OBJECTIVE:  Severe maternal morbidity (SMM) is increasing and characterized by substantial racial and ethnic disparities. Analyzing trends and disparities across time by etiologic or organ system groups instead of an aggregated index may inform specific, actionable pathways to equitable care. We explored trends and racial and ethnic disparities in seven SMM categories at childbirth hospitalization. STUDY DESIGN:  We analyzed California birth cohort data on all live and stillbirths ≥ 20 weeks gestation from 1997 to 2017 (n = 10,580,096) using the Centers for Disease Control and Preventions SMM index. Cases were categorized into seven nonmutually exclusive indicator categories (cardiac, renal, respiratory, hemorrhage, sepsis, other obstetric, and other medical SMM). We compared prevalence and trends in SMM indicator categories overall and by racial and ethnic group using logistic and linear regression. RESULTS:  SMM occurred in 1.16% of births and nontransfusion SMM in 0.54%. Hemorrhage SMM occurred most frequently (27 per 10,000 births), followed by other obstetric (11), respiratory (7), and sepsis, cardiac, and renal SMM (5). Hemorrhage, renal, respiratory, and sepsis SMM increased over time for all racial and ethnic groups. The largest disparities were for Black individuals, including over 3-fold increased odds of other medical SMM. Renal and sepsis morbidity had the largest relative increases over time (717 and 544%). Sepsis and hemorrhage SMM had the largest absolute changes over time (17 per 10,000 increase). Disparities increased over time for respiratory SMM among Black, U.S.-born Hispanic, and non-U.S.-born Hispanic individuals and for sepsis SMM among Asian or Pacific Islander individuals. Disparities decreased over time for sepsis SMM among Black individuals yet remained substantial. CONCLUSION:  Our research further supports the critical need to address SMM and disparities as a significant public health priority in the United States and suggests that examining SMM subgroups may reveal helpful nuance for understanding trends, disparities, and potential needs for intervention. OBJECTIVE: There has been little evidence of the impact of preventive services during pregnancy covered under the Affordable Care Act (ACA) on birthing parent and infant outcomes. To address this gap, this study examines the association between Medicaid expansion under the ACA and birthing parent and infant outcomes of low-income pregnant people. METHODS: This study used individual-level data from the 2004-2017 annual waves of the Pregnancy Risk Assessment Monitoring System (PRAMS). PRAMS is a surveillance project of the Centers for Disease Control and Prevention and health departments that annually includes a representative sample of 1,300 to 3,400 births per state, selected from birth certificates. Birthing parents outcomes of interest included timing of prenatal care, gestational diabetes, hypertensive disorders of pregnancy, cigarette smoking during pregnancy, and postpartum care. Infant outcomes included initiation and duration of breastfeeding, preterm birth, and birth weight. The association between ACA Medicaid expansion and the birthing parent and infant outcomes were examined using difference-in-differences estimation. RESULTS: There was no association between Medicaid expansion and the outcomes examined after correcting for multiple testing. This finding was robust to several sensitivity analyses. CONCLUSIONS FOR PRACTICE: Study findings suggest that expanded access to more complete insurance benefits with limited cost-sharing for pregnant people, a group that already had high rates of insurance coverage, did not impact the birthing parents and infant health outcomes examined.

BACKGROUND: Women in South Asia often return to their natal home during pregnancy, for childbirth, and stay through the postpartum period-potentially impacting access to health care and health outcomes in this important period. However, this phenomenon is understudied (and not even named) in the demographic or health literature, nor do we know how it impacts health. OBJECTIVE: The aim of this study is to measure the magnitude, timing, duration, risk factors and impact on care of this phenomenon, which we name Temporary Childbirth Migration. METHODS: Using data from 9,033 pregnant and postpartum women collected in 2019 in two large states of India (Madhya Pradesh and Bihar) we achieve these aims using descriptive statistics and logistic regression models, combined with qualitative data from community health workers about this practice. RESULTS: We find that about one third of women return to their natal home at some point in pregnancy or postpartum, mostly clustered close to the time of delivery. Younger, primiparous, and non-Hindu women were more likely to return to their natal home. Women reported that they went to their natal home because they believed that they would receive better care; this was born out by our analysis in Bihar, but not Madhya Pradesh, for prenatal care. CONCLUSIONS: Temporary childbirth migration is common, and, contrary to expectations, did not lead to disruptions in care, but rather led to more access to care. CONTRIBUTION: We describe a hitherto un-named, underexplored yet common phenomenon that has implications for health care use and potentially health outcomes.

Fetal age in Quarter Horses can be predicted within 2 weeks from 100- to 200- days of gestation using femur length, biparietal diameter (cranium diameter) and eye approximated volume. However, as pregnancy advances, the femur and cranium become too large to be imaged in their entirety using ultrasound and the corresponding biometric parameters can no longer be measured. In this longitudinal study, the proximal phalanx (P1) was evaluated as a novel biometric parameter for late gestation to predict fetal age and bone maturation. Transrectal ultrasound was performed in ten pregnant mares with known ovulation dates, every two weeks from 240- days of gestation until parturition. P1 was imaged in 69 % of the examinations. Inability to image P1 was due to obstructive positioning such as carpal or fetlock flexion, or posterior presentation of the fetus. Advancing fetal age did not affect visibility of P1. P1 length correlated significantly with days of gestation and a correlation equation was established: y = 0.3837x -69.55 where y is the predicted value of P1 length and x is the day of gestation (with day 0 being the day of ovulation). When P1 length was equal to or larger than the width of the ultrasound image (52.5 mm), 90 % of mares (9/10) were above 300- days of gestation. Ossification of the proximal and distal epiphysis of P1 typically appeared between 277- and 303 -days of gestation (mean: 288 days). The proximal epiphysis did not close before parturition whereas the distal one closed between 306- and 333-days of gestation (mean: 320 days). P1 epiphyseal appearance and closure occurred chronologically reflecting bone maturation. Radiographic findings at birth and prenatal ultrasound findings were in agreement, apart from timing of P1 distal epiphyseal closure. In conclusion, P1 length can be used as a new fetal biometric parameter to assess fetal age and growth after 240- days of gestation. The knowledge of P1 bone maturation process in utero as a marker for fetal bone development, may also be valuable in clinical decision-making when considering inducing parturition in the mare. One of the selective advantages of delayed implantation in carnivores is the flexibility to decouple the time between mating and parturition, allowing both to occur when conditions are most favorable. Terrestrial carnivores with delayed implantation have evolved reduced maternal energy expenditure through shorter gestation length, smaller neonates, and smaller litters, with a possible linkage between maternal body condition and time of birth or litter size. Using data on wolverine (Gulo gulo) females and cubs from management removals covering the entire latitudinal gradient of the Scandinavian population in 2001‐2022, I assessed the effect of extrinsic and intrinsic factors on variation in parturition date and litter size. Timing of parturition varied over 2 months, where 90% of the litters were born between 5 February and 11 March, with a birth peak on 23 February. Females living in winter grazing pastures of semi‐domestic reindeer (Rangifer tarandus) gave birth earlier than those outside, and along an elevation gradient parturition date was delayed at higher altitudes. I did not find evidence of an effect of latitude on parturition date. Furthermore, older females had greater odds than younger females of having litters with 3 cubs rather than 1 cub. To minimize the risk of harvesting lactating females, management regulations should implement harvest seasons that extend no longer than the end of January. [ABSTRACT FROM AUTHOR]

