Abstract

OBJECTIVE: Graphene and derived materials are a new class of fundamental nanomaterials. Thanks to the unique characteristics as excellent electrical conductivity, biocompatibility and thermal properties, this material has aroused a considerable interest in the scientific community from its discovery. The purpose of this work is to pursue the various steps chronologically that led from discovery to single production and in series of graphene, focusing on the advantages and disadvantages of the techniques going to underline the validity and importance of each, on the basis of the searches to be carried out and the products finished to be obtained. In the second section of this work, however, we focused on the evolution of the applications of this super material over time. In fact, graphene and bidimensional materials in graphene have been significantly used in various areas of biomedical research such as fabric engineering, regenerative medicine, bioimaging with application in neural engineering and in the clinic, with different future perspectives still to be completed.

2. A Full Computerized Workflow for Planning Surgically Assisted Rapid Palatal Expansion and Orthognathic Surgery in a Skeletal Class III Patient

Abstract

In the present case report, we present and discuss the digital workflow involved in the orthodontic/orthognathic combined treatment of a skeletal malocclusion correction in a 17-year-old male patient affected by a skeletal class III, facial asymmetry, sagittal and transversal deficiency of the medium third of the skull, dental crowding, and bilateral cross-bite. The first stage of the treatment involved surgically assisted rapid palatal expansion and occlusal decompensation, using fixed self-ligating appliance. An orthodontic software package (i.e., Dolphin 3D Surgery module) was used to perform virtual treatment objective evaluation by integrating data from cone beam computer tomography acquisition, intraoral scan, and extraoral photographs. The software allowed a comprehensive evaluation of skeletal, dento-alveolar, and soft-tissue disharmonies, qualitative and quantitative simulation of surgical procedure according to skeletal and aesthetic objectives, and, consequently, the treatment of the malocclusion. Using a specific function of the software, the surgical splint was designed according to the pre-programmed skeletal movements, and subsequently, the physical splint was generated with a three-dimensional (3D) printing technology. Once a proper occlusal decompensation was reached, a Le Fort I osteotomy of the maxilla and a bilateral sagittal surgical osteotomy of the mandible were executed to restore proper skeletal relations. The whole treatment time was 8 months. The orthodontic/orthognathic combined treatment allowed to correct the skeletal and the dental imbalance, as well as the improvement of facial aesthetics. Accordingly, the treatment objectives planned in the virtual environment were achieved. Virtual planning offers new possibilities for visualizing the relationship between dental arches and surrounding bone and soft structures in a single virtual 3D model, allowing the specialists to simulate different surgical and orthodontic procedures to achieve the best possible result for the patient and providing an accurate and predictable outcome in the treatment of challenging malocclusions.

Abstract

Purpose: The aim of the present study is to evaluate the influence and efficacy of autologous platelets on bone regeneration in a rabbit defects model. Materials and Methods: A total of 12 critical size tibial defects were produced in six New Zealand rabbits: A total of six defects were filled with autologous platelet gel (APG) and six defects were maintained as untreated controls. No membranes were used to cover the bone osteotomies. The histology and histomorphometry were performed at four weeks on retrieved samples of both groups. Results: No complications were reported in any of the animals nor for the defects produced. A significantly higher lamellar and woven bone percentage was reported for the APG group with a lower level of marrow spaces (p < 0.05). Evidence of newly formed bone was found in the superficial portion of the bone defect of APG samples where no aspects of bone resorption were observed. Conclusions: The evidence of the present research revealed that APG increases new bone formation restricted to the cortical portion and induces more rapid healing in rabbit bone defects than in untreated defects.

4. Oral bisphosphonate-induced osteonecrosis complications in patients undergoing tooth extraction: a systematic review and literature updates.

