1. Hear Our Voice: Pediatric Communication Barriers From the Perspectives of Refugee Mothers With Limited English Proficiency.

Abstract

Adverse health outcomes are more common for health consumers with limited English proficiency (LEP). This study examines the consumer experience of refugee mothers with LEP when communicating with paediatric health services. A community-based participatory qualitative study engaging participants from refugee-like backgrounds. Focus groups and in-depth individual interviews (using professional interpreters) were conducted in community settings and analysed using Grounded Theory principles. Fifty ethnolinguistically diverse participants reported universal communication barriers; (i) "Negative health care experiences" (fear, helplessness, lack of safety, trust and dignity), (ii) "Ineffective health service communication and adverse outcomes", (iii) "Logistical access barriers" and (iv) self-sourced solutions". The "importance of professional interpreter utilisation" and subsequent "sense of empowerment" was unanimous. This study highlights gaps in current health interactions which negatively impact care, inclusion, and culturally safe engagement. Recommendations include orgainzational reform enhancing language services, increased cultural competency, long term support, and research with LEP populations. [ABSTRACT FROM AUTHOR]

20. Concurrent improvement and deterioration of epilimnetic water quality in an oligotrophic lake over 37 years

Abstract

Single‐lake studies offer an opportunity for understanding, predicting, and mitigating local or regional threats to lake ecosystems. Our goal was to understand how concurrent environmental stressors such as climate change, eutrophication, and salinization affect long‐term lake water quality. We report epilimnetic changes in 18 water‐quality parameters collected at seven sites from 1980 to 2016 in Lake George, a large oligotrophic lake in the Adirondack Park, New York, USA. Improvements and deteriorations in water quality occurred over 37 years. We observed a 32% increase in chlorophyll aassociated with an increase in orthophosphate, but not total phosphorus or a warming epilimnion (0.05°C/year). Salinization from road deicing salts contributed to the largest deterioration in water quality. However, chloride concentrations and the current rate of increase are low enough that few ecological impacts are likely to occur over the next few decades. Increasing calcium concentrations were not high enough to facilitate the persistence of invasive species in the lake such as zebra mussels (Dreissena polymorpha) but are sufficient for Asian clams (Corbicula fluminea) and the spiny water flea (Bythotrephes longimanus). Similar to other lakes, environmental legislation has supported recovery from acidification, indicated by reduced sulfate and nitrate, and increased alkalinity and pH. Declines in water quality were minor relative to other lakes, suggesting that decades of tourism and development occurred without major deterioration in water quality, but management efforts are needed to curb salinization in the Lake George watershed, particularly as it relates to sodium concentrations to prevent a loss of drinking water quality.

22. The dynamics of Chaoborus americanus in an Adirondack lake following the reintroduction of fish.

Abstract

Acid deposition during the 20th century led to the gradual elimination of fish in Brooktrout Lake (Adirondack Mountains, New York State). Thereafter, the lake was colonized by Chaoborus americanus , a dipteran with an aquatic larval stage that typically resides in the pelagic zone in fishless lakes. During subsequent chemical recovery from acidification, the lake was stocked with Brook Trout ( Salvelinus fontinalis ). For seven years following this reintroduction we examined the re-adaptation of the food chain. The C. americanus abundance and distribution was quantified utilizing a combination of hydroacoustics, traditional vertical net tows and Schindler-Patalas trap collections. Hydroacoustic backscattering signals were repeatable and correlated (r = 0.86, p = 0.003) with C. americanus abundance. Backscattering, depth, month and year were used to develop a random forest model that predicted the C. americanus density (r 2 = 0.67, p< 0.05). The hydroacoustic signal revealed a clear but limited diurnal vertical migration of C. americanus . The signal continued in the presence of the fish population beginning with reintroduction in 2005 and extending through 2011. In 2012, the hydroacoustic signal no longer was present in the lake, suggesting that the fish had eliminated the C. americanus population, which was verified with net tows. Using novel and traditional survey methods, we demonstrate that the reintroduction of fish can alter the lake community structure significantly through the extirpation of the major component of the pelagic zone. Abstract