The white shark (Carcharodon carcharias) is a globally distributed top predator. Due to its ecological importance and historical declining population trends, data contributing to conservation initiatives (e.g. habitat protections and resource management) pertaining to all life stages of this species are essential to facilitate population recovery. Of particular interest, the locations and discrete seasonality of C. carcharias parturition remain uncertain. Understanding C. carcharias parturition in relation to each population is relevant to population recovery since neonate to young-of-the-year (YOY) sharks are more vulnerable to predation and particularly threatened by and susceptible to commercial fishing pressure. Herein, this paper provides a synthesis from published literature across seven well-studied C. carcharias populations to identify common trends associated with parturition location, seasonality, and habitat characteristics. The data reviewed in this study are consistent with previous population-specific hypotheses, that C. carcharias parturition occurs during spring and summer for all populations. Further, this review also indicates that parturition likely occurs in insular shelf waters and water temperatures ranging from15.7 to 23.1 °C. Although discrete parturition sites were not identified, the compiled data are suggestive that C. carcharias parturition may occur over horizontal and vertical spatial scales that exceed the inshore, shallow water environments associated with nursery area habitat to perhaps minimize predation by conspecifics. Due to the vulnerability of C. carcharias, conducting non-lethal technological (e.g., baited remote underwater video systems—BRUVS), morphological (i.e., ontogenetic changes in dorsal fin shape), and reproductive (e.g., blood chemistry and ultrasonography) research that may help identify parturition location and seasonality are thus warranted. Internationally, over one-third of women experience birth trauma, leading to adverse mental health outcomes. Poor communication with healthcare professionals is a primary contributing factor. Paramedics attend various clinical presentations, including childbirth, yet their potential impact on women's birth experiences has been largely overlooked. A systematic literature search was conducted following the Joanna Briggs Institute methodological framework. The search identified 1015 potentially suitable articles, and 5 articles met the inclusion criteria. Data was analysed using reflexive thematic analysis from a feminist standpoint. Three themes were generated: 1. First Impressions Count: paramedic demeanour impacted the woman's sense of safety and perception of paramedic clinical competence. 2. Choice as a Pathway to Control: when paramedics involved women in decision-making, it led to empowerment, while non-involvement led to women becoming passive participants. 3. Exposed, Violated and Disempowered: some paramedics disrespected and abused women, treating them solely as objects for the purpose of producing a baby. This review highlights the influence of paramedic communication on women's birth experiences. While some paramedics communicated respectfully, other paramedics were the perpetrators of Obstetric Violence. Simple Summary: Postbiotics are non-viable probiotic microorganisms, with or without their metabolites, that provide a health benefit to the host. They prevent the colonisation of pathogens by improving the gut environment for beneficial commensal bacteria. This effect reduces the incidence of digestive disorders in calves, which enhances animal growth and boosts the immune system and general health. The aim of this work was to evaluate the addition of postbiotics to dairy cow rations during the transition period on nutrient digestibility and milk productivity. The effects of two postbiotics from calving to two months of lactation were evaluated in twelve Friesian cows. The results suggest that postbiotic supplementation in late gestation and early lactation increases the voluntary intake of dry matter, with higher apparent total tract digestibility of nutrients. The inclusion of postbiotics in the dairy cow diet increases colostral immunoglobulin concentration and milk production with high fat and protein yields. The metabolites secreted by probiotics or released after their lysis are called postbiotics. They provide physiological benefits to the host, preventing the colonisation of pathogens by improving the intestinal environment for beneficial commensal bacteria, which reduces the incidence of digestive disorders and improves the immune system. The aim of this work was to evaluate the addition of postbiotics to dairy cow rations during the transition period on nutrient digestibility, composition, and milk yield. The effects of two postbiotics were evaluated in twelve Friesian cows from 30 days before calving to two months of lactation. The animals were randomly allocated to two treatment groups: control (CT) and supplemented with postbiotics (PC and PR). Feeding was ad libitum with 60/40 of forage/concentrate ratio on dry matter basis. Daily feed intake and milk production were recorded individually throughout the study. Two digestibility balances were performed, one before parturition and one after parturition. Colostrum was sampled at first milking and milk was sampled weekly. Data were analysed using a mixed mode in R software 4.4.1. The results suggest that postbiotic supplementation in late gestation and early lactation increase the voluntary intake of dry matter, especially in the PR treatment, with higher apparent total tract digestibility of dry matter, organic matter and neutral detergent fibre. Both treatments including postbiotics induced an increase in colostral immunoglobulin concentration. Milk production of cows receiving the PC treatment was the highest, with high fat and protein yields and a higher persistence of the production curve throughout the lactation. [ABSTRACT FROM AUTHOR]

Total bacterial count (TBC) and SCC are important quality parameters in goat milk. Exceeding the bulk milk TBC (BMTBC) thresholds leads to price penalties for Dutch dairy goat farmers. Controlling these milk quality parameters can be challenging, especially around kidding. First, we describe the variation and the peaks around kidding of TBC and SCC in census data on Dutch bulk milk over the last 22 yr. Second, to explore causes of these elevations, we studied the variation of TBC and SCC in individual goat milk from 3 wk before to 5 wk after kidding and their association with systemic response markers IFN-γ, calprotectin, BHB, BCS, and fecal consistency. We visited 4 Dutch dairy goat farms weekly for 10 to 16 wk around kidding. Some of the goats had been dried off; other goats were milked continuously throughout pregnancy. A total of 1,886 milk samples from 141 goats were collected for automated flow cytometric quantification of TBC and SCC measurement. IFN-γ, calprotectin, and BHB were determined twice in blood of the same goats; most samples were collected after kidding. The BCS and fecal consistency were scored visually before and after kidding. We found a strong correlation between TBC and SCC (Spearman's rho = 0.87) around kidding. Furthermore, in the third week before kidding, the average TBC (5.67 log 10 cfu/mL) and SCC (6.70 log 10 cells/mL) were significantly higher compared with the fifth week after kidding, where the average TBC decreased to 4.20 log 10 cfu/mL, and the average SCC decreased to 5.92 log 10 cells/mL. In multivariable linear regression models, farm and stage of lactation were significantly associated with TBC and SCC, but none of the systemic response markers correlated with TBC or SCC. In conclusion, TBC and SCC in dairy goats were high in late lactation and decreased shortly after parturition. For SCC, the dilution effect might have caused the decrease, but this was not plausible for TBC. Moreover, the excretion of bacteria and cells in goat milk was not associated with the selected systemic response markers that were chosen as a readout for general immunity status, intestinal health, and metabolic diseases. Therefore, we assume that the TBC increase before kidding and the decrease after parturition are caused by other systemic, possibly hormonal, processes. To reduce BMTBC and bulk milk SCC, it would be advisable to keep milk of goats with highest numbers of bacteria and cells in their milk out of the bulk milk during end lactation. The use of sensor-based measures of rumination time as a parameter for early disease detection has received a lot of attention in scientific research. This study aimed to assess the accuracy of health alerts triggered by a sensor-based accelerometer system within 2 different management strategies on a commercial dairy farm. Multiparous Holstein cows were enrolled during the dry-off period and randomly allocated to conventional (CON) or sensor-based (SEN) management groups at calving. All cows were monitored for disorders for a minimum of 10 DIM following standardized operating procedures (SOP). The CON group (n = 199) followed an established monitoring protocol on the farm. The health alerts of this group were not available during the study but were later included in the analysis. The SEN group (n = 197) was only investigated when the sensor system triggered a health alert, and a more intensive monitoring approach was implemented according to the SOP. To analyze the efficiency of the health alerts in detecting disorders, the sensitivity (SE) and specificity (SP) of health alerts were determined for the CON group. In addition, all cows were divided into 3 subgroups based on their health status and the status of the health alerts in order to retrospectively compare the course of rumination time. Most health alerts (87%, n = 217) occurred on DIM 1. For the confirmation of diagnoses, health alerts showed SE and SP levels of 71% and 47% for CON cows. In SEN cows, SE of 71% and 75% and SP of 48% and 43% were found for the detection of ketosis and hypocalcemia, respectively. Objectives: Previous research shows that sacroiliac (SI) joint fusion is age and sex dependent. Older individuals—specifically starting in the fifth or sixth decade of life—are more likely to develop SI fusion. Females have a lower frequency of SI joint fusion than males, perhaps due to pregnancy or parturition. This study examines the relationship between SI joint fusion with both sex and parity status in females. The issue is whether the prevalence of SI fusion in nulliparous females is more similar to that of males or parous females. Materials and Methods: The sample consists of 46 nulliparous females, 119 parous females, and 158 males from the William M. Bass Donated Skeletal Collection. Ages of the individuals ranged from 50 to 89 years. Sex, age, and parity status were self‐reported. Results: The frequency of SI joint fusion is significantly different among males (13.29%), nulliparous females (6.52%), and parous females (0.84%). Pairwise comparison of the three groups for SI joint fusion shows that parous females and males are significantly different, but nulliparous females are nonsignificantly different