Abstract

Background: Anhedonia and suicidal ideation are symptoms of major depressive disorder (MDD) that are not regularly captured in structured scales but may be captured in unstructured clinical notes. Natural language processing (NLP) techniques may be used to extract longitudinal data on suicidal behaviors and anhedonia within unstructured clinical notes. This study assessed the accuracy of using NLP techniques on electronic health records (EHRs) to identify these symptoms among patients with MDD. Methods: EHR-derived, de-identified data were used from the NeuroBlu Database (version 23R1), a longitudinal behavioral health real-world database. Mental health clinicians annotated instances of anhedonia and suicidal symptoms in clinical notes creating a ground truth. Interrater reliability (IRR) was calculated using Krippendorff's alpha. A novel transformer architecture-based NLP model was trained on clinical notes to recognize linguistic patterns and contextual cues. Each sentence was categorized into one of four labels: (1) anhedonia; (2) suicidal ideation without intent or plan; (3) suicidal ideation with intent or plan; (4) absence of suicidal ideation or anhedonia. The model was assessed using positive predictive values (PPV), negative predictive values, sensitivity, specificity, F1-score, and AUROC. Results: The model was trained, tested, and validated on 2,198, 1,247, and 1,016 distinct clinical notes, respectively. IRR was 0.80. For anhedonia, suicidal ideation with intent or plan, and suicidal ideation without intent or plan the model achieved a PPV of 0.98, 0.93, and 0.87, an F1-score of 0.98, 0.91, and 0.89 during training and a PPV of 0.99, 0.95, and 0.87 and F1-score of 0.99, 0.95, and 0.89 during validation. Conclusions: NLP techniques can leverage contextual information in EHRs to identify anhedonia and suicidal symptoms in patients with MDD. Integrating structured and unstructured data offers a comprehensive view of MDD's trajectory, helping healthcare providers deliver timely, effective interventions. Addressing current limitations will further enhance NLP models, enabling more accurate extraction of critical clinical features and supporting personalized, proactive mental health care. Scientists and engineers alike have long looked to animals in their pursuit of understanding the natural world and how best to interact with it. While researchers have looked across diverse classes, insects have been extensively studied for their rich diversity of life histories and abilities to perform at spatial and temporal scales difficult for engineered systems. Within insects, the fruit fly, Drosophila melanogaster, is a particularly well-studied organism because of its experimental tractability and status as a genetic model organism, providing both detailed descriptions of a broad suite of behaviors and access to and control over specific sets of tissue. In this work, we make use of these tools to study two behaviors in Drosophila, local search, the behavior in which walking flies will search the area around a food site in search of other food sources nearby, and the optomotor response, wherein they will stabilize in response to visual motion during flight. In these studies, we will use modern techniques from both biology and engineering, to exhaustively characterize and describe the observed behaviors and attempt to untangle the underlying algorithms and their neuronal implementation. First, we explore the algorithmic structure of local search in fruit flies. When flies encounter a piece of food, they will often perform walking searches nearby; as food tends to be patchy in natural settings, searches may allow flies to locate other food sites in the area. We induce local search using optogenetic stimulation of sugar-sensing neurons and constrain the flies to a dark, annular arena. These experimental details result in a simplified behavior, as the fly has access to a limited sensory environment, so that the search can be interpreted as an example of idiothetic path integration, and the search itself is one-dimensionalized and therefore more easily analyzed. 20. Talking about Race: Overcoming Fear in the Process of Change

Abstract

Purpose: We investigated the way in which structured, multiyear conversations about race and institutional racism occurred in suburban secondary schools with changing racial demographics. Research Framework: The study draws on interpretive research traditions, in that we assume that how teachers understand race and racism will influence how they work with colleagues and students. As such, the research examines to what extent talking about race and learning about institutional racism affects educators' mental models and their classroom practices. Method: Secondary schools in three districts that had participated in ongoing professional development related to racial equity were selected. Grounded theory methods were used for data collection, coding, and analysis of interviews with teachers and administrators. Findings: This study revealed that principal leadership affected teachers' engagement in this work. When this occurred, teachers made meaningful changes in classroom practices and their school communities. The primary findings of the study are: (1) fear of being considered racist was a barrier for White teachers and administrators that impeded collective focus on racial achievement gaps; and (2) principals' deep personal engagement over a period of several years encouraged a process of confronting and mitigating this fear, and helped teachers engage with the implications of race for school and classroom practices. Significance: Discomfort, a critical element in confronting racial inequities in schools, requires school leaders, particularly principals, to authentically participate and engage in order to foster teacher change.

