1. LLMic: Romanian Foundation Language Model

Abstract

Recent advances in Large Language Models (LLMs) have demonstrated remarkable capabilities across various tasks with commercial models leading the way. While open models usually operate at a smaller scale, they maintain competitiveness through specialization and fine-tuning. However, a significant challenge persists: open models often underperform in low-resource languages due to limited representation in the training corpus. In this paper, we present LLMic, a bilingual foundation language model designed specifically for the Romanian Language. We document the complete process of pretraining a foundation model for a low-resource language, including corpus construction, architecture selection, and hyper-parameter optimization. Our evaluation demonstrates that LLMic can be specialized for tasks in the target language, achieving results comparable to other much larger open models. We show that fine-tuning LLMic for language translation after the initial pretraining phase outperforms existing solutions in English-to-Romanian translation tasks. This opens the path for efficient large-scale processing for the Romanian language community, using the much smaller LLMic model

Abstract

Research in the field of language models is rapidly evolving, with many open models being released to the public. Openly available pretraining corpora usually focus on only a handful of languages, with many others either missing completely or extremely underrepresented. In this report, we introduce FuLG, a hundred-fifty-billion-token Romanian corpus extracted from CommonCrawl. We present our methodology for filtering FuLG and compare it via ablation studies against existing Romanian corpora.

Abstract

Hyperscalers dominate the landscape of large network deployments, yet they rarely share data or insights about the challenges they face. In light of this supremacy, what problems can we find to solve in this space? We take an unconventional approach to find relevant research directions, starting from public plans to build a $100 billion datacenter for machine learning applications. Leveraging the language models scaling laws, we discover what workloads such a datacenter might carry and explore the challenges one may encounter in doing so, with a focus on networking research. We conclude that building the datacenter and training such models is technically possible, but this requires novel wide-area transports for inter-DC communication, a multipath transport and novel datacenter topologies for intra-datacenter communication, high speed scale-up networks and transports, outlining a rich research agenda for the networking community.

Abstract

Rotational dynamics often challenge physical intuition while enabling unique realizations, from the rotor of a gyroscope that maintains its orientation regardless of the outer gimbals, to a tennis racket that rotates around its handle when tossed face-up in the air. In the context of inertial mass sensing, which can measure mass with atomic precision, rotational dynamics are normally considered a complication hindering measurement interpretation. Here, we exploit the rotational dynamics of a microfluidic device to develop a new modality in inertial resonant sensing. Combining theory with experiments, we show that this modality normally measures the volume of the particle while being insensitive to its density. Paradoxically, particle density only emerges when fluid viscosity becomes dominant over inertia. We explain this paradox via a viscosity-driven, hydrodynamic coupling between the fluid and the particle that activates the rotational inertia of the particle, converting it into a viscous flywheel. 6. Classroom Quality, Classroom Composition, and Age at Entry: Experiences in Early Childhood Education and Care and Single and Dual Language Learners' German Vocabulary

Abstract

We examined independent and interactive links among three central characteristics of children's experiences in early childhood education and care and the German receptive vocabulary of single language learners and dual language learners (DLLs). We allowed for possible differential effects depending on children's language background. Our sample included 2,231 children (n = 1,555 single language learners, n = 371 DLLs from families in which German was frequently spoken, n = 305 DLLs from families in which German was less frequently spoken). Children attended 177 classrooms in 95 early childhood education and care centers and were 30 to 80 months old. We found that classroom process quality predicted German vocabulary only for DLLs with low exposure to German in the family. An earlier age at entry was linked to a larger German vocabulary for all children, but the link was stronger for DLLs from families with low exposure to German. Classroom composition did not predict German vocabulary.

7. Does Variation in Early Childhood Education Matter More for Dual Language Learners' than for Monolingual Children's Language Development?

Abstract

Can early childhood education (ECE) support the societal language development of children from linguistically diverse backgrounds? This study examined how existing variation in classroom interaction quality (CLASS Pre-K), classroom composition (percentages of children from low-income backgrounds and dual language learners [DLLs]), and duration of attending German ECE were related to language trajectories from 30 to 73 months of age (n = 519 children in 154 classrooms). DLL status served as a focal moderator and parental education was considered as an additional moderator. Age-based growth models revealed interactions between DLL status and ECE characteristics. There were numerous intercept effects that endured over the age span studied. Interaction quality (emotional support) and classroom composition (percentage of DLLs) were related only to DLLs' German language growth. Parental education did not moderate ECE effects. Hence, the observed range of variation in German ECE may matter for DLLs' societal language development but may be of less relevance for monolingual children.

