Purpose: The purpose of this study was to assess in vitro Er:YAG laser efficacy for composite resin removal, as well as the thermal alterations occurring during laser irradiation.Materials and Methods: Cavity preparations (1.0 mm deep) in bovine teeth were restored with resin and divided into 2 groups: the control group, in which restorations were removed with a high-speed bur, and the experimental group, in which restorations were removed with Er:YAG laser at 250 mJ output energy, 80 J/cm2 energy density, and 6 Hz pulse repetition rate. The removal time was measured and the temperature alteration was checked. After the removal, the specimens were split in the middle, and the surrounding and deep walls were analyzed to check for the presence of restorative material.Results: The results revealed that the temperature rise during composite resin removal in both groups occurred on the substrate underneath the restoration. Although the temperature rose during composite filling removal, none of the groups presented a temperature increase higher than 5.6°C, which is considered the critical temperature increase above which there may be irreversible thermal damage to the pulp. Regarding the time for composite filling removal, it was observed that the laser group required more time than the control group for complete elimination of the material from the cavity walls.Conclusion: Although there was a greater temperature increase during composite resin removal with Er:YAG, it did not reach the critical temperature value above which there may be irreversible thermal damage to the pulp. Introduction: A systematic review and meta-analysis were conducted to evaluate the physicomechanical properties of tertiary monoblock obturation with different obturation techniques., Methods and Materials: PubMed (MEDLINE), Web of Science, Scopus, the Cochrane Library, LILACS, IBECS, and BBO were searched time. PICO question was: "In extracted human teeth (Population), does tertiary monoblock obturation (Intervention) have superior physicomechanical properties (Outcome) compared to conventional obturation systems (Comparison)?". Statistical analyses for push-out bond strength were performed with RevMan software by comparing the mean differences of each study, with a 95% confidence interval. Inverse variance was used as statistical method, random-effects models as analysis model, and heterogeneity between studies was assessed by Cochran's Q test and I 2 statistic ( P  <0.05)., Results: Of 2162 studies retrieved, 31 were included in this review for "Study Characteristics". Ten studies were included in the meta-analysis. Analysis demonstrated that conventional obturation had significantly higher push-out bond strength than tertiary monoblock obturation ( P  <0 .01), with a mean difference of -1.00 (95% CI, -1.41 to -0.58; I 2 =100%). Subgroups using single-cone and cold lateral condensation techniques showed significantly lower push-out bond strength for tertiary monoblock obturation ( P  <0.01), respectively with a mean difference of -0.09 (95% CI, -1.13 to -0.67; I 2 =97%) and of -1.97 (95% CI, -3.19 to -0.75; I 2 =100%). The warm vertical compaction subgroup showed no statistically significant difference between tertiary monoblock and conventional systems ( P  =0.13), with a mean difference of 0.49 (95% CI, -0.14 to 1.12; I 2 =10%)., Conclusion: Tertiary monoblock systems have a push-out bond strength similar to conventional systems when used with warm vertical compaction., Competing Interests: ‘None declared’.

This study presents an overview of the accuracy of cone beam computed tomography (CBCT) compared with micro-computed tomography (μCT) in the assessment of root canal morphology of extracted human permanent teeth. A database search in PubMed, PubMed Central, Embase, Scopus, Opengrey, Scielo and Virtual Health Library was conducted which compared root canal morphology of extracted human permanent teeth on the accuracy of CBCT with μCT. In accordance with PRISMA statement guidelines, data were extracted on study characteristics, target mediators, sampling and assay techniques and the parameters associated with obtaining the image and ability to identify the root canal morphology. Amongst 2734 records, ten fulfilled the inclusion criteria. Four studies compared the accuracy of CBCT and μCT in the assessment of root canal morphology using Vertucci's classification, with at least one CBCT group or subgroup of each study presented high agreement compared to the μCT. Six studies assessed more detailed root canal morphology, including two articles that found a lack of agreement between these imaging systems. Risk of bias was deemed low in three studies, moderate in four and high in three. CBCT can be as accurate as μCT in the assessment of several morphological features of extracted human permanent teeth; however there are some exceptions related to the more detailed morphological aspects. Voxel size likely influences the ability to detect these features, though the different aspects of exposure setting used in studies components may be confounding factors. CBCT may be considered for the assessment of root canal morphology ex-vivo.

The Brazilian Dental Journal (BDJ) was officially launched in 1990, stimulated by the courage and boldness of researchers dedicated to teaching and research in dentistry. The journal was conceived in a worldwide coverage and universal language to allow publication of the results of Brazilian studies, which otherwise would not be accessible to the scientific dental community. In the year we celebrate the thirtieth anniversary of BDJ, this article presents a brief overview of Brazilian dental research and a bibliometric analysis of the articles published in this journal as a contribution to our readers and fellow researchers. The purpose was to identify the mot frequent categories of study, the most published areas of dentistry and BDJ's top 50 most-cited articles in the Scopus and Google Scholar databases. A search was performed on all BDJ online issues published from 1990 to 2019. In this period, BDJ published 1,710 articles. Based on their distribution by category of study, 557 articles were in the basic research/dental materials area, 527 in the basic research/biology area and 280 in the clinical research area. Eight articles were cited more than 100 times in the Scopus database and 266 times in the Google Scholar database. Endodontics was the most published area. This overview of BDJ production over those 30 years allows establishing a profile of the characteristics, impact and trends of the published studies, as well as the journal's contribution to the top 50 most-cited articles in the Scopus and Google Scholar databases.

