Abstract

Objectives Workers in the construction industry have been exposed to asbestos in various occupations. In Italy, a National Mesothelioma Registry has been implemented more than 20 years ago. Using cases selected from this registry and exploiting existing control data sets, we estimated relative risks for pleural mesothelioma (PM) among construction workers.Design Case–control study.Setting Cases from the National Mesothelioma Registry (2000–2018), controls from three previous case–control studies.Methods We selected male PM incident cases diagnosed in 2000–2018. Population controls were taken from three studies performed in six Italian regions within two periods (2002–2004 and 2012–2016). Age-adjusted and period-adjusted unconditional logistic regression models were fitted to estimate odds ratios (OR) for occupations in the construction industry. We followed two approaches, one (primary) excluding and the other (secondary) including subjects employed in other non-construction blue collar occupations for >5 years. For both approaches, we performed an overall analysis including all cases and, given the incomplete temporal and geographic overlap of cases and controls, three time or/and space restricted sensitivity analyses.Results The whole data set included 15 592 cases and 2210 controls. With the primary approach (4797 cases and 1085 controls), OR was 3.64 (2181 cases) for subjects ever employed in construction. We found elevated risks for blue-collar occupations (1993 cases, OR 4.52), including bricklayers (988 cases, OR 7.05), general construction workers (320 cases, OR 4.66), plumbers and pipe fitters (305 cases, OR 9.13), painters (104 cases, OR 2.17) and several others. Sensitivity analyses yielded very similar findings. Using the secondary approach, we observed similar patterns, but ORs were remarkably lower.Conclusions We found markedly increased PM risks for most occupations in the construction industry. 20. Reconstructing eruptive source parameters from tephra deposit: a numerical approach for medium-sized explosive eruptions

Abstract

Since the seventies, several reconstruction techniques have been proposed, and are currently used, to extrapolate and quantify eruptive parameters from sampled deposit datasets. Discrete numbers of tephra ground loadings or stratigraphic records are usually processed to estimate source eruptive values. Reconstruction techniques like Pyle, Power law and Weibull are adopted as standard to quantify the erupted mass (or volume) whereas Voronoi for reconstructing the granulometry. Reconstructed values can be affected by large uncertainty due to complexities occurring within the atmospheric dispersion and deposition of volcanic particles. Here we want to quantify the sensitivity of reconstruction techniques, and to quantify how much estimated values of mass and grain size differ from emitted and deposited ones. We adopted a numerical approach simulating with a dispersal code a mild explosive event occurring at Mt. Etna, with eruptive parameters similar to those estimated for eruptions occurred in the last decade. Then we created a synthetic deposit by integrating the mass on the ground computed by the model over the computational domain (>50000 km2). Multiple samplings of the simulated deposit are used for generating a large dataset of sampling tests afterwards processed with standard reconstruction techniques. Results are then compared and evaluated through a statistical analysis, based on 2000 sampling tests of 100 samplings points. On average, all the used techniques underestimate deposited and emitted mass. A similar analysis, carried on Voronoi results, shows that information on the total grain size distribution is strongly deteriorated. Here we present a new method allowing an estimate of the deficiency in deposited mass for each simulated class. Abstract

Classifications of volcanic eruptions were first introduced in the early twentieth century mostly based on qualitative observations of eruptive activity, and over time, they have gradually been developed to incorporate more quantitative descriptions of the eruptive products from both deposits and observations of active volcanoes. Progress in physical volcanology, and increased capability in monitoring, measuring and modelling of explosive eruptions, has highlighted shortcomings in the way we classify eruptions and triggered a debate around the need for eruption classification and the advantages and disadvantages of existing classification schemes. Here, we (i) review and assess existing classification schemes, focussing on subaerial eruptions; (ii) summarize the fundamental processes that drive and parameters that characterize explosive volcanism; (iii) identify and prioritize the main research that will improve the understanding, characterization and classification of volcanic eruptions and (iv) provide a roadmap for producing a rational and comprehensive classification scheme. In particular, classification schemes need to be objective-driven and simple enough to permit scientific exchange and promote transfer of knowledge beyond the scientific community. Schemes should be comprehensive and encompass a variety of products, eruptive styles and processes, including for example, lava flows, pyroclastic density currents, gas emissions and cinder cone or caldera formation. Abstract

Atrial fibrillation (AF) is common in patients with cardiac amyloidosis (CA) and is a significant risk factor for heart failure hospitalization and thromboembolic events. This study was designed to investigate the atrial electrofunctional predictors of incident AF in CA. A multicenter, observational study was conducted in 4 CA referral centers including sinus rhythm patients with light-chain (AL) and transthyretin (ATTR) CA undergoing electrocardiography and cardiac magnetic resonance imaging. The primary end point was new-onset AF occurrence. Overall, 96 patients (AL-CA, n = 40; ATTR-CA, n = 56) were enrolled. During an 18-month median follow-up (Q1–Q3, 7–29 months), 30 patients (29%) had incident AF. Compared with those without AF, patients with AF were older (79 vs 73 years; P =.001). They more frequently had ATTR (87% vs 45%; P <.001); electrocardiographic interatrial block (IAB), either partial (47% vs 21%; P =.011) or advanced (17% vs 3%; P =.017); and lower left atrial ejection fraction (LAEF; 29% vs 41%; P =.004). Age (hazard ratio [HR], 1.059; 95% CI, 1.002–1.118; P =.042), any type of IAB (HR, 2.211; 95% CI, 1.03–4.75; P =.041), and LAEF (HR, 0.967; 95% CI, 0.936–0.998; P =.044) emerged as independent predictors of incident AF. Patients exhibiting any type of IAB, LAEF <40%, and age >78 years showed a cumulative incidence for AF of 40% at 12 months. This risk was significantly higher than that carried by 1 (8.5%) or none (7.6%) of these 3 risk factors. In patients with CA, older age, IAB on 12-lead electrocardiography, and reduced LAEF on cardiac magnetic resonance imaging are significant and independent predictors of incident AF. Abstract

