72 patients with hormone resistant, progressing prostatic cancer completed a self-administered questionnaire to assess subjective morbidity and quality of life before they were entered into a phase III trial of estramustine (34) vs. mitomycin (38). At least one post-treatment assessment was available in 43 patients. This considerable degree of non-compliance is explained by practical problems related to completion and collection of the questionnaires in these rapidly deteriorating patients. Doctors underestimated subjective morbidity (pain, decreased performance status, nausea) in 30-50% of the cases. Decreased functional status, fatigue and pain were identified as the most frequent major morbidities before study entry. In most patients, treatment did not reduce this morbidity. The routine application of self-administered quality of life questionnaires has considerable practical problems but yields clinically worthwhile information about subjective morbidity. Simple but relevant monitoring of subjective morbidity by the patient should be mandatory in cancer trials where palliation is a major endpoint.

The aim of the study was to determine whether preoperative urodynamic evaluation helps the physiotherapist to adapt preoperative management of patients undergoing radical retropubic prostatectomy (RP) by identifying a group at risk of incontinence.We compared the preoperative urodynamic evaluation of 229 men scheduled for RP with their continence status, evaluated by standardized pad-test and questionnaire, at 6 weeks and 4 months postoperatively.The primary urinary incontinence risk has been obtained for five patient's categories, namely normal, bladder instability, bladder outlet obstruction, hypocontractility, and mixed results. None of the patients diagnosed with detrusor instability and bladder outlet obstruction was continent at six weeks from surgery. At four months, although it improves, the continence status remains significantly poorer than observed in all other groups.Preoperative urodynamic evaluation of patients scheduled for RP allows identifying patients with a high risk of postoperative urinary incontinence.

Androgen ablation has been the standard treatment of symptomatic patients with metastatic prostate cancer for more than 50 years. Within the last 15 years, the introduction of prostate-specific antigen (PSA) has induced a stage migration toward less extensive disease and a dramatic decrease in the proportion of men presenting with N+/M+ disease. Historical studies, conducted during the pre-PSA era, are therefore of limited interest in counseling modern patients. The routine use of radical therapies such as radical prostatectomy and radiotherapy has considerably expanded the problem of timing of endocrine treatment in range and complexity. Advanced disease is now diagnosed in patients with limited involvement of extraprostatic sites and even in patients presenting an isolated elevation of PSA after radical treatment. In the absence of clear guidelines, data from past literature and ongoing modern studies were compiled in the present review in an attempt to generate practical considerations.

To determine the incidence and natural history of stage T1a-T1b prostate cancer in patients undergoing surgery for benign prostatic hypertrophy (BPH), and thus evaluate the effect that recent medical and 'minimally invasive' treatments (which provide no prostate sample for pathological examination) might have on the percentage of patients with unsuspected prostate cancer.A series of 1648 patients undergoing surgery for BPH over a 13-year period were reviewed retrospectively; the period overlapped the introduction of serum prostate specific antigen (PSA) as a detection method.Stage T1 prostate cancer was found in 182 patients (11%), comprising 126 (11%) of 1199 transurethral resections and 56 (12%) of 449 open enucleations. The introduction of systematic PSA assays gradually reduced the mean incidence of T1 cancer from 23% to 7%, with a greater effect on T1b (from 15% to 2%), while the incidence of T1a remained nearly constant (+/-5%). The pathological features of surgical specimens from 43 radical prostatectomies undertaken for T1 tumours were reviewed. Locally advanced disease (stage/=pT3) was apparent in 13% of T1a and 28% of T1b tumours. Amongst the patients electing for surveillance, only 8% of those with T1a progressed within 30-97 months of follow-up (mean progression time 73 months), whereas 29% of those with stage T1b progressed within 36 months of follow-up (mean progression time 17 months).These results show that the use of the PSA assay has decreased but not suppressed the incidence of pT1 prostate cancer, with a greater effect on those tumours at higher risk of progression (T1b). This suggests that the detection of prostate cancer based on PSA and transrectal ultrasonography is appropriate for screening patients and is sufficiently accurate that treatments for BPH that provide no pathological materials can be applied safely.

We report one case of acute testicular torsion following orchidopexy for an undescended testis. A review of the literature reveals only ten similar cases. History of a previous testicular surgery should not preclude the possibility of a torsion in that testicle. We conclude that at orchidopexy for an undescended testis, eversion of the tunica vaginalis is an essential step to avoid any future torsion.

