Abstract

Landmines pose significant humanitarian and strategic challenges, threatening both civilian populations and military operations worldwide. This study presents a practical simulation method based on Rayleigh and Compton scattering ratios to determine the effective atomic number of low atomic number (low-Z) explosives used in landmines. Utilizing the Monte Carlo N-Particle (MCNP) simulation program, Rayleigh/Compton scattering ratios were obtained using a Ge(Li) detector at a scattering angle of 115° within a simulated geometry. The simulation results were found to be in good agreement with experimental data, confirming the reliability of the method. Pure elements with atomic numbers ranging from 3 to 20 and various explosives were irradiated with photons of 59.54 keV energy to obtain scattering spectra. The effective atomic numbers calculated using Rayleigh/Compton scattering ratios were compared with five different theoretical methods, yielding consistent results. These findings demonstrate that the proposed method can reliably determine the effective atomic number of low-Z elements and explosives containing these elements. Abstract

Personalized treatment and precision medicine have become the new standard of care in oncology and radiotherapy. Because treatment outcomes have considerably improved over the last few years, permanent side-effects are becoming an increasingly significant issue for cancer survivors. Five to ten percent of patients will develop severe late toxicity after radiotherapy. Identifying these patients before treatment start would allow for treatment adaptation to minimize definitive side effects that could impair their long-term quality of life. Over the last decades, several tests and biomarkers have been developed to identify these patients. However, out of these, only the Radiation-Induced Lymphocyte Apoptosis (RILA) assay has been prospectively validated in multi-center cohorts. This test, based on a simple blood draught, has been shown to be correlated with late radiation-induced toxicity in breast, prostate, cervical and head and neck cancer. It could therefore greatly improve decision making in precision radiation oncology. This literature review summarizes the development and bases of this assay, as well as its clinical results and compares its results to the other available assays.

Abstract

It has been widely accepted that political regimes have a positive impact on various issues, from the establishment of international peace to the creation of economic welfare. Within this framework, the crucial role democracy plays as a political regime in overcoming the global problems has been studied extensively. However, despite the optimism about the global diffusion of democracy in the post Cold War World, today numerous countries which finalized their democratic transition are far away from being consolidated democracies due to their fragilities and deficiencies. These regimes, which cannot be classified as full- fledged autocracies but fall far short of the standard definition of liberal democracy, have been occupying the agenda of democracy theorists. In this study, we explore three macro theories frequently emphasized in the literature to understand the conditions of democratic consolidation: Institutional approach, socio-economic models and cultural theories. In addition, international factors, which have recently become more and more noticeable, are included in this analysis as a fourth variable. Bu cercevede, ozellikle kuresel sorunlarin ustesinden gelinmesinde siyasal bir rejim olarak demokrasinin oynadigi hayati rol literaturde yaygin bir bicimde tartisilmaktadir. Soguk Savas sonrasi dunyada demokrasinin kuresel yayilimina iliskin olusan iyimserlige karsin, bugun demokratik gecislerini tamamlamis pek cok ulke kirilganliklari ve yetersizlikleri nedeniyle konsolide demokrasiler olmanin cok uzagindadir. Tam anlamiyla otoriter olarak siniflandirilamamakla birlikte, liberal demokrasi taniminin da gerisinde kalan bu siyasal rejimler demokrasi teorisyenlerinin gundemini mesgul etmektedir. Abstract

INTRODUCTION The current phase III EORTC 1420 Best-of trial (NCT02984410) compares the swallowing function after transoral surgery versus intensity modulated radiotherapy (RT) in patients with early-stage carcinoma of the oropharynx, supraglottis and hypopharynx. We report the analysis of the Benchmark Case (BC) procedures before patient recruitment with special attention to dysphagia/aspiration related structures (DARS). MATERIALS AND METHODS Submitted RT volumes and plans from participating centers were analyzed and compared against the gold-standard expert delineations and dose distributions. Descriptive analysis of protocol deviations was conducted. Mean Sorensen-Dice similarity index (mDSI) and Hausdorff distance (mHD) were applied to evaluate the inter-observer variability (IOV). RESULTS 65% (23/35) of the institutions needed more than one submission to achieve Quality assurance (RTQA) clearance. OAR volume delineations were the cause for rejection in 53% (40/76) of cases. IOV could be improved in 5 out of 12 OARs by more than 10 mm after resubmission (mHD). Despite this, final IOV for critical OARs in delineation remained significant among DARS by choosing an aleatory threshold of 0.7 (mDSI) and 15 mm (mHD). CONCLUSIONS This is to our knowledge the largest BC analysis among Head and neck RTQA programs performed in the framework of a prospective trial. Benchmarking identified non-common OARs and target delineations errors as the main source of deviations and IOV could be reduced in a significant number of cases after this process. Due to the substantial resources involved with benchmarking, future benchmark analyses should assess fully the impact on patients' clinical outcome.