Objective: The aim of the present study was to characterize a novel resin-based monoblock endodontic obturation system consisting of a polymeric cone and a resin-based endodontic sealer., Methods: The preliminary tests performed for the experimental cone were as follows: cohesive strength, dimensional stability, standardization of the diameter and taper, calcium ion release, and radiopacity, for the characterization of the experimental sealer, film thickness and flow tests were performed. Tests were performed according to the American National Standards Institute/American Dental Association (ANSI/ADA) Standards Nos. 57 and 78. The experimental cone was compared to gutta-percha, whereas the experimental sealer was compared to AH Plus. Data were analyzed by Student's t -test ( α  = 0.05)., Results: The experimental cone had superior values for cohesive strength and dimensional stability compared to gutta-percha. Regarding calcium ion release, the experimental system continued to release calcium ions after 30 days. Film thickness was similar for both endodontic sealers; conversely, the experimental resin-based sealer achieved higher values for flow compared to AH Plus., Conclusion: The experimental resin-based monoblock obturation system fully met the requirements of the ANSI/ADA Standard No. 78 and the ANSI/ADA Standard No. 57, except for radiopacity. Aim: To evaluate the wear of root wall surfaces, the bond strength of sealers to dentine and the demineralization around root filling materials after canals were exposed to acid challenge in situ., Methodology: Eighty-seven roots of mandibular incisors were selected. Thirty-two were used in the laboratory bond strength study (n = 8), and 55 in the in situ study (n = 11). Root canals were prepared biomechanically and then filled with gutta-percha and AH Plus, MTA Fillapex, Sealapex or Endofill. For 14 days, 11 participants used intra-oral devices with five sterilized roots (four experimental and one control - only canal prepared). Drops of sucrose were dripped onto roots allowing the accumulation of biofilm on canal surfaces. Roots were removed, sectioned and analysed for the following: bond strength of filling material using a push-out test and also wear profile and dentine demineralization using confocal microscopy. Bond strength (MPa) was evaluated by two-way anova and Tukey test (α = 0.05), and wear profile was assessed by Kruskal-Wallis and t-tests (α = 0.05)., Results: AH Plus had the highest bond strength values. Intermediate results were found in roots with MTA Fillapex and Endofill, whilst Sealapex had inferior results (P < 0.05). No significant differences were found amongst root thirds (P > 0.05). For wear profile, samples were associated with degradation of the filling materials after exposure to the oral environment (P < 0.05). Roots had signs of demineralization around the filling material when Sealapex and Endofill were used., Conclusions: Sealers were not able to prevent degradation of the adhesive interface and dentine. Aim: To evaluate the effect of ultrasonic and sonic activation of two root canal sealers on interfacial adaptation and push-out bond strength (BS) to root canal dentine., Methodology: The root canals of seventy-eight roots of maxillary canines were prepared with NiTi rotary instruments under irrigation with 2.5% NaOCl and 17% EDTA. Canals were filled by lateral condensation of gutta-percha and AH Plus (n = 39) or MTA Fillapex (n = 39). Three subgroups (n = 13) were formed according to sealer activation: no activation (NA), sonic activation (SA, 20 s) and ultrasonic activation (US, 20 s). In three specimens of each subgroup, 0.1% rhodamine B was added to the sealer. Three 1-mm-thick slices were obtained from each root third. The first and second were used for push-out BS test and the third for a qualitative analysis of interfacial adaptation (gaps) and voids by confocal laser scanning microscopy (CLSM). Intratubular penetration of rhodamine B-labelled sealer was also assessed by CLSM. Data were analysed by two-way anova and Tukey's test (α = 0.05)., Results: AH Plus (2.58 ± 1.21) had higher push-out BS than MTA Fillapex (1.45 ± 0.71) (P = 0.000). UA (2.64 ± 1.44) had higher BS (P = 0.000) than NA (1.58 ± 0.73) and SA (1.83 ± 0.85), which did not differ significantly from each other (P = 0.3303). The coronal third had higher BS (2.48 ± 1.49) than the middle (2.08 ± 0.94) (P = 0.0003) and apical (1.48 ± 0.58) (P = 0.00) thirds. For both sealers, UA was associated with a more homogeneous filling material with better interfacial adaptation and greater quantity, density and length of tags in dentinal tubules. When UA was used, the tags had similar quantity, density and length to those formed without activation, but interfacial gaps and voids were observed., Conclusion: Ultrasonic activation was associated with higher BS values, deeper intratubular penetration and greater interfacial adaptation to root dentine than sonic activation and no activation techniques, AH Plus had higher BS values than MTA Fillapex irrespective of the type of activation., (© 2017 International Endodontic Journal. The aim of this study was to evaluate morphological features of 70 single-rooted mandibular first premolars with radicular grooves (RG) using micro-CT technology. Teeth were scanned and evaluated regarding the morphology of the roots and root canals as well as length, depth and percentage frequency location of the RG. Volume, surface area and Structure Model Index (SMI) of the canals were measured for the full root length. Two-dimensional parameters and frequency of canal orifices were evaluated at 1, 2, and 3 mm levels from the apical foramen. The number of accessory canals, the dentinal thickness, and cross-sectional appearance of the canal at different root levels were also recorded. Expression of deep grooves was observed in 21.42% of the sample. Mean lengths of root and RG were 13.43 mm and 8.5 mm, respectively, while depth of the RG ranged from 0.75 to 1.13 mm. Mean canal volume, surface area and SMI were 10.78 mm3, 58.51 mm2, and 2.84, respectively. Apical delta was present in 4.35% of the sample and accessory canals were observed mostly at the middle and apical thirds. Two-dimensional parameters indicated an oval-shaped cross-sectional appearance of the root canal with a high percentage frequency of canal divisions (87.15%). Canal configuration type V (58.57%) was the most prevalent. C-shaped configuration was observed in 13 premolars (18.57%), whereas dentinal thickness ranged from 1.0 to 1.31 mm. Radicular grooves in mandibular first premolars was associated with the occurrence of several anatomical complexities, including C-shaped canals and divisions of the main root canal.

Objectives: Chitosan has been widely investigated and used. However, the literature does not refer to the shelf life of this solution. This study evaluated, through the colorimetric titration technique and an analysis of dentin micro-hardness, the shelf life of 0.2% chitosan solution., Materials and Methods: Thirty human canines were sectioned, and specimens were obtained from the second and third slices, from cemento-enamel junction to the apex. A 0.2% chitosan solution was prepared and distributed in 3 identical glass bottles (v1, v2, and v3) and 3 plastic bottles (p1, p2, and p3). At 0, 7, 15, 30, 45, 60, 90, 120, 150, and 180 days, the specimens were immersed in each solution for 5 minutes ( n = 3 each). The chelating effect of the solution was assessed by micro-hardness and colorimetric analysis of the dentin specimens. 17% EDTA and distilled water were used as controls. Data were analyzed statistically by two-way and Tukey-Kramer multiple comparison ( α = 0.05)., Results: There was no statistically significant difference among the solutions with respect to the study time ( p = 0.113) and micro-hardness/time interaction ( p = 0.329). Chitosan solutions and EDTA reduced the micro-hardness in a similar manner and differed significantly from the control group ( p < 0.001). Aim: To evaluate the efficacy of four final irrigation protocols on the reduction of hard-tissue debris accumulated within the mesial root canal system of mandibular first molars using micro-CT analysis., Methodology: Forty mesial roots of mandibular molars with a single and continuous isthmus connecting the mesiobuccal and mesiolingual canals (Vertucci's Type I configuration) were selected and scanned at a resolution of 8.6 μm. Canals were enlarged sequentially using WaveOne Small and Primary instruments activated in reciprocating motion without intracanal irrigation to allow debris to accumulate within the mesial root canal system. Then, specimens were anatomically matched and distributed into four groups (n = 10), according to the final irrigation protocol: apical positive pressure (APP), passive ultrasonic irrigation (PUI), Self-adjusting File (SAF) and XP-endo Finisher (XPF). The final irrigation procedures were performed over 2 min using a total of 5.5 mL of 2.5% NaOCl per canal. Reconstructed data sets were coregistered, and the mean percentage reduction of accumulated hard-tissue debris after the final irrigation procedures was compared statistically between groups using the anovapost hoc Tukey test with a significance level set at 5%., Results: Reduction of accumulated hard-tissue debris was observed in all groups after the final irrigation protocol. Overall, PUI and XPF groups had higher mean percentage reductions of accumulated hard-tissue debris (94.1% and 89.7%, respectively) than APP and SAF groups (45.7% and 41.3%, respectively) (P < 0.05). No significant differences were found when comparing the results of PUI and XPF groups (P > 0.05) or APP and SAF groups (P > 0.05)., Conclusions: The PUI technique and XP-endo Finisher instrument were associated with significantly lower levels of AHTD compared with conventional irrigation and the modified SAF system protocol in mesial root canals of mandibular molars., (© 2016 International Endodontic Journal. Operative procedural errors must be well analyzed in order to avoid influence negatively the root canal treatment (RCT) prognosis. The successful RCT prevents tooth loss and avoids pain and apical periodontitis. This review aimed to categorize common operative procedure errors and clinical factors associated with RCT. Based on this, will be approached common errors of procedures within the clinical operative sequence: endodontic treatment planning, pulp and periapical disease diagnosis, anaesthesia, access cavity preparation, isolation with rubber dam, root canal preparation, root canal filling and retreatment, restoration of endodontically treated teeth, postoperative pain, follow up of endodontically treated teeth. The professional must remind that in each phase of RCT an operative error may have adverse implication on prognosis, and these errors characterize risk factors to failure. The knowledge of probable operative procedural errors and its consequences are essentials to avoid future problems to the tooth health.