from parous females and males. Discussion: Parity status does not appear to be a factor in the sexually dimorphic nature of SI joint fusion in this sample. Rather, biomechanical and hormonal factors may have a greater contribution to higher rates of SI joint fusion in males than females. Research Highlights: Parous females have a lower prevalence of SI joint fusion compared with males of the same age categories.Parity status and SI joint fusion were not statistically significant, suggesting that the constraints of childbirth may not contribute to lower rates of SI joint fusion in females than males. Pregnancy involving intricate tissue transformations governed by the progesterone hormone (P4). P4 signaling via P4 receptors (PRs) is vital for endometrial receptivity, decidualization, myometrial quiescence, and labor initiation. This study explored the role of TCF23 as a downstream target of PR during pregnancy. TCF23 was found to be expressed in female reproductive organs, predominantly in uterine stromal and smooth muscle cells. Tcf23 expression was high during midgestation and was specifically regulated by P4, but not estrogen. The Tcf23 knockout (KO) mouse was generated and analyzed. Female KO mice aged 4-6 months exhibited subfertility, reduced litter size, and defective parturition. Uterine histology revealed disrupted myometrial structure, altered collagen organization, and disarrayed smooth muscle sheets at the conceptus sites of KO mice. RNA-Seq analysis of KO myometrium revealed dysregulation of genes associated with cell adhesion and extracellular matrix organization. TCF23 potentially modulates TCF12 activity to mediate cell-cell adhesion and matrix modulation in smooth muscle cells. Overall, TCF23 deficiency leads to impaired myometrial remodeling, causing parturition delay and fetal demise. This study sheds light on the critical role of TCF23 as a dowstream mediator of PR in uterine remodeling, reflecting the importance of cell-cell communication and matrix dynamics in myometrial activation and parturition. Wildlife conservation often focuses on mitigating disturbance in critical habitats like reproductive ranges. We studied lambing and nursery habitat selection by Stone's sheep (Ovis dalli stonei (J.A. Allen, 1897)) in the Cassiar Mountains in British Columbia. We estimated the timing of parturition and delineated lambing and nursery habitats based on movement behaviours of collared females (n = 18) and vaginal implant transmitters. We identified 23 lambing events in 2018 (n = 4), 2019 (n = 13), and 2020 (n = 6). The median birth date was 17 May and ranged from 3 May to 14 June. Females remained in lambing habitats from 1.5 to 11.3 days with a median of 5.5 days. We examined habitat selection during the lambing and nursery periods at the home-range scale using resource selection functions, and at a finer scale using integrated step-selection analyses. Females selected southwest slopes in rugged terrain at mid-elevations, suggesting selection for warmer micro-climates and features that facilitate predator avoidance. Females avoided habitats near roads during the lambing period but showed selection for habitats near roads during the nursing period. We developed predictive maps of suitable lambing and nursery ranges to inform land planning to help reduce overlap of anthropogenic disturbance with potential reproductive habitats. We comment on a recent study (Vasenkov et al. in Dokl Biol Sci 513:395–399, 2023, https://doi.org/10.1134/S0012496623700746) presenting the movement paths of three greater noctule bats (Nyctalus lasiopterus), two juveniles and an adult, tracked from Russia in September–October 2020 and 2021, with particular emphasis on the unprecedented straight-line migration of a juvenile male to Italy. Notably, the juvenile's extraordinary 2515 km migration marks the longest recorded (as the crow flies) movement in a bat species. We argue that the two juveniles may represent cases of dispersal. Despite the attention garnered by Vasenkov et al.'s findings, caution is warranted in interpreting the broader migration picture. The paper's dissemination underscores a potential oversimplification, neglecting key insights from recent work conducted in Italy that documented the year-round presence, hibernation, and parturition of N. lasiopterus in an area of the Friuli Venezia-Giulia region where the juvenile male tracked from Russia stopped over. Besides, inferring the existence of migratory routes based on small numbers of tracked bats is insufficient. The oversight of these findings may imply a misleading narrative about the species' behaviour, likely representing a case of partial migration, a pattern widespread among migratory bat species. During the gestation and lactation period, the energy demand in pregnant and lactating bitches is elevated. Non‐esterified fatty acids (NEFAs) are utilized either directly from the fed diet or from body fat storage. High NEFA concentration in the blood plasma leads to an increased risk for diseases. Therefore, measuring blood NEFA concentrations may be an indicator for a period of scarcity. The aim of this study is to explore if serum NEFA concentrations in healthy bitches change during gestation and lactation. Healthy pregnant and lactating bitches were sampled on three appointed dates around parturition. NEFA values were examined with a multiparameter clinical chemistry analyser. All statistical analyses were performed using R. Overall, 38 bitches were enrolled in the study. Twenty‐one bitches were sampled on all three appointed dates. The median NEFA concentration antepartum was 0.73 mmol/L (IQR: 0.59, 1.01); during peak lactation, it was 0.57 mmol/L (IQR: 0.44, 0.82); and around weaning, it was 0.58 mmol/L (IQR: 0.46, 0.73). NEFA concentrations rose slightly with litter size in late gestation. Body condition score had no influence on observed NEFA values. We conclude that NEFA concentrations widely remain within reference ranges in well‐fed pregnant and lactating bitches. Background: In the livestock industry, Foreign Body Syndrome is a devastating disease condition. Feeding management, lacking of food discrimination, and eating chopped food increase the risk of swallowing sharp foreign bodies in bovine species. In addition to the honeycomb cells shape of the reticulum, the contractions of the reticular wall, gravid uterine pressure, and parturition efforts, foreign bodies can penetrate the reticular wall, causing cascade of problems including traumatic reticulitis, traumatic reticuloperitonitis, and traumatic pericarditis. The present study was carried out to evaluate the diagnostic significance of cardiac troponin I rapid test cassette and other cardiac biomarkers including serum cardiac troponin I (cTn I), creatine kinase-myocardial band (CK-MB), lactate dehydrogenase (LDH), and aspartate aminotransferase enzyme (AST), in confirmed cases of traumatic pericarditis (TP) and/or traumatic reticuleoperitonitis (TRP) in cattle and buffaloes. Methods: A total number of 30 animals (22 cattle and 8 buffaloes) with different signs such as anorexia, jugular distension, brisket edema, and signs of pain (reluctance to move, arching back, and abduction of the forelimbs) were included in the present study. Based on case history, clinical signs, ferroscopic, pericardiocentesis, radiographic and ultrasonographic examinations, TP were confirmed in cattle (n = 10) and buffaloes (n = 8) while TRP were confirmed only in cattle (n = 12). Additionally, 20 clinically healthy animals (n = 10 cattle and 10 buffaloes) were used as a control group. Blood samples were collected for determination of blood level of Tn-I, and activity of CK-MB, LDH, and AST. Results: The obtained results revealed a highly significant increase in serum cTn I in diseased cattle with TP and TRP (P = 0.00), while buffaloes with TP showed no significant changes in serum cTn I (P = 0.111). Both diseased cattle and buffaloes showed increased serum activities of CK-MB, AST, and LDH enzyme. On the other hand, cardiac troponin I rapid test cassette failed to detect cTn I in diseased animals. Conclusion: The study concluded that the cardiac troponin I rapid test cassette did not have a diagnostic significance and could not be used as a point-of-care under field condition for diagnosis of TP and TRP in large ruminants. However, the serum troponin I level is helpful in diagnosis of TP and TRP in cattle. Although cardiac biomarkers have some diagnostic values in TP and TRP, the traditional diagnostic methods (clinical, radiography and ultrasonography examinations) are crucial for thorough evaluation of TP/TRP cases in bovine. Data on the reproductive biology of elasmobranchs are essential for understanding their life history. Published studies on batoid ray reproductive biology are comparatively scarce, leading to limited understanding and data gaps. The Oceania fantail ray, Taeniura lessoni, is a good example. This Data Deficient nearshore stingray is restricted to Melanesia, with lacking biological and ecological data, including reproduction. To expand upon the limited life-history data for this species, this short paper provides observational data on the reproductive condition in female T. lessoni, at Drawaqa Island, Fiji. Field work involved direct observations and ocean temperature measurements. Over 40 days spanning three months, 105 surveys were conducted across five sites, resulting in 71 sightings of the species. Based on spot patterns and body markings, four female individuals were identified. Between January and March 2024, these females exhibited convex dorsa indicating advanced gestation, transitioning to concave dorsa suggesting parturition. The presence of neonates from early March onwards coincided with the estimated parturition period inferred from the rays' condition. Furthermore, a female previously pregnant was photographed with a dermal abrasion around her pectoral fin, possibly indicating pre-copulatory biting, suggesting a continuous reproductive cycle. The average monthly