Intrafamilial trauma (the physical, emotional, and sexual abuse between two or more family members) can render psychologically deleterious effects not only on abused children, but their non-abused sibling(s) as well (Vitale, Squires, Zuckerbraun & Berger 2010). However, non-abused siblings are rarely the participants of trauma treatment or the focus of research, rendering them the 'forgotten survivors' of childhood maltreatment (Renner 2012; Tavkar & Hansen 2011). Nevertheless, therapists may encounter these siblings as patients as they too present myriad problematic symptoms and grief-related experiences (McMackin, Newman, Fogler & Keane 2012; Van der Kolk 2014). To assist therapists when counselling non-abused children affected by intrafamilial maltreatment, this article aims to describe the psychosocial impact of intrafamilial childhood trauma on non-abused siblings and to discuss the ways that playbased treatment can be incorporated into an empirically supported trauma treatment approach to meet their needs and grief-based concerns adequately. 26. Donor blood perfusion improves myocardial recovery after heart transplantation

Abstract

Improved methods of donor heart preservation may allow for prolonged storage and permit remote procurement. Previous attempts to use oxygenated perfusion circuits during storage have not gained widespread acceptance because they were either too impractical or complicated to use for remote harvest. We hypothesized that collection and perfusion of donor blood during prolonged storage may improve myocardial recovery. Our aim was to devise a safe, simple, cost-effective system that could be used in any hospital setting.Yorkshire pigs (40 to 50 kg) were used to perform 16 orthotopic heart transplantations with either continuous perfusion with donor blood (BL, n = 8) or standard hypothermic storage (CON, n = 8). After administration of heparin, hypothermic (4 degrees C) cardioplegic arrest, and donor heart extraction, donor blood (2688 +/- 166 ml) was harvested in the BL group. After filtration for particulate matter, blood perfusion was initiated via a standard intravenous transfusion apparatus at room temperature (20 degrees C) and a pressure of 60 mm Hg and maintained during storage. Arterial and coronary sinus blood samples were obtained to examine myocardial oxygen extraction, lactate release, and acid production. A Millar micromanometer was used to measure left ventricular developed pressures at an end-diastolic pressure of 2 and 10 mm Hg both before and after transplantation.All pigs (eight of eight) in the BL group were successfully weaned off bypass compared to three of eight in the CON group (p0.01). Developed pressures (at left ventricular end-diastolic pressure = 10 mm Hg) was improved in the BL group (mean +/- SD: baseline: BL: 90 +/- 16 mm Hg vs CON: 83 +/- 12 mm Hg, p = NS; posttransplantation: BL: 66 +/- 8 mm Hg vs CON: 35 +/- 29 mm Hg, p0.05). Similarly, maximum dP/dt was higher in the BL group. Lactate release was higher at cross-clamp removal in the BL group (2.4 +/- 0.3 mmol/L vs 0.7 +/- 0.2 mmol/L, p0.01). There were no differences in oxygen extraction or acid production during reperfusion.Perfusion of donor blood improved the ability to wean off bypass after 4 hours of storage. Blood perfusion permitted persistent myocardial metabolism during the ischemic period, which led to improved functional recovery. Harvesting donor blood for subsequent perfusion during prolonged storage may improve the results of orthotopic heart transplantation and allow for more distant procurement of donor organs.

