Abstract

L-asparaginase is one of the most effective drugs in pediatric and adult acute lymphoblastic leukemia treatment. But drug side effects are an important problem. pancreatitis and pancreatic necrosis are not common (2–18 %) complication, but high chance of severe disease with fatal outcome make to bring in careful attention of pediatric oncologists-hematologists, surgeons, intensivists, radiologists. Recognizing multidisciplinary importance of this problem, at June 21st, 2023 at Morozov Children’s Clinical hospital a Round table on pancreatitis/pancreatic necrosis after L-asparaginase use was organized. This article presents expert recommendations from federal and regional clinics in diagnosis and treatment of such severe complication.

2. Central or distal pancreatectomy in patients with pancreatic tumors: assessment of short‑term outcomes

Abstract

Objective: to compare short-term and long-term postoperative complications between patients who have undergone central pancreatectomy (CP) and distal pancreatectomy (DP).Materials and methods. This retrospective study included patients who had CP for benign pancreatic tumors and tumors of low malignant potential (cases) and patients who had DP for similar reasons (controls). The controls were randomly selected and matched cases for tumor size, presence of diabetes mellitus (Dm), and ASA physical status. we evaluated the incidence of grade ≥III complications (Clavien–Dindo classification), clinically significant pancreatic fistulas, Dm, and impaired exocrine pancreatic function in the late postoperative period.Results. There were 25 cases and 25 controls. Both groups were matched for the main clinical characteristics. Surgeries were significantly longer in the CP groups compared to the DP group (230 min vs 180 min, р < 0.0001). There was no difference in the overall incidence of postoperative complications (9 (36 %) vs 14 (56 %), р = 0.26); there was a trend towards a higher incidence of postoperative complications in the CP group. Two patients after CP (8 %) required repeated surgeries. none of the study participants died. Clinically significant (B and C) pancreatic fistulas were registered in 8 (32 %) and 11 (44 %) patients, respectively (p = 0.56). Two patients in the DP group (8 %) developed impairments of exocrine pancreatic function that required pharmacotherapy. none of the patients developed Dm postoperatively.Conclusion. Despite the fact that CP and DP outcomes were comparable in terms of the main parameters evaluated, severe post-CP complications indicate that there is a need for careful selection of patients for such interventions and further accumulation of experience. Our findings can be used in the subsequent analysis of the experience of different clinics.

3. Safe Interval Randomized Path Planning For Manipulators

Abstract

Planning safe paths in 3D workspace for high DoF robotic systems, such as manipulators, is a challenging problem, especially when the environment is populated with the dynamic obstacles that need to be avoided. In this case the time dimension should be taken into account that further increases the complexity of planning. To mitigate this issue we suggest to combine safe-interval path planning (a prominent technique in heuristic search) with the randomized planning, specifically, with the bidirectional rapidly-exploring random trees (RRT-Connect) - a fast and efficient algorithm for high-dimensional planning. Leveraging a dedicated technique of fast computation of the safe intervals we end up with an efficient planner dubbed SI-RRT. We compare it with the state of the art and show that SI-RRT consistently outperforms the competitors both in runtime and solution cost. 4. The Power of Two in Token Systems

Abstract

In economies without monetary transfers, token systems serve as an alternative to sustain cooperation, alleviate free riding, and increase efficiency. This paper studies whether a token-based economy can be effective in marketplaces with thin exogenous supply. We consider a marketplace in which at each time period one agent requests a service, one agent provides the service, and one token (artificial currency) is used to pay for service provision. The number of tokens each agent has represents the difference between the amount of service provisions and service requests by the agent. We are interested in the behavior of this economy when very few agents are available to provide the requested service. Since balancing the number of tokens across agents is key to sustain cooperation, the agent with the minimum amount of tokens is selected to provide service among the available agents. When exactly one random agent is available to provide service, we show that the token distribution is unstable. However, already when just two random agents are available to provide service, the token distribution is stable, in the sense that agents' token balance is unlikely to deviate much from their initial endowment, and agents return to their initial endowment in finite expected time. Our results mirror the power of two choices paradigm in load balancing problems. Supported by numerical simulations using kidney exchange data, our findings suggest that token systems may generate efficient outcomes in kidney exchange marketplaces by sustaining cooperation between hospitals., Comment: 32 pages

Abstract

This paper considers the initial-boundary value problem for the heat equation with a dynamic type boundary condition. Under some regularity, consistency and orthogonality conditions, the existence, uniqueness and continuous dependence upon the data of the classical solution are shown by using the generalized Fourier method. This paper also investigates the inverse problem of finding a time-dependent coefficient of the heat equation from the data of integral overdetermination condition.

Abstract

We derive relative upper bounds on the effective magnetic moment of Dirac neutrinos from comparison of the standard weak and electromagnetic mechanisms of the neutrino luminosity due to the Compton-like photoproduction of neutrino pairs in a degenerate gas of electrons on the lowest Landau level in a strong magnetic field. Abstract

The potential group method is applied to the n-dimensional Coulomb-Rosochatius potential, whose bound states and scattering states are worked out in detail. As far as scattering is concerned, the S-matrix elements are computed by the method of intertwining operators and an integral representation is obtained for the scattering amplitude. Abstract

In this paper we develop an algebraic technique for building relativistic models in the framework of direct-interaction theories. The interacting mass operator M in the Bakamjian-Thomas construction is related to a quadratic Casimir operator C of a non-compact group G. As a consequence, the S matrix can be gained from an intertwining relation between Weyl-equivalent representations of G.
