The drug albetol (labetalol) was used in 97 patients with stage I-III essential hypertension (according to the WHO classification) and 13 patients with stable arterial hypertension of renal genesis. Albetol exerted a marked hypotensive effect beginning from the 2-3d day of the treatment and peaking on the 6-8th day of the treatment. To normalize the blood pressure, the patients with labile hypertension were given the drug in a dose of 200-400 mg/day whilst in the patients with a stable course the dose used was 600-2200 mg/day (858 mg/day on the average). In patients with the hyperkinetic type of the hemodynamics the blood pressure decreased following albetol administration at the expense of a reduction in the cardiac index and the heart rate whereas in patients with the hypokinetic type it was achieved mostly at the expense of a decreased vascular resistance to the blood flow. The drug was effective in 69% of the patients, it was well tolerated and induced no side effects.

Visken (pindolol), a blocking agent of adrenergic beta-receptors, was used in the treatment of hypertensive crisis in 30 patients (stage I of the disease in 4, stage II in 23, stage III in 3). Intravenous injfection of visken in a dose of 1 mg led to a decrease of arterial pressure in all patients and relieved the clinical manifestations of crisis completely or partly. Significant reduction of arterial pressure and deceleration of pulse were already in evidence in 5 minutes after injection of the drug. In most patients maximum drop in pressure occurred in 30--60 minutes. There were no complications. As compared to obsidan (propranolol), which was used by other authors in hyperkinetic-type crisis, visken had the same effect on pressure and pulse. The peripheral vasodilator hyperstat (diazoxide) was used in the treatment of crisis in 28 patients with hypertensive disease (stage II of the disease in 21, and stage III in 7). In intravenous injection of 300 mg of hyperstat the hypotensive effect occurred in 1--5 minutes and reached maximum in 5 minutes. A hypotensive and clinical effect was noted in all patients. No complications were observed. On comparison with the results obtained by other authors in hyperstat treatment of hypo- and eukinetic types of crisis no essential difference in the hypotensive effect were noted.

The two agents beta-methyldigoxin (medixin), a Soviet medilazide, and bemecor (digicor), a foreign analogue (LEK, Yugoslavia) were comparatively evaluated. An equal high (85%) clinical efficacy of the drugs was found in 81 patients with varying stages of heart failure. A positive therapeutic effect was accompanied by lower heart rate, higher diuresis and natri-and kaliuresis, decreased systolic and diastolic pressures in the pulmonary artery. The incidence of adverse reactions and the causes of their occurrence are analyzed.

Methylated Digoxin preparation--beta-Methyldigoxin (Lanitop) was used in 50 patients with congestive heart failure of different etiology (tablets of 0.3--0.6 mG per day). Its employment proved very effective in the treatment of congestive heart failure. It is not contraindicated to patients with chronic coronary insufficiency with underlying athrosclerotic cardiosclerosis with signs of congestive circulatory insufficiency. Side-effects of beta-Methyldigoxin are mild and noted rather seldom.

The therapeutic efficacy of combined and separate application of a diuretic (brinaldix) and beta-adrenoblocker (visken) was compared in the treatment of 69 patients with essential hypertension running a stable course; the patients were kept on a low-sodium diet (4-6 g NaCl daily) and given a potassium substitute for NaCl.

In 77 patients with postinfarction cardiosclerosis the blood serum immunoglobulin level was studied along with some indices of the lipid and protein metabolism. An increased blood level of immunoglobulins A, M and G was observed in postinfarction cardiosclerosis cases, those with hyper-beta-lipoproteinemia, hypercholesterolemia and C-reactive protein having a significantly higher level of immunoglobulin G and than the rest of the patients. In those having an increased content of immunoglobulin G in the blood serum hypo-beta-globulinemia was observed, as well as a considerable increase of the gamma-globulin fraction as compared to those having a normal or decreased level of immunoglobulin G. Proceeding from these data the authors suggest the presence of autoimmune shifts in the patients with postinfarction cardiosclerosis that are closely connected with lipid and protein metabolism disorders.

Peripheral atherosclerosis was treated in 178 patients: 90 were given pyridinol carbamate for 2--4 months, 40 pentoxyphylline, 30 cetedil, and 18 received butalamine. in intermittent claudication pyridinol carbamate and trental proved most effective, cetedil (straten) was less effective. The favourable effect of pyridinol carbamate is associated with its action both on the state of microcirculation and on the content of lipids. A significant decrease in the level of blood triglycerides was observed during treatment with pyridinol carbamate. Pentoxyphylline reduced blood viscosity and platelet aggregation in patients with peripheral atherosclerosis but had no effect on the blood lipid content. Cetedil did not reduce blood viscosity although it decreased the aggregation of erythrocytes and inhibited the second phase of platelet aggregation. The objective criterion for the improvement of circulation in the affected extremities was increased tolerance to load, particularly in medication with pyridinol carbamate and trental.

Visken (prindolol) was used in 42 patients. Its hypotensive effect was studied by the double-blind method. The hypotensive effect was encountered in 34 patients. With the administration of a placebo the arterial pressure became elevated, while after repeated prescription of the drug it again decreased significantly. Study of hemodynamics by radiocardiography in 18 patients treated with prindolol revealed no essential changes in the cardiac output, total peripheral resistance, rate of venous blood flow and volume of circulation.

The hypotensive effect of captopril was assessed in patients with essential hypertension. Captopril-induced fall in arterial blood pressure was shown to be due to a decrease in total peripheral resistance. Treatment with captopril improved considerably physical stress tolerance, its combinations with a diuretic agent being particularly effective.

In 50 patients with congestive heart failure a compound diuretic drug aldactone--saltucine was used. Diuresis, urine excretion of sodium and potassium, rate of glomerular filtration and tubular reabsorption of water, as shown by endogenous creatinin, were measured in all patients. In 13 patients the study was conducted dynamically in 8 three-hour portions of urine during 24 hours. In 6 patients the drug was administred in combination with furosemid. In 11 patients the effect of several days of therapy with aldactone-saltucine upon the urine excretion of electrolytes and their plasma level was studied. The effect of the compound drug was evaluated separately in patients with IIA, IIB and III stage of circulatory insufficiency. Aldactone-saltucine was shown to execute a distinct diuretic and natriuretic effect that lasts for 24 hours after a single dose. The drug has also a clear potassium-preserving action that manifests itself in a low level of potassium excretion with urine and an elevation of a potassium plasma level. A combination of aldactone-saltucine with furosemid considerably increases diuresis and natriuresis, without any signifcant increase in potassiuresis, which permits to use these drugs in the severest cases of congestive heart failure.
