Abstract

This review investigates the intricate relationship between hyperparathyroidism (HPT) and thyroid carcinoma (TC), aiming to elucidate their coexistence, potential pathogenetic mechanisms, and clinical implications. A systematic search strategy, employing the MeSH terms 'Hyperparathyroidism' and 'Thyroid Carcinoma', spanned publications from 2013 to 2023 across the PubMed, Web of Science, and Scopus databases. Fifteen selected articles were analyzed. Studies unanimously confirm the notable association between primary hyperparathyroidism (PHPT) and thyroid nodules/cancer, with incidences ranging from 2.8% to 47.1%. Key findings reveal a predilection for papillary thyroid carcinoma (PTC) in this association, showcasing varying tumor characteristics and gender disparities. Lower preoperative serum parathyroid hormone (PTH) levels are a potential risk factor for thyroid cancer in PHPT patients. Diverse surgical approaches and tumor characteristics between PHPT and secondary hyperparathyroidism (SHPT) cases were noted. Moreover, this review underscores the scarcity of definitive guidelines in managing concurrent PHPT and thyroid conditions, advocating for comprehensive assessments to enhance diagnostic accuracy and refine therapeutic interventions. Rare coincidental associations, as highlighted by case reports, shed light on unique clinical scenarios. In essence, this review amalgamates evidence to deepen the understanding of the interplay between HPT and TC, emphasizing the need for further research to elucidate underlying mechanisms and guide clinical management. Abstract

Background: Minimally invasive parathyroidectomy (MIP) has gained acceptance as the preferred surgical procedure for management of primary hyperparathyroidism (pHPT). Appropriate selection of patients for a MIP is a crucial step in its utilization. The aim of the study was to evaluate the role of 18F-FCH PET/CT as second-line imaging for accurately directing MIP. Methods: This is a retrospective single-center study. Seventy-two patients with biochemical evidence of pHPT and a non-conclusive or negative first-line imaging (ultrasound and dual isotope subtraction scintigraphy) received 18F-FCH PET/CT between January 2018 and February 2020. All imaging studies were performed at our institution. Assessment of therapeutic changes and outcomes was performed. Results: of the 72 patients imaged with 18F-FCH PET/CT, 54 subsequently underwent parathyroidectomy. When considering the ability of 18F-FCH PET/CT alone to predict a uniglandular disease, the sensitivity, specificity, PPV and NPV were 92.7% (95%CI: 80.1—98.5), 46.2% (19.2–74.9), PPV 87.3% (80.5—92) and NPV 61.2% (31.4—84.5), respectively. When we combined the data provided by 18F-FCH PET/CT with the data already collected from 1st line imaging we were able to complete a minimally invasive surgery in 38 of the 41 (92%) patients with a uniglandular disease. Thirteen patients (24%) had a multiglandular disease, all of them except one underwent bilateral neck exploration based on the data collected by all imaging modalities combined. Overall, cure was achieved in 53 (98%) patients. Conclusion: 18F-FCH PET/CT, interpreted along with first-line imaging results in selected patients, can better facilitate utilization of MIS while maintaining exceptional cure rates. Abstract

Background Primary hyperparathyroidism/(PHPT) is one of the most common endocrinological conditions. Surgery remains the only curative option. We have evaluated the performance of double isotope 123I/99mTc-sestamibi parathyroid scintigraphy/(PS) with subtraction SPECT/CT in PHP for identifying uniglandular disease. Methods Ninety PHPT patients undergoing parathyroidectomy (December 2015–August 2016) were included. All patients were evaluated with neck ultrasound/(US), PS and SPECT/CT with a new protocol. Outcomes from imaging modalities were reported as: uniglandular disease/(UGD), multiglandular disease/(MGD), or negative, and were compared to post-operative diagnoses. Results Post-operatively, 72 and 18 patients had true UGD and MGD, respectively. Sensitivities and specificities of US, pinhole scintigraphy with subtraction, pinhole and SPECT/CT with subtraction, and all modalities combined were 91.7%/38.9%, 88.9%/72.2%, 93%/66.7% and 84.72%/77.78%, respectively: specificity of US + PS superior to US alone, p = 0.074. SPECT/CT enables reclassification of doubtful uptake foci. Conclusions Combination of neck US and PS with subtraction SPECT/CT offers a higher specificity for guiding towards minimally invasive parathyroidectomy.