water temperature at the surveyed sites remained relatively stable throughout the study. Collectively, our findings suggest that Drawaqa Island provides suitable habitat niches for reproductive activities in female T. lessoni. Repeated and long-term data is certainly needed to confirm either a continuous reproductive cycle or seasonal peaks. Metabolic adaptations to negative energy balance, as well as lipomobilization, influence inflammatory responses, immune function, and oxidative stress in animals. This study aimed to evaluate the biochemical profile of Mediterranean buffaloes with different levels of lipomobilization from the prepartum to the postpartum period. A total of 76 Mediterranean buffaloes were enrolled, and a weekly blood sample was taken from 7 weeks before to 6 weeks after calving. The concentration of non-esterified fatty acids (NEFAs) was determined in serum and was used to categorize buffaloes into three lipomobilization groups 7 weeks before calving: mild (NEFA-I; NEFA ≤ 0.29 mEq/L; n = 18), medium (NEFA-II; 0.29 < NEFA < 0.57mEq/L; n = 20), and severe (NEFA-III; NEFA ≥ 0.57mEq/L; n = 38). Two-way repeated measures ANOVA was used to assess changes within and between the groups and over time. Significant differences were found in the concentration levels of NEFA, β-hydroxybutyrate, glucose, cholesterol, protein profile, oxygen radicals, antioxidants, lysozyme, complement, and minerals. These results suggest that both medium and severe lipomobilization groups are associated with metabolic alterations. In conclusion, buffaloes with higher NEFA levels (>0.29mEq/L; NEFA-II and NEFA-III) at 7weeks before calving should be monitored more closely to reduce the risk of metabolic diseases. Furthermore, the medium (NEFA-II) and severe (NEFA-III) lipomobilization groups could be associated with differences in the animals' ability to manage their metabolic status. Specifically, the severe mobilization group was most associated with a greater energy deficit during both the prepartum and postpartum periods without oxidative stress. On the contrary, the medium mobilization group was associated with a less severe energy deficit but was also associated with an inflammatory status and oxidative stress during the prepartum period. Background: Fatty liver in dairy cows is a common metabolic disease defined by triglyceride (TG) buildup in the hepatocyte. Clinical diagnosis of fatty liver is usually done by liver biopsy, causing considerable economic losses in the dairy industry owing to the lack of more effective diagnostic methods. Therefore, this study aimed to investigate the potential utility of blood biomarkers for the diagnosis and early warning of fatty liver in dairy cows. Results: A total of twenty-four lactating cows within 28 days after parturition were randomly selected as experimental animals and divided into healthy cows (liver biopsy tested, n = 12) and cows with fatty liver (liver biopsy tested, n = 12). Inductively coupled plasma mass spectrometry (ICP-MS) was used to determine the macroelements and microelements in the serum of two groups of cows. Compared to healthy cows (C), concentrations of calcium (Ca), potassium (K), magnesium (Mg), strontium (Sr), selenium (Se), manganese (Mn), boron (B) and molybdenum (Mo) were lower and copper (Cu) was higher in fatty liver cows (F). Meanwhile, the observed differences in macroelements and microelements were related to delivery time, with the greatest major disparity between C and F occurring 7 days after delivery. Multivariable analysis was used to test the correlation between nine serum macroelements, microelements and fatty liver. Based on variable importance projection and receiver operating characteristic (ROC) curve analysis, minerals Ca, Se, K, B and Mo were screened as the best diagnostic indicators of fatty liver in postpartum cows. Conclusions: Our data suggested that serum levels of Ca, K, Mg, Se, B, Mo, Mn, and Sr were lower in F than in C. The most suitable period for an early-warning identification of fatty liver in cows was 7 days after delivery, and Ca, Se, K, B and Mo were the best diagnostic indicators of fatty liver in postpartum cows. The Western Three-toed Skink (Chalcides striatus) is a viviparous species found in arid Mediterranean climates. Using artificial shelters, we captured five pregnant females (snout vent length = 139.2 ± 7.9 mm [SD], 131 to 150 mm; total length = 286.0 ± 9.6 mm, 275 to 292 mm [N = 3, two females with damaged tails]); body mass 15.72 ± 3.89 g, 11.55 to 19.88) and kept them in captivity until parturition. Mean litter size and litter mass were 6.2 ± 1.3 (5 to 8 neonates) and 3.85 ± 0.88 g (2.99 to 5.13 g), respectively. Mean neonatal size and mass were 81.2 ± 4.1 mm (total length = 70 to 90 mm, N = 26) and 0.62 ± 0.07 g (0.49 to 0.82 g, N = 31), respectively. On average, the relative litter mass represented 32 ± 3% (29% to 36%) of the postpartum female body mass. The estimated mass of water lost at birth (total mass loss at parturition minus litter mass) was 0.37 ± 0.39 g (0.00 to 0.98 g), representing 10 ± 8% (0 to 26%) of the individual neonate mass. This low value is in line with that expected for viviparous squamates from arid climates, which is much lower than that observed in oviparous or in viviparous squamates from temperate climates. These results help to fill a gap, as the reproductive output of Chalcides striatus has been poorly documented, but mostly because the importance of extraembryonic water mass has been neglected. Parturition timing has long been a topic of interest in ungulate research. However, few studies have examined parturition timing at fine scale (e.g., <1 day). Predator activity and environmental conditions can vary considerably with diel timing, which may result in selective pressure for parturition to occur during diel times that maximize the likelihood of neonate survival. We monitored parturition events and early‐life survival of elk (Cervus canadensis) and mule deer (Odocoileus hemionus) in Utah, USA to better understand diel timing of parturition in temperate ungulates. Diel timing of parturition was moderately synchronous among conspecifics and influenced by environmental variables on the date of parturition. For elk, parturition events were most common during the morning crepuscular period and generally occurred later (i.e., closer to 12:00) when a relatively large proportion of the moon was illuminated. For mule deer, parturition events were most common during the diurnal period and generally occurred later (i.e., closer to 15:00) on cold, wet dates. Diel timing of parturition did not influence neonate survival, but larger datasets may be required to verify the apparent lack of influence. Although additional work could evaluate alternative variables that might affect parturition timing, our data provide an improved and finer scale understanding of reproductive ecology and phenology in ungulates. Context: There is some evidence that physiological stages such as breeding, pregnancy, parturition and lactation may trigger oxidative stress. It has also been observed in several species, including sheep, that age affects their vulnerability to oxidative stress. Aims: This study investigated the effects of supplemental β-carotene and age on the oxidative status of grazing ewes around breeding, pregnancy, parturition and early lactation as well as on their production and reproduction performance. Methods: Hundred and four ewes were divided into two broad age groups (young = 1–3 years, and old = 4–6 years). Within age groups, ewes of similar age were randomly assigned to four treatment groups (A1, A2, A3, C) by using a completely randomised block design. The young (n = 13) and old (n = 13) sheep in Groups A1, A2 and A3 were daily drenched with 100 mg, 75 mg and 50 mg β-carotene respectively. Group C was unsupplemented (Control). Supplementation lasted 28 days before oestrus synchronisation; 14 days during oestrus synchronisation (CIDR) and hand-mating; 18 days post-hand-mating; 30 days before lambing; and 15 days after lambing. All animals were grazed on Kikuyu grass (Pennisetum clandestinum). Six ewes from each group (total = 24) were sampled for blood to measure oxidative and hormonal status during the experiment. Key results: Supplemental β-carotene at ≥75 mg/day may act as a pro-oxidant in ruminants under oxidatively stressed conditions such as parturition. Age did not affect the ewes' oxidative status. The supplemental β-carotene did not affect bodyweight, oestrus parameters, corpus luteum size, conception rate and litter size. Except for a tendency of lambs from supplemented ewes to have heavier weight at 15 days old (P = 0.080), age group and supplemental β-carotene did not affect the lamb mortality rate or weight at birth or 15 days old. Older ewes showed earlier and longer duration of oestrus than did younger ewes. Conclusions: Supplementation of grazing ewes with β-carotene should not be ≥75 mg/day, especially at parturition period, because it may work as a pro-oxidant. The older ewes seem to have earlier oestrus with a longer duration than do the younger ones. Implications: Caution is urged when administering high doses of β-carotene to ewes during the peri-parturient period. Objectives: At our center, natural home‐like delivery settings have been established in or near conventional labor wards, for the care of pregnant women who prefer little or no medical intervention during labor and birth. We compared obstetrical and neonatal outcomes of women in active spontaneous labor, between those who chose to deliver in a natural‐delivery setup and those who chose a conventional setting. Methods: This retrospective study included low‐risk women who delivered at term between March 1, 2020 and December 31, 2022, in a single tertiary university affiliated medical center. Birth outcomes were compared between 124 women who delivered by natural birth (the study group) and 244 who gave birth in a conventional setting (the control group). Results: No cesarean deliveries