1. Open Resources for Biology Education (ORBE): A Resource Collection

Abstract

In undergraduate life sciences education, open educational resources (OERs) increase accessibility and retention for students, reduce costs, and save instructors time and effort. Despite increasing awareness and utilization of these resources, OERs are not centrally located, and many undergraduate instructors describe challenges in locating relevant materials for use in their classes. To address this challenge, we have designed a resource collection (referred to as Open Resources for Biology Education, ORBE) with 89 unique resources that are primarily relevant to undergraduate life sciences education. To identify the resources in ORBE, we asked undergraduate life sciences instructors to list what OERs they use in their teaching and curated their responses. Here, we summarize the contents of the ORBE and describe how educators can use this resource as a tool to identify suitable materials to use in their classroom context. By highlighting the breadth of unique resources openly available for undergraduate biology education, we intend for the ORBE to increase instructors' awareness and use of OERs.

2. Elementary Teachers' Self-Efficacy and Its Role in STEM Implementation

Abstract

To equip students with 21st-century skills, teachers must have both deep STEM content knowledge and the confidence to implement and teach appropriate STEM content. Many elementary teachers have inadequate STEM background knowledge, low confidence, and STEM self-efficacy for implementing STEM in the classroom; as a result, teachers' classroom practices are affected. The study examined how elementary teachers perceive their ability to implement STEM in the classroom. The STEM Efficacy Survey was sent to a randomized pool of 100 elementary educators, and 18 of them agreed to participate in the study. This instrument was designed to elicit responses related to the teachers' previous background in STEM, their beliefs about their ability to implement STEM, and their actual STEM implementation in the elementary classroom. The results revealed that participants were confident in their understanding of the engineering design process and problem-based learning. However, teachers were unwilling to apply the engineering design process in the classroom. From this research, the researchers concluded that higher levels of training in STEM education may influence how teachers perceive their ability to implement STEM in the classroom. Further research should focus on exploring how STEM training affects teachers' self-efficacy in STEM implementation.

Abstract

Intermediate mass black holes (10^2 < M_BH< 10^5 Msun) are an open question in our understanding of black hole evolution and growth. They have long been linked to dense star cluster environments thanks to cluster dynamics, but there are a limited number of secure detections. We leverage existing X-ray observations from Chandra X-ray Observatory and optical catalogs from Hubble Space Telescope with new radio observations from the Karl G. Jansky Very Large Array to search for any evidence of accreting black holes in young massive clusters in the nearby galaxy M51. We find that of 43 bright ($L_X > 10^{38}$ erg/s) X-ray point sources in M51, 24 had probable matches to objects including possible associated star clusters in the HST Legacy Extragalactic UV Survey catalog, seven of which were classified as contaminants (background galaxies or foreground stars). We explore the optical properties of the remaining 17 sources, including cluster age and mass estimates, and search for radio counterparts in the 8-12 GHz band. The lack of radio counterparts to X-ray sources we know to be associated with young massive clusters in M51 suggests that we do not significantly detect hard-state IMBHs ~ 10^4 Msun or above. However, more sensitive radio facilities like the Square Kilometre Array and next generation Very Large Array may be able to provide evidence for IMBHs with masses down to ~ 10^3 Msun., Comment: 18 pages, 7 figures, accepted to ApJ

4. Connecting Compassion: Empathy's Role in STEM and Literacy Integration

Abstract

Empathy, an unsung, yet critical element of learning, can be strategically interlaced with literacy instruction and engineering design to create a rich and authentic learning experience for students. Integrated STEM education rests on the promise of engaging students and providing deep understandings through the intentional practice of delivering science and math content through the application of technology and engineering skills. Using children's literature to activate empathy and design thinking can help students become better problem-solvers, critical thinkers, and caring members of society. Empathy facilitates a deeper connection to the human experience, ensuring that solutions not only meet the design requirements, but also address the emotions and concerns of the end user. Building upon story grammar, students can understand the needs of characters in narratives and create empathetic solutions to the challenges that characters in a book may face, ultimately helping students develop confidence and embrace their future possibilities.

5. Exploring First-Year Engineering Student Perceptions of Peer-Led Study Groups in a Global South Context

Abstract

The purpose of the study is to explore the advantages of study groups for first-year engineering students in a challenged Global South context. The research aims to explore how the support and resources provided by the peer-led study groups can help students cope with the stressors and challenges that are often associated with the transition to university and the demands of an engineering programme. Educational institutions prioritise student development, yet many student initiatives overlook student agency and that understanding students' learning approaches is necessary for effective support initiatives. A total of 40 registered engineering students, who voluntarily enrolled in the peer-led study groups, were the subjects of this study at the University of Pretoria. The research adopted a qualitative approach and aimed to explore the advantages of study groups. Data were collected through interviews and surveys with both students and study group leaders to gain their respective views on the strengths and areas for improvement of the learning community experience. The research was conducted using qualitative methods to gain a deeper understanding of students' experiences with study groups and the associated benefits. Through participation in study groups, students benefited from the support of their peers, opportunities for alternative problem-solving methods, and improved academic performance. Furthermore, the smaller group size and collaborative nature of these groups created a supportive and empowering learning environment, where students felt confident to ask questions and engage in meaningful learning opportunities.