Abstract

The effect of lead accumulation on photopigment production, mineral nutrition, and Anabaena vegetative cell size and heterocyst formation in Azolla caroliniana was investigated. Plants were exposed to 0, 1, 5, 10, and 20mgL−1 lead acetate for ten days. Lead accumulation increased when plants were treated with higher lead concentrations. Results revealed a statistically significant decline in total chlorophyll, chlorophyll a, chlorophyll b, and carotenoids in 5, 10, and 20mgPbL−1 treatment groups as compared to plants with 0 or 1mgPbL−1 treatments. No statistically significant change in anthocyanin production was observed. Calcium, magnesium, and zinc concentrations in plants decreased in increasing treatment groups, whereas sodium and potassium concentrations increased. Nitrogen and carbon were also found to decrease in plant tissue. Anabaena vegetative cells decreased in size and heterocyst frequency declined rapidly in a Pb dose-dependent manner. These results indicate that, while A. caroliniana removes lead from aqueous solution, the heavy metal causes physiological and biochemical changes by impairing photosynthesis, changing mineral nutrition, and impeding the growth and formation of heterocysts of the symbiotic cyanobacteria that live within leaf cavities of the fronds. 30. Evidence for water temperature increase in Lake George, NY: impact on growing season duration and degree days

Abstract

AbstractLake George (NY) water temperature increased significantly between 1980 and 2009, leading to a longer aquatic growing season and increased degree-days. Surface water (0–10 m) temperatures increased by 0.063 and 0.051 C/yr, and degree-days increased by 229 and 195 degree-days in the North and South basins, respectively. The aquatic growing season increased by ∼2 weeks, with extensions in both the spring and fall. The rate of warming, degree-days, and the duration of the growing season were consistently greater in the North Basin, due in part to a greater input of groundwater in the South Basin. Weather variables over the same time period changed significantly with wind speed and cloud cover decreasing, humidity and precipitation increasing, and no significant change identified for air temperature. Wind and cloud cover were correlated to the onset of the growing season and degree-days in both the North and South basins, suggesting they are influencing the lake thermal regime through reduced evaporative cooling in the fall, diminished spring mixing and increased solar radiation absorption. Warmer water temperatures have a myriad of consequences for aquatic ecosystems from increased primary production and shifting species composition to uncoupling of trophic dynamics and increased mineralization of organic carbon. While management practices are available for some of these issues, others have no management protocols, creating a foreseeable problem for lake managers in the future.

31. Road salt application differentially threatens water resources in Lake George, New York

Abstract

AbstractRoad salt (NaCl) application around Lake George, New York, resulted in nearly tripling in-lake salt concentration between 1980 and 2009. Salt concentrations measured in 8 streams between 2007 and 2009 ranging in development from pristine to moderate resulted in 4 significantly different groups based on chloride concentrations. Chloride concentrations were significantly correlated to the amount of roadway surface within each sub-watershed, with chloride concentrations in the most impacted stream approaching 200 mg/L. In the more impacted streams, chlor­ide concentrations were significantly and inversely correlated to discharge rate. The high road density around the more developed south end accounted for ∼30% of the estimated road salt application, which created a chloride gradient within the lake that decreased as water flowed north to the single outlet. The continual and disproportionate input of road salt near the southern end of the lake strengthened the gradient over time to create 4 significantly different regions within the lake. While the consistent lake-wide increase seems to be open-ended, a steady-state salt concentration within the lake may occur in the near future. Based on change of lake chloride mass and export from the lake (2007–2009), an average lake-wide steady-state chloride concentration of ∼17 mg/L is expected, with some variation anticipated due to interannual variation in precipitation and the salt gradient within the lake.

Abstract

The article discusses a stream assessment in the Oswegatchie and Black River drainages. Acidification was investigated with the base-cation surplus (BCS) and the conventional acid-neutralizing capacity by Gran titration. Results showed that the contribution of acidic deposition to stream acidification was greater than that of strongly acidic organic acids in each of the surveys. The percentage attributed to acidic deposition during snowmelt was 81%, whereas the percentage was 66% during the October 2003 survey, when dissolved organic carbon concentrations were highest.