Abstract

OBJECTIVE: Bisphosphonates, the most common anti-resorptive medications, are internalized by osteoclasts, where they inhibit the macrophage colony-stimulating factor (M-CSF) pathway, preventing their differentiation, inhibiting anchorage to the cell membrane, and inducing apoptosis. In patients undergoing oral bisphosphonate therapy, oral surgery involves a high risk of developing drug-related osteonecrosis of the jaws (BRONJ/MRONJ), among the possible complications. MATERIALS AND METHODS: A systematic search was carried out on the PubMed, Scopus and Cochrane Library search engines, using the keywords “oral bisphosphonates AND tooth extraction”, “third molar extraction AND oral bisphosphonates”. In addition, we manually evaluated the articles included in references from other sources and an analysis of the Gray Literature was performed. A secondary outcome was to evaluate the assessment of pharmacological (antibiotics) use in the BRONJ/MRONJ management. The revision protocol followed the indications of the Cochrane Handbook, and was registered in the INPLASY database, while the drafting of the manuscript was based on PRISMA. RESULTS: The results of the systematic review, after the study identification and selection process, included a total of 7 studies: 4 retrospective studies, 2 prospective studies and 1 case report. The main complication was represented by osteonecrosis of the jaws, which appears to be related to the duration of treatment with bisphosphonates; in addition, data regarding the anatomical location of post-extraction sites, the sex and age of patients, comorbidities and various systemic risk factors were extrapolated. The most frequent post-extraction complication in patients treated with oral bisphosphonates is osteonecrosis of the jaws, with a significant prevalence in the posterior region of the mandible. In some cases, delayed healing of the surgical wound was also found; moreover, the duration of exposure to oral bisphosphonates influences the onset of complications. CONCLUSIONS: Ongoing studies continue to unravel the role of the oral environment response in alveolar bone homeostasis and how it might contribute to the induction of BRONJ/MRONJ. Approaching the problem from this perspective could provide new directions for the prevention of BRONJ/MRONJ and expand our understanding of the unique oral microenvironment. [ABSTRACT FROM AUTHOR]

Abstract

In this study, real marine sediments polluted by petroleum compounds were treated by means of a bioslurry pilot scale reactor. The treatment performance was evaluated by measuring the removal of total petroleum hydrocarbon (TPH), coupled to further analyses required to understand the mechanisms involved in the biodegradation process. The maximum TPH-removal efficiency reached 86 % at the end of experiments. Moreover, high throughput 16S RNA gene sequencing was used to describe the microbiome composition in sediment prior to, and after, bioslurry treatment, in order to identify the taxa mostly entailed in the TPH removal process. The raw sediment was mostly colonized by members of Sulfurimonas genus; after bioslurry treatment, it was noticed a shift in the microbial community composition, with Proteobacteria phylum dominating the remediation environment (high increase in terms of growth for Hydrogenophaga and Sphingorhabdus genera) along with the Phaeodactylibacter genus (Bacteroidetes). Furthermore, the assessment of gaseous emissions from the system allowed to quantify the volatile hydrocarbon component and, consequently, to obtain a more accurate evaluation of TPH-removal pathway by the bioslurry system. Finally, phytotoxicity tests on sediment samples highlighted an increase of the treated sample quality status compared to the untreated one.

Abstract

Background The independent prognostic impact of diabetes mellitus ( DM ) and prediabetes mellitus (pre‐ DM ) on survival outcomes in patients with chronic heart failure has been investigated in observational registries and randomized, clinical trials, but the results have been often inconclusive or conflicting. We examined the independent prognostic impact of DM and pre‐ DM on survival outcomes in the GISSI ‐HF (Gruppo Italiano per lo Studio della Sopravvivenza nella Insufficienza Cardiaca‐Heart Failure) trial. Methods and Results We assessed the risk of all‐cause death and the composite of all‐cause death or cardiovascular hospitalization over a median follow‐up period of 3.9 years among the 6935 chronic heart failure participants of the GISSI ‐ HF trial, who were stratified by presence of DM (n=2852), pre‐ DM (n=2013), and non‐ DM (n=2070) at baseline. Compared with non‐ DM patients, those with DM had remarkably higher incidence rates of all‐cause death (34.5% versus 24.6%) and the composite end point (63.6% versus 54.7%). Conversely, both event rates were similar between non‐ DM patients and those with pre‐ DM . Cox regression analysis showed that DM , but not pre‐ DM , was associated with an increased risk of all‐cause death (adjusted hazard ratio, 1.43; 95% CI , 1.28–1.60) and of the composite end point (adjusted hazard ratio, 1.23; 95% CI , 1.13–1.32), independently of established risk factors. In the DM subgroup, higher hemoglobin A1c was also independently associated with increased risk of both study outcomes (all‐cause death: adjusted hazard ratio, 1.21; 95% CI , 1.02–1.43; and composite end point: adjusted hazard ratio, 1.14; 95% CI , 1.01–1.29, respectively). Conclusions Presence of DM was independently associated with poor long‐term survival outcomes in patients with chronic heart failure. Clinical Trial Registration URL : http://www.clinicaltrials.gov . 8. Laser surgical approach to impacted maxillary incisors: case series and brief review.

