Abstract Background Unicompartmental knee arthroplasty (UKA) is a surgical procedure for managing osteoarthritis of one joint compartment, most commonly the medial side. This systematic review investigates the causes of UKA revision. The outcomes of interest were establishing the revision rate, time to revision, and the most common causes of revision in the long- and midterm follow-up. Methods This study was conducted according to the 2020 PRISMA statement. In October 2024, the following databases were accessed: PubMed, Web of Science, Google Scholar, and Embase. All the clinical studies investigating the rate and causes of revision in UKA were accessed. Only studies with a minimum of 10 years of follow-up were considered. Results Data from 56 studies (13,540 patients) were collected. Of them, 65.6% were women. The mean length of the follow-up was 13.1 ± 3.0 years. The mean age of the patients was 65.6 ± 5.6 years, and the mean BMI was 28.5 ± 2.2 kg/m2. Revisions were performed in 8.8% (2641 of 30,140) of implanted UKAs. The mean time to revision was 6.5 ± 2.6 (range, 2.5 to 13.0) years. Conclusion 8.8% (2641 of 30,140) of UKAs were revised at a mean time of 6.5 ± 2.6 years. Through the case of the amateur painter and engraver Gerolamo Imperiale, a Genoese patrician who, in the Parma of Ranuccio I Farnese, alternated university lectures at the Studium with attendance at the painters' workshops, the article intends to reflect on how amateurs were trained and their relationship with painters and the art market. Ubiquitous in seventeenth-century art literature, young members of the nobility who dabbled in painting, training alongside house painters or in the schools set up in the artists' studios, had the opportunity to engage in close contact with the work of their masters, but without being burdened by the professional constraints affecting collaborators and pupils, or by the impositions of patronage. With this in mind, we intend to examine the particular combinatorial freedom that Gerolamo Imperiale demonstrates in the invention engravings of his Parma years.

Soil salinity is considered one of the most limiting factors in large agricultural areas, and tomato (Solanum lycopersicum L.) is highly sensitive to this abiotic stress, which exacerbates under drought. The use of halotolerant PGPR is a promising strategy to enhance tomato tolerance and productivity in saline soils. The present study tested the capability of the recently discovered bacterial strain PVr_9 to increase tolerance to salinity in tomato plants in vitro, greenhouse and open field conditions. When inoculated with PVr_9, in vitro seedlings showed a significant increase in primary root length, number of secondary roots and fresh weight both in absence of stress and when exposed to 150 mM NaCl. In plants exposed to salt treatment, PVr_9 upregulated the salt tolerance genes SOS1 and NHX1, and the enzyme prolyl aminopeptidase involved in proline metabolism. Proteins associated with resistance against pathogens were also upregulated by PVr_9 in absence of salt stress. In greenhouse, PVr_9-inoculated tomato plants treated for 1-week with 150 mM NaCl showed a significant increase in shoot fresh biomass, chlorophyll, proline content, and APX activity. Tomato fruits from PVr_9 inoculated plants, exposed in the field to salt stress during the flowering stage, showed higher levels of carotenoids, lycopene, and l-ascorbic acid, as well as an increased antioxidant capacity compared to fruits of uninoculated plants. These results indicate PVr_9 as a potential biostimulant in tomato for better tackling soil salinization in a context of climate change and expansion of coastal salinity, while there is large scope for the improvement of the nutritional characteristics of tomato fruits.

ObjectiveThe urgent transfer of an intensive care unit (ICU) is particularly challenging because it carries a high clinical and infectious risk and is a critical node in a hospital’s patient flow. In early 2017, exceptional rainfall damaged the roof of the tertiary hospital in Udine, necessitating the relocation of one of the three ICUs for six months. We decided to assess the impact of this transfer on quality of care and patient safety using a set of indicators, primarily considering the incidence of healthcare-associated infections (HAIs) and mortality rates.MethodsWe performed a retrospective, observational analysis of structural, process, and outcome indicators comparing the pre- and posttransfer phases. Specifically, we analyzed data between July 2016 and June 2017 for the transferred ICU and examined mortality and the incidence of HAI.ResultsDespite significant changes in structural and organizational aspects of the unit, no differences in mortality rates or cumulative incidence of HAIs were observed before/after transfer. We collected data for all 393 patients (133 women, 260 men) admitted to the ICU before (49.4%) and after transfer (50.6%). The mortality rate for 100 days in the ICU was 1.90 (34/1791) before and 2.88 (37/1258) after transfer (p = 0.063). The evaluation of the occurrence of at least one HAI included 304 patients (102 women and 202 men), as 89 of them were excluded due to a length of stay in the ICU of less than 48 h; again, there was no statistical difference between the two cumulative incidences (13.1% vs. 6.9%, p = 0.075).ConclusionIn the case studied, no adverse effects on patient outcomes were observed after urgent transfer of the injured ICU. The indicators used in this study may be an initial suggestion for further discussion.