6. Utility of smart watch in expediting diagnosis of cold drink-triggered atrial fibrillation: a case report.

Abstract

BACKGROUND: Patients presenting to the emergency department with recent palpitations are a diagnostic challenge when the arrhythmia and its symptoms have resolved prior to arrival. Newer smart watch technology, adept at detecting atrial fibrillation, can assist in the diagnostic evaluation of transitory palpitations. We report a case of cold drink-triggered atrial fibrillation whose diagnosis would not have been possible without the assistance of the patients smart watch. CASE PRESENTATION: A middle-aged man without cardiac history developed sudden rapid, irregular palpitations with shortness of breath while drinking a glass of cold juice. He activated his smart watch with 1-lead electrocardiography technology which detected rapid atrial fibrillation. He sought medical care, but while waiting, his symptoms-then 90 min in duration-spontaneously resolved. His initial diagnostic evaluation documented only sinus rhythm, as did several follow-up evaluations with cardiology the next several weeks. Had it not been for his smart watch, the etiology of his transitory palpitations would have remained undiagnosed. His physicians encouraged trigger avoidance. In the following months, he avoided rapid ingestion of cold drink, taking instead small sips. The atrial fibrillation has not recurred. CONCLUSIONS: The case illustrates the valuable contribution of smart watch technology in the diagnostic evaluation of patients with short-lived palpitations. The case also educates clinicians about cold drink and food as a trigger of paroxysmal atrial fibrillation. This trigger, like alcohol, can be modified. Cold drink trigger avoidance has been reported by patients to reduce atrial fibrillation recurrence and is a low-risk, cost-effective strategy that is often successful.

Abstract

During human development, a temporary organ is formed, the placenta, which invades the uterine wall to support nutrient, oxygen, and waste exchange between the mother and fetus until birth. Most of the human placenta is formed by a syncytial villous structure lined by syncytialized trophoblasts, a specialized cell type that forms via cell-cell fusion of underlying progenitor cells. Genetic and functional studies have characterized the membrane protein fusogens Syncytin-1 and Syncytin-2, both of which are necessary and sufficient for human trophoblast cell-cell fusion. However, identification and characterization of upstream transcriptional regulators regulating their expression have been limited. Here, using CRISPR knockout in an in vitro cellular model of syncytiotrophoblast development (BeWo cells), we found that the transcription factor TFEB, mainly known as a regulator of autophagy and lysosomal biogenesis, is required for cell-cell fusion of syncytiotrophoblasts. TFEB translocates to the nucleus, exhibits increased chromatin interactions, and directly binds the Syncytin-1 and Syncytin-2 promoters to control their expression during differentiation. Although TFEB appears to play a critical role in syncytiotrophoblast differentiation, ablation of TFEB largely does not affect lysosomal gene expression or lysosomal biogenesis in differentiating BeWo cells, suggesting a previously uncharacterized role for TFEB in controlling the expression of human syncytins.

8. Expanding outpatient management of low-risk pulmonary embolism to the pregnant population: a case series.

Abstract

BACKGROUND: Outpatient treatment of pregnant patients with acute pulmonary embolism (PE) is recommended by some obstetric and haematology societies but has not been described in the literature. Little is known about patient selection and clinical outcomes. CASE SUMMARY: We report two cases of pregnant patients diagnosed with acute PE. The first, at 9 weeks of gestational age, presented to the emergency department with 12 h of pleuritic chest pain and was diagnosed with segmental PE. She was normotensive and tachycardic without evidence of right ventricular dysfunction. She received multispecialty evaluation, was deemed suitable for outpatient management, and, after 12 h of monitoring, was discharged home on enoxaparin with close follow-up. The second case, at 30 weeks of gestational age, presented to obstetrics clinic with 3 days of dyspnoea. Vital signs were normal except for tachycardia. She was referred to labour and delivery, where she was diagnosed with segmental PE. Her vital signs were stable, and she had no evidence of right ventricular dysfunction. After 6 h of monitoring, she was discharged home on enoxaparin with close follow-up. Neither patient developed antenatal complications from their PE or its treatment. DISCUSSION: This case series is the first to our knowledge to describe patient and treatment characteristics of pregnant patients with acute PE cared for as outpatients. We propose a definition for this phenomenon and discuss the benefits of and provisional selection criteria for outpatient PE management, while engaging with professional society guidelines and the literature. This understudied practice warrants further research.

