7. 'Back to School after Corona Virus Disease of 2019': New Relationships, Distance Schooling, and Experienced Routine

Abstract

The Corona Virus Disease of 2019 (COVID-19) pandemic has upset the students' daily routine, forcing them at first into a sudden transition to distance learning and then to a return to school modelled on the basis of infection containment measures. The present research involved 157 students from schools in central Italy with a mean age of 13.58 years old to investigate the affective impact of the pandemic on the school experience and its components (recess, oral testing, relationships with classmates, and relationship pupils-teachers). The results show that only a few have experienced school interruption in a traumatic way: they have appreciated neither distance learning, nor the return to school; for these teenagers, the school of the past has died. Other adolescents and pre-adolescents tried to replace the face-to-face mode with distance learning, maintaining certain attention to the school even during the quarantine. However, the online mode did not keep its promise. Those who have invested more in digital innovation find it difficult to return to normality today. For all of them, socialization mediated by school experience is decisive in supporting the return to ordinary life after the pandemic.

Abstract

There is rising interest globally with respect to the health implications of vegetarian or plant-based diets. A growing body of evidence has demonstrated that higher consumption of plant-based foods and the nutrients found in vegetarian and plant-based diets are associated with numerous health benefits, including improved blood pressure, glycemic control, lipid levels, body mass index, and acid-base parameters. Furthermore, there has been increasing recognition that vegetarian and plant-based diets may have potential salutary benefits in preventing the development and progression of chronic kidney disease (CKD). While increasing evidence shows that vegetarian and plant-based diets have nephroprotective effects, there remains some degree of uncertainty about their nutritional adequacy and safety in CKD (with respect to protein-energy wasting, hyperkalemia, etc.). In this review, we focus on the potential roles of and existing data on the efficacy/effectiveness and safety of various vegetarian and plant-based diets in CKD, as well as their practical application in CKD management.

9. Eliciting the Double-edged Impact of Digitalisation: a Case Study in Rural Areas

Abstract

Designing systems that account for sustainability concerns demands for a better understanding of the \textit{impact} that digital technology interventions can have on a certain socio-technical context. However, limited studies are available about the elicitation of impact-related information from stakeholders, and strategies are particularly needed to elicit possible long-term effects, including \textit{negative} ones, that go beyond the planned system goals. This paper reports a case study about the impact of digitalisation in remote mountain areas, in the context of a system for ordinary land management and hydro-geological risk control. The elicitation process was based on interviews and workshops. In the initial phase, past and present impacts were identified. In a second phase, future impacts were forecasted through the discussion of two alternative scenarios: a dystopic, technology-intensive one, and a technology-balanced one. The approach was particularly effective in identifying negative impacts. Among them, we highlight the higher stress due to the excess of connectivity, the partial reduction of decision-making abilities, and the risk of marginalisation for certain types of stakeholders. The study posits that before the elicitation of system goals, requirements engineers need to identify the socio-economic impacts of ICT technologies included in the system, as negative effects need to be properly mitigated. Abstract

While dialysis has been the prevailing treatment paradigm for patients with advanced chronic kidney disease (CKD), emphasis on conservative and preservative management in which dietary interventions are a major cornerstone have emerged. Based on high-quality evidence, international guidelines support the utilization of low-protein diets as an intervention to reduce CKD progression and mortality risk, although the precise thresholds (if any) for dietary protein intake vary across recommendations. There is also increasing evidence demonstrating that plant-dominant low-protein diets reduce the risk of developing incident CKD, CKD progression, and its related complications including cardiometabolic disease, metabolic acidosis, mineral and bone disorders, and uremic toxin generation. In this review, we discuss the premise for conservative and preservative dietary interventions, specific dietary approaches used in conservative and preservative care, potential benefits of a plant-dominant low-protein diet, and practical implementation of these nutritional strategies without dialysis.

To establish a closer contact between school and experimental sciences, Sapienza Universit\`a di Roma and the Istituto Nazionale di Fisica Nucleare (INFN) launched the Lab2Go project. Lab2Go has the goal of spreading laboratory practice among students and teachers in high schools. In this article, it is presented a measurement, carried out in the framework of the Lab2Go project, of the ratio hc/e where h, c and e are respectively the Planck constant, the speed of light in the vacuum, and the electric charge.

Abstract

We design a multi-purpose environment for autonomous UAVs offering different communication services in a variety of application contexts (e.g., wireless mobile connectivity services, edge computing, data gathering). We develop the environment, based on OpenAI Gym framework, in order to simulate different characteristics of real operational environments and we adopt the Reinforcement Learning to generate policies that maximize some desired performance.The quality of the resulting policies are compared with a simple baseline to evaluate the system and derive guidelines to adopt this technique in different use cases. The main contribution of this paper is a flexible and extensible OpenAI Gym environment, which allows to generate, evaluate, and compare policies for autonomous multi-drone systems in multi-service applications. This environment allows for comparative evaluation and benchmarking of different approaches in a variety of application contexts.