Abstract

ABSTRACTDrinking water standards and recreational demands on lakes and ponded waters place a high priority on the management of water quality. As stakeholders, volunteer citizen monitors can provide considerable assistance to state agencies and research organizations charged with monitoring freshwater resources by accumulating valuable data for water quality evaluation and lake assessment. The Lay Monitoring Program on Lake George (NY) was initiated in 1980 concomitant with the establishment of a long-term chemical monitoring program. By 2002, 23 years of Secchi transparency and surface temperature data had been collected. Results have revealed potential relationships between variations in Secchi transparency and trophic indicators (chlorophyll a and total phosphorus) and have assisted in evaluating the potential influences of both increased recreational use and precipitation regime. Lay monitor participation provides a valuable and cost effective enhancement for ongoing lake chemical and biotic effect studies. The integration of lay monitor Secchi data improves the breadth and consistency of long-term datasets, thereby increasing the program effectiveness in detecting incremental changes in water quality.

42. Wegener’s Granulomatosis

Abstract

We report a case of classic Wegener’s granulomatosis. Direct immunofluorescent study showed finely granular deposits of IgG and IgM in some of the alveolar walls, and of IgM in the maxillary sinus arteries. Electron-microscopic study of the lung and maxillary sinus showed intravascular fibrin, but failed to demonstrate electron-dense deposits in the blood vessel walls. Our patient also had circulating cryoglobulins, consisting of IgG, IgM, Clq, and C3, and evidence of circulating immune complexes as demonstrated by the Clq-binding test. These findings suggest that circulating immune complexes may play an important role in the pathogenesis of the respiratory lesions of Wegener’s granulomatosis.

43. Stromal fibroblasts influence oral squamous-cell carcinoma cell interactions with tenascin-C

Abstract

In this study we identified tenascin-C (TN-C) and one of its integrin receptors, αvβ6, in oral squamous-cell carcinoma (SCC) specimens. Neither TN-C nor αvβ6 are expressed in normal oral mucosa. We also studied 2 human oral squamous-cell carcinoma cell lines: the highly invasive HSC-3 cells, and the poorly invasive SCC-25 cells. We determined that adhesion of these cells to TN-C involves both α2 and αv integrins. Migration on TN-C by oral SCC cells required fibroblast-conditioned medium and did not occur in its absence. This migration was blocked by anti-α2 and anti-αv antibodies and was partially inhibited by antibodies to hepatocyte growth factor, epidermal growth factor and transforming growth factor-β1. When seeded on TN-C, the poorly invasive SCC-25 cells formed αvβ6-positive focal contacts; the HSC-3 cells did not. HSC-3, SCC-25 and PTF cells secrete TN-C into the culture medium, as determined by Western blot. However, when HSC-3 cells were inoculated into the floor of the mouth of nude mice, only murine TN-C could be identified in the reactive stroma adjacent to the resulting tumor nests, demonstrating that in vivo, HSC-3 cells do not secrete TN-C. Our results demonstrate that αvβ6 and tenascin-C are neo-expressed in oral squamous-cell carcinoma, and that the tumor stromal environment is influential in oral SCC behavior. Int. J. 44. Pneumocystis cariniiPneumonia in the Acquired Immunodeficiency Syndrome (AIDS)

Abstract

Sixty-one diagnostic biopsies for Pneumocystis cariniipneumonia were performed on 40 homosexual male patients with acquired immunodeficiency syndrome (AIDS), using flexible fiberoptic bronchoscopy. Bronchial brushings and bronchoalveolar lavage were performed in conjunction with the biopsy in 58 and 29 bronchoscopies, respectively. Using a rapid methenamine silver stain, P cariniipneumonia was diagnosed in 27 (68 percent) of the patients. Twenty of these patients had a repeat biopsy one or more times for evaluation of therapy. Eighteen of the biopsies following two to three weeks of therapy were positive. There was an 84 percent correlation between findings on transbronchial brushing and biopsy (89 percent on initial biopsy before treatment) and an 86 percent correlation between bronchoalveolar lavage and biopsy. Additionally, transbronchial brushing permitted demonstration of Pneumocystis organisms in four follow-up bronchoscopies in which the biopsy was negative or inadequate. Rapid methenamine silver stain of transbronchial brushings permits diagnosis of P cariniipneumonia in patients with AIDS within one-half hour of bronchoscopy.