were performed in the study group, compared to 18 (7.4%) of the control group, p = 0.004. Intrapartum fever, postpartum hemorrhage, and uterotonic administration were similar between the groups. For the study compared to the control group, breastfeeding was more common (71.3% vs. 12.3%, p < 0.001), analgesia administration within 48 h delivery was lower (4.1% vs. 10.7%, p = 0.033), and maternal and neonatal length of hospitalization were shorter. Of the women initially admitted to the natural‐delivery room, 14 (11.5%) were transferred to a conventional‐delivery room. Conclusions: Birth in a hospital natural‐delivery setting was associated with increased likelihood of vaginal birth, increased immediate breastfeeding and breastfeeding at discharge, and lower postpartum pain. Parturition is a complex physiological process that must occur in a reliable manner and at an appropriate gestation stage to ensure a healthy newborn and mother. To this end, hormones that affect the function of the gravid uterus, especially progesterone (P4), 17β-estradiol (E2), oxytocin (OT), and prostaglandins (PGs), play pivotal roles. P4 via the nuclear P4 receptor (PR) promotes uterine quiescence and for most of pregnancy exerts a dominant block to labor. Loss of the P4 block to parturition in association with a gain in prolabor actions of E2 are key transitions in the hormonal cascade leading to parturition. P4 withdrawal can occur through various mechanisms depending on species and physiological context. Parturition in most species involves inflammation within the uterine tissues and especially at the maternal-fetal interface. Local PGs and other inflammatory mediators may initiate parturition by inducing P4 withdrawal. Withdrawal of the P4 block is coordinated with increased E2 actions to enhance uterotonic signals mediated by OT and PGs to promote uterine contractions, cervix softening, and membrane rupture, i.e., labor. This review examines recent advances in research to understand the hormonal control of parturition, with focus on the roles of P4, E2, PGs, OT, inflammatory cytokines, and placental peptide hormones together with evolutionary biology of and implications for clinical management of human parturition. Background: Interaction between nutrition and reproduction has been established to play an important role in reproductive performance of animals. Zinc is considered to be an essential element required for reproduction. The present study was conducted to evaluate the efficiency of nano zinc (NZn) as feed supplementation on reproductive performance of Assam Hill goats. Methods: A total of twenty-four numbers of 7 days post kidding doe maintained at Goat Research Station, Assam Agricultural University, Burnihat were randomly selected and divided into 4 groups comprising 6 animals in each group. The control group animals were fed with basal diet without zinc supplementation however, all the animals of the treatment groups received 25 mg NZn, 35 mg NZn and 50 mg NZn/kg concentrate mixture with basal diet for a period of three months. Does were bred naturally at 24 hours from the onset of oestrus and confirmed for pregnancy after two months. Parturition behaviour was observed closely from two days before the expected date of kidding upto the period of placental expulsion. Result: Supplementation of 50 mg NZn to the basal diet of postpartum Assam Hill Goat significantly improved the reproductive performance by decreasing postpartum oestrus interval and increasing conception rate (83.33%). The occurrence of wound infection following a Caesarean section procedure poses a substantial clinical obstacle. Transcutaneous Electrical Nerve Stimulation (TENS) has been identified as a promising supplementary treatment option for improving the healing process and decreasing the incidence of infections. This study assessed the efficacy of TENS therapy in the postoperative care of patients who have had Caesarean section. We randomly assigned a total of 108 women who had Caesarean sections to either a TENS group (n = 54) or control (n = 54). The TENS therapy was provided twice daily for a duration of 30 min for the period of 14 days following the surgery. The main measure of interest in this study was the occurrence of wound infection during 30‐day period. Additionally, secondary measures included the rate of wound healing, levels of pain experienced and level of patient satisfaction. In comparison to the control (22.2%, p < 0.05), the TENS group had notably reduced occurrence of wound infection, with the rate of 7.4%. TENS group had superior wound healing results, as measured by REEDA scale, at 7 days (2.1 ± 0.8 vs. 2.5 ± 1.0, p < 0.04), 14 days (1.2 ± 0.5 vs. 1.9 ± 0.7, p < 0.05) and 30 days (0.3 ± 0.5 vs. 0.7 ± 0.6, p < 0.05). Furthermore, TENS group had reduced pain levels on the Visual Analog Scale (VAS) at all evaluation intervals (p < 0.05). TENS group exhibited significantly higher levels of patient satisfaction, as evidenced by 64.8% of participants rating high satisfaction, in contrast to 40.7% in the control group (p < 0.05). The incidence of adverse effects was found to be minor, as indicated by a skin irritation rate of 3.7% and reported discomfort rate of 1.9% at the electrode location. TENS therapy effectively decreased the occurrence of post‐Caesarean wound infections, expedited the healing process and enhanced pain control. This treatment was well‐received by patients and had little negative consequences. The aforementioned results provided evidence in favour of incorporating TENS into post‐Caesarean care regimens, which may have significant consequences for improving patient outcomes and maximizing healthcare resources. Background: Accurate prediction of parturition is paramount to ensuring monitoring of delivery and preventing complications. Assessing the pH and electrolytes of the mammary gland secretions (MGS) helps detect impending parturition. As conductivity is related to electrolyte concentrations and pH, it could be a useful alternative for predicting impending parturition; however, this hypothesis warrants a critical assessment. Objectives: To assess the ability of conductivity, pH, and Brix in the MGS to predict parturition and to investigate their associations. Study design: Field study. Methods: The MGS of periparturient mares (n = 241) was assessed daily for conductivity, pH, and Brix index from 320d until parturition. Receiving operating curve cut‐off values for conductivity (≤4.8 mS/cm), pH (≤6.4), and Brix index (>23.6%) were used to calculate sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) for predicting parturition in ≤24 h. Results: Impending parturition was associated with a reduction in conductivity and pH (p < 0.05), and conductivity was strongly correlated with pH (r = 0.88) and Brix (r = −0.80) (p < 0.05). Sensitivity, specificity, PPV, and NPV for parturition in ≤24 h for conductivity (82%, 91%, 77%, and 92%, respectively), pH (79%, 84%, 81%, and 71%, respectively), and Brix (72%, 79%, 66%, and 83%, respectively) were determined separated and pairwise. Of interest, the sensitivity, specificity, PPV, and NPV, of combining conductivity and pH, were 80%, 95%, 90%, and 88%, respectively. Conductivity (≤4.8 mS/cm) presented the greatest odds ratio for predicting parturition in ≤24 h, and coupling it with pH (≤6.4 pH units) doubled its odds ratio (i.e., 25.4–62.3). Main limitations: Field study. Conclusion: The conductivity of MGS is a sensitive and specific method to predict parturition. This is the first large‐scale study showing that a combination of conductivity and pH is useful for predicting parturition in mares. The advancement of technology paved the way for the wearable tail sensor to be developed as livestock owners depend on analog measurements for detecting fever, estrus, and parturition. Hence, these advancements help farm and livestock management. This study aimed to design and develop a wearable tail sensor that integrates heat, motion, and pulse sensors for the early detection of livestock fever, estrus, and parturition. This study utilized a Research and Development (R&D) design employing the 4D Model of device development, which encompasses four distinct phases: define, design, develop, and disseminate. The study successfully designed and developed a device that integrates four sensors to detect fever, estrus, and parturition in livestock. The results revealed that the device has a high level of acceptability and adaptability. Statistical analysis also showed that there is no significant difference between the measurements of the analog instruments and the wearable tail sensor in terms of temperature and pulse rate for both cattle and pigs. The researchers successfully constructed a wearable device integrating four sensors to detect fever, estrus, and parturition in livestock that transmits real-time data over the web server and accurate measurements of temperature, pulse, and motion of livestock. The adoption of various practices for higher milk production from the animals is very pertinent, which generally increases peripartum disorders. In the period of transition phase (3 weeks before parturition and 3 weeks after parturition), an animal is in huge demand of energy for the developing fetus, colostrum production, and subsequent milk production for the postnatal development of the calf, and thus animals undergo a complex set of physiological and metabolic adaptations. The transition stress drastically has an impact on immunological function which leads the metabolic disorders in animals. In many experiments, it has been reported that immune dysfunction causes early lactation disorders during the period around the dryoff. The stress and dietary changes occur during the period of transition phase which may affect immunity, metabolic and physio-biochemical changes which finally affect the productive efficiency of animals. Several nutritional strategies and mental strategies have been adopted to minimize the detrimental consequences on the immune system during the