24. Empowering Nursing Students: Giving Voice to the Next Generation

This poster presentation aims to answer the question, "Why is it important for nursing students to have a voice?" Despite the government's commitment to bringing 50,000 new registered nurses to the NHS by 2024 (NHS England, 2019), the annual rate of attrition from nursing courses has typically been around 24% (Nursing Standard, 2018). To understand why, the researcher surveyed 122 nursing students using four primary questions and conducted a thematic analysis to identify the key themes. The findings show that there are clear barriers to nursing students having a voice, including poor communication with the university and placement, lacking confidence to speak up, and fear of repercussions. However, having a voice is crucial as nursing students are the next generation of nurses and have a duty of care going into the profession. The findings also provide recommendations to improve the retention of a generation of student nurses, such as providing support, advocacy, active listening, passionate mentors, a student platform, and training for students and staff.

Abstract

In this paper, we present a student-developed and led program implemented in our graduate department to help students begin preparation for oral examinations on more equal footing. Despite the common use of oral qualifying (preliminary) examinations to assess students' ability to succeed and continue in engineering PhD programs, the communication skills needed to be successful in such settings are not always explicitly taught in courses students take to prepare. We give a brief overview of historical and current qualifying exam practices, discuss the benefits and potential inequities of oral qualifying exams specifically, and finally, present one resource educators may use to address the gap in students' preparedness for this particular exam format. During this department's oral qualifying exams, students entering their second year solve technical problems on the blackboard while professors engage with and evaluate not only their final answer but also their approach and problem-solving process. While in theory evaluating a student's process alongside the final answer can be more equitable, the oral exam format itself may actually amplify inequality via implied expectations that go beyond demonstrating mastery of technical concepts (the nominal goal). Indeed, to succeed in an oral examination, students must also exhibit confident communication and skill working at the board, but these skills are not currently taught, practiced, or assessed during our first-year courses. We created a progressive qualifying exam preparatory program to bridge the gap between the technical knowledge developed during the first year and the critical communication skills required to effectively demonstrate that knowledge in an oral exam format. Our program, led by graduate student facilitators, consisted of voluntary hour-long weekly sessions with components targeting board work preparation, mental well-being, and study skills. Abstract

To maintain stable flight, animals continuously perform trimming adjustments to compensate for internal and external perturbations. Whereas animals use many different sensory modalities to detect such perturbations, insects rely extensively on optic flow to modify their motor output and remain on course. We studied this behavior in the fruit fly,Drosophila melanogaster, by exploiting the optomotor response, a robust reflex in which an animal steers so as to minimize the magnitude of rotatory optic flow it perceives. Whereas the behavioral and algorithmic structure of the optomotor response has been studied in great detail, its neural implementation is not well-understood. In this paper, we present findings implicating a group of nearly homomorphic descending neurons, the DNg02s, as a core component for the optomotor response in flyingDrosophila. Prior work on these cells suggested that they regulate the mechanical power to the flight system, presumably via connections to asynchronous flight motor neurons in the ventral nerve cord. When we chronically inactivated these cells, we observed that the magnitude of the optomotor response was diminished in proportion to the number of cells silenced, suggesting that the cells also regulate bilaterally asymmetric steering responses via population coding. During an optomotor response, flies coordinate changes in wing motion with movements of their head, abdomen, and hind legs, which are also diminished when the DNg02 cells are silenced. Using two-photon functional imaging, we show that the DNg02 cells respond most strongly to patterns of horizontal motion and that neuronal activity is closely correlated to motor output. However, unilateral optogenetic activation of DNg02 neurons does not elicit the asymmetric changes in wing motion characteristic of the optomotor response to a visual stimulus, but rather generates bilaterally symmetric increases in wingbeat amplitude. Abstract

Background: The evidence suggests an increased risk of developing anxiety problems in children of anxious parents. The current study explored the feasibility and acceptability of an intervention with anxious parents of young children, to inform the possibility of further trials. Methods: Participants were recruited through primary and secondary care psychological services and social media. Participants who had a current or recent anxiety disorder and a child aged 12–47 months were included. Assessments of parental and child outcomes occurred at baseline, after the intervention (week‐2) and follow‐up (week‐8). The intervention was delivered in a small group format, in two sessions, one week apart, using videoconferencing. Results: Out of 32 participants, 30 (94%) attended the full intervention. All found the intervention acceptable and reported it as useful and relevant. There was a reduction in parental depression (MD = 2.63, 95%CI 1.01–4.26), anxiety (MD = 3.93, 95%CI 2.49–5.37) and stress (MD = 4.60, 95% CI 3.02–6.18) and increases in parenting confidence. Conclusions: The online group intervention was feasible and acceptable. There were moderate to large effects on parental mental health and no adverse effects on children (decline on outcome measures). This indicates that intervening early in parenting with anxious parents is possible and warrants further investigation to establish prevention efficacy with a larger, controlled trial.