Abstract

BACKGROUND AND AIMS: Home treatment is considered safe in acute pulmonary embolism (PE) patients selected by a validated triage tool (e.g. simplified PE severity index score or Hestia rule), but there is uncertainty regarding the applicability in underrepresented subgroups. The aim was to evaluate the safety of home treatment by performing an individual patient-level data meta-analysis. METHODS: Ten prospective cohort studies or randomized controlled trials were identified in a systematic search, totalling 2694 PE patients treated at home (discharged within 24 h) and identified by a predefined triage tool. The 14- and 30-day incidences of all-cause mortality and adverse events (combined endpoint of recurrent venous thromboembolism, major bleeding, and/or all-cause mortality) were evaluated. The relative risk (RR) for 14- and 30-day mortalities and adverse events is calculated in subgroups using a random effects model. RESULTS: The 14- and 30-day mortalities were 0.11% [95% confidence interval (CI) 0.0-0.24, I2 = 0) and 0.30% (95% CI 0.09-0.51, I2 = 0). The 14- and 30-day incidences of adverse events were 0.56% (95% CI 0.28-0.84, I2 = 0) and 1.2% (95% CI 0.79-1.6, I2 = 0). Cancer was associated with increased 30-day mortality [RR 4.9; 95% prediction interval (PI) 2.7-9.1; I2 = 0]. Pre-existing cardiopulmonary disease, abnormal troponin, and abnormal (N-terminal pro-)B-type natriuretic peptide [(NT-pro)BNP] at presentation were associated with an increased incidence of 14-day adverse events [RR 3.5 (95% PI 1.5-7.9, I2 = 0), 2.5 (95% PI 1.3-4.9, I2 = 0), and 3.9 (95% PI 1.6-9.8, I2 = 0), respectively], but not mortality. At 30 days, cancer, abnormal troponin, and abnormal (NT-pro)BNP were associated with an increased incidence of adverse events [RR 2.7 (95% PI 1.4-5.2, I2 = 0), 2.9 (95% PI 1.5-5.7, I2 = 0), and 3.3 (95% PI 1.6-7.1, I2 = 0), respectively]. CONCLUSIONS: The incidence of adverse events in home-treated PE patients, selected by a validated triage tool, was very low. Patients with cancer had a three- to five-fold higher incidence of adverse events and death. Patients with increased troponin or (NT-pro)BNP had a three-fold higher risk of adverse events, driven by recurrent venous thromboembolism and bleeding.

10. Scalable, Trustworthy Generative Model for Virtual Multi-Staining from H&E Whole Slide Images

Abstract

Chemical staining methods are dependable but require extensive time, expensive chemicals, and raise environmental concerns. These challenges highlight the need for alternative solutions like virtual staining, which accelerates the diagnostic process and enhances stain application flexibility. Generative AI technologies are pivotal in addressing these issues. However, the high-stakes nature of healthcare decisions, especially in computational pathology, complicates the adoption of these tools due to their opaque processes. Our work introduces the use of generative AI for virtual staining, aiming to enhance performance, trustworthiness, scalability, and adaptability in computational pathology. The methodology centers on a singular H&E encoder supporting multiple stain decoders. This design focuses on critical regions in the latent space of H&E, enabling precise synthetic stain generation. Our method, tested to generate 8 different stains from a single H&E slide, offers scalability by loading only necessary model components during production. We integrate label-free knowledge in training, using loss functions and regularization to minimize artifacts, thus improving paired/unpaired virtual staining accuracy. To build trust, we use real-time self-inspection with discriminators for each stain type, providing pathologists with confidence heat-maps. Automatic quality checks on new H&E slides ensure conformity to the trained distribution, ensuring accurate synthetic stains. Recognizing pathologists' challenges with new technologies, we have developed an open-source, cloud-based system, that allows easy virtual staining of H&E slides through a browser, addressing hardware/software issues and facilitating real-time user feedback. We also curated a novel dataset of 8 paired H&E/stains related to pediatric Crohn's disease, comprising 480 WSIs to further stimulate computational pathology research.