transition phase. A suitable environment and a healthy balanced diet should be given to the transition cow to minimize transition phase challenges. Just after parturition, Dry matter (DM) consumption increases more slowly than milk energy output, therefore the animals cannot meet their nutritional needs only through feed consumption. During the last 3 weeks of pregnancy, nutrient demands by the fetal calf and placenta are at their greatest, and in a few animals, DM consumption may decrease by 10 to 30% as compared with consumption during the initial phase of the dry period. In such conditions, animals are exposed to negative energy balance and most of the animals attempt to make up energy balance by lipolysis of stored body fat. This leads to an increase in the non-esterified fatty acid (NEFA) in the blood and further, there is an accumulation of NEFAS in the liver causing fatty liver, which in turn stimulates cytokines. Cytokines activate receptors on different target cells leading to systemic inflammatory reactions resulting in several biochemical changes, such as elevated glucocorticoid and adrenocorticotrophic hormone production, the stimulation of the blood clotting process, decreased serum concentrations of calcium, zinc, copper, iron, and vitamin E. Ascorbic acid and vitamin E are typical antioxidant substances that break the chain of lipid radicals and transform them into more stable metabolites. Cu-Zn superoxide dismutase (SOD) and ceruloplasmin are two enzymes that copper and zinc are involved in, and these enzymes participate in the antioxidant system. In the cytosol, superoxide radicals are transformed into hydrogen peroxide by Cu-Zn SOD. Selenium aids in boosting neutrophil activity. Just before parturition, there is an increase in the level of serum cortisol and estrogen, which suppresses the immunity of animals by reducing the phagocytic index and lymphocyte proliferation in the animals. The animals become immune compromised and under such situations animals used to have different types of metabolic diseases viz, metritis, mastitis and retained placenta. The successful transition phase is of prime importance and concern to animal keepers by reducing macro-mineral deficiencies, lipid mobilization and immune suppression. The animal owner can get rid of transition problems of animals by supplementation of Vit E, Se, Cu, Zn, Chromium and 20-25% additional energy. This supplement may help animals to pass the transition phase successfully with minimal incidence of having metabolic diseases like mastitis, metritis and retained placenta. A growing body of research has documented disrespectful, abusive, and neglectful treatment of women in facilities during childbirth, as well as the drivers of such mistreatment. Yet, little research exists on effective interventions to improve Person-Centred Maternal Care (PCMC)-care that is respectful and responsive to individual women's preferences, needs, and values. We sought to extend knowledge on interventions to improve PCMC, with a focus on two factors - provider stress and implicit bias - that are driving poor PCMC and contributing to disparities in PCMC. In this paper we describe the process towards the development of the intervention. The intervention design was an iterative process informed by existing literature, behaviour change theory, formative research, and continuous feedback in consultation with key stakeholders. The intervention strategies were informed by the Social Cognitive Theory, Trauma Informed System framework, and the Ecological Perspective. This process resulted in the 'Caring for Providers to Improve Patient Experience (CPIPE)' intervention, which has 5 components: provider training, peer support, mentorship, embedded champions, and leadership engagement. The training includes didactic and interactive content on PCMC, stress, burnout, dealing with difficult situations, and bias, with some content integrated into emergency obstetric and neonatal care (EmONC) simulations to enable providers apply concepts in the context of managing an emergency. The other components create an enabling environment for ongoing individual behavior and facility culture change. The pilot study is being implemented in Migori County, Kenya. The CPIPE intervention is an innovative theory and evidence-based intervention that addresses key drivers of poor PCMC and centers the unique needs of vulnerable women as well as that of providers. This intervention will advance the evidence base for interventions to improve PCMC and has great potential to improve equity in PCMC and maternal and neonatal health.

ObjectiveTo describe the anti-hepatitis B virus (HBV) efficacy, HBeAg serologic changes, HBV perinatal transmission, and safety in pregnant women who are living with human immunodeficiency virus (HIV) and HBV co-infection who were randomized to various antiretroviral therapy (ART) regimens.MethodsThe PROMISE (Promoting Maternal and Infant Survival Everywhere) trial was a multicenter randomized trial for ART-naive pregnant women with HIV infection. Women with HIV and HBV co-infection at 14 or more weeks of gestation were randomized to one of three ART arms: one without HBV treatment (group 1) and two HBV treatment arms with single (group 2) or dual anti-HBV activity (group 3). The primary HBV outcome was HBV viral load antepartum change from baseline (enrollment) to 8 weeks; safety assessments included alanine aminotransferase (ALT) level, aspartate aminotransferase (AST) level, and anemia (hemoglobin less than 10 g/dL). Primary comparison was for the HBV-active treatment arms. Pairwise comparisons applied t test and the Fisher exact tests.ResultsOf 3,543 women, 3.9% were HBsAg-positive; 42 were randomized to group 1, 48 to group 2, and 48 to group 3. Median gestational age at enrollment was 27 weeks. Among HBV-viremic women, mean antepartum HBV viral load change at week 8 was -0.26 log 10 international units/mL in group 1, -1.86 in group 2, and -1.89 in group 3. In those who were HBeAg-positive, HBeAg loss occurred in 44.4% at delivery. Two perinatal HBV transmissions occurred in group 2. During the antepartum period, one woman (2.4%) in group 1 had grade 3 or 4 ALT or AST elevations, two women (4.2%) in group 2, and three women (6.3%) in group 3.ConclusionOver a short period of time, HBV DNA suppression was not different with one or two HBV-active agents. HbeAg loss occurred in a substantial proportion of participants. Perinatal transmission of HBV infection was low. Pelvic floor disorders, including pelvic organ prolapse and urinary and fecal incontinence, affect millions of women globally and represent a major public health concern. Pelvic floor muscle (PFM) dysfunction has been identified as one of the leading risk factors for the development of these morbid conditions. Childbirth, specifically vaginal delivery, has been recognized as the most important potentially modifiable risk factor for PFM injury; however, the precise mechanisms of PFM dysfunction after parturition remain elusive. In this study, we demonstrated that PFMs exhibit atrophy and fibrosis in parous women with symptomatic pelvic organ prolapse. These pathological alterations were recapitulated in a preclinical rat model of simulated birth injury (SBI). The transcriptional signature of PFMs after injury demonstrated an impairment in muscle anabolism, persistent expression of genes that promote extracellular matrix (ECM) deposition, and a sustained inflammatory response. We also evaluated the administration of acellular injectable skeletal muscle ECM hydrogel for the prevention of these pathological alterations. Treatment of PFMs with the ECM hydrogel either at the time of birth injury or 4 weeks after injury mitigated PFM atrophy and fibrosis. By evaluating gene expression, we demonstrated that these changes are mainly driven by the hydrogel-induced enhancement of endogenous myogenesis, ECM remodeling, and modulation of the immune response. This work furthers our understanding of PFM birth injury and demonstrates proof of concept for future investigations of proregenerative biomaterial approaches for the treatment of injured pelvic soft tissues.

BackgroundPhthalates, a group of pervasive endocrine-disrupting chemicals found in plastics and personal care products, have been associated with a wide range of developmental and health outcomes. However, their impact on biomarkers of aging has not been characterized. We tested associations between prenatal exposure to 11 phthalate metabolites on epigenetic aging in children at birth, 7, 9, and 14 years of age. We hypothesized that prenatal phthalate exposure will be associated with epigenetic age acceleration measures at birth and in early childhood, with patterns dependent on sex and timing of DNAm measurement.MethodsAmong 385 mother-child pairs from the CHAMACOS cohort, we measured DNAm at birth, 7, 9, and 14 years of age, and utilized adjusted linear regression to assess the association between prenatal phthalate exposure and Bohlin's Gestational Age Acceleration (GAA) at birth and Intrinsic Epigenetic Age Acceleration (IEAA) throughout childhood. Additionally, quantile g-computation was utilized to assess the effect of the phthalate mixture on GAA at birth and IEAA throughout childhood.ResultsWe found a negative association between prenatal di (2-ethylhexyl) phthalate (DEHP) exposure and IEAA among males at age 7 (-0.62 years; 95% CI:-1.06 to -0.18), and a marginal negative association between the whole phthalate mixture and GAA among males at birth (-1.54 days, 95% CI: -2.79 to -0.28), while most other associations were nonsignificant.ConclusionsOur results suggest that prenatal exposure to certain phthalates is associated with epigenetic aging in children. Additionally, our findings suggest that the influence of prenatal exposures on epigenetic age may only manifest during specific periods of child development, and studies relying on DNAm measurements solely from cord blood or single time points may overlook potential relationships.