Standard radical nephrectomy entails en bloc removal of the kidney together with Gerota's fascia and the ipsilateral adrenal. Thanks to the refinement of imaging techniques (ultrasound, CT and MRI), smaller tumors are being diagnosed. In addition, direct extension to the adrenal gland or adrenal metastasis can be detected in most cases. This is why several authors reserve adrenalectomy for large and/or upper pole tumors or abnormal appearing glands on preoperative CT-scan. However, preoperative diagnosis is not always accurate. Furthermore, micrometastatic adrenal invasion at the time of nephrectomy and late recurrences in the persistent adrenal have been documented, so that partisans of adrenalectomy only spare the adrenal in exceptional cases. The authors have reviewed several series in the litterature as well as there own, and conclude that ipsilateral adrenalectomy can be omitted for small middle- or lower pole tumors when the adrenal appears normal on CT and during the surgical intervention.

Seventy-seven hypospadias, selected from a series of 504 treated in our institution, were operated using the pediculated transverse "onlay" preputial graft. We try to demonstrate that this technique produces excellent functional and cosmetic results. Chordee can be corrected using extensive dissection of the urethral plate with association, if necessary, of a Nesbit type of dorsal plication. Complications rate, in our hands, is lower in comparison to the technique described by John Duckett, where the urethral plate is divided and a complete urethral tube is performed. We report 4 fistulas, one distal end breakdown of the repair and one postoperative hematoma that had to be revised. Because of this low complication rate, in many cases, the "onlay" pediculated graft replaces advantageously the Duckett type of repair.

The effectiveness of a vacuum erection device was evaluated in a 3-year prospective trial involving 110 men with erectile disorders. The patients ranged from 36 to 75 years of age (average: 61). Fifty-seven had chosen the vacuum device as a first treatment attempt; 27 were on intracavernous self-injections with a vasoactive agent but wanted another treatment; and several treatment modalities had failed in the remaining 26 patients. At the 3-month follow-up, the patients were mailed a questionnaire to evaluate the quality of sexual intercourse with the vacuum constriction device. Partner satisfaction was evaluated in parallel. Improvement in sexual parameters as well as difficulties and side effects induced by the device were investigated. Our results confirm the efficacy of the vacuum constriction device in the treatment of erectile dysfunction in selected patients.

Unexpected severe electrolyte imbalance in the postoperative period. Report two cases. Two cases of inappropriate secretion of antidiuretic hormone are reported. In each patient, the physiopathology is reviewed. Differential diagnosis and treatment of hyponatremia is discussed.

Antenatal and incidentally diagnosed megaureters can be safely treated conservatively after careful work up by combined imaging technics. Serial and repeated imaging are non the less usually necessary to confirm the diagnosis. This aspect could be fastidious and expensive. In case of symptoms, breakthrough infections or loss of kidney function, surgical treatment should be considered and good results can be expected. Refluxing megaureter has to be considered as high grade reflux and surgical approach is more often suggested. The authors have reviewed the experience of 139 patients with megaureters treated in the last decade to illustrate those facts.

We report a case of multiple and bilateral renal angiomyolipomas demonstrated and followed up by urography, sonography, and computed tomography during a period of 17 years. The diagnosis has been made by computed tomography. Follow-up examinations showed first a stability of the lesions, and afterwards, a rather fast increase of the volume of various masses, without symptoms, followed by a new period of stabilization.

Administration of intravesical chemotherapy by mitomycin C (MMC) is an adjuvant treatment of the recurrent superficial bladder cancer. Since use of MMC in 1967, many different toxicities were reported. Those are local toxicities. Systemic toxicity is rare. At the hand of 2 cases reports and recent literature, we discuss the possible complications of intravesical therapy with MMC.

Bladder wall calcification is a rare complication of intravesical therapy with mitomycin C. We report a new case and discuss the pathogenic mechanism of this complication.

A nephrogenic adenoma in a urethral diverticulum has been observed in a 32 years old black woman. The association of both of these abnormalities is relatively uncommon, however symptomatic and source of complications.

Urethral prolapse represent a rare cause of urological consultation. Frequent in older women, it also happens in under 10-year-old little girls with a racial predominance. For most cases, medical treatment and follow-up are sufficient, but failure of these or possible complications may necessitate a surgical correction. Circumferential excision and muco-mucous suture are effective treatments with very few complications. About four case-reports, we are reviewing here the literature and discussing etiopathogenic mechanisms and therapeutic options for this disease.