Introduction: Metallic antibacterial nanoparticles have been shown to provide distinct antibacterial advantage and antibiofilm efficacy when applied in infected root canals. The purpose of this study was to evaluate the influence of incorporating zinc oxide nanoparticles (ZnO-Np) on the physicochemical properties of Grossman sealer., Methods: Grossman sealer was prepared according to its original formula. Additionally, 4 experimental sealers were prepared by replacing the zinc oxide component of the powder with ZnO-Np (average size of 20 nm) in different amounts (25%, 50%, 75%, and 100%). Characterization of the setting time, flow, solubility, dimensional changes, and radiopacity were performed according to American National Standards Institute (ANSI)/American Dental Association (ADA) Specification 57. Scanning electron microscopic and energy-dispersive X-ray spectroscopic analyses were conducted to assess the ultrastructural and chemical characteristics of experimental sealers subjected to the solubility test. Statistical analyses were performed with analysis of variance and post hoc Tukey-Kramer tests with a significance level of 5%., Results: A statistically significant difference in the setting time was observed among groups (P < .05), but only 25% ZnO-Np sealer complied with ANSI/ADA requirements. There was a significant difference in the flow characteristics between the control and 25% and 75% ZnO-Np experimental sealers (P < .05), but all sealers conformed to ANSI/ADA standardization; 25% ZnO-Np sealer showed significantly less solubility (1.81% ± 0.31%) and dimensional change (-0.34% ± 0.12%) than other sealers (P < .05). All sealers showed ultrastructural changes with increasing solubility., Conclusions: ZnO-Np decreased the setting time and dimensional changes characteristic of Grossman sealer; 25% ZnO-Np improved the physicochemical properties of Grossman sealer in accordance with ANSI/ADA requirements., (Copyright © 2016 American Association of Endodontists. The morphology, microhardness and dentin permeability of the furcation region of maxillary premolars were evaluated. Ten premolars were cut lengthwise and the furcation region was delimited. In one group (n=10) microhardness was measured in the buccal, central and palatal regions, in the outer middle and inner layers of the furcation, and in the buccal and palatal regions adjacent to the furcation. Knoop hardness was tested with 10 g load for 15 s. Data were analyzed statistically by ANOVA and Tukey-Kramer test (a=0.05). In the other group (n=10) confocal microscopy was used to study dentin morphology. Other 10 specimens were instrumented with ProTaper and immersed in 10% copper sulfate for analysis of permeability under light microscopy. About microhardness, there was no significant difference (p>0.05) among the buccal (39.9±3.1), central (39.5±4.4) and palatal (39.7±5.0) regions of the furcation, or between these regions and the adjacent buccal (39.1±5.8) and palatal (39.7±5.8) regions (p>0.05). The inner layer (42.3±3.7) had significantly higher microhardness (p<0.05) than the outer layer (37.1±3.9). There was a tendency of calcification of the dentinal tubules from the outer towards the inner layer. The percentage of stained area was 12.45±2.0%, restricted to the outer layer. The buccal, central and palatal regions of the furcation as well as the buccal and palatal adjacent regions had similar microhardness values. In conclusion, the inner dentin layer is harder than the outer dentin layer. The dentinal tubules are sinuous and intertwine towards the middle layer. Dye penetration is restricted to the outer layer.

Objective: To describe the morphological aspects of middle mesial canals (MMC) in mandibular first molars using micro-CT., Design: Mandibular first molars collected from the Brazilian (n=136) and Turkish (n=122) populations were scanned (voxel size: 9.9μm) and mesial roots with MMC (n=48) evaluated regarding several morphological aspects. The incidence of MMC in each population was statistically compared using Chi-square test (α=0.05)., Results: Overall, the incidence of MMC was 18.6% (48 out of 258 molars) and was significantly higher in the Brazilian (n=30; 22.1%) than in the Turkish (n=18; 14.8%) population (p<0.05). In both populations, confluent configuration of the MMC was the most frequent anatomy. Most of the specimens with MMC had 3 independent orifices (n=26; 54.2%) and 3 apical foramina (n=21; 43.8%). The mean minor diameter of the MMC orifice (0.16mm) was 3 times less than the other orifices (∼0.50mm). In mesial roots with independent configuration (n=3; 6.3%), the mean volumes (mm(3)) of the MMC, mesiobuccal (MBC) and mesiolingual (MLC) canals were 0.20±0.10, 0.75±0.28, and 0.88±0.19, respectively. In the specimens with canal confluence (n=26; 54.2%), MMC merged to the MBC (n=8; 16.7%), MLC (n=4; 8.3%), or to both MBC and MLC (n=14; 29.2%). Double mesial canal was observed in only 1 specimen. MMC with an independent foramen was observed mostly in Brazilian specimens., Conclusions: Incidence of MMC was higher in the Brazilian molars. Confluent configuration was the most prevalent anatomic variation, while independent and fin configurations, as well as, double MMC, were found only in a few specimens., (Copyright © 2015 Elsevier Ltd. The volume of sealer in the apical 1 mm of teeth filled using different techniques was evaluated by micro-commuted tomography (micro-CT). Sixty-four maxillary central incisors were prepared using NiTi rotary instruments. Teeth were randomly distributed into four groups according to root canal sealers (AH Plus, Endofill, Sealapex, and Sealer 26) and subdivided into two subgroups according to the filling techniques (active and passive lateral condensation; n = 8 each). Subsequently, teeth were examined using the 1174 SkyScan micro-CT device. Images were reconstructed using the NRecon software, and the sealer volume (mm3) in the apical region was analyzed using the two-way ANOVA and post-hoc Student-Newman-Keuls test (α = 0.05). The lowest volume of sealer was observed in teeth filled with Sealapex (0.100 ± 0.009) and Endofill (0.103 ± 0.010). The highest volume was observed in teeth filled with AH Plus (0.112 ± 0.008) and Sealer 26 (0.109 ± 0.018) (p > 0.05). Regarding the filling technique, a lower sealer volume was observed using the active lateral condensation technique compared with that using the passive lateral condensation technique (0.100 ± 0.010 vs. 0.111 ± 0.012) (p < 0.05). Therefore, the lowest volume of sealer was observed in teeth filled with Sealapex and Endofill using the active lateral condensation technique.

This study evaluated the anatomical relationship between posterior teeth root apices and maxillary sinus floor (MSF) on 202 cone beam computed tomography (CBCT) exams. The distance between the root apices and the MSF, as well as the MSF thickness of the cortical bone closest to root apices and furcation regions were measured. The vertical and horizontal relationships of the MSF with the molar roots were classified into categories adapted from the criteria proposed by Kwak et al. (14). The shortest distances between MSF and the root apices were observed in the mesiobuccal root of the second molar (0.36±1.17 mm) and the palatal root of the first molar (0.45±1.10 mm) and the widest in buccal roots of the first premolars (5.47±4.43 mm). Significant differences were observed between the distance of MSF to the root apices of single-rooted first and second premolars. The cortical thickness ranged from 0.65±0.41 mm over the mesiobuccal root of the second molar to 1.28±0.42 mm over the buccal root of the first premolar. The most observed vertical and horizontal relationships were type II and 2H, respectively. The maxillary molar roots showed greater proximity to the MSF. The thickness of the MSF cortical bone closest to the apices and furcation regions was found to be similar only for premolars.

Aim: To investigate changes in three-dimensional geometry, in various cross-sectional morphological parameters and in the centring ability of root canals prepared with different preparation systems using microcomputed tomographic imaging technology., Methodology: Sixty-four mesial canals of mandibular molars were matched based on similar morphological dimensions using micro-CT evaluation and assigned to four experimental groups (n = 16), according to the canal preparation technique: Reciproc, WaveOne, Twisted File and HyFlex CM systems. Changes in several 2D (area, perimeter, form factor, roundness, minor and major diameter) and 3D [volume, surface area, structure model index (SMI)] morphological parameters, as well as canal transportation, were compared with preoperative values using Kruskal-Wallis and anovapost hoc Tukey's tests with the significance level set at 5%., Results: Preparation significantly increased all tested parameters in the experimental groups. No significant differences were observed between groups regarding changes in volume, surface area, SMI, form factor and roundness of the root canal after preparation (P > 0.05). In the apical third, the Reciproc group had significantly greater changes in canal area, perimeter, major and minor diameters than the other groups (P < 0.05). Overall, the Twisted File and HyFlex CM systems were associated with significantly less transportation than the reciprocating instruments, Reciproc and WaveOne (P < 0.05)., Conclusions: Shaping procedures led to the enlargement of the root canal space with no evidence of significant preparation errors. Changes in 3D parameters were not different between groups whilst, in the apical third, Reciproc was associated with significantly greater changes in several 2D parameters compared to the other groups. Twisted File and HyFlex CM systems were able to maintain the original canal anatomy with less canal transportation than Reciproc and WaveOne; however, these differences are unlikely to be of clinical significance., (© 2014 International Endodontic Journal. The aim of this study was to evaluate the frequency of roots, root canals and apical foramina in human permanent teeth using cone beam computed tomography (CBCT). CBCT images of 1,400 teeth from database previously evaluated were used to determine the frequency of number of roots, root canals and apical foramina. All teeth were evaluated by preview of the planes sagittal, axial, and coronal. Navigation in axial slices of 0.1 mm/0.1 mm followed the coronal to apical direction, as well as the apical to coronal direction. Two examiners assessed all CBCT images. Statistical data were analyzed including frequency distribution and cross-tabulation. The highest frequency of four root canals and four apical foramina was found in maxillary first molars (76%, 33%, respectively), followed by maxillary second molars (41%, 25%, respectively). The frequency of four root canals in mandibular first molars was 51%. Mandibular first premolars had two root canals and two apical foramina in 29% and 20% of the cases, respectively. Mandibular central and lateral incisors and canines presented two root canals in 35%, 42% and 22% of the cases, respectively. The navigation strategy in CBCT images favors a better identification of frequency and position of roots, root canals and apical foramina in human permanent teeth.