BackgroundThe phenomenon violence against health professionals has received increasing attention in recent years because of its frequency and significant impact on victims’ mental health and disruption of health services. Despite this attention, little is known about the incidence of workplace violence in the highly politicized immunization services. Therefore, we decided to examine the prevalence of workplace violence in the COVID-19 immunization campaign, the risk and protective factors, and the impact on victims’ mental health.MethodsBetween March and April 2022, we conducted an anonymous online survey among health professionals working in COVID-19 vaccination centers in the Friuli-Venezia Giulia Region (Italy). We used the Questionnaire for Workplace Violence in Healthcare Settings and the Impact of Event Scale–Revised.ResultsOf the 200 participants, 93 (46.5%) reported being victims of an act of violence during the vaccination campaign, 60 of them verbally and 7 physically. In 35.5% of cases, the IES score indicated a possible post-traumatic stress reaction in the victim. The Ornstein–Uhlenbeck (OU) process describes the dynamics of Brownian particles in a confining harmonic potential, thereby constituting the paradigmatic model of overdamped, mean-reverting Langevin dynamics. Despite its widespread applicability, this model falls short when describing physical systems where the confining potential is itself subjected to stochastic fluctuations. However, such stochastic fluctuations generically emerge in numerous situations, including in the context of colloidal manipulation by optical tweezers, leading to inherently out-of-equilibrium trapped dynamics. To explore the consequences of stochasticity at this level, we introduce a natural extension of the OU process, in which the stiffness of the harmonic potential is itself subjected to OU-like fluctuations. We call this model the OU ^2 process. We examine its statistical, dynamic, and thermodynamic properties through a combination of analytical and numerical methods. Importantly, we show that the probability density for the particle position presents power-law tails, in contrast to the Gaussian decay of the standard OU process. In turn, this causes the trapping behavior, extreme value statistics, first passage statistics, and entropy production of the OU ^2 process to differ qualitatively from their standard OU counterpart. Due to the wide applicability of the standard OU process and of the proposed OU ^2 generalisation, our study sheds light on the peculiar properties of stochastic dynamics in random potentials and lays the foundation for the refined analysis of the dynamics and thermodynamics of numerous experimental systems.

Se evaluó el efecto de distintos tratamientos de secado (natural, 50 y 70 °C) y manejo agronómico (sin y con re-fertilización en pre-floración) sobre la dureza endospérmica en cuatro híbridos de maíz (AW 190, NK 940, Cóndor y Mill 522). La dureza se evaluó mediante peso hectolítrico (PH), test de flotación (TF) en NaNO3 y CCl4 y relación de molienda (RM). El ensayo se basó en un diseño factorial. Generalmente, altos valores de PH y RM y bajos de TF están asociados con maíces más duros. Se observó que el secado artificial (50 y 70 °C) disminuyó RM, aumentó TF y no produjo mayores cambios en PH, disminuyendo así la dureza. Los híbridos Cóndor (secado natural) y con re-fertilización (secado a 50 °C), Mill 522 (secado natural y a 50 °C) y en el manejo control (secado a 70 °C) lograron valores de dureza para calidad ‘Premium’. Las técnicas empleadas permitieron clasificar los híbridos por su dureza y evaluar el efecto de la temperatura de secado sobre la misma.

Static nonreciprocal forces between particles generically drive persistent motion reminiscent of self-propulsion. Here, we demonstrate that reciprocity-breaking fluctuations about a reciprocal mean coupling strength are sufficient to generate this behavior in a minimal two-particle model, with the velocity of the ensuing active bound state being modulated in time according to the nature of these fluctuations. To characterize the ensuing nonequilibrium dynamics, we derive exact results for the time-dependent center of mass mean-square displacement and average rate of entropy production for two simple examples of discrete- and continuous-state fluctuations in one dimension. We find that the resulting dimer can exhibit unbiased persistent motion akin to that of an active particle, leading to a significantly enhanced effective diffusivity.