Background Health care workers (HCWs) are on the frontline, playing a crucial role in the prevention of infection and treatment of patients. Aims This study was aimed to evaluate the prevalence of hospital-acquired coronavirus disease 2019 (COVID-19) infection at work and related factors at the University Hospital of Trieste workers exposed to COVID-19 patients. Methods From March 1 to May 31, of 4216 employees, 963 were in contact with COVID-19 patients or colleagues and were followed up. Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) in nasopharyngeal swabs was determined every 3 days, by RT-PCR. Results During the follow-up period, 193 workers were positive for COVID-19 (5%), and 165 of these (86%) were symptomatic. We identified five major cluster outbreaks of COVID-19 infection in Trieste Hospitals, four of which occurred before the implementation of universal masking for HCWs and patients (1–14 March 2020). COVID-19 infection was significantly higher in high-risk ward workers (Infectious Diseases, and Geriatric and Emergency Medicine, odds ratio [OR] 13.4; 95% confidence interval [CI] 5.8–31), in subjects with symptoms (OR 5.4; 95% CI 2.9–10) and in those with contacts with COVID-19 patients and colleagues (OR 2.23; 95% CI 1.01–4.9). Conclusions Hospital workers were commonly infected due to contact with COVID-19 patients and colleagues, mainly in the first 15 days of the pandemic, before the implementation of universal mask wearing of HCWs and patients. Abstract

ABSTRACTWe investigate the role of homeostatic mechanisms involved in acute, postprandial nutrient metabolism and nutrient-induced systemic inflammation in CKD presence and progression in Metabolic dysfunction-associated steatohepatitis (MASH). We assessed postprandial incretins (GLP-1 and GIP), intestinotropic hormone GLP-2, endotoxin LPS, Zonulin (a marker of intestinal permeability), hepatokines, adipokines and NF-kB activation in circulating MNCs during a meal tolerance test in 52 biopsy proven MASH patients randomized to curcumin Meriva or placebo and 26 matched controls. At baseline, MASH-CKD had a lower GLP-2 response and a 2-fold higher postprandial LPS and NF-kB activation in MNCs than MASH patients without CKD, but similar remaining postprandial or fasting parameters. Postprandial IAUC GLP-2 predicted the presence of CKD in MASH (OR = 0.43, 95%CI:0.32-0.80, p = 0.008) independently of liver histology and traditional risk factors. After 72 weeks, changes in IAUC GLP-2 independently predicted the presence of CKD (OR = 0.49, 95%CI:0.21-0.73, p = 0.010) and eGFR changes [β(SE) = 0.510(0.007, p = 0.006] at end-of-treatment, In MASH, an impaired GLP-2 response to meals is associated with intestinal barrier dysfunction, endotoxemia and NF-kB-mediated systemic inflammation and may promote renal dysfunction and CKD. Abstract

Explosive volcanic eruptions inject hot mixtures of solid particles (tephra) and gasses into the atmosphere. Entraining ambient air, these mixtures can form plumes rising tens of kilometers until they spread laterally, forming umbrella clouds. While the largest clasts tend to settle in proximity to the volcano, the smallest fragments, commonly referred to as ash (≤2 mm in diameter), can be transported over long distances, forming volcanic clouds. Tephra plumes and clouds pose significant hazards to human society, affecting infrastructure, and human health through deposition on the ground or airborne suspension at low altitudes. Additionally, volcanic clouds are a threat to aviation, during both high‐risk actions such as take‐off and landing and at standard cruising altitudes. The ability to monitor and forecast tephra plumes and clouds is fundamental to mitigate the hazard associated with explosive eruptions. To that end, various monitoring techniques, ranging from ground‐based instruments to sensors on‐board satellites, and forecasting strategies, based on running numerical models to track the position of volcanic clouds, are efficiently employed. However, some limitations still exist, mainly due to the high unpredictability and variability of explosive eruptions, as well as the multiphase and complex nature of volcanic plumes. In the next decades, advances in monitoring and computational capabilities are expected to address these limitations and significantly improve the mitigation of the risk associated with tephra plumes and clouds. During explosive volcanic eruptions, fragmented material known as tephra is forcefully injected into the atmosphere through volcanic plumes. While the largest clasts tend to settle in proximity to the volcano, the smallest fragments, commonly referred to as ash, can be transported over long distances, forming volcanic clouds. On the ground, tephra poses a significant hazard to human activities, both near the volcano and at distances up to hundreds of kilometers. In contrast, volcanic clouds present a serious threat to aviation. For these reasons, we are motivated to understand the dynamics of tephra plumes and clouds and how we can mitigate their hazards. Today, we have the capability to monitor tephra plumes and clouds using a plethora of techniques, ranging from ground‐based methods to satellite instruments. Moreover, forecasting of tephra dispersion through computer simulations has become a valuable tool used to mitigate the risk of explosive eruptions. In this paper, we provide a comprehensive review of the current understanding of tephra plumes and clouds, covering their generation, the factors that influence their characteristics, and the latest strategies for monitoring and forecasting.