8. Do Peers Matter? Peer Effects on Young Children's Vocabulary Gains in German Classrooms

Abstract

Children in early childhood education and care (ECEC) spend a considerable amount of time interacting with their peers. However, open questions remain on whether and how children influence their peers' language development. The present study examined effects of peers' German receptive vocabulary (n = 1,871) on individual children's (n = 431) receptive vocabulary gains. Target children were between 30 and 48 months old at the beginning of the study. Findings revealed no links between peers' vocabulary skills and individual children's vocabulary gains, neither for all children nor depending on children's prior vocabulary skills. However, for children with lower prior skills, there was a negative association between the percentage of dual language learners in the classroom and children's vocabulary gains. This link was not attributable to peers' vocabulary skills. A number of possible mediators and moderators for peer effects were examined but none of them proved to be significant. In sum, these (null-) findings inform future research on compositional and peer effects as well as their underlying processes. Moreover, they have important practical implications for policymakers and ECEC professionals.

9. Longitudinal Interrelations between Nonword Repetition and Vocabulary from Age Three to Five: Evidence for Within-Child Processes?

Abstract

The relation between nonword repetition and vocabulary has been the focus of a theoretical controversy for several decades. The point of contention is whether the ability underlying nonword repetition drives vocabulary growth or vice versa. The present study examines longitudinal interrelations between nonword repetition and vocabulary from age 3 to 5 with random intercept cross-lagged panel models (RI-CLPMs). RI-CLPMs have the advantage of separating within-child dynamic processes from more stable differences between children, including time-stable unmeasured confounders. For n = 260 monolingual German-speaking children assessed at three time points with a lag of eleven months, RI-CLPM and, for comparison purposes, "classical" cross-lagged panel models (CLPMs) were estimated. The ill-fitting CLPMs in which cross-lagged effects combine within-child processes and stable differences between children yielded evidence consistent with reciprocal effects between nonword repetition and vocabulary (without covariates) or from nonword repetition to vocabulary (with covariates). Adding a random intercept markedly improved model fit. All within-child cross-lagged effects in the RI-CLPM were nonsignificant. Thus, the results provided no evidence consistent with within-child processes such as nonword repetition affecting vocabulary or vice versa for preschool-age children. Instead, results are more consistent with, for example, third variable explanations, within-child processes fading out by age 3 or occurring on a time frame that is not captured with a lag of approximately 1 year.

10. Family Literacy Activities and Their Interplay with Family and Preschool Language Environments: Links to Gains in Dual Language Learners' German Vocabulary

Abstract

Research Findings: Are family literacy activities linked to gains in preschool-aged dual language learners' (DLLs') societal language vocabulary? To understand connections between literacy activities and vocabulary, we separately considered literacy activities in the respective heritage language and in the societal language, German, and accounted for heterogeneity in DLLs' family and preschool language environment. DLLs (n = 115) were administered the German PPVT-4 twice (at 40 and 51 months). Literacy activities as well as other family language environment variables were reported by mothers. Classroom language stimulation was observed with the CLASS Pre-K language modeling scale. Two different models of change revealed that family literacy activities in the heritage language predicted gains in German vocabulary for children with low levels of classroom language stimulation (albeit at the p = 0.10-level in one set of models). There was no evidence that literacy activities in German were positively connected to gains in German vocabulary. Practice or Policy: This study suggests that family literacy activities in the heritage language may have the potential to support societal language vocabulary. More research is needed to understand under what circumstances family literacy activities in the societal language are supportive of DLLs' societal language vocabulary.

