Abstract

As a preliminary approach to define the object of larger case-control studies, the distribution of potential risk factors coming from environmental pollution among patients affected by superficial bladder cancer (TCCB) has been analyzed. Material e Methods. The analysis included only patients affected by medium risk superficial TCCB. Forty Italian urological centres joined the study. Detailed information about age, sex, residency, employment, active and passive cigarette smoking, water resource, hair-dye use were centralized. All patients underwent TUR and early intravesical chemotherapy. The distribution of the above mentioned environmental factors was related to tumor characteristics such as multiplicity and previous natural history. Results. Until today 474 patients have been recruited, 182 (38.4%) with primary tumors and 293 (61.8%) with multiple lesions. Over 80% of the patients lived in urban areas, 20% were employed in industry at risk for bladder cancer and 8% used hair dye. Forty percent of the patients were smokers, with a median smoking period of 30 years. Bottled water was the only water resource in 42% of cases. Abstract

The diagnosis of prostate carcinoma has been monitored in Sicily and Calabria for one year. Material e Methods Twenty-seven urological centres of Sicily and Calabria joined the study. Detailed informations about age, familiarity, PSA, Gleason score and TNM stage at diagnosis were centralized between november 2001 and november 2002. Results 721 patients have been recruited. The median age was 73 years. More than 80% of the patients were older than 65 years. Median PSA was 14 ng/ml. The clinical stage at diagnosis was T1c in 33% of patients and 67% had an organ confined prostate cancer (T1-T2). Familiarity was detected in 8% of cases. Age and PSA at diagnosis in these patients were similar to those of the remaining population. However, only 18% of them had a T1 tumor, emphasizing the delay in diagnosis in these patients. Although only 10% of the patients shows at diagnosis a metastatic tumor, 29% only are fit for radical prostatectomy in according to age, PSA and stage.

Abstract

Introduzione e obiettivi Caratterizzare, in uno studio prospettico longitudinale su pazienti (pz) con disfunzione erettile (DE) trattati per 6 mesi con Tadalafil (Tad), efficacia, sicurezza, e grado di soddisfazione per il trattamento del pz e della partner. Materiali e metodi Furono arruolati 155 pz (età media 67 anni, range 26-75) con DE, mai trattati (naïve) nel 74% dei casi. La DE, presente da < 1 anno (37%) o da 1-5 anni (63%), era organica (59%), mista (26%), psicogena (11%), idiopatica (4%). Il numero dei FR (fumo, diabete, ipertensione, dislipidemia), presenti nel 71% dei pz, era: 1 (37%), 2 (26%), 3 (8%); le comorbilità erano: 0 (28%), 1 (39%), 2 (23%), > 2 (10%). Del piano terapeutico previsto (totale 9 m) (posologia Tad: iniziale, costante, 20 mg x 2/sett. x 6 m.; proseguimento: 20 mg x 1-2/sett. x altri 3 m. in base a risposta dei primi 6 m. e/o opzione motivata dal pz) riportiamo i dati su efficacia (IIEF5) e soddisfazione (EDITS-paziente; EDITS partner) al 3° (T3) e del 6° mese (T6). Risultati Efficacia: A T0, l’IIEF5 score (mediana 11, range 1-20): era 0-10 (DE severa) (46%), 11-16 (DE moderata) (43%), 17-20 (DE lieve) (11%). L’IIEF5 score aumentò (mediana 19, range 1-25) a T3 ed a T6 (mediana 21, range 7-24), in virtù di una risposta > 5 punti nel 50% (a T3) e nel 58% (a T6) dei pz. Soddisfazione: l’EDITS-paziente score registrò una risposta piena (score < 11) (nel 50% a T3; nel 67% a T6) con valori di mediana 10 (range 1-33) a T3 e 8 (range 3-19) a T6; risposta assente (score > 22) nel 6% (a T3) e nello 0% (a T6) dei pzt. L’EDITS-partner fece riscontrare una risposta piena (score < 5) (nel 25% a T3; nel 42% a T6) con valori di mediana 6 (range 0-18) a T3 e 5 (range 1-9) a T6; risposta assente (score > 22) nel 9% (a T3) e nello 0% (a T6) delle partner. Compliance- Sicurezza: a T3 una bassa percentuale di pzt modificò la posologia (9,7%), interruppe (10,3%) o descrisse effetti indesiderati (3%) dopo Tad; a T6, nessun pzt modificò la posologia, e solo 1 pzt interruppe Tad per effetti collaterali. Conclusioni La terapia long-term con Tad per DE ha fatto registrare efficacia di risposta (IIEF5 > 5 punti) del 50% e 58% a T3 e T6, influenzata dall’etiopatogenesi organica e dalla durata della DE, e buona compliance-sicurezza. I punteggi EDITS-pazienti e -partner sottolineano un alto grado di soddisfazione (che continua a crescere anche dopo il 3° mese, raggiungendo punteggi più alti a T6) per la terapia eseguita. Abstract

The Erectile Dysfunction Observational Study (EDOS) is a 6-months observational prospective multicentric study enrolling men with erectile dysfunction (ED) who asked, to be started on a treatment or to change a previous treatment. Aims of the study were to analyse the pattern of treatment and compare the efficacy of treatments used. Patients were enrolled during a normal hospital visit and were prescribed a treatment for ED. They were asked at baseline and after 3 and 6 months, to answer a set of questions from the International Index of Erectile Function, Erectile Dysfunction Inventory of Treatment Satisfaction (EDITS) and Short Form of the Psychological and Interpersonal Relationships Scale questionnaires (SF-PAIRS). Clinicians were free to prescribe any therapy for ED available in the market, and to change therapy at any time during the study. Out of 1 338 patients, available for analysis at 6 months, 624 (47%) changed their treatment during the study and 714 (53%) continued with the drug prescribed at baseline. Patients assuming tadalafil had a significantly higher probability of maintaining the same treatment compared to sildenafil or vardenafil. There was no clinically significant difference in terms of efficacy, patient satisfaction, self-confidence and spontaneity between the different inhibitors of PDE5. The 'time concerns' domain score of SF-PAIRS, was statistically better in patients assuming tadalafil. In conclusion sildenafil, vardenafil and tadalafil show similar efficacy in the clinical practice. However, patients receiving tadalafil display a lower risk to discontinue or change the treatment.