Abstract

Requirements engineering (RE) is a key area to address sustainability concerns in system development. Approaches have been proposed to elicit sustainability requirements from interested stakeholders before system design. However, existing strategies lack the proper high-level view to deal with the societal and long-term impacts of the transformation entailed by the introduction of a new technological solution. This paper proposes to go beyond the concept of system requirements and stakeholders' goals, and raise the degree of abstraction by focusing on the notions of drivers, barriers and impacts that a system can have on the environment in which it is deployed. Furthermore, we suggest to narrow the perspective to a single domain, as the effect of a technology is context-dependent. To put this vision into practice, we interview 30 cross-disciplinary experts in the representative domain of rural areas, and we analyse the transcripts to identify common themes. As a result, we provide drivers, barriers and positive or negative impacts associated to the introduction of novel technical solutions in rural areas. This RE-relevant information could hardly be identified if interested stakeholders were interviewed before the development of a single specific system. This paper contributes to the literature with a fresh perspective on sustainability requirements, and with a domain-specific framework grounded on experts' opinions. The conceptual framework resulting from our analysis can be used as a reference baseline for requirements elicitation endeavours in rural areas that need to account for sustainability concerns.

Abstract

The Academy of Nutrition and Dietetics and the National Kidney Foundation collaborated to provide an update to the Clinical Practice Guidelines (CPG) for nutrition in chronic kidney disease (CKD). These guidelines provide a valuable update to many aspects of the nutrition care process. They include changes in the recommendations for nutrition screening and assessment, macronutrients, and targets for electrolytes and minerals. The International Society of Renal Nutrition and Metabolism assembled a special review panel of experts and evaluated these recommendations prior to public review. As one of the highlights of the CPG, the recommended dietary protein intake range for patients with diabetic kidney disease is 0.6-0.8 g/kg/day, whereas for CKD patients without diabetes it is 0.55-0.6 g/kg/day. The International Society of Renal Nutrition and Metabolism endorses the CPG with the suggestion that clinicians may consider a more streamlined target of 0.6-0.8 g/kg/day, regardless of CKD etiology, while striving to achieve intakes closer to 0.6 g/kg/day. For implementation of these guidelines, it will be important that all stakeholders work to detect kidney disease early to ensure effective primary and secondary prevention. Once identified, patients should be referred to registered dietitians or the region-specific equivalent, for individualized medical nutrition therapy to slow the progression of CKD. As we turn our attention to the new CPG, we as the renal nutrition community should come together to strengthen the evidence base by standardizing outcomes, increasing collaboration, and funding well-designed observational studies and randomized controlled trials with nutritional and dietary interventions in patients with CKD.

Purpose of reviewIn advanced chronic kidney disease (CKD) patients with progressive uremia, dialysis has traditionally been the dominant treatment paradigm. However, there is increasing interest in conservative and preservative management of kidney function as alternative patient-centered treatment approaches in this population.Recent findingsThe primary objectives of conservative nondialytic management include optimization of quality of life and treating symptoms of end-stage renal disease (ESRD). Dietetic-nutritional therapy can be a cornerstone in the conservative management of CKD by reducing glomerular hyperfiltration, uremic toxin generation, metabolic acidosis, and phosphorus burden. Given the high symptom burden of advanced CKD patients, routine symptom assessment using validated tools should be an integral component of their treatment. As dialysis has variable effects in ameliorating symptoms, palliative care may be needed to manage symptoms such as pain, fatigue/lethargy, anorexia, and anxiety/depression. There are also emerging treatments that utilize intestinal (e.g., diarrhea induction, colonic dialysis, oral sorbents, gut microbiota modulation) and dermatologic pathways (e.g., perspiration reduction) to reduce uremic toxin burden.SummaryAs dialysis may not confer better survival nor improved patient-centered outcomes in certain patients, conservative management is a viable treatment option in the advanced CKD population.

Abstract

Computer-aided detection (CADe) systems for colonoscopy have been shown to increase small polyp detection during colonoscopy in the general population. People with Lynch syndrome represent an ideal target population for CADe-assisted colonoscopy because adenomas, the primary cancer precursor lesions, are characterised by their small size and higher likelihood of showing advanced histology. We aimed to evaluate the performance of CADe-assisted colonoscopy in detecting adenomas in individuals with Lynch syndrome.