Abstract

Background: The Global Anticoagulant Registry in the FIELD–Atrial Fibrillation (GARFIELD-AF) is an ongoing prospective noninterventional registry, which is providing important information on the baseline characteristics, treatment patterns, and 1-year outcomes in patients with newly diagnosed non-valvular atrial fibrillation (NVAF). This report describes data from Indian patients recruited in this registry. Methods and results: A total of 52,014 patients with newly diagnosed AF were enrolled globally; of these, 1388 patients were recruited from 26 sites within India (2012–2016). Abstract

Employee well-being is a critical concern in the contemporary workplace, as highlighted by the American Psychological Association's 2021 report, indicating that 71% of employees experience stress or tension. This stress contributes significantly to workplace attrition and absenteeism, with 61% of attrition and 16% of sick days attributed to poor mental health. A major challenge for employers is that employees often remain unaware of their mental health issues until they reach a crisis point, resulting in limited utilization of corporate well-being benefits. This research addresses this challenge by presenting a groundbreaking stress detection algorithm that provides real-time support preemptively. Leveraging automated chatbot technology, the algorithm objectively measures mental health levels by analyzing chat conversations, offering personalized treatment suggestions in real-time based on linguistic biomarkers. The study explores the feasibility of integrating these innovations into practical learning applications within real-world contexts and introduces a chatbot-style system integrated into the broader employee experience platform. This platform, encompassing various features, aims to enhance overall employee well-being, detect stress in real time, and proactively engage with individuals to improve support effectiveness, demonstrating a 22% increase when assistance is provided early. 19. Edge-sharing quasi-one-dimensional cuprate fragments in optimally substituted Cu/Pb apatite

Abstract

The flurry of theoretical and experimental studies following the report of room-temperature superconductivity at ambient pressure in Cu-substituted lead apatite Cu$_x$Pb$_{10-x}$(PO$_4$)$_6$O (`LK99') have explored whether and how this system might host strongly correlated physics including superconductivity. While first-principles calculations at low doping ($x\approx1$) have indicated a Cu-$d^{9}$ configuration coordinated with oxygen giving rise to isolated, correlated bands, its other structural, electronic, and magnetic properties diverge significantly from those of other known cuprate systems. Here we find that higher densities of ordered Cu substitutions can result in the formation of contiguous edge-sharing Cu-O chains, akin to those found in some members of the cuprate superconductor family. Interestingly, while such quasi-one-dimensional edge-sharing chains are typically ferromagnetically coupled along the chain, we find an antiferromagnetic ground-state magnetic order for our cuprate fragments which is in proximity to a ferromagnetic quantum critical point. This is a result of the elongated Cu-Cu distance in Cu-substituted apatite that leads to larger Cu-O-Cu angles supporting antiferromagnetism, which we demonstrate to be controllable by strain. Finally, our electronic structure calculations confirm the low-dimensional nature of the system and show that the bandwidth is driven by the Cu-O plaquette connectivity, resulting in an intermediate correlated regime., Comment: 6 pages plus Supplemental. 22. Teaching End-of-Life Care in Speech-Language Pathology: A Tutorial

Abstract

Speech-language pathologists often lack preparation for working with patients near the end of life (EoL). Few academic training programs offer dedicated or sufficient content in the area of end-of-life care (EoLC). Furthermore, traditional knowledge-focused outcomes are not the most effective pedagogical strategy in teaching EoL and palliative care to students. Instead, a focus on self-awareness and personal beliefs in relation to death and dying have been shown to have a positive effect on attitudes toward EoLC. The purposes of this paper are three-fold: First, to explore what is known about the teaching and training of EoL in the discipline of speech-language pathology; second, to explore successful pedagogical strategies being used to teach this content; and third, to provide evidence-based suggestions for the inclusion of this content into speech-language pathology curriculum.

23. U.S. Attending Anesthesiologist Burnout in the Postpandemic Era.

Abstract

BACKGROUND: Anesthesiologists are experiencing unprecedented levels of workplace stress and staffing shortages. This analysis aims to assess how U.S. attending anesthesiologist burnout changed since the onset of the COVID-19 pandemic and target well-being efforts. METHODS: The authors surveyed the American Society of Anesthesiologists U.S. attending anesthesiologist members in November 2022. Burnout was assessed using the Maslach Burnout Inventory Human Services Survey with additional questions relating to workplace and demographic factors. Burnout was categorized as high risk for burnout (exhibiting emotional exhaustion and/or depersonalization) or burnout syndrome (demonstrating all three burnout dimensions concurrently). The association of burnout with U.S. attending anesthesiologist retention plans was analyzed, and associated factors were identified. RESULTS: Of 24,680 individuals contacted, 2,698 (10.9%) completed the survey, with 67.7% (1,827 of 2,698) at high risk for burnout and 18.9% (510 of 2,698) with burnout syndrome. Most (78.4%, n = 2,115) respondents have experienced recent staffing shortages, and many (36.0%, n = 970) were likely to leave their job within the next 2 yr. Those likely to leave their job in the next 2 yr had higher prevalence of high risk for burnout (78.5% [760 of 970] vs. 55.7% [651 of 1,169], P < 0.001) and burnout syndrome (24.3% [236 of 970] vs. 13.3% [156 of 1,169], P < 0.001) compared to those unlikely to leave. On multivariable analysis, perceived lack of support at work (odds ratio, 9.2; 95% CI, 7.0 to 12.1), and staffing shortages (odds ratio, 1.96; 95% CI, 1.57 to 2.43) were most strongly associated with high risk for burnout. Perceived lack of support at work (odds ratio, 6.3; 95% CI, 3.81 to 10.4) was the factor most strongly associated with burnout syndrome. CONCLUSIONS: Burnout is more prevalent in anesthesiology since early 2020, with workplace factors of perceived support and staffing being the predominant associated variables. Interventions focused on the drivers of burnout are needed to improve well-being among U.S. attending anesthesiologists.