12. Linking Quantity and Quality of Early Childhood Education and Care to Children's Socio-Emotional Adjustment: A German Cross-Sectional Study

Abstract

This study examined how quantity and quality of early childhood education and care (ECEC) are related to the socio-emotional adjustment of children born in Germany (30-48 months old, N = 395). Previous research focused on a small set of ECEC features, used broad measures, and yielded inconclusive results. We assessed ECEC quantity (age at entry, hours per week), process quality (CLASS Pre-K), structural quality (child-teacher ratio), and classroom composition (percentage of immigrant children). Controlling for child, family, and teacher characteristics, we tested main and interaction effects in regression analyses. Research Findings: More hours per week predicted lower adjustment only on specific indicators (hyperactivity, conduct problems) and only in classrooms with a high percentage of immigrant children or a high child-teacher ratio. In addition, a higher child-teacher ratio predicted more conduct problems, but only for children who entered ECEC at a very young age. Process quality did not predict adjustment. Practice or policy: In sum, this implies that German ECEC does not pose a major risk for children's socio-emotional adjustment, as only certain aspects of quantity and structural quality are linked to very specific socio-emotional outcomes under very specific circumstances.

Abstract

We have altered the superconductivity of a suspended rope of single walled carbon nanotubes, by coating it with organic polymers. Upon coating, the normal state resistance of the rope changes by less than 20 percent. But superconductivity, which on the bare rope shows up as a substantial resistance decrease below 300 mK, is gradualy suppressed. We correlate this to the suppression of radial breathing modes, measured with Raman Spectroscopy on suspended Single and Double-walled carbon nanotubes. This points to the breathing phonon modes as being responsible for superconductivity in carbon nanotubes.

Cet article de l'Académie européenne d'allergologie et d'immunologie clinique (EAACI) fournit des recommandations selon la méthodologie Grading of Recommendations, Assessment, Development and Evaluations (GRADE) pour le diagnostic de l'allergie alimentaire IgE-médiée. Le diagnostic de l'allergie alimentaire commence par une anamnèse clinique axée sur l'histoire allergique du patient, suivie de tests visant à déterminer la sensibilisation tels que la recherche d'IgE sériques spécifiques de l'allergène (IgEs) et les tests cutanés (TC), ainsi que le test d'activation des basophiles (TAB), si ce dernier est disponible. La preuve d'une sensibilisation IgE-médiée doit être recherchée pour tout aliment suspecté. Le diagnostic d'allergie à certains aliments, tels que l'arachide et la noix de cajou, est bien étayé par les TC et les IgEs, alors que l'on dispose de moins de données et que les performances de ces tests sont moins bonnes pour d'autres aliments, tels que le blé et le soja. La mesure des IgE spécifiques aux allergènes moléculaires tels que Ara h 2 de l'arachide, Cor a 14 de la noisette et Ana o 3 de la noix de cajou peut être utile pour affirmer le diagnostic, en particulier chez les personnes sensibilisées aux pollens. Les TAB à l'arachide et au sésame peuvent être utilisés en complément. Le test de référence pour le diagnostic de l'allergie alimentaire est le test de provocation alimentaire orale (TPO). Le TPO doit être réalisé dans les cas douteux. Pour des raisons pratiques, les tests ouverts conviennent dans la plupart des cas. This European Academy of Allergy and Clinical Immunology guideline provides recommendations for diagnosing IgE-mediated food allergy and was developed using the Grading of Recommendations, Assessment, Development and Evaluations (GRADE) approach. Food allergy diagnosis starts with an allergy-focused clinical history followed by tests to determine IgE sensitization, such as serum allergen-specific IgE (sIgE) and skin prick test (SPT), and the basophil activation test (BAT), if available. Evidence for IgE sensitization should be sought for any suspected foods. The diagnosis of allergy to some foods, such as peanut and cashew nut, is well supported by SPT and serum sIgE, whereas there are less data and the performance of these tests is poorer for other foods, such as wheat and soya. The measurement of sIgE to allergen components such as Ara h 2 from peanut, Cor a 14 from hazelnut and Ana o 3 from cashew can be useful to further support the diagnosis, especially in pollen-sensitized individuals. BAT to peanut and sesame can be used additionally. The reference standard for food allergy diagnosis is the oral food challenge (OFC). OFC should be performed in equivocal cases. For practical reasons, open challenges are suitable in most cases. Reassessment of food allergic children with allergy tests and/or OFCs periodically over time will enable reintroduction of food into the diet in the case of spontaneous acquisition of oral tolerance. 37. Multidimensional endotyping using nasal proteomics predicts molecular phenotypes in the asthmatic airways.