Abstract

OBJECTIVE: Report Authors' clinical experience on impacted maxillary incisors treatment with laser technology. PATIENTS AND METHODS: Studied population was composed by 6 male and 12 female undergoing orthodontic treatments for lack of eruption of 28 upper incisors; eight impacted teeth (4 patients) spontaneously erupted after orthodontic creation of the correct eruptive space. Twenty maxillary incisors were surgically exposed; in 5 patients seven un-erupted incisors were exposed through an apically positioned flap technique; in two patients a guided closed eruption technique was performed on a central and two lateral maxillary incisors; in seven patients laser exposure technique was applied on ten incisors. All impacted teeth resulted correctly aligned at the end of therapy. RESULTS: Impaction of incisors is not a frequent event in dental practice. The positioning of the incisors was obtained by creating the eruptive space and aligning the un-erupted teeth with orthodontic treatment or after removing any obstacle or after their exposure with different surgical methods. In all patients the treatment allowed the restoration of both the aesthetic and function aspect of the stomatognathic apparatus. CONCLUSIONS: In this case series, patients undergoing exposure of the dental crown using diode laser showed the best postoperative course. [ABSTRACT FROM AUTHOR]

Abstract

Cardiac resynchronisation therapy (CRT) is a non-pharmacological treatment for patients with severe congestive heart failure (CHF) due to systolic dysfunction who also present an intraventricular conduction abnormality and a QRS duration greater than 120 ms [1]. CRT has been shown to improve functional status, quality of life, and exercise tolerance and to decrease hospitalisation frequency in these patients: recent results of the MUSTIC [2] and MIRACLE [3] trials are consistent in showing an improvement in NYHA class, quality of life scores, and distance covered during a 6-min walking test. A decrease in dynamic mitral regurgitation, increase in left ventricular ejection fraction, and reverse remodelling effect on the left ventricle has also been noted. The COMPANION study has recently demonstrated that CRT provides significantly better results regarding a combined endpoint of mortality and heart failure hospitalisation [4].

The beneficial use of biventricular pacing is reported in a patient with end-stage hypertrophic cardiomyopathy, intraventricular conduction delay and echocardiographic evidence of intraventricular dyssyncrony. Marked improvement in clinical status, left ventricular ejection fraction and peak VO2 were observed. As far as we know, this is the first report of a beneficial effect of a biventricular device in this subset of patients, and may be worth further investigation.

Abstract

Low exercise capacity is considered predictive for postoperative complications or death after thoracic and general surgery. However, in recent literature no agreement has been found about the predictive cut-off values for preoperative exercise parameters. The aim of this work was to investigate whether peak oxygen consumption (V'o2) and noninvasive anaerobic threshold (AT) determined by gas exchange threshold (GET) can be reliable preoperative predictors of mortality and morbidity after lung resection in patients with mild-to-moderate (forced expiratory volume in one second (FEV1)50% predicted) chronic obstructive pulmonary disease (COPD). Fifty tour COPD patients were studied before lung surgery: 12 had severe complications, 16 had mild and 26 had no complications. Peak V'O2 sensitivity and specificity in predicting severe postoperative complications were 41.6% and 95.5% respectively (using 75% of the predicted value as cut-off), while for GET they were 91.6% and 97.6% respectively (using 14.5 mL.kg-1.min-1 as cut-off value). Only one patient (3.5%) with a peak V'O220 mL.kg-1.min-1 suffered severe complications. On the other hand 11 out of the 26 patients (42.3%) with peak V'O220 mL.kg-1.min-1 had serve complications. In patients with peak V'O220 mL.kg-1.min-1, 11 out of 12 (91.6%) with a GETor = 14.5 mL.kg-1.min-1 suffered severe complications, whereas 15 out of 15 (100%) with a GET14.5 mL.kg-1.min-1 showed no or mild complications. In conclusion, peak oxygen consumption values20 mL.kg-1.min-1 can be considered a safe upper cut-off limit for pulmonary resection. In patients with a peak oxygen consumption value20 mL.kg-1.min-1, gas exchange threshold determination can improve significantly the predictivity of a cardiopulmonary test for severe complications and must be routinely considered.

