Objetivo: Describir las características topográficas, paquimétricas y aberrométricas en pacientes con queratocono (QC), queratocono subclínico (QCS) y córneas normales y del QC según la limitación visual (LV). Elaborar un modelo diagnóstico de QCS y otro modelo de LV para el QC.Métodos: Estudio transversal en el que se han analizado las variables topográficas, paquimétricas y aberrométricas obtenidas por cámara de Scheimpflug rotatoria (tipoPentacam). Se incluyeron 205 ojos repartidos en 82 córneas normales, 40 QCS y 83 QC establecidos. Además, los 123 QCS y QC se distribuyeron según la presencia o no de LV (agudeza visual con corrección > 0,05 unidades logMAR o < 0,9 en escala decimal). Se realizó un análisis descriptivo y bivariante de los datos registrados. Se elaboró un modelo diagnóstico de QCS y otro para la LV del QC con las variables obtenidas con el topógrafo corneal. Resultados: Se obtuvieron diferencias estadísticamente significativas al comparar córneasnormales con QC en casi todas las variables recogidas, pero al comparar los controles con QCS, solo se vieron diferencias estadísticamente significativas en variables de asimetría vertical y espesor corneal. El modelo para el diagnóstico de QCS propuesto incluiría elespesor corneal mínimo, el coma anterior a 90º y el coma posterior a 90º. El modelo de LV en QC incluiría el equivalente esférico, la queratometría más curva de la cara posterior y el coma posterior a 90º. Conclusiones: El diagnóstico de QCS vendrá dado por los parámetros paquimétricos de adelgazamiento corneal y aberrométricos como el coma anterior a 90º y coma posterior a 90º. El modelo de LV en QC vendrá dado por parámetros refractivos (equivalenteesférico) y por las características de la superficie corneal posterior (queratometría más curva y coma a 90º). Abstract: Propose: To describe the topographic, pachymetric and aberrometric characteristics in patients with keratoconus (KC), subclinical keratoconus (SKC) and normal corneas and KC according to visual limitation (VL). To elaborate a diagnostic model of SKC and another model of keratoconus VL Methods: Cross-sectional study in which topographic, pachymetric and aberrometric variables obtained by rotating Scheimpflug camera (Pentacam type) have been studied. Itincluded 205 eyes distributed in 82 normal corneas, 40 SKC and 83 established KC. Moreover, the 123 SKC and KC were distributed according to the presence or not of VL(visual acuity with correction > 0.05 logMAR units or < 0.9 in decimal scale). A descriptive and bivariate analysis of the recorded data was performed. A diagnostic modelof SKC and another one for the KC VL with the variables obtained with the corneal topographer was elaborated.Results: Statistically significant differences were obtained when comparing normal corneas with KC in almost all variables collected, but when comparing controls with SKC, statistically significant differences were only seen in variables of vertical asymmetry and corneal thickness. The model for the diagnosis of SKC proposed would include minimum corneal thickness, anterior coma to 90º and posterior coma to 90º. The KC VL model would include spherical equivalent, posterior steepest keratometry and posterior coma to 90º. Conclusions: The diagnosis of SKC will be given by the pachymetric parameters of corneal thinning and aberrometry parameters like anterior coma to 90º and posterior coma to 90º. The KC VL model will be given by refractive parameters (spherical equivalent)and by the characteristics of the posterior corneal surface (steepest keratometry and coma to 90º).