Abstract

Unsupervised clustering of biomarkers derived from noninvasive samples such as nasal fluid is less evaluated as a tool for describing asthma endotypes. We sought to evaluate whether protein expression in nasal fluid would identify distinct clusters of patients with asthma with specific lower airway molecular phenotypes. Unsupervised clustering of 168 nasal inflammatory and immune proteins and Shapley values was used to stratify 43 patients with severe asthma (endotype of noneosinophilic asthma) using a 2 "modeling blocks" machine learning approach. This algorithm was also applied to nasal brushings transcriptomics from U-BIOPRED (Unbiased Biomarkers for the Prediction of Respiratory Diseases Outcomes). Feature reduction and functional gene analysis were used to compare proteomic and transcriptomic clusters. Gene set variation analysis provided enrichment scores of the endotype of noneosinophilic asthma protein signature within U-BIOPRED sputum and blood. The nasal protein machine learning model identified 2 severe asthma endotypes, which were replicated in U-BIOPRED nasal transcriptomics. Cluster 1 patients had significant airway obstruction, small airways disease, air trapping, decreased diffusing capacity, and increased oxidative stress, although only 4 of 18 were current smokers. Shapley identified 20 cluster-defining proteins. Forty-one proteins were significantly higher in cluster 1. Pathways associated with proteomic and transcriptomic clusters were linked to T H 1, T H 2, neutrophil, Janus kinase-signal transducer and activator of transcription, TLR, and infection activation. Gene set variation analysis of the nasal protein and gene signatures were enriched in subjects with sputum neutrophilic/mixed granulocytic asthma and in subjects with a molecular phenotype found in sputum neutrophil-high subjects. Protein or gene analysis may indicate molecular phenotypes within the asthmatic lower airway and provide a simple, noninvasive test for non–type 2 immune response asthma that is currently unavailable. Allergen immunotherapy, applied for more than a century, is currently supported by novel modalities such as mAb-based therapies or small molecules targeting the key nodes of the allergic inflammation network. In this review, a summary of the most significant advances in immunotherapy is presented, addressing not only novel approaches to stratifying patients but also major controlled clinical trials and real-world evidence that strengthen the role of immunotherapy in the treatment of allergies. Validated questionnaires are used to assess asthma control over the past 1–4 weeks from reporting. However, they do not adequately capture asthma control in patients with fluctuating symptoms. Using the Mobile Airways Sentinel Network for airway diseases (MASK-air) app, we developed and validated an electronic daily asthma control score (e-DASTHMA).

Abstract

Human activities, primarily the burning of fossil fuels, widespread deforestation, soil erosion or machine-intensive farming methods, manufacturing, food processing, mining, construction, and the iron, cement, steel, and chemicals industries, have been the main drivers of the observed increase in Earth’s average surface temperature and climate change. Rising global temperatures, extreme weather events, ecosystems disruption, agricultural impacts, water scarcity, problems in access to good quality water, food and housing, and profound environmental disruptions such as biodiversity loss and extreme pollution are expected to steeply increase the prevalence and severity of acute and chronic diseases. Its long-term effects cannot be adequately predicted or mitigated without a comprehensive understanding of the adaptive ecosystems. Studying the complex interaction between environmental aggressors and the resilient adaptive responses requires the exposomic and the One Health approaches. The problem is broad and affects the whole ecosystem, plants, pets, and animals in addition to humans. The central role of the epithelial barrier, microbiome, and diet as key pillars for an adaptive tolerogenic immune response should be explored for increasing resilience at the individual level. A radical change in mindset worldwide, with sustainable solutions and adaptive strategies and climate resilience and health equity policies at their center, should be achieved quickly through increased awareness based on solid scientific data.

Abstract

Background: It is unclear whether asthma may affect susceptibility or severity of coronavirus disease 2019 (COVID-19) in children and how pediatric asthma services worldwide have responded to the pandemic. Objective: To describe the impact of the COVID-19 pandemic on pediatric asthma services and on disease burden in their patients. Methods: An online survey was sent to members of the Pediatric Asthma in Real Life think tank and the World Allergy Organization Pediatric Asthma Committee. It included questions on service provision, disease burden, and the clinical course of confirmed cases of COVID-19 infection among children with asthma. Results: Ninety-one respondents, caring for an estimated population of more than 133,000 children with asthma, completed the survey. COVID-19 significantly impacted pediatric asthma services: 39% ceased physical appointments, 47% stopped accepting new patients, and 75% limited patients' visits. Consultations were almost halved to a median of 20 (interquartile range, 10-25) patients per week. Virtual clinics and helplines were launched in most centers. Better than expected disease control was reported in 20% (10%-40%) of patients, whereas control was negatively affected in only 10% (7.5%-12.5%). Adherence also appeared to increase. Only 15 confirmed cases of COVID-19 were reported among the population; the estimated incidence is not apparently different from the reports of general pediatric cohorts. Conclusions: Children with asthma do not appear to be disproportionately affected by COVID-19. Outcomes may even have improved, possibly through increased adherence and/or reduced exposures. Clinical services have rapidly responded to the pandemic by limiting and replacing physical appointments with virtual encounters. © 2020 American Academy of Allergy, Asthma & Immunology