45. Respiratory Symptoms and Functional Impairment from Acute (Cross-Shift) Exposure to Welding Gases and Fumes

Abstract

One hundred forty-five male welders from a West Coast shipyard were studied cross-sectionally and across a Monday work shift by pulmonary function tests and a questionnaire. Ten years of welding was associated with chronic bronchitis in 23.3% of nonsmokers compared to 3.3% in male controls, shortness of breath in 31.5% of nonsmokers compared to 1.5% in controls, and chest pain or heaviness in 38.4% compared to 4.4% in controls. Men who welded aluminum but had never smoked had more frequent wheezing, chest tightness, phlegm, feverishness and fatigue than those welding mild (black) or stainless steel. There were no significant cross-shift effects from welding exposure on measurements of pulmonary function. Although baseline expiratory flows were reduced slightly when compared to Caucasian-predicted values, ethnic specific comparisons for the largest subgroup showed only that FEF25−75was reduced to 92.9 percentage of predicted values. Diffusing capacities for carbon monoxide were significantly reduced as compared to referents. The pulmonary function values of 25 current smokers were indistinguishable from the 41 who had never smoked, which probably reflects their low consumption of cigarettes.

47. Cocaine toxicity and isoflurane anesthesia: Hemodynamic, myocardial metabolic, and regional blood flow effects in swine

Abstract

Objectives:: Increasing numbers of people use cocaine recreationally and may require anesthesia care, having recently abused the drug. However, no data currently exist concerning potential interactions between toxic levels of cocaine and volatile anesthetic agents. This study investigated the effects of cocaine infusion on systemic hemodynamics, myocardial metabolism, and regional organ blood flow in relation to depth of isoflurane anesthesia. Design:: Prospective, randomized, controlled trial. Setting:: A laboratory at a university medical center. Participants:: Twelve miniature pigs. Interventions:: An open-chest swine model was used. Isoflurane (ISO) was the sole anesthetic, administered at 0.75 and 1.5 minimum alveolar concentration (MAC), and cocaine was infused (n = 6) at a rate of 0.5 mg/kg/min. Control animals (n = 6) received an equivalent amount of normal saline. Measurements and Main Results:: Systemic and pulmonary arterial pressures and thermodilution cardiac output data were collected at 0.75 MAC and 1.5 MAC ISO. Regional myocardial and blood flows to other organs were measured using radiolabeled microspheres. Arrhythmias and altered ventricular conduction were noted only in the cocaine group, along with significant elevations in diastolic arterial pressure, coronary perfusion pressure, and systemic vascular resistance. Increased subendocardial blood flow occurred during cocaine infusion (p = 0.03); subepicardial perfusion was unchanged. Cerebral (p < 0.01) and spinal cord (p < 0.05) blood flows were reduced in animals receiving cocaine. Other organ blood flows were unchanged with depth of anesthesia or cocaine administration, with the exception of splenic blood flow (p < 0.04). Conclusions:: Moderately toxic cocaine levels occurring during isoflurane at 0.75 MAC and 1.5 MAC are associated with hemodynamic abnormalities, a marked increase in systemic vascular resistance, and a tendency to produce cardiac arrhythmias. A reversal of endo/epicardial myocardial perfusion ratio occurs associated with cocaine infusion during ISO anesthesia. This is probably not related to a primary redistribution of subendocardial blood flow and may be related to a combination of increased myocardial oxygen demand and epicardial coronary vasoconstriction. The reductions in cerebral and spinal cord perfusion observed may explain, in part, the neurologic sequelae of cocaine toxicity.