Introduction: This study evaluated the frequency of root canal isthmi (RCIs) in human permanent teeth by using cone-beam computed tomography., Methods: A sample of 1400 teeth of 618 patients (394 women; mean age, 43.4 years) was selected. RCIs were detected longitudinally on 0.1-mm/0.1-mm axial slices of cone-beam computed tomography images of roots scanned from the pulp orifice to the apex, and findings were classified into 7 categories according to RCIs beginning and end: (1) both in the cervical third, (2) begin in the cervical third and end in the middle third, (3) begin in the cervical third and end in the apical third, (4) both in the middle third, (5) begin in the middle third and end in the apical third, (6) both in the apical third, or (7) no isthmus. A χ(2) test with Yates correction or the Fisher exact test was used to analyze categorical variables, described as frequencies (%). The Student t test was used to compare quantitative variables., Results: RCI is a common anatomic structure in human permanent teeth, except in maxillary anterior teeth., Conclusions: The higher frequencies of RCIs (87.9%) were found in mandibular first molars. The frequencies of RCIs according to mean age and tooth group were not significantly different (P > .05), except in mandibular central incisors. To determine the frequency of apical and cervical curvatures in human molars using the radius method and cone-beam computed tomography (CBCT) images. Four hundred images of mandibular and maxillary first and second molars were selected from a database of CBCT exams. The radius of curvature of curved root canals was measured using a circumcenter based on three mathematical points. Radii were classified according to the following scores: 0 - straight line; 1 - large radius (r > 8 mm, mild curvature); 2 - intermediate radius (r > 4 and r < 8 mm, moderate curvature); and 3 - small radius (r ≤ 4 mm, severe curvature). The frequency of curved root canals was analyzed according to root canal, root thirds, and coronal and sagittal planes, and assessed using the chi-square test (significance at α = 0.05). Of the 1,200 evaluated root canals, 92.75% presented curved root canals in the apical third and 73.25% in the cervical third on coronal plane images; sagittal plane analysis yielded 89.75% of curved canals in the apical third and 77% in the cervical third. Root canals with a large radius were significantly more frequent when compared with the other categories, regardless of root third or plane. Most root canals of maxillary and mandibular first and second molars showed some degree of curvature in the apical and cervical thirds, regardless of the analyzed plane (coronal or sagittal).

Aim: The aim of this study was to determine the root canal area before and after the instrumentation 1 mm short of the apical foramen by clinical and cone beam computed tomography (CBCT) methods, and to evaluate the cleanliness of the apical region in mesiodistal flattened teeth by using optical microscopy., Materials and Methods: Forty-two human single-canal mandibular incisors were instrumented using the Free Tip Preparation technique up to three, four or five instruments from the initial. Cone beam computed tomography scans were acquired of the samples before and after root canal preparation (RCP). Irrigation was performed by conventional or hydrodynamic means, using 2.5% sodium hypochlorite. The samples were prepared for observation under an optical microscope. Images were digitally obtained, analyzed and the results were submitted to statistical analysis (two-way ANOVA complemented by Bonferroni's post-test)., Results: There was no significant difference between the studied anatomical areas with both CBCT and clinical methods. There were no differences between irrigation methods. It was verified differences between instrumentation techniques. Instrumentation with four instruments from the initial instrument determined a significant increase in the contact area when compared to preparation with three instruments, but RCP with 5 instruments did not result in a better cleanliness., Conclusion: The analysis with CBCT was not capable to determine the precise shape of surgical apical area comparing to the clinical method., Clinical Significance: Both the conventional and hydrodynamic irrigation techniques were not able to promote root canals debris-free. The instruments action in root canal walls was proportional to the number of instruments used from the initial apical instrument.

Aim: To evaluate the use of a dissolved oxygen microsensor (DOMS) for assessing the viability and thickness of microbial biofilms on the apical external surface of contaminated human tooth roots., Methodology: Apical biofilm formation was evaluated in 15 roots contaminated in vitro with a polymicrobial mixture of Staphylococcus aureus, Enterococcus faecalis, Pseudomonas aeruginosa and Candida albicans for 7, 21 and 60 days and in three freshly extracted roots with associated radiographically visible periapical lesions. In each root, the thickness and viability (measured by the amount of dissolved oxygen) of biofilm formed on the apical 2 mm were examined with the DOMS. Scanning electron microscopy (SEM) was used as an auxiliary analysis to confirm the existence of the biofilms detected by the DOMS., Results: The DOMS detected dissolved oxygen on the biofilms formed on the three residual roots up to thickness of 375 μm, 480 μm and 1650 μm. In the 15 roots contaminated in vitro, the DOMS detected dissolved oxygen in six specimens up to thicknesses from 75 to 250 μm, and the intensity of the metabolic activity (biofilm thickness) was directly proportional to the contamination time. SEM confirmed the presence of biofilm in all roots., Conclusion: The dissolved oxygen microsensor allowed the measurement of the amount of dissolved oxygen in the biofilm, which is indicative of the intensity of the microbial metabolic activity (viability), correlating the results with biofilm thickness. The DOMS was effective in freshly extracted roots, but had limitations in roots contaminated in vitro after short periods (7 and 21 days) of contamination., (© 2014 International Endodontic Journal. The aim of this study was to evaluate thermal changes, dentin ablation removal capacity, and morphological aspects of sound and demineralized human dentin surface irradiated with different output energies of an erbium: yttrium-aluminium-garnet (Er:YAG) laser. Eighty sound human tooth specimens were assigned into two groups: demineralized dentin and sound dentin (control group). The dentin groups were subdivided into four subgroups (n = 10) according to the irradiation energy used (120, 160, 200, or 250 mJ) at a constant frequency level of 6 Hz, in focused mode, and under refrigeration. Quantitative analysis of the sound and carious dentin ablation was performed using light microscopy (LM) by measuring (mm) the remaining demineralized tissue with the Axion Vision™ software. Qualitative analysis was performed using the images obtained with a scanning electron microscope (SEM), and the temperature increase was recorded with an infrared digital thermometer. The Er:YAG laser promoted a gradual increase in temperature for all groups, and no difference was observed between the sound and demineralized dentin. The groups of 200 and 250 mJ showed the highest values, yet a variation in temperature did not exceed 5 °C. The energy output of 120 mJ selectively removed demineralized tissue when compared to 250 mJ, while also providing more regular surfaces in the cavity preparation. It was concluded that the temperature increase during sound and demineralized dentin removal had a strong positive correlation with the Er:YAG laser energy level output. However, the higher energies used did not present selectivity to the demineralized tissue, and the parameters used did not cause an increase in temperature over 5 °C.

