Abstract

Phenylketonuria is a metabolic disease caused by phenylalanine hydroxylase deficiency. Treatment is based on a strict natural protein-restricted diet that is associated with the risk of malnutrition and severe psychosocial burden. Oral administration of tetrahydrobiopterin can increase residual enzyme activity, but most patients with severe clinical phenotypes are nonresponders. We performed liver cell transplantation in a 6-year-old boy with severe tetrahydrobiopterin nonresponsive phenylketonuria who failed to comply with diet prescriptions. The transplanted hepatocytes were obtained in part from an explanted glycogen storage type 1b liver. Following two infusions, blood phenylalanine levels returned within the therapeutic target while the phenylalanine half-life assessed by loading tests decreased from 43 to 19 h. However, 3 months later, blood phenylalanine concentrations increased and the phenylalanine intake had to be reduced. Cell-based therapy is a promising therapeutic option in phenylketonuria, and the domino concept may solve the issue of cell sources for hepatocyte transplantation.

Abstract

In his 1971's Dynamic Models of Segregation paper, the economist Thomas C. Schelling showed that a small preference for one's neighbors to be of the same color could lead to total segregation, even if total segregation does not correspond to individual preferences and to a residential configuration maximizing the collective utility. The present work is aimed at deepening the understanding of the properties of dynamic models of segregation based on Schelling's hypotheses. Its main contributions are (i) to offer a comprehensive and up-to-date review of this family of models; (ii) to provide an analytical solution to the most general form of this model under rather general assumptions; to the best of our knowledge, such a solution did not exist so far; (iii) to analyse the effect of two devices aimed at decreasing segregation in such a model., Comment: 52 pages, 21 figures, working paper

14. Utility of A2L2score in acute ischemic stroke patient triage: the “H.uni” experience

Abstract

Background and aims: Two or more National Institutes of Health Stroke Scale (NIHSS) points on each motor items (A2L2score) have shown good accuracy in predicting large vessel occlusion (LVO) in the prehospital setting of acute ischemic stroke (AIS) care. We aimed to study this score for LVO prediction in our stroke network and predictors of poor outcome (PO) after mechanical thrombectomy (MT). Methods: From our Safe Implementation of Thrombolysis in Stroke (SITS) registry including patients receiving reperfusion therapy for AIS, we retrospectively computed the A2L2score from the admission NIHSS to test the diagnostic accuracy for LVO prediction. Multivariable analysis for independent predictors of LVO on the entire cohort and PO from patients with LVO were performed. Results: From the 853 patients with AIS (67% LVO), A2L2was positive in 52%. A2L2score (Odds ratio [OR] 4.6;95%CI 3.36-6.34), smoking (OR 2.1;95%CI 1.14-3.85), atrial fibrillation (OR 1.6;95%CI1.1-2.4) and younger age (OR 0.98;95%CI0.97-0.99) were independent predictors of LVO. A2L2score showed 82%/49% positive/negative predictive values with 66% accuracy (64%/72% sensitivity/specificity) for LVO prediction. Age (OR 1.05;95%CI 1.03-1.07), atrial fibrillation (OR 4.85;95%CI 1.5-15.7), diabetes (OR 2.62;95% CI 1.14-6.05), dyslipidemia (OR 2;95% CI 1.04-3.87), A2L2score (OR 2.68;95% CI 1.45-4.98), longer onset-to-groin time (OR 1.003;95% CI 1.001-1.01), MT procedure (OR 1.01;95%CI 1.003-1.02) general anaesthesia (OR 2.06;95% CI 1.1-3.83) and symptomatic intracranial hemorrhage (OR 12.10;95%CI 3.15-46.44) were independent predictors of PO. Conclusions: A2L2score independently predicted LVO and PO after MT. Patient characteristics and procedural factors determined PO of LVO patients after MT.

Abstract

We report the history of a patient and his daughter, both affected with hypoplasia of the abdominal aorta and its branches, leading to early and dramatic complications. In the index patient, renal ischaemia as a result of severe hypoplasia of the abdominal aorta and the origin of renal arteries led to progressive renal failure and end-stage renal disease at the age of 32 years. Other vascular abnormalities included hypoplasia of the celiac trunk (CT) and superior mesenteric artery (SMA). After a successful kidney transplantation at the age of 40 years, he eventually deceased following an episode of possibly ischaemic acute pancreatitis at 47 years. The patient's daughter suffered from an haemorrhagic stroke at the age of 7 years, which led to the discovery of severe hypertension caused by bilateral narrowing of renal arteries, as well as hypoplasia of CT, SMA, subclavian and pulmonary arteries. Biopsy of the narrowed renal artery of the daughter showed a particular form of fibrodysplasia characterized by an unusual fibrosis of the inner part of the media, just beneath the internal elastic lamina. To our knowledge, this is the first report of familial hypoplasia of the abdominal aorta. It might be the cardinal manifestation of a familial form of fibromuscular dysplasia (FMD). Interestingly, the histological lesions described in the daughter's renal artery differ from the classical form of medial FMD.