Abstract Purpose In patients with monocompartmental knee osteoarthritis, unicompartmental knee arthroplasty (UKA) can be performed. This study compared the medial versus lateral UKA in patients with monocompartimental knee arthroplasty. It was hypothesised that both implants achieve a similar outcome in OKS. Methods The UKAs were fixed‐bearing medial PPK (Zimmer‐Biomet, Warsaw, Indiana, USA) and fixed‐bearing lateral Zuk (Lima Corporate, Udine, Italy). An intraarticular drain was placed and removed on the first postoperative day. Enoxaparin sodium 4000 units subcutaneously daily for 45 days was used as thromboembolic prophylaxis. The Italian version of the OKS was used for the clinical assessment. The following complications were also recorded: anterior knee pain, infection and revision surgeries. Results Data from 203 patients were collected. The mean age of the patients was 68.9 ± 6.7 years and the mean BMI was 28.1 ± 4.1 kg/m2. The mean OKS on admission was 22.1 ± 4.5 points. On admission, women, patients older than 70 years, and those with a BMI lower than 28 kg/m2 who underwent lateral UKA evidenced lower OKS. At the last follow‐up, 26.7 and 26.9 months for the lateral and medial UKA, respectively, no between groups difference in OKS was evidenced. No patients experienced complications. Conclusion Medial and lateral UKA achieve similar outcomes in OKS at a minimum of two years of follow‐up.

Abstract Background The coverage for influenza vaccination among healthcare workers (HCWs) is inadequate in many countries despite strong recommendations; is there evidence that influenza vaccination is effective in preventing absenteeism? Aim of the study is to evaluate the influenza vaccination coverage and its effects on absences from work among HCWs of an Italian academic healthcare trust during the 2017–2018 influenza season. Methods We performed a retrospective study to identify predictive characteristics for vaccination, and a retrospective cohort study to establish the effect of vaccination on absences among the vaccinated and non-vaccinated cohorts between December 2017 and May 2018. Overall absence rates over the whole observation period and sub-rates over 14-days intervals were calculated; then comparison between the two groups were conducted applying Chi-square test. Results Influenza vaccination coverage among 4419 HCWs was 14.5%. Age, university degree, medical care area and physician profile were positively associated with vaccine uptake. Globally during influenza season non-vaccinated HCWs lost 2.47/100 person-days of work compared to 1.92/100 person-days of work among vaccinated HCWs (p

The current pandemic caused by COVID-19 has underlined the importance of a joint effort and approach to ensure patient and health care worker safety in medical care throughout Europe. In addition, the recent flood disasters in Germany and other countries called for immediate joint action, in this case with regard to the prevention of water-borne infections. Environmental disasters will increase with consequences for hospitals and nursing homes. Cooperative efforts are needed for preventing and controlling associated infection outbreaks, new pathogens will appear and a geographic shift of infectious diseases previously not detected in certain areas has already been observed.This approach to infection prevention and control must entail structural as well as regulatory aspects. The principle of equal protection against infections in all European countries must be implemented. Prevention and control of infections, including nosocomial infections, infections caused by antibiotic-resistant bacteria as well as pandemics, need to be based on equal standards in all of Europe.Protection against infections and other public health risks in all European countries is the best guarantor for building trust and identification of citizens in our common Europe.Experts in the fields of hygiene, microbiology, infectiology and epidemiology have to pool the expertise on the prevention and control of infections from different European countries and define key targets for achieving a high standard of hygiene measures throughout Europe. Objective: Chemotherapy-induced alopecia is a common and distressful side effect, especially among breast cancer patients. Scalp cooling (SC) can reduce hair loss during anthracycline- and taxane-based chemotherapy. This study assessed the effectiveness of SC in daily clinical practice in three Italian oncology units. Methods: From 2014 to 2016, we prospectively included 220 female early-stage breast cancer patients undergoing curative chemotherapy in combination with SC using the Paxman device. A key tool for monitoring breast cancer patients under neoadjuvant treatment is the identification of reliable predictive markers. Ki67 has been identified as a prognostic and predictive marker in ER-positive breast cancer. Ninety ER-positive, HER2 negative locally advanced breast cancer patients received letrozole (2.5 mg daily) and cyclophosphamide (50 mg daily) with/without Sorafenib (400 mg/bid daily) for 6 months before undergoing surgery. Ki67 expression and tumor size measured with caliber were determined at baseline, after 30 days of treatment and at the end of treatment. Patients were assigned to a clinical response category according to Response Evaluation Criteria in Solid Tumors, both at 30 days and before surgery and further classified as high-responder and low-responder according to the median variation of Ki67 values between biopsy and 30 days and between biopsy and surgery time. The predictive role of Ki67 and its changes with regard to clinical response and survival was analyzed. No differences in terms of survival outcomes emerged between the arms of treatment, while we observed a higher percentage of women with progression or stable disease in arm with the combination containing Sorafenib (20.5% vs 7.1%, p = 0.06). Clinical complete responders experienced a greater overall variation in Ki67 when compared with partial responders and patients with progressive/stable disease (66.7% vs 30.7%, p = 0.009). High responders showed a better outcome than low responders in terms of both disease-free survival ( p = 0.009) and overall survival ( p = 0.002). ΔKi67 score evaluated between basal and residual tumor at definitive surgery showed to be highly predictive of clinical complete response, and a potential parameter to be used for predicting disease-free survival and overall survival in luminal breast cancer treated with neoadjuvant endocrine-based therapy.