Objectives: The aim of this study was to introduce a methodology to map irrigant spreadability within the root canal space using micro-computed tomography (micro-CT)., Materials and Methods: Mandibular molars presenting Vertucci's types I and II canal configurations were selected, and four scans using isotropic resolution of 19.5 μm were accomplished per tooth: prior to treatment (S1), after glide path (S2) and after root canal preparation (S3 and S4). A contrast solution (CS) was used to irrigate the canals at stages S2 and S4. The touched and untouched surface areas of the canals, the volume of irrigant-free areas and the percentage volume occupied by the CS were calculated. Density, surface tension and the spread pattern of the CS and 2.5% NaOCl were also evaluated., Results: In the type I mesial root, there was an increase in the percentage volume of free-irrigated areas from S2 to S4 preparation steps, whilst in the distal roots and type II mesial root, a decrease of irrigant-free areas was observed. The use of CS allowed the quantification of the touched surface area and the volume of the root canal occupied by the irrigating solution. Density (g/mL) and surface tension (mN/m) of the CS and 2.5% NaOCl were 1.39 and 47.5, and 1.03 and 56.2, respectively. Besides, a similar spread pattern of the CS and 2.5% NaOCl in a simulated root canal environment was observed., Conclusions: This study introduced a new methodology for mapping the irrigating solution in the different stages of the root canal preparation and proved useful for in situ volumetric quantification and qualitative evaluation of irrigation spreading and irrigant-free areas., Clinical Relevance: Micro-computed tomographic technology may provide a comprehensive knowledge of the flush effectiveness by different irrigants and delivery systems in order to predict the optimal cleaning and disinfection conditions of the root canal space.

Objective: This study determined the chemical components derived from degradation of 2% chlorhexidine (CHX) gel and solution by using gas chromatography-mass spectrometry., Materials and Methods: Three 2% CHX gels were used to identify the products of CHX gel degradation using gas chromatography-mass spectrometry. A solution of CHX was also evaluated to compare the degradation between gel and solution. Degradation was evaluated in four storage situations (on the worktable with light: on the worktable without light; in the Pasteur oven at 36.5°C without light; and in the refrigerator at 8°C without light). Measurements were made at four time points: initial analysis and 1, 3 and 6 months after. The conversion of CHX into para-chloroaniline in storage situations and in different periods was analyzed statistically using chi-square test (α = 5%)., Results: The 2% CHX gel or solution had already degraded vial found within the period of validity, at all time points and for all storage conditions. The amount of para-chloroaniline (pCA) was directly proportional to time in the case of CHX solution, but not in CHX gel due to lack of homogeneity. CHX homogeneity in hydroxyethylcellulose gel was directly dependent on compounding mode., Conclusions: Degradation products, such as para-chloroaniline (pCA), orto- chloroaniline (oCA), meta-chloroaniline (mCA), reactive oxygen species (ROS) and organochlorines (ortho-chlorophenyl isocyanate and 2-amino-5-clorobenzonitrila) were found in 2% CHX gel and solution, regardless of storage conditions or time. In relationship to gel homogenization an alternative to produce 2% CHX gel and a new homogenization method have been developed.

Introduction: The morphology of the supernumerary third root (radix) in mandibular first molars was examined by micro-computed tomography (μCT) scanning., Methods: Nineteen permanent mandibular first molars with radix were scanned in a μCT device to evaluate their morphology with respect to root length, root curvature direction, location of radix, apical foramen, accessory canals and apical deltas, and distance between canal orifices as well as 2- and 3-dimensional parameters of the canals (number, area, roundness, major/minor diameter, volume, surface area, and structure model index). Quantitative data were analyzed by 1-way analysis of variance and the Tukey test (α = 0.05)., Results: The mean length of the mesial, distal, and radix roots was 20.36 ± 1.73 mm, 20.0 ± 1.83 mm, and 18.09 ± 1.68 mm, respectively. The radix was located distolingually (n = 16), mesiolingually (n = 1), and distobuccally (n = 2). In a proximal view, most radix roots had a severe curvature with buccal orientation and a buccally displaced apical foramen. The spatial configuration of the canal orifices on the pulp chamber floor was mostly in a trapezoidal shape. The radix root canal orifice was usually covered by a dentinal projection. The radix differed significantly from the mesial and distal roots for all evaluated 3-dimensional parameters (P < .05). The radix canal had a more circular shape in the apical third, and the mean size of the minor diameter 1 mm short of the foramen was 0.25 ± 0.10 mm., Conclusions: The radix root is an important and challenging anatomic variation of mandibular first molars, which usually has a severe curvature with a predominantly distolingual location, and a narrow root canal with difficult access., (Copyright © 2014 American Association of Endodontists. Introduction: This study compared the bond strength, interfacial ultrastructure, and tag penetration of resin-based sealers applied to smear-free radicular dentin using 70% isopropyl alcohol as the active final rinse., Methods: Eighty root canals were prepared and assigned to 2 groups (n = 40) according to the drying protocol: paper points or 70% isopropyl alcohol. Then, roots were divided into 4 subgroups (n = 10) with respect to the sealer and obturation material: AH Plus (Dentsply De Trey GmbH, Konstanz, Germany) and gutta-percha (AH/GP), Hybrid Root SEAL (Sun Medical, Tokyo, Japan) and gutta-percha (HR/GP), Epiphany SE (Pentron Clinical Technologies, Wallingford, CT) and gutta-percha (EP/GP), and Epiphany SE and Resilon (EP/RS). Roots were sectioned, and the push-out test was performed. Failure modes were examined under stereomicroscopy and sealer penetration into the dentinal tubules under scanning electron microscopy. Data were statistically analyzed by 2-way analysis of variance post hoc Tukey tests with a significant level of 5%., Results: Overall, canals dried with isopropyl alcohol showed significantly higher bond strength values (2.11 ± 1.74 MPa) than with paper points (1.81 ± 1.73 MPa) (P < .05). The HR/GP group showed lower bond strength than the AH/GP group (P < .05) but higher than the EP/GP and EP/RS groups (P < .05). The most frequent type of failure was cohesive in the AH/GP and HR/GP groups and adhesive in the EP/GP and EP/RS groups. Scanning electron microscopic evaluation revealed better adaptation of the adhesive interface in the AH/GP and HR/GP groups in comparison with the EP/GP and EP/RS groups., Conclusions: A final rinse with EDTA and 70% isopropyl alcohol improved the bond strength and penetration of the sealers into dentinal tubules of the root., (Copyright © 2014 American Association of Endodontists. Introduction: This study aimed to describe the anatomy of mandibular central and lateral incisors using micro-computed tomographic imaging., Methods: One hundred mandibular incisors were scanned in a micro-computed tomographic device using an isotropic resolution of 22.9 μm. The anatomy of each tooth (length of the roots, presence and location of accessory canals and apical deltas, and number of canals) as well as the 2- and 3-dimensional morphologic aspects of the canal (area, roundness, diameter, volume, surface area, and structure model index) were evaluated. Data were statistically compared using the Student t test (alpha = 0.05)., Results: The mean lengths of the mandibular central and lateral incisors were 20.71 and 21.56 mm, respectively. Most of the central (60%) and lateral (74%) incisors had no accessory canals. An apical delta was observed in only 1 specimen. The cross-section analysis of the apical third showed the presence of 1, 2, or 3 canal orifices. No statistical difference was observed in the comparison of the 2- and 3-dimensional morphologic parameters between central and lateral incisors (P < .05). The qualitative analyses of the 3-dimensional models of the root canal systems of the central and lateral incisor teeth confirm that the most prevalent configurations were Vertucci's types I (50% and 62%, respectively) and III (28%)., Conclusions: Overall, mandibular central and lateral incisors were similar in terms of the 2- and 3-dimensional analyzed parameters. Purpose: To evaluate the influence of long-term water storage and thermocycling on the shear bond strength of a glass-ionomer cement to Er:YAG-irradiated and bur-prepared dentin., Materials and Methods: Fifteen bovine incisors were selected and the roots removed. Crowns were sectioned into four pieces, resulting in 60 samples that were individually embedded in polyester resin (n = 15) and ground to plane the enamel and expose the dentin. The bonding site was delimited and samples were randomly assigned according to the method of cavity preparation: Er:YAG laser (250 mJ/4 Hz) or high-speed handpiece (diamond bur #2096). Samples were fixed to a metallic device, where glass-ionomer cement (GIC) cylinders were prepared. Subsequently, they were subdivided according to the duration of water storage (WS) and number of thermocycles (TCs) - 24 h WS/no TCs and 6 months WS/12,000 TCs - and subjected to a shear bond strength test (500 N at 0.5 mm/min)., Results: The duration of water storage and number of thermocycles tested had no statistically significant effect on the shear bond strength to laser-irradiated dentin (p > 0.05). For bur-prepared substrate, the long-term degradation process promoted a decrease in shear bond strength values (p < 0.05)., Conclusion: Long-term water storage and thermocycling did not affect shear bond strength of glass-ionomer cement bonded to Er:YAG laser-prepared dentin.