OBJECTIVE: Most outpatients with pulmonary embolism (PE) are diagnosed in the emergency department (ED). The relationship between means of arrival, site of diagnosis, and disposition in ED patients with PE is unknown. We compared discharge home between patients arriving by emergency medical services (EMS) and those arriving by other means. Within the EMS cohort, we compared those with a recent PE diagnosis in the outpatient clinic setting to those who were diagnosed with PE in the ED. METHODS: This study was a secondary analysis of a retrospective cohort that included all adult, non-pregnant ED patients treated for acute PE across 21 community EDs from January 2013 to April 2015. The primary outcome was discharge home within 24 h of ED registration; we also examined mortality. We described associations with patient arrival method and other patient characteristics. RESULTS: Among 2996 ED patient encounters with acute PE, 644 (21.5%) arrived by EMS. Abstract

Hexagonal boron nitride (hBN) is a highly selective catalyst for the oxidative dehydrogenation of propane (ODHP) to propylene. Using a variety of ex situ characterization techniques, the activity of the catalyst has been attributed to the formation of an amorphous boron oxyhydroxide surface layer. The ODHP reaction mechanism proceeds via a combination of surface mediated and gas phase propagated radical reactions with the relative importance of both depending on the surface-to-void-volume ratio. Here we demonstrate the unique capability of operando X-ray Raman spectroscopy (XRS) to investigate the oxyfunctionalization of the catalyst under reaction conditions (1 mm outer diameter reactor, 500 to 550 °C, P = 30 kPa C3H8, 15 kPa O2, 56 kPa He). We probe the effect of a water cofeed on the surface of the activated catalyst and find that water removes boron oxyhydroxide from the surface, resulting in a lower reaction rate when the surface reaction dominates and an enhanced reaction rate when the gas phase contribution dominates. Computational description of the surface transformations at an atomic-level combined with high precision XRS spectra simulations with the OCEAN code rationalize the experimental observations. This work establishes XRS as a powerful technique for the investigation of light element-containing catalysts under working conditions.

Abstract

Using well-calibrated experimental data we validate theoretical X-ray absorption spectroscopy (XAS) as well as X-ray emission spectroscopy (XES) calculations for titanium (Ti), titanium oxide (TiO), and titanium dioxide (TiO$_2$) at the Ti K- and L-edges as well as O K-edge. XAS and XES in combination with a multi-edge approach offer a detailed insight into the electronic structure of materials since both the occupied and unoccupied states, are probed. The experimental results are compared with ab initio calculations from the OCEAN package which uses the Bethe-Salpeter equation (BSE) approach. Using the same set of input parameters for each compound for calculations at different edges, the transferability of the OCEAN calculations across different spectroscopy methods and energy ranges is validated. Thus, the broad applicability for analysing and interpreting the electronic structure of materials with the OCEAN package is shown.

Abstract

Flare frequency distributions represent a key approach to addressing one of the largest problems in solar and stellar physics: determining the mechanism that counter-intuitively heats coronae to temperatures that are orders of magnitude hotter than the corresponding photospheres. It is widely accepted that the magnetic field is responsible for the heating, but there are two competing mechanisms that could explain it: nanoflares or Alfv\'en waves. To date, neither can be directly observed. Nanoflares are, by definition, extremely small, but their aggregate energy release could represent a substantial heating mechanism, presuming they are sufficiently abundant. One way to test this presumption is via the flare frequency distribution, which describes how often flares of various energies occur. If the slope of the power law fitting the flare frequency distribution is above a critical threshold, $\alpha=2$ as established in prior literature, then there should be a sufficient abundance of nanoflares to explain coronal heating. We performed $>$600 case studies of solar flares, made possible by an unprecedented number of data analysts via three semesters of an undergraduate physics laboratory course. This allowed us to include two crucial, but nontrivial, analysis methods: pre-flare baseline subtraction and computation of the flare energy, which requires determining flare start and stop times. We aggregated the results of these analyses into a statistical study to determine that $\alpha = 1.63 \pm 0.03$. 29. How I Make It Through: An Exploratory Study of the Experiences of Black High School Males in a Predominantly White Community