Abstract

The diagnosis of prostate carcinoma has been monitored in Sicily and Calabria for one year.Material e Methods Twenty-seven urological centres of Sicily and Calabria joined the study. Detailed informations about age, familiarity, PSA, Gleason score and TNM stage at diagnosis were centralized between november 2001 and november 2002.Results 721 patients have been recruited. The median age was 73 years. More than 80% of the patients were older than 65 years. Median PSA was 14 ng/ml. The clinical stage at diagnosis was T1c in 33% of patients and 67% had an organ confined prostate cancer (T1-T2). Familiarity was detected in 8% of cases. Age and PSA at diagnosis in these patients were similar to those of the remaining population. However, only 18% of them had a T1 tumor, emphasizing the delay in diagnosis in these patients. Although only 10% of the patients shows at diagnosis a metastatic tumor, 29% only are fit for radical prostatectomy in according to age, PSA and stage.

Abstract

As a preliminary approach to define the object of larger case-control studies, the distribution of potential risk factors coming from environmental pollution among patients affected by superficial bladder cancer (TCCB) has been analyzed.Material e Methods. The analysis included only patients affected by medium risk superficial TCCB. Forty Italian urological centres joined the study. Detailed information about age, sex, residency, employment, active and passive cigarette smoking, water resource, hair-dye use were centralized. All patients underwent TUR and early intravesical chemotherapy. The distribution of the above mentioned environmental factors was related to tumor characteristics such as multiplicity and previous natural history.Results. Until today 474 patients have been recruited, 182 (38.4%) with primary tumors and 293 (61.8%) with multiple lesions. Over 80% of the patients lived in urban areas, 20% were employed in industry at risk for bladder cancer and 8% used hair dye. Forty percent of the patients were smokers, with a median smoking period of 30 years. Bottled water was the only water resource in 42% of cases. At multivariate statistical analysis a significant correlation between tumor multiplicity and employment in industry (p<0.01) and between past natural history and period of cigarette smoking (p<0.05) was found. Preliminar/y, an interesting trend was detected for a correlation between water resource and bladder cancer in non-smoking patients. The municipal water system was the main water resource more frequently in non-smokers (75% vs 53%). This might imply a pathogenic role of water pollution and chlorination.

Abstract

Uric acid calculi can also be treated without surgery, with simple medical lytic therapy. After appropriate dietary adjustments and add of mineral water, the needed amount of alkali supplementation can increase pH values of the urine in order to dissolve the stones. Treatment should be prolonged to prevent stone recurrence. A case of bilateral renal uric acid stones that were successfully treated by alakalizing treatment was presented.

Abstract

About 10% of the people is the subject of an episode of kidney stones during their lifetime, about 70% of these people undergoes relapses. About 80% of the urinary stones contains calcium, of wich 80% is formed of calcium oxalate, in pure form or associated with calcium phosphate. Therefore we can saythat in most cases (about 65%) the urinary stones are composedof calcium oxalate. Use of supplements of potassium citrate and magnesium citrate can help in the prevention of kidney stones of calcium oxalate, but mostly they can be used in the days before a shockwaves lithotripsy treatment to make the stones more fragile to the effect of the shock waves. A case of successful treatment with magnesium potassium citrate of a SWL resistant ureteral stone is presented.

Abstract

Introduction: To evaluate PCA3 score accuracy in prostate cancer (PCa) diagnosis in patients undergoing repeat saturation prostate biopsy (SPBx)., Material and Methods: From January 2010 to March 2012, 177 patients (median 64 years) with primary negative extended biopsy underwent a SPBx (median 28 cores) for persistent suspicion of PCa. The indications for repeat biopsy were: PSA > 10 ng/mL, PSA values between 4.1-10 or 2.6-4 ng/mL with free/total PSA < 25% and < 20%, respectively; moreover, before performing SPBx PCA3 score was evaluated., Results: Median PSA was 9.5 ng/mL (range: 3.7-28 ng/mL): in 74 (41.8%) cases PSA was > 10 ng/mL, in 99 (56%) and 4 (2.2%) was included between 4-10 and 2.6-4 ng/mL, respectively. Median PCA3 score was equal to 52 (range 3-273); 140 (79%) and 100 (56.5%) patients had a PCA3 score greater than 20 and 35, respectively. A T1c PCa was found in 48 patients (27.1%); PCA3 score was 60 (median; range: 7-208) in the presence of PCa and 34 (median; range: 3-268) in the absence of cancer (p < 0.05). Diagnostic accuracy, sensitivity, specificity, PPV and NPV of PCA3 score cut-off of 20 vs. 35 in PCa diagnosis were 43.5 vs. 50.2%, 91.7 vs. 73%, 25.6 vs. 41.8%, 31.5 vs. 35% and 89.5 vs. 80.6%, respectively., Conclusions: PCA3 score reduce number of unnecessary repeat SPBx; using a PCA3 cut-off of 20 vs 35 would have avoided 21% vs. 37.8% of biopsies while missing 8.4% (4 cases) vs. 27% (13 cases) of significant PCa, respectively.