Knowing the outcome of root canal treatment (RCT) is determinant to substantiate the clinical decision making process, especially when RCT is weighed against the extraction of natural teeth or replacement by prosthetic elements. The ideal scenario in all clinical situations should combine healing/prevention of disease (apical periodontitis) and the functional retention of the tooth. Understanding the risk factors associated with endodontic failure is a key factor to increase the chances of success. The logical action is to reverse the existing disease, which requires intervention to neutralize the bacterial invasion and disrupt the bacterial biofilm within the complex anatomy. Success is more predictable when the immune host defenses are favorable. However, success has different meanings to the dentist, to the patient and to the tooth itself. The life of an endodontically treated tooth depends on the accuracy of the diagnosis and planning, excellence of disinfection, instrumentation and filling procedures (antimicrobial strategies, root canal shaping and coronal and apical seal) and finally the rehabilitation management. The interpretation of constant or intermittent pain and/or discomfort associated with apical periodontitis (AP) in endodontically treated tooth may be suggestive of endodontic failure. The success features of RCT, namely absence of pain, regression of AP, tight seal of canal and coronal spaces, and recovery of tooth function, must be reevaluated over time. In case of doubt between success and failure, cone beam computed tomography (CBCT) could be indicated for detection and precise localization of AP. The possibility of map reading on CBCT images characterizes the real multidimensional structure, providing accurate information on the presence, absence or regression of AP. The survival of an endodontically treated tooth implies understanding the biological and mechanical outcomes as multifactorial events over the individual's life span. The objective of this review of literature is to discuss relevant factors associated with patient's health, tooth and dentist that could account for a successful RCT.

Introduction: This study evaluated dentin morphology and microhardness at the furcation area of mandibular molars., Methods: Tooth segments of mandibular molars were embedded in resin blocks and bisected longitudinally (n = 20), and the furcation area was delimited by tracing 2 orthogonal lines from the most concave point of the outer surface of the furcation toward the mesial and distal canal entrances. In half of the specimens, Knoop microhardness was measured in 2 directions in the furcation area: vertically, parallel to the orthogonal lines and the bisector of the angle formed by them; and horizontally, in the outer (close to the pulp chamber floor), inner (close to the cementum), and middle dentin layers. Data were analyzed statistically by one-way analysis of variance and Tukey-Kramer test (α = 0.05). The other half of the specimens were examined by scanning electron microscopy to evaluate dentin morphology and trajectory of the tubules., Results: No statistically significant difference (P > .05) was found among the mesial (46.5 ± 6.4), central (47.3 ± 8.1), and distal (49.7 ± 6.5) orthogonal lines. The inner dentin layer (51.7 ± 2.5) was statistically similar to the middle (46.3 ± 2.9) (P > .05), which was similar to the outer layer (41.4 ± 2.4) (P > .05). The inner layer was significantly harder than the outer layer (P < .05). The morphologic analysis revealed a tendency of calcification of the tubules from the outer toward the inner layer., Conclusions: Dentin microhardness at the furcation area is uniform in its 3 vertical axes, but the inner dentin layer is harder than the outer layer. This study used gas chromatography-mass spectrometry (GC-MS) to detect the products formed during the contact of sodium hypochlorite (NaOCl) with bovine pulp and dentin. For analysis of the products formed in the volatile phase, 11 mg of bovine pulp tissue were placed in contact with 0.5%, 2.5% and 5.25% NaOCl until complete tissue dissolution occurred. The solid phase microextraction (SPME) fiber was exposed inside the container through the cover membrane and immediately injected into the GC-MS system. 30 mg of the of dentin were kept in contact with NaOCl, and then the SPME fiber was exposed inside the container through the cover membrane for adsorption of the products and injected into the GC-MS system. The same protocol was used for the aqueous phase. For analysis of the volatile compounds, the final solution was extracted using pure ethyl ether. The suspended particulate phase of the mixture was aspirated, and ether was separated from the aqueous phase of the solution. The ether containing the products that resulted from the chemical interaction of dentin and pulp with the NaOCl was filtered and then injected into the GC-MS system for analysis of the aqueous phase. The aqueous and volatile phases of both dentin and pulp showed the formation of chloroform, hexachloroethane, dichloromethylbenzene and benzaldehyde. In conclusion, organochlorine compounds are generated during the contact of dentin and pulp with NaOCl at concentrations of 0.5%, 2.5% and 5.25%.

The aim of this study was to detect root isthmuses in maxillary and mandibular molars and evaluate their frequencies using map-reading dynamics in CBCT images. Two hundred extracted human maxillary and mandibular molars were used in ex vivo assay. A consecutive sample of two hundred maxillary and mandibular molars (first and second) was selected from CBCT exams. The isthmuses were detected from the pulp orifice to the apex and were recorded according to their beginning and their end, into categories: 1. begin and end in cervical third; 2. beginning in cervical third and end in middle third; 3. beginning in cervical third and end in apical third; 4. beginning and end in middle third; 5. begin in middle third and end in apical third; 6. beginning and end in apical third; 7. no isthmus. The scans were obtained in different planes with map-reading in axial slices of 0.5 mm/0.5 mm involved the coronal to apical direction. The frequencies of isthmus were analyzed according to the level of root and evaluated by Chi-square test. The level of significance was set at α=0.05. The presence of isthmus detected in maxillary molars was 86% in ex vivo assay and 62% in vivo assay, whereas in mandibular molars was observed 70% in ex vivo assay and 72% in vivo assay. The frequency of isthmus was high in both study models. The map-reading dynamics in CBCT images was found to be precise to detect the localization of isthmus.

Introduction: This study investigated procedural errors made during root canal preparation using stainless steel and nickel-titanium (NiTi) instruments by undergraduate students, using two diagnostic imaging methods., Materials and Methods: Sixty human molars were divided into three groups (n=20; group 1: K-Flexofile, group 2: K3, and group 3: BioRace). The root canals were filled with gutta-percha and AH Plus. Periapical radiographs and cone beam computed tomography (CBCT) images were obtained to detect procedural errors made by undergraduate students during root canal preparation. Two examiners evaluated the presence or absence of fractured instruments, perforations and canal transportations. The agreement between observers was assessed using the kappa coefficient. The Kolmogorov-Smirnov, Fisher exact, ANOVA and Tukey tests were used for statistical analysis. The level of significance was set at 5%., Results: There were no significant differences in detecting procedural errors between two- and three-dimensional diagnostic imaging methods. There were no significant differences in procedural errors between stainless steel and NiTi instruments. Mean preparation time was recorded in minutes, and results were significantly different between the three groups. NiTi instruments had the lowest mean preparation time., Conclusion: Both periapical radiographs and CBCT identified procedural errors, however, three-dimensional images offered more diagnostic resources. The frequency of procedural errors was low for any of the endodontic instruments despite being used by inexperienced operators.