Questo studio si propone di ripercorrere i contatti tra la scena artistica parmense e quella romana nei primi decenni del XVII secolo per fare luce sui canali attraverso i quali il pittore modenese può aver conosciuto e studiato l'opera del Merisi.

The rate of entropy production by a stochastic process quantifies how far it is from thermodynamic equilibrium. Equivalently, entropy production captures the degree to which global detailed balance and time-reversal symmetry are broken. Despite abundant references to entropy production in the literature and its many applications in the study of non-equilibrium stochastic particle systems, a comprehensive list of typical examples illustrating the fundamentals of entropy production is lacking. Here, we present a brief, self-contained review of entropy production and calculate it from first principles in a catalogue of exactly solvable setups, encompassing both discrete- and continuous-state Markov processes, as well as single- and multiple-particle systems. The examples covered in this work provide a stepping stone for further studies on entropy production of more complex systems, such as many-particle active matter, as well as a benchmark for the development of alternative mathematical formalisms.

Stabilization of ohmic pretreated peach cubes (ohm) in syrup, representative of semifinished fruit products, was finalized by ohmic heating (OHM) and high pressure processing (HPP), proposed respectively as thermal and nonthermal pasteurization, in comparison to a conventional pasteurization treatment (DIM). The samples were then studied in terms of histological, physical (dimensional distribution, tenderometry, texture, viscosity of syrup and colour), chemical (total phenolic and ascorbic acid content), and sensorial (triangle test) properties. Severe modifications of the cell walls were observed in ohm-DIM and ohm-OHM samples, with swelling and electroporation, respectively. From chemical analyses, significant reduction of ascorbic acid and simultaneous increase of total phenolics content were observed for ohm-DIM and ohm-OHM, probably in relation to the cell wall damages. ohm-HPP showed the best preservation of the dimensional characteristics and hardness, followed by ohm-OHM and ohm-DIM. In addition, textural and colour parameters evidenced similar results, with ohm-HPP as the less different from ohm. Finally, the sensorial analysis confirmed ohm-HPP and ohm-OHM samples as the most similar to ohm as well as the most appreciated in terms of colour and consistency.

The 2018 West Nile Virus (WNV) season in Europe was characterized by an extremely high infection rate and an exceptionally higher burden when compared to previous seasons. Overall, there was a 10.9-fold increase in incidence in Italy, with 577 human cases, 230 WNV neuroinvasive diseases (WNNV) and 42 WNV-attributed deaths. Methods: in this paper we retrospectively reported the neurological presentation of 7 patients admitted to University Hospital of Udine with a diagnosis of WNNV, especially focusing on two patients who presented with atypical severe brain stem involvement. Conclusions: the atypical features of some of these forms highlight the necessity to stay vigilant and suspect the diagnosis when confronted with neurological symptoms. We strongly encourage clinicians to consider WNNV in patients presenting with unexplained neurological symptoms in mild climate-areas at risk.

Among observables characterizing the random exploration of a graph or lattice, the cover time, namely, the time to visit every site, continues to attract widespread interest. Much insight about cover times is gained by mapping to the (spaceless) coupon collector problem, which amounts to ignoring spatiotemporal correlations, and an early conjecture that the limiting cover time distribution of regular random walks on large lattices converges to the Gumbel distribution in d≥3 was recently proved rigorously. Furthermore, a number of mathematical and numerical studies point to the robustness of the Gumbel universality to modifications of the spatial features of the random search processes (e.g., introducing persistence and/or intermittence, or changing the graph topology). Here we investigate the robustness of the Gumbel universality to dynamical modification of the temporal features of the search, specifically by allowing the random walker to “accelerate” or “decelerate” upon visiting a previously unexplored site. We generalize the mapping mentioned above by relating the statistics of cover times to the roughness of 1/f^{α} Gaussian signals, leading to the conjecture that the Gumbel distribution is but one of a family of cover time distributions, ranging from Gaussian for highly accelerated cover, to exponential for highly decelerated cover. While our conjecture is confirmed by systematic Monte Carlo simulations in dimensions d>3, our results for acceleration in d=3 challenge the current understanding of the role of correlations in the cover time problem.