15. Future New Indications for CRT: Which Patients Might Benefit?

Abstract

Cardiac resynchronisation therapy (CRT) is a non-pharmacological treatment for patients with severe congestive heart failure (CHF) due to systolic dysfunction who also present an intraventricular conduction abnormality and a QRS duration greater than 120 ms [1]. CRT has been shown to improve functional status, quality of life, and exercise tolerance and to decrease hospitalisation frequency in these patients: recent results of the MUSTIC [2] and MIRACLE [3] trials are consistent in showing an improvement in NYHA class, quality of life scores, and distance covered during a 6-min walking test. A decrease in dynamic mitral regurgitation, increase in left ventricular ejection fraction, and reverse remodelling effect on the left ventricle has also been noted. The COMPANION study has recently demonstrated that CRT provides significantly better results regarding a combined endpoint of mortality and heart failure hospitalisation [4]. Consequently, CRT has been included in the current American Heart Association/American College of Cardiology/North American Society of Pacing and Electrophysiology guidelines for the implantation of permanent pacemakers as a therapeutic option for patients with systolic heart failure, NYHA class ≥ 3, QRS duration ≥ 130 ms, left ventricular end-diastolic diameter ≥ 55 mm and ejection fraction < 35% [5]. According to these criteria, between 13% and 35% of patients with heart failure would be eligible for CRT [6, 7]. However, a growing body of evidence seems to suggest that a larger proportion of heart failure patients could benefit of CRT. Some of the ‘new' indications for CRT are discussed in this paper. 21. Effects of Oral Lacidipine on Cardiopulmonary Function at Rest and During Exercise in Normal Subjects

Abstract

A randomized, double-blind, crossover, placebo-controlled study was carried out to evaluate the effects of a single oral 4-mg dose of lacidipine vs. placebo on cardiopulmonary circulation at rest and during exercise. Twelve healthy volunteers were randomized to receive either placebo or 4 mg of lacidipine once daily for 2 days, followed by a 3-day washout period, after which they received alternate treatment. Patients were assessed before and at 60, 90, and 180 min after dosing. At 120 min, a maximum exercise test with a treadmill was per-formed according to the Bruce protocol. No relevant changes with placebo or lacidipine were observed in the respiratory function tests whereas 4 mg of lacidipine in-creased pulmonary effective blood flow (Qp.eff.) and stroke volume index (SVI) at 60 min, reaching a peak at 90 min: at 180 min, these effects, although diminished, were still present. The arteriovenous oxygen difference [C(a)O2] decreased, but reverted to normal values by 180 min. No differences in maximum attained p.effand oxygen consumption (O2) during exercise were observed. Only the heart rate was higher both before and after treatment with lacidipine. Lacidipine increased p.effin these normal subjects without relevant effects on respiratory function. Performance on exercise testing after dosing was normal, although drug-induced vasodilation was present.

Cases of acute pneumothorax admitted in the period 1975-1978 were examined to determine the presence of changes in ventricular repolarization. A significant elongation of the QT segment was noted in 23/126 cases. The series did not include subjects with chronic heart and /or bronchopulmonary disease, or other clinical features that might explain such elongation. The ECG changes bore no relation to the site and extent of the pneumothorax. Agreement is expressed with the view that their explanation lies in the existence of a reflex mechanism acting on the autonomous nervous system.

Abstract

In a 54 year old woman affected by infective endocarditis, with bicuspid aortic valve and aneurysm of sinus of Valsalva protruding into the right atrium, two-dimensional echocardiography helped to identify altered myocardial echo patterns of the interventricular septum at basal level. Because of the proximity with valvular lesions due to endocarditis, we could interpret these appearances as produced by invasion of the infective process into the septum. The autopsy report of septum rupture in the same area further supports our interpretation of the echocardiographic finding.

Abstract

In this study, real marine sediments polluted by petroleum compounds were treated by means of a bioslurry pilot scale reactor. The treatment performance was evaluated by measuring the removal of total petroleum hydrocarbon (TPH), coupled to further analyses required to understand the mechanisms involved in the biodegradation process. The maximum TPH-removal efficiency reached 86 % at the end of experiments. Moreover, high throughput 16S RNA gene sequencing was used to describe the microbiome composition in sediment prior to, and after, bioslurry treatment, in order to identify the taxa mostly entailed in the TPH removal process. The raw sediment was mostly colonized by members of Sulfurimonas genus; after bioslurry treatment, it was noticed a shift in the microbial community composition, with Proteobacteria phylum dominating the remediation environment (high increase in terms of growth for Hydrogenophaga and Sphingorhabdus genera) along with the Phaeodactylibacter genus (Bacteroidetes). Furthermore, the assessment of gaseous emissions from the system allowed to quantify the volatile hydrocarbon component and, consequently, to obtain a more accurate evaluation of TPH-removal pathway by the bioslurry system. Abstract