46. Astegolimab (anti-ST2) efficacy and safety in adults with severe asthma: A randomized clinical trial.

Abstract

The IL-33/ST2 pathway is linked with asthma susceptibility. Inhaled allergens, pollutants, and respiratory viruses, which trigger asthma exacerbations, induce release of IL-33, an epithelial-derived "alarmin." Astegolimab, a human IgG 2 mAb, selectively inhibits the IL-33 receptor, ST2. Approved biologic therapies for severe asthma mainly benefit patients with elevated blood eosinophils (type 2-high), but limited options are available for patients with low blood eosinophils (type 2-low). Inhibiting IL-33 signaling may target pathogenic pathways in a wider spectrum of asthmatics. This study evaluated astegolimab efficacy and safety in patients with severe asthma. This double-blind, placebo-controlled, dose-ranging study (ZENYATTA [A Study to Assess the Efficacy and Safety of MSTT1041A in Participants With Uncontrolled Severe Asthma]) randomized 502 adults with severe asthma to subcutaneous placebo or 70-mg, 210-mg, or 490-mg doses of astegolimab every 4 weeks. The primary endpoint was the annualized asthma exacerbation rate (AER) at week 54. Enrollment caps ensured ∼30 patients who were eosinophil-high (≥300 cells/μL) and ∼95 patients who were eosinophil-low (<300 cells/μL) per arm. Overall, adjusted AER reductions relative to placebo were 43% (P =.005), 22% (P =.18), and 37% (P =.01) for 490-mg, 210-mg, and 70-mg doses of astegolimab, respectively. Adjusted AER reductions for patients who were eosinophil-low were comparable to reductions in the overall population: 54% (P =.002), 14% (P =.48), and 35% (P =.05) for 490-mg, 210-mg, and 70-mg doses of astegolimab. Adverse events were similar in astegolimab- and placebo-treated groups. Astegolimab reduced AER in a broad population of patients, including those who were eosinophil-low, with inadequately controlled, severe asthma. Astegolimab was safe and well tolerated. Abstract

Pharmacists are trusted health care professionals. Many patients use over-the-counter (OTC) medications and are seen by pharmacists who are the initial point of contact for allergic rhinitis management in most countries. The role of pharmacists in integrated care pathways (ICPs) for allergic diseases is important. This paper builds on existing studies and provides tools intended to help pharmacists provide optimal advice/interventions/strategies to patients with rhinitis. The Allergic Rhinitis and its Impact on Asthma (ARIA)-pharmacy ICP includes a diagnostic questionnaire specifically focusing attention on key symptoms and markers of the disease, a systematic Diagnosis Guide (including differential diagnoses), and a simple flowchart with proposed treatment for rhinitis and asthma multimorbidity. Key prompts for referral within the ICP are included. The use of technology is critical to enhance the management of allergic rhinitis. However, the ARIA-pharmacy ICP should be adapted to local healthcare environments/situations as regional (national) differences exist in pharmacy care. © 2018 EAACI and John Wiley and Sons A/S. Abstract

Allergen immunotherapy (AIT) is a proven therapeutic option for the treatment of allergic rhinitis and/or asthma. Many international or national practice guidelines have been produced, but the evidence-based method varies and they do not usually propose care pathways. The present article considers the possible role of mobile health in AIT for allergic rhinitis/asthma. There are no currently available validated biologic biomarkers that can predict AIT success, and mobile health biomarkers have some relevance. In the current article, the following aspects will be discussed: patient stratification for AIT, symptom-medication scores for the follow-up of patients, clinical trials, as well as the approach of the European Academy of Allergy and Clinical Immunology.