Abstract

International audience; PURPOSE: Succinate dehydrogenase B (SDHB) associated pheochromocytomas (PHEOs) are associated with a higher risk of tumor aggressiveness and malignancy. The aim of the present study was to evaluate (1) the frequency of germline SDHB mutations in apparently sporadic patients with PHEO who undergo preoperative genetic testing and (2) the ability to predict pathogenic mutations.METHODS: From 2012 to 2016, 82 patients underwent a PHEO surgical resection. Sixteen were operated in the context of hereditary PHEO and were excluded from analysis. Among the 66 remaining cases, 48 were preoperatively screened for an SDHB mutation. In addition to imaging studies with specific radiopharmaceuticals (123I-MIBG or 18F-FDOPA) for exclusion of multifocality/metastases, 36 patients underwent 18F-FDG PET/CT.RESULTS: From the 48 genetically screened patients, genetic testing found a germline SDHB variant in two (4.2%) cases: a variant of unknown significance, exon 1, c.14T>G (p.Val5Gly), and a most likely pathogenic mutation, exon 5, c.440A>G (p.Tyr147Cys), according to in silico analysis. Structural and functional analyses of the protein predicted that p.Tyr147Cys mutant was pathogenic. Both tumors exhibited moderate 18F-FDG PET uptake with similar uptake patterns to non-SDHB mutated PHEOs. The two patients underwent total laparoscopic adrenalectomies. Of the remaining patients, 44 underwent a laparoscopic adrenalectomy, and two had an open approach. Pathological analysis of the tumors from patients bearing two germline SDHB variants revealed a typical PHEO (PASS 0 and 2). Ex-vivo analyses (metabolomics, SDHB immunohistochemistry, loss of heterozygosity analysis) allowed a reclassification of the two SDHB variants as probably non-pathogenic variants.CONCLUSIONS:This study illustrates that SDHx mutational analysis can be misleading, even if structural and functional analyses are done. Surgeons should be aware of the difficulty of classifying new SDHB variants prior to implementing SDHB mutation status into a tailored surgical management strategy of a patient.

Abstract

Background: adrenal tumor-to-liver uptake value (Tmx:Lmx) on 18F-FDG PET/CT is an accurate and reproducible PET parameter in the distinction between benign and malignant adrenal masses. The potential impact of steroid hormone secretion on 18F-FDG uptake is still debatable. The aim of this study was to evaluate this relationship. Methods: 2010-2015: 73 patients who underwent adrenalectomy for adrenocortical tumors [49 secreting/(SA) and 24 non-secreting/(NSA)] were retrospectively included in the study. Fourteen were malignant. All patients underwent hormonal evaluation, functional and anatomical imaging, Weiss scoring and Ki 67 evaluation. Results: malignant tumors exhibit higher SUVmax than benign tumors (median 7.75 vs 3.06 respectively, p < 0.001) and Tmx:Lmx was 2.7 vs 1.17 for benign tumors, p < 0.001. Tmx:Lmx was positively correlated to Weiss score (p < 0.001). No significant difference was observed for Tmx:Lmx between SA and NSA overall (p = 0.851), regardless of the subgroup of tumors analyzed. Tmx:Lmx was not correlated to tumor size (p < 0.508) or 24 h free urinary cortisol level (p < 0.522). Conclusions: no correlation was observed between Tmx:Lmx and hormonal status, however the correlation between ratio, malignancy and Weiss score confirm the utility of 18F-FDG PET/CT for the differentiation of benign from malignant adrenal lesions, irrespective of the hormone secretory status of the tumor. 18F-FDG PET/CT is a useful biomarker in the diagnosis of adrenal tumors, regardless of the secretion status. 40. Galectin-3 immunodetection for improving the performance of conventional thyroid cytology: preliminay results