Contaminated marine sediments represent a critical threat towards human health and ecosystems, since they constitute a potential reservoir of toxic compounds release. In the present study, a bioslurry reactor was studied for the treatment of real marine sediments contaminated by petroleum hydrocarbons. The experimental campaign was divided in two periods: in the first period, microcosm trials were carried out to achieve useful indicators for biological hydrocarbon removal from sediments. The microcosm trials highlighted that the inoculum of halotolerant allochthonous bacteria provided the highest performance followed by autochthonous biomass. Based on the achieved results, in the second experimental period a bioslurry reactor was started up, based on a semisolid stirred tank reactor (STR) operated in batch mode. The process performances have been evaluated in terms of total petroleum hydrocarbon (TPH) removal, coupled with the characterization of microbial community through a Next Generation Sequencing (NGS) and phytotoxicity tests through the Germination Index (GI) with Lepidium Sativum seeds. The achieved results showed good hydrocarbons removal, equal to 40%, with a maximum removal rate of 220 mg TPH kg -1 d -1 , but highlighting that high contaminant concentrations might affect negatively the overall removal performance. In general, the observed results were encouraging towards the feasibility of biological treatment of marine sediments contaminated by hydrocarbons. Abstract

Oxidized low density lipoproteins (oxLDLs) may exert several pro-inflammatory effects that can contribute to the development of coronary artery disease (CAD). Evaluating a possible correlation between oxLDLs and clinical expression of CAD, we measured specific lipid peroxidation indices in healthy subjects and in patients at different clinical stages of CAD. We observed a slight, but not significant, increase in plasma content of cholesterol oxidation products, i.e. oxysterols, in all CAD patients, and a slight, but not significant, increase of 4-hydroxynonenal-protein adducts only in subjects with acute CAD. Moreover, CAD patients showed a plasma rise of specific inflammatory proteins, i.e. C-reactive protein, intercellular adhesion molecule-1, and interleukin-8, but not of monocyte chemotactic protein-1. These preliminary data, without excluding an involvement of oxidative stress and inflammation in CAD, do not show a strict correlation between relevant plasma markers, other than C-reactive protein, and acute phase of the disease.

Abstract

We report two brothers, aged 22 and 34, affected by dilated cardiomyopathy. At the first examination both patients were in NYHA functional class I. Their echocardiograms showed dilated and hypokinetic ventricles. Their clinical condition remained stable for three years, during which the peripheral immunophenotyping of T-lymphocyte subsets showed T-helper/T-suppressor ratio values lower than normal (normal value for our laboratory = 1.64 +/- 0.53) due both to low T-helper and to high T-suppressor subsets. Subsequently, the clinical condition of both patients deteriorated, exercise tolerance decreased and echocardiographic ventricular diameters increased. T-helper/T-suppressor ratio progressively increased from the initial values of 0.8 and 0.42 to 2.5 and 2.24, respectively. A few months later, both patients died, one because of refractory heart failure and the other one suddenly. Histologic examination of myocardial tissue in one patient showed a diffuse necrotizing disease involving myocytes with focal lymphocyte infiltrates, granulation tissue and endocardial thrombosis. The reported data underline the importance of genetic and familial factors in the pathogenesis and evolution of dilated cardiomyopathies. The increasing T-helper/T-suppressor ratio value which matched the deterioration of the clinical status, is proposed as a useful evolutive and prognostic marker.

Abstract

On the basis of some personal cases and with reference to world literature on the subject, the surgical treatment of liver and lung localized echinococcus cyst is discussed. Special attention is paid to 1) thoracotomy which may in some cases become a thoraco-phreno-laparatomy, the only approach which leads to really effective control of the field; 2) problems of suction drainage, particularly as regards diaphragm mobility in cases of thoraco-phreno-laparatomy.

Abstract

In a 54 year old woman affected by infective endocarditis, with bicuspid aortic valve and aneurysm of sinus of Valsalva protruding into the right atrium, two-dimensional echocardiography helped to identify altered myocardial echo patterns of the interventricular septum at basal level. Because of the proximity with valvular lesions due to endocarditis, we could interpret these appearances as produced by invasion of the infective process into the septum. The autopsy report of septum rupture in the same area further supports our interpretation of the echocardiographic finding.