With the wide adoption of the Internet, organizations establish collaborative networks to execute Collaborative Business Processes (CBPs). Current approaches of Process-Aware Information Systems (PAISs) to implement and execute CBPs have shortcomings: high costs and complexity of IT infrastructure to deploy the PAISs; poor support for organization autonomy, decentralized execution, global view of message exchange, and peer-to-peer interactions; and rigid platforms for generating and deploying PAISs on-demand according to the CBPs agreed in collaborative networks. To overcome these issues, this work proposes a cloud-based platform for the management of CBPs. The platform provides cloud services that enable the generation and deployment on-demand of the PAISs required to implement the agreed CBPs, as well as the execution on-demand of CBPs by fulfilling the abovementioned issues. The use of cloud computing also implies to deal with privacy, elasticity, and portability issues; the platform offers proper approaches to deal with them.

Os fatores naturais e, em especial os antrópicos, que contribuem para a variação espacial e temporal da qualidade da água superficial nas bacias hidrográficas do município de Caxias do Sul foram determinados com uso de técnicas multivariadas de análise de dados. Foi utilizada a técnica de Análise do Componente Principal (ACP) como base para o estudo. O monitoramento foi realizado em 30 pontos de monitoramento no período compreendido entre setembro de 2012 a janeiro de 2014. A amostragem foi realizada bimestralmente, abrangendo seis bacias hidrográficas, onde foram analisados 21 parâmetros físicos, químicos e biológicos. Os resultados obtidos demonstraram que com o emprego da ACP, foi possível explicar uma variância total de 71,3% para os dados de qualidade de água. Ainda, constatou-se que os principais fatores que contribuem para a variação da qualidade da água nas bacias hidrográficas da região são a poluição doméstica e industrial, sobretudo do setor galvonotécnico. Foi verificada por fim, uma tendência à atenuação dos poluentes nos corpos hídricos à jusante das áreas urbanas e de grande influência antrópica, especialmente na medida em que há menor pressão das áreas urbanizadas sobre as bacias que drenam a região.

Este trabajo forma parte de una tesis de Licenciatura en Enseñanza de las Ciencias Naturales que se propone diagnosticar la situación de estudiantes preuniversitarios de la ciudad de Olavarría en relación con la enseñanza y el aprendizaje de estas ciencias. El análisis pretende identificar factores que, según otros estudios realizados en contextos diferentes, parecerían estar relacionados a la elección de una carrera de Ingeniería-Tecnología. La muestra con la que se trabajó incluyó alrededor de 400 estudiantes de educación secundaria. Se utilizó una encuesta especialmente diseñada. En este trabajo se muestra una parte del análisis, referido a los resultados obtenidos para los aspectos: Experiencia de aprendizaje en Ciencias Naturales y Elección vocacional.

The aim of this study was to explore prematurity as a psycho-social process from the point of view of both parents and healthcare professionals. We conducted a Grounded Theory study through semi-structured interviews. Participants were from an Italian Neonatal Intensive Care Unit, including parents of premature children, and health professionals. Interviews were analyzed according to Grounded Theory coding strategies, to hypothesize a theoretical model. On one side, behaviors, emotions, and feelings of parental couples; on the other one, the activities and reactions of professionals have been interpreted in a four-phase negotiated process: 1) the initial breakup; 2) the floating family; 3) the event processing; 4) the reconstruction of a new family. Managing prematurity is a negotiated process which could be better managed through a family-centered approach as fundamental in neonatal and pediatric services. Being open in listening, coherent in the given answers, and knowing the entire process and story of being suddenly parents of a premature child can make the difference in the future definition of a new family. Perinatal care would benefit from the following suggested practical implications: i) reducing feelings of separation and solitude by integrating diverse professionals around the family systems; ii) expediting prematurity-related changes through peer-support during hospitalization and attention to rooms' organization; iii) enhancing information exchange between all professionals involved in the care of dyads/parents and new-borns, starting since the pregnancy and continuing after the child's birth by implementing multidisciplinary meetings or appropriate care pathways. • Prematurity represents the leading cause of death in first weeks of life. • Parents face hard-hitting challenges and NICU may represent a place where their expectations are dashed. • Literature on prematurity is limited to distinct perspectives, leaving out the involved actors' process. • A Grounded Theory has been conceptualized to explain the parents and professionals' process around prematurity. • This study may improve perinatal care by suggesting integrating professionals into the family systems.