Abstract

This dissertation utilizes a basic qualitative exploratory approach to examine the schooling experiences of eight Black high school males in a predominantly White community. It seeks to understand how they define and make sense of their own mental health and well-being through a theoretical framework of Mental Health and Well-being: A Socio-Ecological Model (Michaels et al., 2022). The findings of this dissertation revealed that the schooling experiences of Black males is multifaceted, with both positive and negative aspects that influence their understanding and management of their mental health and well-being. Despite this complexity, participants generally expressed a positive overall mental health, attributing this to a range of coping skills they would employ when faced with challenging situations. While peers and school staff were perceived as sources of support in navigating challenges, they were also identified as contributors to issues related to school belonging, safety, and racial identity. Participants discussed the pressures of acceptance while also navigating instances of racism and microaggressions from their peers and school staff. However, participants were able to acknowledge that their involvement with extracurricular activities and the support provided by their school regarding mental health and well-being, had a positive impact, mitigating some of the challenges faced by Black males. The implications of this study are relevant for school officials, administrators, teachers, researchers, and families, providing insights into how to better support Black males attending schools in predominantly White communities. 30. Systematic Review of Sport Coaches' and Teachers' Perceptions and Application of Game-Based and Constraints-Led Pedagogy: A Qualitative Meta-Study

Abstract

This study represents the first comprehensive qualitative systematic review on sport coaches' and teachers' perceptions and application of Game-Based Approaches (GBA) and Constraints-Led Approach (CLA). From searching 12 electronic academic databases from 1982 to 2020, 29 studies met the eligibility criteria and were included in the meta-study. Data revealed studies were conducted in several different countries, and case study design was the predominant methodology. The main data collection method reported was cross-sectional interviews. There were inconsistencies in the quality of reporting methodologies, sampling procedures, data analysis, and assessing quality. The meta-theory analysis identified weaknesses in the methodological and conceptual approaches, and a low number of studies stated philosophical perspectives. The meta-synthesis identified assumptions about learning, pedagogical knowledge and skills, content knowledge, and support as the overarching themes reported to impact coaches' and teachers' perceptions and application of GBAs and CLA. Finally, some recommendations for future research and practice are provided.

Abstract

Introduction There is considerable variation in the approach to infants presenting to the emergency department (ED) with fever. The authors primary aim was to develop a robust set of algorithms using community ED data to inform modifications of broader clinical guidance. Methods The authors report the development of California Febrile Infant Risk Stratification Tool (CA FIRST) using key components of the Roseville Protocol (ROS) and American Academy of Pediatrics (AAP) Clinical Practice Guideline (CPG). Expanded guidance was derived using a retrospective analysis of a cohort of 3527 febrile infants aged 7-90 days presenting to any Kaiser Permanente Northern California ED between 2010 and 2019 who underwent a core febrile infant evaluation. Results Melding ROS and AAP CPG algorithms in infants 7-60 days old, CA FIRST Algorithms had comparable performance characteristics to ROS and AAP CPG. CA FIRST enhancements included guidance on febrile infants 61-90 days old, high-risk infants, infants with bronchiolitis, and infants who received immunizations within the prior 48 hours. This retrospective analysis revealed that of 235 febrile infants 22-90 days old with respiratory syncytial virus and 221 who had fever in the 48 hours following vaccination, there were no cases of invasive bacterial infection. Discussion CA FIRST is a set of 13 algorithms providing a thoughtful and flexible approach to the febrile infant while minimizing unnecessary interventions. Conclusions CA FIRST Algorithms empower clinicians to manage most febrile infants. Algorithms are being modified as new data become available, imparting useful and ever-current educational information within a learning health care system.

STUDY OBJECTIVE: Some patients with acute pulmonary embolism (PE) will suffer adverse clinical outcomes despite being low risk by clinical decision rules. Emergency physician decisionmaking processes regarding which low-risk patients require hospitalization are unclear. Higher heart rate (HR) or embolic burden may increase short-term mortality risk, and we hypothesized that these variables would be associated with an increased likelihood of hospitalization for patients designated as low risk by the PE Severity Index. METHODS: This was a retrospective cohort study of 461 adult emergency department (ED) patients with a PE Severity Index score of fewer than 86 points. Primary exposures were the highest observed ED HR, most proximal embolus location (proximal vs distal), and embolism laterality (bilateral vs unilateral PE). The primary outcome was hospitalization. Abstract

