The term Open Educational Resources (OER) was coined in 2002 in discussions at the Forum on the Impact of Open Courseware UNESCO. It describes "the provision of educational resources on open license, enabled by information technologies and communication, for consultation, use and adaptation by a community of users with non-commercial purposes".In June 2012, Community OER and UNESCO celebrated 10 years in the area of Open World Congress of Educational Resources in Paris, where the Declaration REA Paris 2012 was formally adopted. This calls on governments around the world to establish the adoption of open licenses for sharing knowledge produced with public funds.This trilingual Compendium (written in English, Spanish and Portuguese) aims to fill part of the need for institutionalized information, and aims to discuss in a clear, didactic and realistic way, the experiences of selected higher education institutions who have offered OER. This document provides a unique approach. It combines remarkable initiatives in the institutional development of REA in Higher Education institutions in the United Kingdom, the Netherlands and Spain with experiences from Higher Education institutions and universities from Brazil, Mexico, and Ecuador, which serve to enlighten the compendium with their innovative ideas and projects.This document has been developed in close collaboration with experts, researchers and decision makers from more than 10 universities in order to provide a global perspective of the OER movement. Some of the more relevant trends identified in this study are exemplified by a broad range of initiatives, including: change in the organizational culture; flexible certification; new business models; middle term institutional strategy; incentives policy; use of non-commercial open source or self-develop platforms; focus on champions; decentralized – federated solutions; open standard, bibliometric criteria; search engine optimization; community building and peer-based collaboration; quality assurance; repurposing and licensing; and open publishing policies.

Bacterial peritonitis is a severe complication in patients with cirrhosis and ascites and despite antibiotic treatment, the inflammatory response to infection may induce renal dysfunction leading to death. This investigation evaluated the effect of TNF-a blockade on the inflammatory response and mortality in cirrhotic rats with induced bacterial peritonitis treated or not with antibiotics. Sprague-Dawley rats with carbon-tetrachloride-induced cirrhosis were treated with an intraperitoneal injection of 109 CFU of Escherichia coli diluted in 20 mL of sterile water to induce bacterial peritonitis and randomized to receive subcutaneously-administered placebo, ceftriaxone, anti-TNF-a mAb and ceftriaxone, or anti-TNF-a mAb alone. No differences were observed between groups at baseline in respect to renal function, liver hepatic tests, serum levels of nitrite/nitrate and TNF-a. Treatment with ceftriaxone reduced mortality (73.3%) but differences did not reach statistical significance as compared to placebo. Objetivo: la hemorragia digestiva es una complicación frecuente en pacientes con cirrosis hepática. La tasa de recidiva tras un primer episodio oscila en torno al 60%, motivo por el cual la profilaxis está recomendada. Esta puede realizarse mediante fármacos (betabloqueantes y nitratos) combinados o no con ligadura endoscópica con bandas. El objetivo de este estudio es valorar el coste-efectividad de la medición del gradiente de presión venosa hepática (GPVH) previo a la elección de la profilaxis secundaria. Métodos: creamos un árbol de decisión para calcular el coste-efectividad de dos estrategias: grupo 1: pacientes a los que se les determinó el GPVH; cuando tras la administración de propranolol hubo una disminución del gradiente ≥ al 20% respecto al inicial o disminuyó por debajo de 12 mmHg, los pacientes fueron tratados con propranolol. Si no hubo tal variación del GPVH, se realizó ligadura endoscópica de las varices. Grupo 2: en este grupo no se monitorizó el GPVH. Los pacientes con varices grado I recibieron tratamiento con betabloqueantes más nitratos y los que presentaban varices grandes (II, III, IV) fueron tratados con betabloqueantes y LEB. Resultados: en el grupo del estudio hemodinámico respondió un 36%, estos recibieron tratamiento betabloqueante, la tasa de resangrado fue del 0%. En los no respondedores la tasa de resangrado fue de un 17%. En el grupo sin estudio se trató con propranolol más nitratos al 28,42% y resangraron un 25%; la tasa de resangrado en el grupo que recibió tratamiento con betabloqueantes más ligadura endoscópica fue de un 13%. El coste total en el grupo al que se realizó el estudio hemodinámico fue de 14.100,49 euros y de 14.677,16 euros para el grupo sin estudio hemodinámico. Conclusiones: la realización del estudio hemodinámico es una herramienta coste-efectiva en la profilaxis de la hemorragia digestiva varicosa en pacientes cirróticos y mantiene una relación coste efectiva favorable comparado con la no realización del mismo. Objective: variceal rebleeding is common following a first episode of hemorrhage in cirrhotic patients. The objective of this study was to determine the cost-effectiveness of monitoring hepatic venous pressure gradient (HVPG) to guide secondary prophylaxis. Methods: we created a Markov decision model to calculate cost-effectiveness for two strategies: Group 1: HVPG monitoring to decide treatment -when portal pressure was reduced by at least 20 percent or HVPG was less than 12 mmHg after beta-blocker administration, patients received beta-blockers; when portal pressure did not meet these criteria therapy was endoscopic band ligation. Group 2: in this group there was no monitoring of HVPG. Patients with large varices received treatment with beta-blockers combined with EBL; patients with small varices received beta-blockers plus isosorbide mononitrate. Results: there was no recurrent variceal bleeding in group 1 for good responders, and for 17% of poor responders. In group 2 a 25% rebleeding rate was detected in patients with small varices and 13% for those with big varices. Overall cost in group 1 was 14,100.49 euros, and 14,677.16 in group 2. Conclusions: HVPG measurement is cost-effective for the secondary prophylaxis of variceal bleeding.

Background: the association of somatostatin (SMT) with endoscopic therapy in patients with cirrhosis and variceal bleeding significantly improves the control of the bleeding episode, and hemodynamic data have shown that a dosage of 500 µg/h allows a more marked reduction of portal pressure versus the usual dosage of 250 µg/h. Aim: to assess if the 500 µg/h dosage is associated with an improved outcome. Methods: sixty-two patients with variceal bleeding were included in the study. Patients were randomized to receive the usual dosage of SMT (group I: 250 µg/h), or a double dosage (group II: 500 µg/h), together with emergency endoscopic sclerotherapy. Results: the control of the bleeding episode was similar in both groups of patients. Early rebleeding was less frequent in patients receiving double vs. single dosage of SMT (p = 0.06). When considering patients with advanced liver disease (Child-Pugh B or C) early rebleeding was significantly less frequent in patients receiving the 500 µg/h dose of SMT (39 vs. 13%, p = 0.03). Conclusions: the perfusion of higher doses of SMT (500 µg/h) in association with emergency sclerotherapy in patients with cirrhosis and esophageal hemorrhage significantly decreases the rate of early rebleeding in patients with more advanced stages of liver disease.