Abstract

Objective: To evaluate the technical performance and delivery characteristics of the Palmaz-Corinthian stent for endovascular therapy of atherosclerotic renovascular disease. Methods: 61 patients underwent implantation of 76 Palmaz-Corinthian (PC) stents in 72 arteries. 50 original PC and 26 PC stents with the modified IQ- design were employed. The indications comprised primary stenting of ostial (n=49) or truncal (n=1) stenosis or occlusion (n=3), and selective stenting following complicated (dissection, n=4) or unsuccessful (n=8) angioplasty. The remaining stents were placed in patients with recurrent stenosis (n=5) or acute aortic dissection (n=2) involving the renal artery. Mean severity and length of stenosis were 81,3% and 9.8 mm, respectively. 39 lesions were rated eccentric or calcified. Data on technical success, complication rate, delivery characteristics and ease of placement compared to standard renal stents were retrieved from a prospective multi-center registry. Results: Stent delivery was successful in all patients, major complications were not reported. Stent placement was suboptimal in 7 of 72 cases: 4 stents were located too distally in the renal artery, necessitating proximal coaxial overstenting in 2 cases. The distal part of the stenosis was incompletely covered and the orifice of a segmental branch inappropriately overstented in one case each. One stent was dislodged from the balloon, resulting in stent protrusion in the aortic lumen. Significnt residual stenosis after stenting was not observed. Overall stent deliverability, trackability and potential repositioning inside the stenosis were rated positive, radio-opacity was rated superior for the IQ design, Conclusion: Technical performance and delivery characteristics of the Palmaz-Corinthian stent have been significantly improved compared to the Palmaz design, allowing mostly correct placement in renal artery stenoses with a low complication rate.

Abstract

Cerebral artery fenestrations are rare anatomical variants usually detected incidentally on cross-sectional imaging or cerebral angiography. Although considered benign findings, many reports have described their association with vascular abnormalities such as aneurysms or arteriovenous malformations, and to a lesser extent with ischemic or hemorrhagic complications. We report a case of middle cerebral artery fenestration associated with subarachnoid hemorrhage and middle cerebral artery thrombosis. To our knowledge, there has been no prior report of middle cerebral artery fenestration with a similar presentation.

Abstract

The possibility of treating intra-cranial aneurysms (ICA) through an endovascular approach is a great progress. But, as any technique, it has its own limitations. Multidisciplinary neurovascular teams are regularly confronted with ICA where embolization is a poor option or even failed (a residue of more than 5% at six months follow-up or after recanalization). Another potential failure is a coil extrusion into the parent vessel with thrombo-embolic risks. Our team and others in the world developed strategies to manage these complex cases. After a brief review of the literature, we describe our experience and present a modified Gurian classification. This classification allows a better identification of the various failed coiled aneurysms types and their potential surgical treatments.

50. Intérêt du cathétérisme veineux sélectif des veines cervicales dans le bilan de localisation des adénomes parathyroïdiens : à propos de deux cas

Abstract

Introduction L’hyperparathyroidie primaire (HPP) est une pathologie frequente dont le premier traitement reste actuellement la chirurgie parathyroidienne. Dans la majorite des cas, les examens de localisation non invasifs ou l’exploration chirurgicale permettent de localiser la (ou les) glande(s) responsable(s) et la resequer. Chez un petit nombre de patients, l’adenome ne peut etre retrouve et l’hypercalcemie persiste. Dans le nouveau bilan de localisation, la realisation d’un catheterisme des veines cervicales (CVC) peut etre utile. Observations Nous souhaitons rapporter l’histoire medicale de deux patients presentant une HPP symptomatique et persistante apres chirurgie cervicale, pour lesquels un nouveau de bilan de localisation non invasif est reste negatif. Dans chaque cas, la realisation d’un CVC a permis de nous orienter vers une localisation inhabituelle de l’adenome parathyroidien (mediastinal superieur droit dans un cas, postero-inferieur para-œsophagien gauche dans l’autre), qui a ete confirmee par un scan mediastinal centre (cas n° 1) et par la tomographie a emission de positrons (TEP)-methionine (cas n° 2). A chaque fois, l’exerese de cet adenome a permis la guerison de l’HPP. Discussion Le bilan de localisation d’un adenome parathyroidien peut etre complexe quand celui-ci est situe en position inhabituelle ou ectopique ou en presence d’une maladie multiglandulaire. Les examens classiques non invasifs (echographie cervicale, scintigraphie de soustraction, CT-scan cervico-mediastinal ou IRM) ne permettent une localisation fiable que dans 90 % des cas environ. Bien qu’invasif, le CVC avec dosages etages de la parathormone peut s’averer tres utile chez ces patients qui doivent beneficier d’une nouvelle intervention chirurgicale.