Aim: To investigate the anatomy of single-rooted mandibular canine teeth using microcomputed tomography (μCT)., Methodology: One hundred straight single-rooted human mandibular canines were selected from a pool of extracted teeth and evaluated using μCT. The anatomy of each tooth (length of the roots, presence of accessory canals and apical deltas, position and major diameter of the apical foramen and distance between anatomical landmarks) as well as the two- and three-dimensional morphological aspects of the canal (area, perimeter, form factor, roundness, major and minor diameter, volume, surface area and structure model index) were evaluated. The results of the morphological analysis in each canal third were compared statistically using Friedman's test (α = 0.05)., Results: The length of the roots ranged from 12.53 to 18.08 mm. Thirty-one specimens had no accessory canals. The location of the apical foramen varied considerably. The mean distance from the root apex to the major apical foramen was 0.27 ± 0.25 mm, and the major diameter of the major apical foramen ranged from 0.16 to 0.72 mm. Mean major and minor diameters of the canal 1 mm short of the foramen were 0.43 and 0.31 mm, respectively. Overall, the mean area, perimeter, form factor, roundness, major and minor diameters, volume, surface area and structure model index (SMI) were 0.85 ± 0.31 mm(2) , 3.69 ± 0.88 mm, 0.70 ± 0.09, 0.59 ± 0.11, 1.36 ± 0.36 mm and 0.72 ± 0.14 mm, 13.33 ± 4.98 mm(3) , 63.5 ± 16.4 mm(2) and 3.35 ± 0.64, respectively, with significant statistical difference between thirds (P < 0.05)., Conclusions: The anatomy and morphology of the root canal of single-rooted canines varied widely in different levels of the root., (© 2013 International Endodontic Journal. Introduction: The newly developed single-file systems claimed to be able to prepare the root canal space with only 1 instrument. The present study was designed to test the null hypothesis that there is no significant difference in the preparation of oval-shaped root canals using single- or multiple-file systems., Methods: Seventy-two single-rooted mandibular canines were matched based on similar morphologic dimensions of the root canal achieved in a micro-computed tomographic evaluation and assigned to 1 of 4 experimental groups (n = 18) according to the preparation technique (ie, Self-Adjusting File [ReDent-Nova, Ra'anana, Israel], WaveOne [Dentsply Maillefer, Ballaigues, Switzerland], Reciproc [VDW, Munich, Germany], and ProTaper Universal [Dentsply Maillefer] systems). Changes in the 2- and 3-dimensional geometric parameters were compared with preoperative values using analysis of variance and the post hoc Tukey test between groups and the paired sample t test within groups (α = 0.05)., Results: Preparation significantly increased the analyzed parameters; the outline of the canals was larger and showed a smooth taper in all groups. Untouched areas occurred mainly on the lingual side of the middle third of the canal. Overall, a comparison between groups revealed that SAF presented the lowest, whereas WaveOne and ProTaper Universal showed the highest mean increase in most of the analyzed parameters (P < .05)., Conclusions: All systems performed similarly in terms of the amount of touched dentin walls. Introduction: Chlorhexidine (CHX) is likely to decompose into reactive by-products. This study evaluated the generation of 4-chloroaniline (pCA), reactive oxygen species (ROS), and 1-chloro-4-nitrobenzene in high concentrations of CHX and in a mixture of CHX and calcium hydroxide at different time points., Methods: A gas chromatography method was developed to detect pCA and CHX by-products. Mass spectroscopy was used to elucidate the structure of compounds. The samples, which were kept at 36.5°C and 95% relative humidity during the study, were analyzed immediately and 7 days after preparation., Results: pCA was detected in the 2% CHX solution and in the mixture of CHX and calcium hydroxide at all time points. pCA concentrations increased after storing under those conditions. The 2% CHX solution alone and the mixture of CHX and calcium hydroxide released ROS at all time points, but 1-chloro-4-nitrobenzene was not found., Conclusions: pCA and ROS were identified as by-products of the 2% CHX aqueous solution alone and as ointment base of calcium hydroxide paste., (Copyright © 2013 American Association of Endodontists. Aim: To evaluate, by scanning electron microscopy (SEM), the efficacy of smear layer removal using chitosan compared with different chelating agents, and to quantify, by atomic absorption spectrophotometry with flame (AASF), the concentration of calcium ions in these solutions after irrigation., Methodology: The root canals of twenty-five canines were prepared using a crown-down technique and irrigated with 1% sodium hypochlorite. The teeth were randomly divided into groups (n = 5), according to the type of final irrigation: 15% EDTA, 0.2% chitosan, 10% citric acid, 1% acetic acid and control (without final irrigation). The total volume of each chelating solution was collected from the canals and analysed by AASF for quantification of calcium ions in the solutions. Then, the roots were split longitudinally and examined by SEM for evaluation of smear layer removal in the middle and apical thirds. Cleaning scores were attributed and analysed statistically using the Kruskal-Wallis and Dunn tests. The AASF data were analysed by one-way anova and Tukey-Kramer test. A significant level of α = 0.05 was adopted., Results: 15% EDTA, 0.2% chitosan and 10% citric acid had similar smear layer removal capacity with a significant difference (P < 0.05) from 1% acetic acid and the control group. There was no significant difference (P > 0.05) between the smear layer remaining in the middle and apical thirds. The highest calcium ion concentration was observed with 15% EDTA (121.80 ± 5.13) and 0.2% chitosan (104.13 ± 19.23), with no significant difference. The lowest calcium ion concentration was obtained with 1% acetic acid (25.62 ± 7.68), whilst 10% citric acid (70.38 ± 11.15) had intermediate results, differing significantly from the other solutions (P < 0.01)., Conclusions: 15% EDTA, 0.2% chitosan and 10% citric acid effectively removed smear layer from the middle and apical thirds of the root canal. 15% EDTA and 0.2% chitosan were associated with the greatest effect on root dentine demineralization, followed by 10% citric acid and 1% acetic acid., (© 2012 International Endodontic Journal.)

The purpose of this study was to analyze, correlate, and compare the demineralization and permeability of dentin remaining after caries removal with either an Er:YAG laser, a bur, or a curette. Thirty human dentin fragments were immersed in a demineralizing solution for 20 days and were randomly divided into three groups (n = 10) for the removal of the demineralized lesion. The groups were G1-Er:YAG laser (200 mJ/6 Hz; noncontact at 12 mm; spot: 0.63 mm), G2-Bur, and G3-Curette. The specimens were then immersed in a 10% copper sulfate solution, then in a 1% dithiooxamide alcoholic solution for 30 min and kept in ammonia vapor for 7 days. Next, the specimens were examined with optical microscopy. The amount of demineralized dentin and the level of copper ion infiltration in the dentin were quantified in μm using Axion Vision software. Data were analyzed with the Kruskal-Wallis test (p < 0.05) and Pearson's Correlation test. The analysis revealed no significant differences between the three caries removal methods in terms of their capacity to remove demineralized tissue (G1: 10.6 μm; G2: 8.4 μm; G3: 11 μm), although the laser removal generated more tissue permeability than the others methods (G1: 17.6 μm; G2: 6.6 μm; G3: 5.5 μm). The correlation between the remaining demineralized dentin and the dentin permeability was moderate for the conventional methods and higher for the Er:YAG laser. Introduction: Cleaning and shaping of root canals are essential steps for the success of endodontic therapy. The purpose of this study was to evaluate the tissue debridement efficacy of the self-adjusting file (SAF) protocol in the apical third of oval-shaped canals of mandibular incisors in comparison with a nickel-titanium rotary system preparation., Methods: Twenty-six single-rooted human mandibular incisor teeth were selected and assigned to a control (n = 4) and 2 experimental groups (n = 11) according to 1 of 2 instrumentation techniques, SAF and nickel-titanium rotary systems. After root canal preparation, the apical thirds of the specimens were submitted to histologic processing and analyzed by optical microscopy regarding the percentage of debris and uninstrumented root canal walls. The data were statistically compared by using unpaired t test with Welch's correction, and the level of significance was set at 5%., Results: The percentage of remaining debris and uninstrumented canal perimeter was significantly lower in SAF group (2.18 ± 2.71 and 12.33 ± 7.85, respectively) than in rotary group (13.11 ± 12.98 and 53.54 ± 15.95, respectively) (P < .05). In the SAF group most of specimens were completely free of debris, whereas in the rotary group 53% of the canals presented debris., Conclusions: SAF had significantly more contact to the dentin walls and removed more debris than rotary instrumentation in the apical third of mandibular incisors., (Copyright © 2013 American Association of Endodontists. Objective: The aim of this in situ study was to analyze the influence of the Er:YAG, Nd:YAG, and CO(2) lasers on the enamel acid resistance of pits and fissures., Background Data: The laser tissue interaction has been studied as a method of preventing occlusal caries., Methods: Thirteen volunteers wore palatal acrylic appliances containing human occlusal enamel blocks that were divided into four groups (G1, control; G2, Er:YAG; G3, Nd:YAG; G4, CO(2)). Each palatal acrylic appliance was used in the four studied groups and was used for 14 consecutive days. A sucrose solution was applied to the specimens six times per day. The specimens were then sectioned in half, and a microhardness test was applied. The other halves were analyzed using polarized light microscopy to measure the caries-like lesion areas, and a morphological analysis was conducted using a scanning electron microscope (SEM)., Results: For the statistical analysis of the data obtained from the microhardness test (Knoop hardness number. [KHN]) (α=5%), Fisher's exact test was performed, and the group means were as follows: G1, 247±71; G2, 258±70; G3, 272±73; and G4, 298±56. The results demonstrated that the control group was significantly different from G3 and G4, which presented higher microhardness values. The Wilcoxon signed-rank test was used to analyze the data obtained from the caries-lesion area measurements (mm(2)) (α=5%) (G1, 0.01±1.08; G2, 0.13±0.18; G3, 0.05±0.17; and G4, 0.09±0.22). The results no showed significant differences among the groups in this analysis., Conclusions: Based on the results from the present study, it may be concluded that the CO(2) and Nd:YAG lasers increased the enamel acid resistance in pits and fissures.