Varicocele is a benign pathology, for which the less invasive treatment is percutaneous embolisation. Literature reports results from 70 to 85% (1,2). Laparoscopic treatment seems to be a good alternative for patients suffering from a recurrence, or patients who can not be embolised. We report our experience concerning the first 13 patients treated with laparoscopic technic. In all 13 cases it was possible to practice the technic, and patients were cured.

The growth of any tissue depends on the quantitative relationship between the rate of cell proliferation and cell death. In normal adults tissues, this steady-state balance is regulated by a series of both systemic hormones and local growth factors. Programmed cell death, also called apoptosis, is one of the two pathways for cell death. In programmed cell death, specific intracellular signals induce the cell to undergo an active, energy-dependent process leading to a cascade of biochemical and morphologic events that result in the irreversible fragmentation of genomic DNA and then of the cell itself. Biochemical and morphological studies have demonstrated that the involution of the normal, hyperplastic and cancerous prostate after castration is the result of programmed cell death. Numerous group have studied the role of calcium in programmed cell death's triggering. In androgen-independent prostatic cell it had been proved that it was possible to trigger programmed cell death by inducing a sustained elevation of the intracellular calcium. Actually, some features tend to explain more precisely the exact role of calcium in apoptosis of prostatic cells.

In our Institution, 23 patients were selected for percutaneous resection of tumors of the upper urinary tract. Two patients underwent a second procedure: the first for ipsilateral recur and the second for an heterolateral tumor. Among these 23 patients, 11 presented a benign tumor, 9 were treated for a transitional carcinoma of whom 2 invasive state. Both underwent a nephro-ureterectomy. For the 7 superficial transitional carcinomas, the mean follow-up period is 30.4 months. The ipsilateral local recurrencee is 14.3% (1/7), and concern a poorly differentiated tumor (G III). The advantage of the endoscopic techniques to explore and treat the upper urinary tract tumors is double. With these techniques, we can diagnose and treat adequately benign tumors which don't justify radical surgery. In the case of transitional tumors, the percutaneous way is an acceptable method of management when it appears better to avoid a radical treatment (solitary kidney, bilateral synchronous tumors, impairment of total renal function, general injuries). However, it must be performed only in selected cases, and patients should be carefully selected.

The authors describe their experience with the Swash technique. It consists of the measurement of the latency over the distal segment of the motor branch of the pudendal nerve following a transrectal stimulation of the pudendal nerve during a rectal examination. The response is picked up from the anal sphincter by the use of a surface electrode and from the bulbocavernosus muscle with a needle electrode.

If the rate of tumoral relapse after partial nephrectomy for hypernephroma is 8-10%, this element is rarely reported in the literature after radical nephrectomy.159 patients (pts) have undergone radical nephrectomy for kidney adenocarcinoma. Mean age is 61.3 years (26-85 years). The mean duration of follow-up is 50.8 months (12-183 months). The patients were regularly followed by abdominal CT scan at 6 months and than yearly by CT scan and chest X-ray.Considering the poor incidence of local relapse (1.3%) after nephrectomy and in a period where economical aspect weigh on our practice, a new policy of post-operative follow-up should be carried.

We report here on a case of cholesteatoma of the upper urinary tract. This rare, but recurrent disease was managed successfully by means of repeated endourological treatment. Diagnosis was confirmed by histological examination of samples obtained by percutaneous extraction.

The authors report two cases of retroperitoneal fibrosis secondary to rectal perforation occurring during barium enema. In view of the variable interval between the radiological accident and the urological complications, a long patient follow-up is recommended. If ureteric obstruction occurs, ureteric catheterisation is a temporary alternative, but surgery is the treatment of choice. When ureterolysis cannot be performed, the authors propose ureteroileoplasty to restore continuity of the urinary tract.

Urinary continence after radical prostatectomy is frequently evaluated subjectively. The authors describe their technique of vesicourethral anastomosis and report their method of objective evaluation of postoperative urinary continence. At the 6th postoperative week, 15% of patients presented with severe incontinence. This percentage fell to 3.3% by the 16th postoperative week.

The authors describe the principles and indications of sympathetic evoked potentials recorded from the pelvic floor. The technique consists of an electrical stimulation of a somatic nerve (median nerve at the wrist or dorsal nerve of penis) with recording of the electrical activity induced by the sympathetic nerve terminals of the sweat glands from the perineal skin, using surface electrodes.