Abstract

Introduction: Most of differentiated thyroid carcinomas should be easily revealed by fine-needle aspiration cytology but sometimes preoperative diagnosis is doubtful. Galactin-3 is a carbohydrate –binding protein with affinity for beta-galactosides that plays a significant role in cell-cell and cell matrix adhesion, growth, neoplastic transformation and metastasis. Moreover, galectin-3 immunodetection is related to follicular thyroid malignancies. Our aim is to test Galectin-3 as potential preoperative marker for follicular and papillary carcinomas. Methods: Expression of galectin -3 was tested by fluorescence staining on 40 fresh cytological thyroid samples obtained preoperatively; the samples were fixed, stained using gal-3-specific monoclonal antibodies and treated with Rhodamine Red-conjugated anti-mouse. Counterstaining was performed using Hoechst 33342. After thyroidectomy, we analyzed correlation between gal-3-expression in FNAB and histology. Results : In our series, definitive diagnoses were: 17 multinodular goiter (A), 13 benign follicular lesions (B), 9 papillary carcinomas (C), 1 follicular carcinoma (D). Group A had shown at the FNAB a gal-3-positivity of cells from 0% to 15% (median: 10%); group B from 2% to 95 (median: 15%); group C from 30% to 90% (median: 46%). D had a 100% positivity. Conclusions: Galectin-3 immunodetection could be an useful method to improve conventional cytology’s performance in pre-operative identification of thyroid differentiated carcinomas: one “suspected” FNAB case showing 3% of positive cells proved to be benign at histology ; one “indefinite” FNAB case with 40% of positive cells proved to be malign. Stronger data are needed to establish a cut-off for malignancy.

Abstract

Introduction: In spite of a low incidence of complications during thyroidectomy, there is a widespread tendency to extend hospitalization at least until postoperative second day. The need of a rebalancing between costs and profits had led us to revise the hospitalization’s duration. Methods: We reviewed our early experience (129 patients candidated to total thyroidectomy as ordinary hospitalization from 2001 to 2002) identifying the causes of hospitalization’s delay both preoperatively and in discharge. We excluded from our analysis re-operations, substernal goitre requiring sternotomy, carcinomas needing a systematic lymphadenectomy and some cases of internal medical interest. Between June 2003 and June 2007, 708 patients meeting the same criteria were referred to a short hospitalization (one-day surgery) after their expressed consent; during surgical operation hemostasis was improved with special remedy in 545 cases; after thyroidectomy all patients were subjected to a empirical protocol to prevent hypocalcaemia. Results: Between 2003 and 2007 the average of postoperative hospitalization was 1.08 days; the causes of extension were: severe/symptomatic hypocalcaemia (45 patients), re-operation because of hematoma (1 case), one tracheotomy for transient bilateral paralysis of recurrent nerve; 2 patients were readmitted for local sepsis. This procedure has been successful in 92.8% of cases. 701 patients properly interrogated have declared to be satisfied. Conclusions: A rational management of patients to undergo a total thyroidectomy with a careful emphasis on prevention of hypocalcaemia makes sure the short stay. The advantages in terms of economics are obvious. Additional benefits seem to come from the introduction of “mini invasive” techniques.

Abstract

INTRODUCTION: myxoid liposarcoma approximately constitutes 20% of all mesenchymal malignancies preferably occurring in adults with prevalent localization in arts even if there are rare intra-abdominal locations: only 3 cases reported in the literature. We present a case of myxoid liposarcoma with colic localization outlining a diagnostic and therapeutic procedure through a literature’s review. CASE REPORT: We observed a 37 year-old man in January 2007 because of colic pain in the middle abdomen, dyspepsia and constipation. The CT with contrast agent showed a capsulated solid mass with net margins, lobules and adipose tissue component compressing and displacing small bowel. After incision we found a polypoid mass of 24 cm with extraluminal development arising from right colon. Total mass, terminal ileum and right colon were excised; an ileo-colic anastomosis was performed. According to the literature it was not practised any adjuvant treatment. After one year the patient presented a local peritoneal recurrences surgically excised. CONCLUSIONS: the diagnostic process is not yet well defined. Only surgery and histologic examination are able to clarify lesion’s nature. Also surgical treatment is not yet well established, although many authors suggest a total excision of lesion without reference to lymphadenectomy. As for the role of chemotherapy only one case was reported in the literature; myxoid liposarcoma is considered an histotype with intermediate degree of malignancy therefore it should be considered a potential malignant neoplasm and chemotherapy would be justified. Total surgical resection with or without adjuvant radio-chemiotherapy could have a strong impact on the prognosis and follow-up.