BackgroundThere has been substantial progress in developing approaches to measure mistreatment of women during childbirth. However, less is known about the differences in measurement approaches. In this study, we compare measures of mistreatment obtained from the same women using labour observations and community-based surveys in Ghana, Guinea and Nigeria.MethodsExperiences of mistreatment during childbirth are person-centred quality measures. As such, we assessed individual-level and population-level accuracy of labour observation relative to women's self-report for different types of mistreatment. We calculated sensitivity, specificity, percent agreement and population-level inflation factor (IF), assessing prevalence of mistreatment in labour observation divided by 'true' prevalence in women's self-report. We report the IF degree of bias as: low (0.75Results1536 women across Ghana (n=779), Guinea (n=425) and Nigeria (n=332) were included. Most mistreatment items demonstrated better specificity than sensitivity: observation of any physical abuse (44% sensitive, 89% specific), any verbal abuse (61% sensitive, 73% specific) and presence of a labour companion (19% sensitive, 93% specific). Items for stigma (IF 0.16), pain relief requested (IF 0.38), companion present (IF 0.32) and lack of easy access to fluids (IF 0.46) showed high risk of bias, meaning labour observations would substantially underestimate true prevalence. Other items showed low or moderate bias.ConclusionUsing self-report as the reference standard, labour observations demonstrated moderate-to-high specificity (accurately identifying lack of mistreatment) but low-to-moderate sensitivity (accurately identifying presence of mistreatment) among women. For overall prevalence, either women's self-report or observations can be used with low-moderate bias for most mistreatment items. However, given the dynamicity, complexity, and limitations in 'objectivity', some experiences of mistreatment (stigma, pain relief, labour companionship, easy access to fluids) require measurement via women's self-report. More work is needed to understand how subjectivity influences how well a measure represents individual's experiences.

Monitoring equine parturition effectively is essential for preemptive intervention in periparturient issues and ensuring the overall well-being of both mares and foals. However, its implementation in breeding farms is challenging due to variable gestational lengths and nocturnal births. Predictive techniques have the potential to streamline the monitoring process, reduce labor intensity, and minimize costs. Research on foaling prediction in mares carrying mule or equine clone fetuses is scarce. Therefore, this study aimed to comparatively analyze foaling prediction parameters in mares pregnant with mule, equine, or equine clone fetus. The study included vulvar relaxation, sacroiliac ligament tension, pH, BRIX index, and concentrations of calcium, phosphorus, magnesium, sodium, and potassium in prepartum mammary secretions. Sixty pregnant mares were used for this study and grouped as follows: 25 mares with mule fetuses (MF), 20 with equine clone fetuses (CF), and 15 with equine control fetuses (EF). Results showed significant differences in vulvar relaxation and sacroiliac ligament tension only in MF group (p < 0.05) on the day of parturition compared to the other days evaluated, different from the other groups. Levels of pH notably decreased on parturition day (mean 5.7 ± 0.04, p < 0.0001), with lower values in MF (6.05 ± 0.02) and CF (6.08 ± 0.04) compared to EF (6.26 ± 0.04) (p < 0.03). The BRIX index showed variation across mares and was not a good parameter for foaling prediction. Electrolytes correlated positively with impending parturition, showing no significant differences among groups. The MF and CF groups exhibited a substantial increase (102.13 % and 110.66 %, respectively) in mean calcium concentrations on the day before foaling, unlike EF (38.29 %). In conclusion, the pH values were different in mammary secretions between mares carrying mule and clone fetuses, in contrast to equine control fetuses. Nevertheless, there was a trend of decreasing pH values closer to parturition in all groups. Conversely, the BRIX index serves as a valuable indicator of colostrum quality yet does not offer insights into the proximity of parturition. While electrolyte concentrations did not reveal significant differences among groups, it is worth noting that the evaluation of phosphorus emerges as a new parameter to explore in mares nearing parturition, since it obtained a pattern similar to calcium. • Mule had an increasing relaxation of physical parameter on the day of birth. • PH values were lower in mammary secretions of mares carrying mule and clone foals. • BRIX does not offer insights into the proximity of parturition. • Electrolyte concentrations did not reveal significant differences among pregnancies. • Phosphorus emerges as a new parameter to explore in mares nearing parturition. Introduction: One of the most common obstetrical interventions is the induction of labour. Cervical ripening drugs are typically used before conventional methods of induction since they are linked to an increased risk of protracted labour, chorioamnionitis, and Caesarean Sections (CS) during labour induction when an unfavourable cervix is present. The two major techniques for cervical ripening are mechanical interventions, such as the insertion of balloon catheters, and the application of pharmacologic agents, such as prostaglandins. It is widely accepted that the induction of labour, which ultimately leads to vaginal delivery, is largely dependent on the condition of the uterus’ cervix. Aim: To compare the efficacy of a combination of Foley bulb catheter and intracervical Dinoprostone and intracervical Dinoprostone alone for the induction of labour. Materials and Methods: A prospective observational study was performed over 150 pregnant mothers from May 2018 to April 2019. in the labour room of North Bengal Medical College and Hospital, a tertiary care institution in Darjeeling, West Bengal, India. The pregnant women were divided into two groups: intracervical Dinoprostone gel alone (Group-1) and a combination of intracervical Foley catheter and intracervical Dinoprostone gel (Group-2) with n=75 patients in each group. Group-1 received intracervical Dinoprostone gel 0.5 mg, which was repeated every 6 hours, a maximum of three doses, or until she went into active labour, whichever occurred earlier. In Group-2, a 16 F Foley catheter was inserted into the cervix, inflated, and a single dose of intracervical Dinoprostone gel 0.5 mg was given alongside it. Participant demographic characteristics, medical and pregnancy history, indication for labour induction, labour course, and outcomes were collected. The collected data were analysed with the mean and standard deviation for numerical variables and counts and percentages for categorical variables. Odds ratio was calculated where deemed relevant. Results: The induction to delivery time was considered the primary outcome and was shorter in Group-2 by 28 minutes (p=0.342). Among the secondary outcomes, the duration of the latent phase of labour was shorter in Group-2 by 57 minutes, and the proportion of patients delivering within 12 hours was also higher (28%) but statistically insignificant (p>0.05). The rates of CS were unaffected by the mode of induction. None of the two methods were found to be inferior to the other in respect to neonatal outcomes or complications of labour like chorioamnionitis and postpartum haemorrhage. Conclusion: The combination of Foley catheter with intracervical Dinoprostone gel did not prove to be more efficient than intracervical Dinoprostone alone for labour induction in the current study.

Objective Parturition is crucial for dams, their calves, and cow managers. The prediction of calving time, which assists cow managers to decide on the relocation of cows to maternity pens and necessity of human supervision, is a pivotal aspect of livestock farming. However, existing methods of predicting calving time in dairy cows based on hormonal changes and clinical symptoms are time-consuming and yield unreliable predictions. Accordingly, we investigated whether heart rate variability (HRV) which is a non-invasive assessment of autonomic nervous system (ANS) activity and behavior during the prepartum period would be useful for predicting calving time in dairy cows. Methods Eight pregnant cows were surveilled under electrocardiogram and video recordings for HRV and behavioral analyses, respectively. HRV parameters in time and frequency domains were evaluated. A 24-h time budget was calculated for each of six types of behavior (standing and lying with or without rumination, sleeping, and eating). Results Heart rate on calving day is considerably higher than those recorded on the days preceding calving. Low frequency power declined, whereas high frequency power escalated on the calving day compared to the period between 24 and 48 h before calving. The time budget for ruminating while lying decreased and that while standing increased markedly on the calving day compared to those allocated on the preceding days; nonetheless, the total time budget for ruminating did not differ during the prepartum period. Conclusion We elucidated the ANS activity and behavioral profiles during prepartum period. Our results confirm that HRV parameters and behavior are useful for predicting calving time, and interestingly indicate that the time budget for ruminating while standing (or lying) may serve as a valuable predictor of calving. Collectively, our findings lay the foundation for future investigations to determine other potential predictors and formulate an algorithm for predicting calving time.