48. Macromolecular Synthesis and Degradation in ArthrobacterDuring Periods of Nutrient Deprivation

Abstract

Cells of Arthrobacter atrocyaneusand A. crystallopoietes, harvested during their exponential phase, were starved in 0.03 M phosphate buffer (pH 7.0) for 28 days. During this time, the cells maintained 90 to 100% viability. Experimental results were similar for both organisms. Total cellular deoxyribonucleic acid was maintained. Measurable degradation rates for deoxyribonucleic acid as determined by radioisotope techniques were not observed, and only during the initial hours of starvation could a synthetic rate be determined. Total ribonucleic acid levels remained stable for the first 24 h of starvation, after which slow, continuous loss of orcinol-reactive material occurred. Synthetic and degradative rates of ribonucleic acid, as determined by radioisotope techniques, dropped quickly at the onset of starvation. Constant basal rates were attained after 24 h. In A. atrocyaneus, total cell protein was degraded continuously from the onset of starvation. In A. crystallopoietes, total cell protein remained stable for the first 24 h, after which slow continuous loss occurred. After 28 days, the total protein per cell was similar for both organisms. In the first week, amino acid pools stabilized at about 50% of the values characteristic of growth. Rates of degradation of protein decreased rapidly for the first 24 h for both organisms, but leveled to a constant basal rate thereafter. Rates of new protein synthesis dropped during the first 24 h and by 48 h achieved a constant basal rate.

49. Hematologic Management of Hemophilia A for Surgery

Abstract

From mid-1967 to mid-1983, three hundred fifty surgical operations were performed on 163 patients with hemophilia A, without factor VIII inhibitor. One death occurred, in a patient with a serious head injury. Postoperative hemorrhages occurred after 23% of operations, but the incidence after surgery on the knee, 40%, was decidedly higher than the 15% incidence after operations at other sites. Concurrent plasma factor VIII levels were over 0.40 units/mL in 72% of instances and under 0.30 units/mL in only 15% of instances. The incidence of postoperative hemorrhage did not change over the study period despite a threefold increase in typical dosage of factor VIII (from 600 to 2,000 units/kg per operation) and doubling of typical trough factor VIII levels (from 0.37 units/mL to 0.70 units/mL). Circulating factor VIII levels apparently are not the sole determinants of postoperative bleeding in hemophilia A.(JAMA 1985;253:1279-1283)

50. Division of Labor of Schwann Cell Integrins during Migration on Peripheral Nerve Extracellular Matrix Ligands

Abstract

Myelination of the peripheral nervous system (PNS) requires the migration of Schwann cells during both development and regeneration. We have characterized the expression pattern of Schwann cell integrins and analyzed their role in migration on different ECM substrates known to be present within the PNS. We found that Schwann cells in cell culture express four β1 integrins, α1β1, α2β1, α6β1, and another unidentified β1 integrin, as well as two αv integrins, αvβ3 and αvβ8. Using the Varani migration assay, we found that laminin-1, laminin-2 (merosin), and fibronectin increased Schwann cell migration, while vitronectin and collagen did not increase migration compared to an uncoated plastic substrate. Schwann cell migration on laminin-1 and laminin-2 (merosin) was blocked by antibodies against β1 integrins, but not affected by RGD peptides or antibodies against β3 integrins. In contrast, migration on fibronectin was unaffected by antibodies against β1 and β3 integrins but was blocked by RGD peptides. Thisin vitrostudy shows that there is a division of labor of Schwann cell integrins in the regulation of migration on peripheral nerve ECM components; β1 integrins mediate migration on laminin-1 and laminin-2 (merosin), while αv integrins mediate migration on fibronectin. Taken together, these results suggest that multiple interactions between Schwann cell integrins and ECM within the PNS will contribute to Schwann cell migration during myelination of the PNS.