X-ray absorption spectroscopy (XAS) is an element-specific materials characterization technique that is sensitive to structural and electronic properties. First-principles simulated XAS has been widely used as a powerful tool to interpret experimental spectra and draw physical insights. Recently, there has also been growing interest in building computational XAS databases to enable data analytics and machine learning applications. However, there are non-trivial differences among commonly used XAS simulation codes, both in underlying theoretical formalism and in technical implementation. Reliable and reproducible computational XAS databases require systematic benchmark studies. In this work, we benchmarked Ti K-edge XAS simulations of ten representative Ti-O binary compounds, which we refer to as the Ti-O-10 dataset, using three state-of-the-art codes: xspectra, ocean and exciting. We systematically studied the convergence behavior with respect to the input parameters and developed a workflow to automate and standardize the calculations to ensure converged spectra. Our benchmark comparison shows: (1) the two Bethe-Salpeter equation (BSE) codes (ocean and exciting) have excellent agreement in the energy range studied (up to 35 eV above the onset) with an average Spearman's rank correlation score of 0.998; (2) good agreement is obtained between the core-hole potential code (xspectra) and BSE codes (ocean and exciting) with an average Spearman's rank correlation score of 0.990. Jackson Women’s Health, it is expected that there are far-reaching effects that will disproportionately affect historically marginalized populations. Access to equitable health care is met with many barriers for these groups, which include racial and sexual minorities, and people with mental health conditions, substance abuse disorders and trauma. Disparity in pregnancy outcomes currently occur in these groups and the Dobbs decision is expected to exacerbate this.

Abstract

This article details the challenges we faced in collecting sensitive information in an ethnically and religiously divided community that has experienced recent violence. The discussion is based on a summer 2016 survey experiment we conducted in Jos, Nigeria, to gather information regarding residents' perceptions of local communal violence. We discuss the challenges of such research and our approach to randomized sampling, constructing treatments that minimize the stress to respondents, debriefing to lower the possibility of spreading rumors of conflict, and utilizing computer tablets to increase access to the study for respondents with varying languages and levels of literacy. In particular, we discuss a geographic sampling method used for randomization, which we hope will prove useful to others facing similar randomization challenges.

Abstract

First-principles computational spectroscopy is a critical tool for interpreting experiment, performing structure refinement, and developing new physical understanding. Systematically setting up input files for different simulation codes and a diverse class of materials is a challenging task with a very high barrier-to-entry, given the complexities and nuances of each individual simulation package. This task is non-trivial even for experts in the electronic structure field and nearly formidable for non-expert researchers. Lightshow solves this problem by providing a uniform abstraction for writing computational x-ray spectroscopy input files for multiple popular codes, including FEFF, VASP, OCEAN, EXCITING and XSPECTRA. Abstract

BackgroundManagement of adults with atrial fibrillation (AF) or atrial flutter in the emergency department (ED) includes rate reduction, cardioversion, and stroke prevention. Different approaches to these components of care may lead to variation in frequency of hospitalization and stroke prevention actions, with significant implications for patient experience, cost of care, and risk of complications. Standardization using evidence-based recommendations could reduce variation in management, preventable hospitalizations, and stroke risk.MethodsWe describe the rationale for our ED-based AF treatment recommendations. We also describe the development of an electronic clinical decision support system (CDSS) to deliver these recommendations to emergency physicians at the point of care. We implemented the CDSS at three pilot sites to assess feasibility and solicit user feedback. We will evaluate the impact of the CDSS on hospitalization and stroke prevention actions using a stepped-wedge cluster randomized pragmatic clinical trial across 13 community EDs in Northern California.DiscussionWe hypothesize that the CDSS intervention will reduce hospitalization of adults with isolated AF or atrial flutter presenting to the ED and increase anticoagulation prescription in eligible patients at the time of ED discharge and within 30 days. 43. Bilateral Tubal Pregnancies Presenting 11 Days Apart: A Case Report

Abstract

Introduction: Ectopic pregnancy is the most common cause of maternal mortality in the first trimester. Bilateral tubal pregnancy is the rarest subset with an estimated incidence of one in 725 to 1,580 ectopic pregnancies. Of the cases of bilateral tubal pregnancy reported in the literature, most were associated with the use of assisted reproductive techniques. Here we present the case of a patient, without a prior history of reproductive technology use, who underwent treatment for a tubal pregnancy and was subsequently found to have a second, contralateral tubal pregnancy 11 days later.Case Report: A 35-year-old female gravida eight para two with a history of left tubal pregnancy and salpingectomy 11 days prior, presented to the emergency department (ED) with two days of left lower and upper quadrant abdominal pain. The patient’s last menstrual period had been several months prior. A physical examination revealed left lower quadrant abdominal tenderness, rebound, guarding, and left adnexal tenderness. Her vital signs were unremarkable, and her laboratory studies revealed normal white blood cell and hemoglobin values. Her human chorionic gonadotropin had tripled from her last presentation 11 days prior. Transvaginal ultrasound showed a possible ectopic pregnancy adjacent to the right ovary. She promptly underwent a right salpingectomy. Pathology findings confirmed a tubal pregnancy, and the patient’s postoperative course was uneventful.Conclusion: This case highlights the importance of maintaining a high index of suspicion for ectopic pregnancy in all biologically female patients of reproductive age who present to the ED with abdominal pain.