Objectives: To investigate the frequency, position, number and morphology of enamel pearls (EPs) using micro-CT (µCT) and to report a case of an EP mimicking an endodontic-periodontic lesion., Methods: Cone beam CT (CBCT) was performed in a patient to evaluate a radio-opaque nodule observed on the left maxillary first molar during the radiographic examination. Additionally, 23 EPs were evaluated regarding frequency, position, number and morphology by means of µCT. The results were statistically compared using the Student's t-test for independent samples., Results: 1 pearl was presented in 13 specimens, while 5 specimens presented 2 pearls. The most frequent location of the EPs was the furcation between the disto-buccal and the palatal roots of the maxillary molars. Overall, the mean major diameter, volume and surface area were 1.98 ± 0.85 mm, 1.76 ± 1.36 mm³ and 11.40 ± 7.59 mm², respectively, with no statistical difference between maxillary second and third molars (p > 0.05). In the case report, CBCT revealed an EP between the disto-buccal and the palatal roots of the maxillary first left molar associated with advanced localized periodontitis. The tooth was referred for extraction., Conclusions: EPs, located generally in the furcation area, were observed in 0.74% of the sample. The majority was an enamel-dentin pearl type and no difference was found in maxillary second and third molars regarding diameter, volume and surface area of the pearls. In this report, the EP mimicked an endodontic-periodontic lesion and was a secondary aetiological factor in the periodontal breakdown.

This study evaluated in vitro the influence of thermocycling and water storage (WS) on the shear bond strength (SBS) of composite resin in cavities prepared in primary tooth enamel with conventional bur or Er:YAG laser. The test surfaces were obtained from 48 primary molars and randomly assigned to 2 groups (n=24), according to cavity preparation: A: bur-preparation and B: Er:YAG laser irradiation. The specimens were restored with an etch-and-rinse adhesive system and composite resin. Each group was divided into 4 subgroups (n=6) according to WS duration and number of thermal cycles (TCs): I: 24 h WS/no thermocycling; II: 7 days WS/500 TCs; III: 1 month WS/2,000 TCs; IV: 6 months WS/12,000 TCs. The specimens were tested to failure in shear strength at a crosshead speed of 0.5 mm/min. Data were analyzed statistically by two-way ANOVA and Tukey's test. SBS means (S.D.) in MPa were: AI: 17.45 (2.03), AII:16.38 (1.49), AIII: 6.88 (0.66), AIV: 7.77 (1.53), BI: 12.32 (0.99), BII: 15.37 (2.24), BIII: 15.05 (2.01) and BIV-5.51 (1.01). WS duration and number of TCs influenced significantly the SBS values only for BIV (p<0.05). AI presented the highest SBS value, which was statistically similar to those of AII, BII and BIII. In conclusion, the adhesion of an etch-and-rinse adhesive to Er:YAG laser-irradiated primary tooth enamel was affected by the methods used to simulate degradation of the adhesive interface only when 6 months WS/12,000 TCs were employed.

Purpose: To investigate the laboratory effect of Er:YAG laser on ablation rate and morphological changes in human enamel and dentin with varying water flow., Methods: 23 human third molars were sectioned in mesio-distal and buccal-lingual directions. The slabs were flattened and weighted on an analytical laboratory balance (control). A 4-mm2 area was demarcated and the samples were randomly assigned into three groups according to water flow employed during the laser irradiation (1.0, 1.5, and 2.0 mL/minute). An Er:YAG laser was used to ablate enamel (80.22-J/cm2, 300 mJ/4Hz) and dentin (96.26-J/cm2, 250 mJ/4Hz). After irradiation, the samples were immersed in distilled water for 1 hour and then weighted again. The final mass was obtained and laser-irradiated substrate mass loss was calculated by the difference between the initial and final mass. Afterwards, specimens were prepared for SEM., Results: Data were submitted to ANOVA and Tukey's test (P < 0.05). It was observed that the 2.0 mL/minute resulted in a higher mass loss, 1.0 mL/minute showed a lower mass loss, and 1.5 mL/minute demonstrated intermediate results (P < 0.05). The increase of water flow promoted less melting areas and cracks. Furthermore, dentin was more ablated than enamel. It may be concluded that the water flow of Er:YAG laser and the substrates affected the ablation rate. Among the tested parameters, 2.0 mL/minute improved the ability of ablation in enamel and dentin, with less morphologic surface alteration.

This study reports the effects on micromorphology and temperature rise in human dentin using different frequencies of Er:YAG laser. Sixty human dentin fragments were randomly assigned into two groups (n = 30): carious or sound dentin. Both groups were divided into three subgroups (n = 10), according to the Er:YAG laser frequency used: 4, 6, or 10 Hz (energy: 200 mJ; irradiation distance: 12 mm; and irradiation time: 20 s). A thermocouple adapted to the tooth fragment recorded the initial temperature value (°C); then, the temperature was measured after the end of the irradiation (20 s). Morphological analysis was performed using images obtained with scanning electron microscope. There was no difference between the temperatures obtained with 4 and 6 Hz; the highest temperatures were achieved with 10 Hz. No difference was observed between carious and sound dentin. Morphological analyses revealed that all frequencies promoted irregular surface in sound dentin, being observed more selectively ablation especially in intertubular dentin with tubule protrusion. The caries dentin presented flat surface for all frequencies used. Both substrates revealed absence of any signs of thermal damage. It may be concluded that the parameters used in this study are capable to remove caries lesion, having acceptable limits of temperature rise and no significant morphological alterations on dentin surface., (Copyright © 2012 Wiley Periodicals, Inc.)

Introduction: This study examined the anatomy of 4-rooted maxillary second molars by using micro-computed tomography., Methods: Twenty-five 4-rooted maxillary second molars were scanned to evaluate the size and curvature of the roots; the distance and spatial configuration between some anatomical landmarks; the number of root canals and the position of apical foramina; the occurrence of fusion of roots and enamel pearls; the configuration of the canal at the apical third; the cross-sectional appearance, the volume, and surface area of the root canals. Data were compared by using analysis of variance post hoc Tukey test (α = 0.05)., Results: The specimens were classified as types I (n = 16), II (n = 7), and III (n = 2). The size of the roots was similar (P > .05), and most of them presented straight with 1 canal, except the mesiobuccal that showed 2 canals in 24% of the samples. The configuration of the pulp chamber was mostly irregular quadrilateral-shaped. The lowest mean distance of the orifices was observed between the buccal roots (P < .05). Accessory canals were present mostly in the apical third. Location of the apical foramina varied considerably. Fusion of roots and enamel pearls occurred in 44% and 8% of the samples, respectively. Mean distance from the pulp chamber floor to the furcation was 2.15 ± 0.57 mm. No statistical differences were found in the bi-dimensional and 3-dimensional analyses (P > .05)., Conclusions: All analyzed parameters showed differences between roots, except for the length of the roots, the configuration of the canals at the apical third, cross-sectional appearance, volume, and surface area of the canals., (Copyright © 2012 American Association of Endodontists.