Optimal management of testicular cancer requires a multidisciplinary approach. This paper reviews the present role of surgery at three different stages of therapy. Radical inguinal orchidectomy is the treatment of the primary tumor, retroperitoneal lymphadenectomy is an essential tool in staging and treatment of low stage non-seminomatous germ cell cancer, and cytoreductive surgery is an indispensable adjunct in post-chemotherapeutic residual masses. These surgical aspects are placed in the perspective of modern therapeutic strategies.

The Authors are reviewing their experience of Obstructive Uropathies diagnosed and treated surgically in neonates, the last 8 years in their Institution. 67 cases were reviewed, in which 37 presented with ureteropelvic junction obstruction (UPJ). 13 with posterior urethral valves, 11 with primary megaureter and 6 with ureterocele. Prenatal ultrasonography allowed the diagnosis to be made in two third of the patients. UPJ is the most common obstructive uropathy observed. Posterior urethral valves the most severe because of high pulmonary and renal (dysplasia) complication rate. Surgery, when indicated, has no more complications to be expected than in general population, if oriented prophylactic measures are taken in the early peri- and postoperative period.

Benign prostatic hypertrophy is a highly prevalent disease, with a significant morbidity. Classical surgical therapy (open or transurethral adenomectomy) is very effective, but not without risks and complications. Better understanding of the pathogenesis of the disease have led to the development of several alternative treatments. Hormonal treatments are briefly reviewed, especially the preliminary results obtained with the 5-alpha-reductase inhibitors.

We report the case of a woman who was found to be HIV positive after a sexual assault, in the absence of other risk factors for HIV infection. This observation and the report of one similar case support the view that HIV may be transmitted to a victim of a single sexual assault. We believe that all victims of rape should be offered a test for HIV and offered counselling. In addition to the current recommendations, the semen obtained should be tested for HIV antibodies by Western blot analysis. In the absence of clear data on safety and efficacy, we should recommend neither routine application of virucidal agents nor systematic Zidovudine (AZT) prophylaxis.

A prospective study comparing a new radioimmunologic and a classical enzymatic assay for prostatic acid phosphatase was done to evaluate their respective roles in patients with prostatic diseases. We studied 50 patients with cancer of the prostate, 101 with benign prostatic hypertrophy and 17 with prostatitis as well as patients with nonprostatic malignancy, and various hematological and bone diseases. The results showed a low incidence of elevated values in patients with early cancer of the prostate and a high incidence of false positive values with the radioimmunoassay in patients with benign prostatic diseases, especially prostatitis. These data suggest that tests for serum prostatic acid phosphatase levels remain disappointing in the assessment of prostatic disease regardless of the technique used.

The authors describe a simple means of destroying the venous valves that oppose countercurrent perfusion of the distal venous segment of a latero-lateral arterio-venous fistula.

Neuromotor pathways from the brain to the pelvic floor have been poorly documented. The recent development of Motor Evoked Potentials may well fill this gap in our basic knowledge. Our technique consists of transcutaneous stimulation of the motor cortex and sacral roots with a magnetic device while recording the evoked response from the bulbocavernosus muscle and anal sphincter. Cortical stimulation is performed first at rest and then during voluntary contraction of the examined muscles ("facilitation" procedure). Sacral root stimulation is performed at rest. Stimulation at 2 different levels allows measurement of the total transit time (brain to muscle transit time) and the peripheral transit time (sacral roots to muscle). By subtracting the latter from the former, the central transit time (brain to sacral roots) is obtained. The technique is painless, and to our knowledge no side effects have been reported. The authors present the preliminary results of this new technique.

Ambiguous genitalia require prompt decision in the neonatal period to give the child the best opportunity to avoid later major problems of sexual identification. A team approach including the neonatologist, the pediatric endocrinologist, the geneticist and the pediatric urologist seems to be the best way to make the right decision. This paper is a review of 48 cases of ambiguous genitalia referred to our center for management. The diagnosis of female pseudohermaphroditism was established in 30 cases with 21 presented with adrenogenital syndrome. Ten patients were considered as having a diagnosis of male pseudohermaphroditism, six presented with gonadal dysgenesis and two were true hermaphrodites. Principles of surgical management for each category of patients is exposed, discussed and will be the way to treat subsequent cases.