Abstract

BACKGROUND: The management and outcomes of patients diagnosed with acute pulmonary embolism in primary care have not been characterized. OBJECTIVE: To describe 30-day outcomes stratified by initial site-of-care decisions DESIGN: Multicenter retrospective cohort study PARTICIPANTS: Adults diagnosed with acute pulmonary embolism in primary care in a large, diverse community-based US health system (2013-2019) MAIN MEASURES: The primary outcome was a composite of 30-day serious adverse events (recurrent venous thromboembolism, major bleeding, and all-cause mortality). The secondary outcome was 7-day pulmonary embolism-related hospitalization, either initial or delayed. KEY RESULTS: Among 652 patient encounters (from 646 patients), median age was 64 years; 51.5% were male and 70.7% identified as non-Hispanic white. Overall, 134 cases (20.6%) were sent home from primary care and 518 cases (79.4%) were initially referred to the emergency department (ED) or hospital. Among the referred, 196 (37.8%) were discharged home from the ED without events. Eight patients (1.2%; 95% CI 0.5-2.4%) experienced a 30-day serious adverse event: 4 venous thromboemboli (0.6%), 1 major bleed (0.2%), and 3 deaths (0.5%). Seven of these patients were initially hospitalized, and 1 had been sent home from primary care. All 3 deaths occurred in patients with known metastatic cancer initially referred to the ED, hospitalized, then enrolled in hospice following discharge. Overall, 328 patients (50.3%) were hospitalized within 7 days: 322 at the time of the index diagnosis and 6 following initial outpatient management (4 clinic-only and 2 clinic-plus-ED patients). CONCLUSIONS: Patients diagnosed with acute pulmonary embolism in this primary care setting uncommonly experienced 30-day adverse events, regardless of initial site-of-care decisions. Over 20% were managed comprehensively by primary care. Delayed 7-day pulmonary embolism-related hospitalization was rare among the 51% treated as outpatients. Primary care management of acute pulmonary embolism appears to be safe and could have implications for cost-effectiveness and patient care experience.

49. Calipers: A Criticality-aware Framework for Modeling Processor Performance

Abstract

Computer architecture design space is vast and complex. Tools are needed to explore new ideas and gain insights quickly, with low efforts and at a desired accuracy. We propose Calipers, a criticality-based framework to model key abstractions of complex architectures and a program's execution using dynamic event-dependence graphs. By applying graph algorithms, Calipers can track instruction and event dependencies, compute critical paths, and analyze architecture bottlenecks. By manipulating the graph, Calipers enables architects to investigate a wide range of Instruction Set Architecture (ISA) and microarchitecture design choices/"what-if" scenarios during both early- and late-stage design space exploration without recompiling and rerunning the program. Calipers can model in-order and out-of-order microarchitectures, structural hazards, and different types of ISAs, and can evaluate multiple ideas in a single run. Modeling algorithms are described in detail. We apply Calipers to explore and gain insights in complex microarchitectural and ISA ideas for RISC and EDGE processors, at lower effort than cycle-accurate simulators and with comparable accuracy. For example, among a variety of investigations presented in the paper, experiments show that targeting only a fraction of critical loads can help realize most benefits of value prediction.

Abstract

There is growing interest in the role of sentiment in economic decision-making. However, most research on the subject has focused on positive and negative valence. Conviction Narrative Theory (CNT) places Approach and Avoidance sentiment (that which drives action) at the heart of real-world decision-making, and argues that it better captures emotion in financial markets. This research, bringing together psychology and machine learning, introduces new techniques to differentiate Approach and Avoidance from positive and negative sentiment on a fundamental level of meaning. It does this by comparing word-lists, previously constructed to capture these concepts in text data, across a large range of semantic features. The results demonstrate that Avoidance in particular is well defined as a separate type of emotion, which is evaluative/cognitive and action-orientated in nature. Refining the Avoidance word-list according to these features improves macroeconomic models, suggesting that they capture the essence of Avoidance and that it plays a crucial role in driving real-world economic decision-making.
