISO 7027 turbidity sensing standard. The findings show that the PEDD technique was superior in all aforementioned aspects. It is therefore more ideal for low-cost, low-power, IoT deployed sensors. The significance of these findings can lead to environmental deployments that greatly lower the device and per-measurement costs.

A start-to-end optimization simulation toolkit for the CLIC positron source has been developed using multiple simulation codes to model adequately different areas of the positron source. The positron source includes the targets using channeling and pair-production processes, the adiabatic matching device and the pre-injector and injector linacs. The primary electron beams with energies of 5 GeV and 3 GeV are optimized with this toolkit as well as the target parameters. Finally rf parameters or the linear accelerators are optimized involving a particle tracking code. Using this approach the yield of the CLIC positron source has been increased by a factor 2 resulting in significant cost savings. [ABSTRACT FROM AUTHOR]

The automated techniques of measurement of water parameters are developed based on flow analysis (flow injection analysis, sequential injection analysis). PEDD detectors (paired emitter- detector diode), built with the use of two paired light-emitting diodes (LEDs) - emission and detection, can serve that purpose. The aim of the studies was to optimize the system for determination of the dissolved organic substances in coloured water with the use of PEDDs in the flowing-stream measurement system. The investigations were conducted for a number of concentrations of the humic, fulvic and tannic acids, which were measured as the dissolved organic carbon (DOC). Three arrangements of diodes (PEDD) with a selected emission spectrum and absorption characteristics were used. A two-beam flow cell was designed for the measurements. The signal from the detection diode was recorded and converted into absorbance through software written for that purpose. The results obtained showed a high sensitivity in relation to the concentration as well as the selectivity in the type of substances studied. A miniature and cheap photometric flow analysis set was proposed, adapted to work with one or several light wavelengths and steered by a microprocessor unit. [ABSTRACT FROM AUTHOR]

Abstract: With recent improvements in wireless sensor network hardware there has been a concurrent push to develop sensors that are suitable in terms of price and performance. In this paper a low-cost gas sensor is detailed, and significant improvements in sensor characteristics have been achieved compared to previously published results. A chemical sensor is presented based on the use of low-cost LEDs as both the light source and photodetector, coupled with a sensor slide coated with a pH sensitive colorimetric dye to create a simple gas sensor. Similar setups have been successfully used to detect both acetic acid and ammonia. The goal of this work was to optimise the system performance by integration of the sensing technique into a purposely deigned flowcell platform that holds the colorimetric slide and optical detector in position. The reproducibility of the sensor has been improved through this arrangement and careful control of deposited film thickness. The enhanced reproducibility between sensors opens the potential of calibration-free measurement, in that calibration of one sensor can be used to model the characteristics of all sensors in a particular batch. Abstract: Only two computer-controlled microsolenoid devices, namely two micropumps or one micropump and one microvalve, are sufficient for the construction of on-line dilution modules useful in several flow analytical systems for the calibration using single standard. Three simple constructions of such modules were tested and compared. The most promising is the one based on the concept of a microvalve controlling dilution ratio of the standard and a solenoid micropump playing a double role: solution pumping device and mixing segments homogenizer. All investigated modules were tested with paired emitter detector diode (PEDD) as photometric flow-through detector and bromothymol blue as a model analyte. The best module was implemented into more advanced flow-injection system dedicated for optical detection of alkaline phosphatase activity using UV-PEDD-based flow-through detector for the enzyme reaction product.