Objective To investigate the characteristics of thyroid function and its influencing factors in preterm infants born to mothers with preeclampsia. Methods A total of 222 preterm infants, who were admitted to the neonatal intensive care unit of our hospital from January to December 2019, were enrolled, and according to the presence or absence of preeclampsia in the mother, they were divided into experimental group with 120 infants and control group with 102 infants. Basic clinical data were collected from the preterm infants and their mothers, as well as the results of thyroid function test using peripheral venous blood of preterm infants at 10-14 d after birth. The Student’s t-test, a one-way analysis of variance, and the multiple stepwise regression analysis were used to investigate the influencing factors for thyroid function in preterm infants. Results There was no significant difference in maternal thyroid function between the two groups (P>0.05). Abstract Background Despite maternal flavivirus infections’ linkage to severe maternal and fetal outcomes, surveillance during pregnancy remains limited globally. Further complicating maternal screening for these potentially teratogenic pathogens is the overwhelming subclinical nature of acute infection. This study aimed to understand perinatal and neonatal risk for poor health outcomes associated with flaviviral infection during pregnancy in El Salvador. Methods Banked serologic samples and clinical results obtained from women presenting for labor and delivery at a national referent hospital in western El Salvador March to September 2022 were used for this study. 198 samples were screened for dengue and Zika virus IgM, and statistical analyses analyzed demographic and clinical outcome associations with IgM positivity. Results This serosurvey revealed a high rate of maternal flavivirus infection—24.2% of women presenting for labor and delivery were dengue or Zika virus IgM positive, suggesting potential infection within pregnancy. Specifically, 20.2% were Zika virus IgM positive, 1.5% were dengue virus IgM positive, and 2.5% were both dengue and Zika virus IgM positive. Women whose home had received mosquito abatement assistance within the last year by the ministry of health were 70% less likely to test IgM positive (aOR = 0.30, 95%CI: 0.10, 0.83). Further, statistical geospatial clustering revealed transmission foci in six primary municipalities. Pregnancy complications and poor birth outcomes were noted among the dengue and/or Zika virus maternal infection group, although these outcomes were not statistically different than the seronegative group. None of the resulting neonates born during this study were diagnosed with congenital Zika syndrome. Conclusions The high rate of Zika virus detected among pregnant women and the lack of Zika-specific neonatal outcomes monitoring during a non-outbreak year highlights the need for continued surveillance in Central America and among immigrant mothers presenting for childbirth from these countries. As changing climatic conditions continue to expand the range of the disease vector, asymptomatic screening programs could be vital to early identification of outbreaks and clinical management of cases.

A four-year-old pregnant African lioness with dystocia was referred to the university teaching hospital. History-taking revealed the parturition had started four hours ago, but the cubs couldn't come out due to incomplete dilation of the cervix. This article describes the procedures before, during, and after the surgical operation, including the anesthesia protocol performed and performing a Cesarean section through the midline celiotomy to remove a dead and two live cubs from the uterus and successfully heal the dam without any complications. This surgery was performed about three years ago. After the operation to date, the lioness is in perfect health and she gave birth to 2 and 3 healthy cubs, respectively in two of her last parturition with no need for any help.

The process of human parturition involves inflammation at the interface where fetal chorion trophoblast cells interact with maternal decidual stromal (DS) cells and maternal immune cells in the decidua (endometrium of pregnancy). This study tested the hypothesis that inflammation at the chorion-decidua interface (CDI) induces labor by negating the capacity for progesterone (P4) to block labor and that this is mediated by inactivation of P4 in DS cells by aldo-keto reductase family 1 member C1 (AKR1C1). In human, Rhesus macaque, and mouse CDI, AKR1C1 expression increased in association with term and preterm labor. In a human DS cell line and in explant cultures of term human fetal membranes containing the CDI, the prolabor inflammatory cytokine, interleukin-1ß (IL-1ß), and media conditioned by LPS-stimulated macrophages increased AKR1C1 expression and coordinately reduced nuclear P4 levels and P4 responsiveness. Loss of P4 responsiveness was overcome by inhibition of AKR1C1 activity, inhibition of AKR1C1 expression, and bypassing AKR1C1 activity with a P4 analog that is not metabolized by AKR1C1. Increased P4 activity in response to AKR1C1 inhibition was prevented by the P4 receptor antagonist RU486. Pharmacologic inhibition of AKR1C1 activity prevented parturition in a mouse model of inflammation-induced preterm parturition. The data suggest that inflammatory stimuli at the CDI drive labor by inducing AKR1C1-mediated P4 inactivation in DS cells and that inhibiting and/or bypassing of AKR1C1-mediated P4 inactivation is a plausible therapeutic strategy to mitigate the risk of inflammation-associated preterm birth. Parturition in dogs is subjected to complex hormonal regulation, with the involvement of prostaglandin F2α (PGF2α) still not fully understood. To investigate uterine inertia (UI), the most prevalent maternal reason for dystocia in the bitch, a better understanding of undisturbed uterine, especiallymyometrial function, is crucial. Our aim was to gain deeper insights into the role of PGF2α in the canine parturient myometrium. Uterine biopsies were obtained during medically indicated cesarean sections. To test for stimulatory effects of PGF2α in vitro, circular and longitudinal myometrial layer tissue strips were challenged with 50 pM, 0.5 µM, and 50 µM PGF2α. Prostaglandin-endoperoxide synthase 2 (PTGS2) and PGF2α-receptor (PTGFR) mRNA expressions were compared between primary UI (PUI) and obstructive dystocia (OD) samples in isolated parturient myometrium. PTGFR protein expression was assessed in full thickness uterine samples. PGF2α concentrations were analyzed in canine interplacental tissue around term. In the organ bath, the contractile response to PGF2α was limited to the circular layer at the highest dosage. Correspondingly, PTGFR immunohistochemical staining was significantly stronger in the circular layer (p ≤ 0.01). PTGS2 gene expression did not differ between PUI and OD, whereas PTGFR gene expression could not be quantified. Local uterine PGF2α concentrations correlated negatively with serum P4 levels and were the highest during prepartum luteolysis while being significantly lower in PUI. Conclusively, despite the significant increase in local PGF2α concentrations at birth, confirming the interplacental tissue as a production site, our results suggest that PGF2αmight affect uterine contractility during labor, mainly indirectly. This clinical study investigates various metabolic and physiological parameters in dairy cows during puerperium. Retained fetal membranes (RFM) is a significant postpartum complication that can affect the overall health, fertility and productivity of dairy cattle. The research focuses on changes in total proteins, albumin, glucose, triglycerides, total cholesterol, aspartate aminotransferase (AST) and alanine aminotransferase (ALT), cortisol, insulin, and insulin-like growth factor 1 (IGF-1) levels among cows experiencing normal post-partum period (NP) and those with RFM. A significant increase in protein levels was noted during the post-partum period in the RFM group, indicating physiological impacts of RFM at this stage. Albumin levels showed significant differences, highlighting a significant biological effect of RFM in the post-partum period. Glucose levels varied significantly in the weeks leading to parturition, suggesting altered metabolic states in cows that suffered RFM. Triglyceride and cholesterol levels were significantly higher during the antepartum period in the group that experienced reproductive failure, indicating substantial alterations in lipid metabolism which could herald the apparition of RFM. AST and ALT levels provided insights into cellular stress and liver function, with significant increases noted around parturition which could be attributed to the substantial physiological strain of parturition itself. Cortisol levels were higher in RFM cows 2 weeks before parturition, which could indicate an increasing stress response or a physiological preparation for the upcoming labor, and may be more pronounced in cows predisposed to RFM. Insulin levels decreased significantly before and at parturition in RFM cows, indicating a strong energy deficit. IGF-1 levels decreased significantly in RFM cows after parturition. Significant changes in metabolic parameters, such as glucose, triglycerides, and cholesterol levels, delineate the pronounced metabolic challenges faced by cows with RFM. The study elucidates that while some variations are noted as parturition approaches, the most substantial impacts attributable to RFM on metabolic and physiological parameters occur after parturition. These changes may have implications for the health, recovery, and productivity of cows postpartum, suggesting the need for targeted management strategies to mitigate the effects of RFM.
